Ok, this is gonna be a rant about the things I didn't like about the ending. Not the whole series, just the ending arc or episode. Although it does effect the whole series. So be prepared for criticisms 😬
I just finished watching the last part of AOT.
TL;DR: While it was overall entertaining, I'm unsatisfied with some of plotlines, characters, and how things turned out at the end of the story.
SPOILER ALERT if you haven't watched it already.
First thing comes in my mind, Eren killing his own mother. Why was it necessary? To ignite the flames of hatred within his past self? That titan eating his mother in front of him could have been a random occurrence or unforeseen incidence and that should have been enough to ignite any kind of hatred inside him. I mean if he was trying to protect his friends why would he kill his mother? She wasn't his friend? This just felt so out of character.
How, in the end, did everything turned out to be a 'romance' or love story? Ymir loved king Fritz? That just came out of nowhere. Ymir going to war, sacrificing herself to save the king. These just show that she is a slave who lost or lacks her will. There wasn't any love or romantic feeling in that. But who knows the king might have loved her in offscreen. But I didn't know that. It was more like a 'stockholm syndrome'. But there was no mention of it, nor was there any explanation.
Also Eren loved mikasha so much that he wants her to stay unmarried for ten years after his death and mourn for him? Why! Where did that even come from? These plots felt like what some might call an 'asspull'.
But what I disliked most is all these 'asspulls' becoming central plot points. How do you stop rumbling and end titans? By showing that mentally ill girl that it is possible to kill the person you love who is your abuser. Who knew in the end AOT is all about overcoming stockholm syndrome.
But I still don't get it why Ymir stopped Titans? What did she think about Mikasha kissing Eren's severed head? The main issue I have with it is that they didn't have any direct control over all these titan thing. It was completely dependent on Ymir's will. What if she didn't wish for it even after seeing Eren's death. It just felt so unconvincing.
There is nothing to talk about Mikasha. Her whole character revolves around 'Ereh'. She barely has any purpose for 90% of the story. She is like a walking mannequin. I enjoyed Gabi character far more than her.
I already talked about Eren. That whole time travel thing 'mindfucked' him upside down. Did he really think that the people who hate them to death for their titan blood will suddenly accept some of his friends after they stop him, a literally titan god with an over 300 thousands titan army, who massacred 80% of humanity? You just gave them more reasons to Assassinate your people. Morally grey? He didn't even have any moral. He is villain, straight up. His character is very inconsistent. And author tried to redeem it by saying, he is stupid. Although it was effective it wasn't convincing enough because you can literally make a character do any random out of character staff and just call them 'stupid'.
Lastly Levi's character handled poorly. He was overpowered so ofcourse the author had to nerf him to bring the plot in Eren's favour. He didn't kill him that time . But in the end his death resting on that rock would have been best conclusion for his character. But the author still kept him alive and decided to give him a wheelchair instead. You know 'Jujutsu Kaisen' has this same kind of issue, an excessive OP character who isn't protagonist. I wonder what's going to happen to him. Although I've seen few other series handle it properly.
OST, voice acting, animation, all the chaos going on in the background elevate the anime and make it more entertaining and kind of distract viewers from the plots. That's why I'll give the anime adaptation a 9/10 and plot wise the ending was 4/10 for me. And because "All is well that ends well", even though it didn't end well for me, it was still entertaining and full of thrill, I'll give the whole AOT a 7.5/10.

So me and my 5 friends (all 14/15) were on a small school residential trip to Paris and one of the days we went to disneyland.
We were in the queue for the Hollywood tower and its around 8ish. For context the ride is enclosed, 10 people in a room kind of thing. A man by himself jumps the queue by going under the railings and ends up infront of us. We immediately get bad vibes and notice how much he was staring at us and hung around near us.
We end up at part where you enter a room with lots of people and learn about the plot of the ride. He is standing at the back of the room and my friends and I stand near the front trying to keep away from him. The room goes dark and the TV turns on playing a video. By the end of it, the lights turn on again and he is wayyy closer to us than before.
At this point we were all nervous but we had been queuing for over an hour and didnt want to have wasted our time so carried on through to the inside queue. We hang back in the room before leaving so were not near him but he also hangs back in the room so he is near us in the queue again. We talk it through between ourselves and decide to go up the fast pass queue stairs to get away because we think "surely he wont follow us up here".
A few minutes go by and we think were in the clear but then behind my friends shoulder i see him walking up the stairs. My heart drops and I point him out and we all literally run through the queue to an employee and he lets us out the queue through a back door. We are all relieved and think we're fine. Walt Disney Studios closes and we leave, heading back into disneyland and to tell our teachers. I am constantly checking over my shoulder and then i see him behind us. i grab my friends and say "he's behind us, he's found us." We double back around and we lose him and we immediately go to where the teachers were hanging about.
After that a teacher stayed with us the rest of the night and thankfully didnt see him again. It was just a weird place to be followed considering you would assume its a safe place for kids.

I started twin star exorcist many years ago and after anime i caught upto manga then was reading monthly and at that time around chapter 76 came where Time Skip happens, so i thought this was good time to let few chapters release and binge read it then.
But then after that i delayed it again and again to read and i started reading many new manga/manhua/manhwa ,and i forgot about it . For some days i have been clearing my backlog of manga that were on hold to read so now i am catching upto them some have ended like Dr.stone and some have continued like MHA.. i easily found videos for MHA to catch upto but i could not fing about Twin star exorcist (found some but only covered anime).
I can't read upto ch 76 again and it will consume much time ,after i think 3-4 years i have forgotten many plot points so if anyone can recommend site or video about it ,cause i just skimmed through some chapter and remembered how amazing art is and also world building and characters

Please don’t come for me!! I’m a big fan and collect all her books and will always read them. Please don’t tell me “just don’t read them” the books were very comforting to me at a time I needed them but that doesn’t mean I can’t be open to discussion about her recent work and how her new direction or perhaps rush to put out books might be affecting quality.

Here goes:
I guess I expected Sloane to be a character who looked like Bridget but with the personality of Jules. Instead she behaved exactly like Allesandra, Isabel and Vivian. Speaking of Bridget, the Queen was more icy than Sloan (the series' designated Ice Queen) and she was supposed to be Rhy's sunny counterpart. We were constantly TOLD how icy and emotionless she was and never really shown it. Instead, we find out she's actually insecure and has a lot of issues, which yes is realistic to how Ice Queens typically form their avoidant personalities, but it would have been great to see a few chapters of her being icy before we peel the layers and find out why she puts up these icy walls.
If the book was supposed to be about how she’s NOT the ice queen people think she is…that wasn’t exactly shown either. She does admit it herself that she is icy, just hates that she is and hates being called that.
Xavier is the Jojo Siwa of bad boys. He was a bad boy, he did some bad things. The things that made him Colombia’s biggest bad boy with a bad boy reputation known all over the word: Parties with other rich kids, spends his wealthy father’s money, refuses to behave at publicity events like galas, refuses to inherit his family’s company, gets into shenanigans that land him in a night in jail with other rich kids, got a tattoo of his family rival’ crest….this one’s more stupid than “sticking it to his family”, slept around with whether model/It Girl was around. In the words of Phoebe Buffay “Okay, who hasn’t?!” Even Jules, Ava and Stella have landed themselves in jail after some shenanigan gone wrong. It’s clear that despite being a “spicy” book, AH created a Disney character version of a bad boy.
I feel like Ana forgot how to write from the male's POV after the Twisted series ended. The point of the dual POV is that you feel like you're reading two different people's POV, and honestly it all just blends together. I think Josh's book (Twisted Hate) was the best example of a male's perspective and thoughts/dialogue that a guy in that situation would be saying.
I noticed since King of Greed at least, these Alpha males have been carrying dialogue that mention the most ridiculous things that "Alpha billionaire" males wouldn't even notice. Example: In King of Greed, when Dominic makes a comment about the expression a Chinese food delivery person makes when he realizes he just dropped off a ton of food for two people.....I think that was an Ana Huang thought, not a Domenic thought. While we’re at it, Domenic was just as bad a Xavier…. Dante was the last good “Alpha” she wrote and Kai was such a different character (modeled clearly after the lead in Crazy Rich Asians) that he was hard to mess up ..
The Spanish in King of Sloth was important given the character's background, but felt like it was written with Google Translate. It felt very formal and boring, and doesn't feel like a Spanish-language consultant was brought in to make sure the random sentences and random Spanish words made sense. It was giving the same energy as the Netflix original shows that are set in Los Angeles neighborhoods and the writer's idea of Spanglish is sprinkling random Spanish words into English sentences. There was a little Spanish dialogue in Twisted Games that was way less cringier- maybe because the Spanish was in full sentences and then translated right after….. Latino English speaking people don’t add random “Tios” or “hermano” to their sentences. Look up @Leogonzall “how Latinos talk in movies” videos to see a visual representation of how the Spanish dialogue in these books feel.
Neither Xavier or Sloane had to work very hard to get together. We've seen small bits and pieces of how Sloan is essentially his babysittePR person and he's a chaotic playboy and I get the
Some random things that appear in every single book: - "A thought niggling......" - A table groaning under the weight of food - The first kiss consists of the male's mouth "crashing" into the female's - Every single female character has parental and sibling issues of some sort. - Every male character has parental/abusive parents of some sort. I'm beginning to feel like AH cannot write complex characters that don't rely solely on parental drama, as relatable as it can be...not counting her "If Love..." series, we've seen this 7 times in a row, for EACH main character. - Ana Huang writes parents and siblings the way Amy Sherman-Palladino writes children in Gilmore Girls, just very one-dimensional and for the plot. - Except for Isabel's good relationship with one of her brothers, Alessandra (and Ava and Josh) every female character has severe sibling drama, usually a case where their sibling is an over-achiever and conforms to their parent's expectations (and that's horrifying because it affects the main character and not because that sibling is also a victim that may have used conformity to survive their abusive parents actions)
Things I wanted to learn more about Sloan other than the same repetitive things over and over - How she started her PR agency and the staff she hires- what a girl boss! We only get a vague background about how she started her agency but nothing about how she built experience prior, and how she managed to build a reputation to become one of the best PR agencies in the world. We just have to take the author's word for it, and there's been so much build-up of Sloan in the three previous books.

I always imagined Sloane having a luxury apartment in NYC, something completely immaculate and stone-cold like her personality. We hear nothing about her apartment despite so many scenes taking place there.
We didn't get that many scenes of Sloane doing anything by herself…anything she eats or wears or anything. I feel we got more of these smaller details about the heroines in the earlier books.
I think Sloane got the Stella treatment- Both heroines's have been teased since the first book in the series but were a bit lackluster once the book came out. The both have parental and sibling drama….but they do yoga

The things I enjoyed about KOS and Ana Huang books in general - The world building in NYC and DC and other countries. - I love the scenes that take place in both DC and NYC - they feel so cosmopolitan but I feel we no longer get smaller details about the weather, street details, etc - we got plenty more of this in Twisted Hate and Twisted Games...even details about it being a misty or cold day, make a huge difference in creating imagery in the reader's mind. - The Valhalla scenes are typically entertaining and beautifully written, it's clear how lovely and gorgeous this country club is. - Any cameos where Alex shows up, are well-written. I think Twisted Love was one of her strongest books, so it makes sense he's one of her strongest characters who is stable and can carry any appearance.

Hello. I have had HS for most of my life. 55 years old male. Stage 3. Within the last year I had to stop avsola infusions because they were not working and I developed paradoxical psoriasis and other problems like sores in my mouth. Then I started cosentyx after fda approval. The disease kept getting worse. Cysts became larger and deeper under the skin. Also way more frequent. The only way to stop it was to stop eating any foods. Which I cannot do long term.
I started going to a surgery clinic to get I & D's when the cysts were very deep and scary and the pain was unbearable. ERs and various doctors in dermatology prescribed amoxicillin & clindimycin. But it seems like everything just got worse the more drugs I took. But I kept taking them like a good patient.
I had a new surgeon on a particular day with the last bad abscess on my side torso and the color bothered her because it was so green and large. So she had a culture taken. It came back somewhat worrying. (although I only see so much in mychart) I forwarded the results to dermatology and they immediately switched my medications to doxycycline and rifampin and thanked me for sharing the information. He explained to me in person a few weeks later that I am evidently one of the rare H.S. patients where the infections are real and are not some kind of disease thing. And the bacteria in them are resistant to the other antibiotics I was being given. Such as clindamycin. (It also said MRSA +3 in the mychart thing whatever that means)
I was given the two antibiotics to try to hold off the bacteria getting more resistant to these medications he told me. He also wants to increase the cosentyx to twice per month? Even though it makes one more susceptible to infections? And I have to take the antibiotics very long term. Not for inflammation reduction but to literally stave off infections.
Anyways, since I started the new antibiotics regimen I can function and work again. I don't want to kill myself anymore. I still have some problem areas that never fully healed. But mostly things are calm now with no new lesions. If I eat a lot of carbs like bread or rice the old wounds leak a little bit. But I don't have to go to the ER or anything. Maybe just take a bath a squeeze them hard. So I'm still avoiding carbs but I can actually eat foods now and not worry about it so much. I am supposed to stay on these antibiotics long term. But I am deathly afraid it will stop working at some point and I will be back in the hellscape of needing an I & D every 6 or 12 days.
Has anyone ever heard of a situation like mine? I know in the distant past some cultures came back "sterile" but not in recent memory. (Mostly they never culture it)
Has anyone any advice for me? I feel like a diseased patient with scars / wounds over my whole central body that are very slowly healing. Most have appeared in the last year. I am no longer in severe pain. Thank you and have a nice day.

I dyed my hair dark brown from bright blonde a little over a year ago, since then I have learned that I am a True Summer. I have had to put so many different color depositing conditioners, toners, etc on my hair to either keep it from being too red or too green, and having to try to strip it of color from being dyed too dark. All I want is my natural hair color, which last time I checked yearssss ago was a light brown/dark blonde. But the ends of my hair turn green and the lengths turn warm (multiple people have pointed me out as a red head or having auburn hair!). Have y’all gone through anything similar? I want to slap some more toner on because it doesn’t look good, but I also don’t want to further the build up or whatever is preventing my hair from being natural (I cut it above my shoulder so it all should have grown back). Help!

From what I know its a point of no return until some far point in plot

Being a writer, I often struggle to sit down and actually write. My often mind wanders somewhere else, and I find myself scrolling through Wikipedia or other useful, but distracting pages. The only time I can sit and enjoy writing is if I haven't done any preparation beforehand, and just do whatever I want. No fact check. Nothing. However, at some point I have to look something up or google given time period to get ideas. When I come back, my head buzzes with everything I learned and the need to figure out the details. I inevitably fall down the loophole of the endless planning. I believe, it has something to do with the next chapter being emotionally draining, so I keep avoiding it at all costs. Unfortunately, there's no other way but to sit down and write.

I am starting a new business. As part of this I am doing all the new business admin work such as creating social media accounts. I opened an IG account and almost immediately received an email saying my account was suspended. The purpose of the account is for business marketing and community interaction so I used my business name and email when creating the account. The suspension email states that I have violated the terms related to account integrity and authenticity. I guess because I used a business name instead of human name they think I’m a bot. When I use the emailed link to appeal, they say give us your phone number to receive a code. I enter my number and I get an error “code not sent, try again later, or use a different number”. I’ve searched online for this code and I’m guessing that IG won’t send to the number because it doesn’t match the number in my IG account. There is NO number in my account yet! They suspended it before I had any time to go in and do any actions at all. The only contact info attached to the account is my email. So at this point, how do I reach IG and appeal this suspension?? I am completely blocked, can’t open my page or anything. And, seriously, how on earth is anyone with a business opening up an IG account? If they are suspending automatically based on name only, how do people have accounts? This is really strange. It seems they are making this impossible for legitimate people in their effort to block bots.

I've been a fast and furious fan ever since 2013 when i first saw "Fast Five" on my fathers old laptop. It was perfect. Everything a child with a hot wheels obsession could ask for. Of course I was too young to understand the plot of the movie back then but the action was absolutely amazing. After rewatching the series I have come to the conclusion that the franchise has lost a lot of what it stands for. It went from being about street racing to Heisting (no complains there) then to being a detective style expendables wannabe and finally to literal science fiction. Another problem that the franchise faces is its obsession with bringing people back from the dead. Considering how invincible the characters already are it just seems overkill (Almost all of them having had survived accidents that would otherwise be fatal). Bringing back Letty was one thing but then bringing back every character that died after her was just plane stupid. I mean Look at how many people came back to life- Shaw, Han, Gisele and Letty. That compared with two characters that were supposedly a part of the story from a long time ago is just obnoxious. The whole story of Jakob Toretto was simply uninspired and forced. Furthermore, the lack of originality with Dante Reyes(Jason Momoa) put the final nail in the coffin for my anticipation of the next movie. Dante Reyes was not only unoriginal but also trying to follow this "Wannabe Joker From Dark knight" archetype. And what was worse still was the casting choice of Jason Momoa. This is not to insult his acting skills at all, its just that this character (A poorly written one) just doesn't match him very well. Coming back to my rant about the story itself. The amount of science fiction elements in the movies has increased stupendously and it is hurting the franchise. Like, Tej and Roman literally got to space, that too in a bloody car. I understand that the movie is supposed to be exciting and absurd but at this point it just seems disappointing and the plot seems braindead. Unfortunately as much as I love this franchise, I am definitely not excited for fast X part 2.

Hello everyone! I'm looking for a notebook with specific dimensions and qualities that so far I haven't been able to find anywhere. I'm probably gonna pour out my entire life story but please do me a solid and hear me out.
In my next notebook I'm planning to map out and document my life for the coming 6 to 12 months, literally writing the book of my life with the odd doodle or chart. For this reason I'm looking for a larger size, high page count notebook. I've used A5 notebooks my whole life, but I'm planning to upsize to B5 if possible, to add some heft and make it feel more like a book. Additionally I'm looking for something with a load of pages, 400, 500 maybe more, would be ideal. In terms of paper quality I'm looking for something ideally at least 80gsm, blank, low transparency, as I like to write fairly densely and with a medium nib fountain pen that writes quite wet.
To give you an idea of where I'm coming from here's what I have tried in the past and what I liked/disliked about them:

Moleskine - I know they have a 400 page notebook but it's smaller than A5 and the paper quality is hot garbage imo, out of question.
Clairefontaine flying spirit notebooks - I loved the smoothness of the paper but doesn't come in high page counts. I considered getting a few, stripping the cover, and stitching and them together to form a big notebook but as it's a heavy paper my desired page count would make a notebook over 5 cm thick.
Rhodia white paper / hard cover notebook, forgot the exact name: again great quality smooth paper but a bit too bright and white for my eyes, low sheet count.
Peter Pauper press: my favourite by far in terms of writing experience, very heavy, smooth and dense paper with zero feathering and little to no transparency even with gushing pen and ink and still dries very quickly. My only gripe if I have to name one is paper colour, it's a bit too ivory for a vivid blue ink (Diamine sargasso sea) to look great on it. Also they only come with heavily designed covers, no subtle black or navy options and low-ish sheet counts. But the same property that makes its quality is why it wouldn't be practical in high page counts, even if it were available.
Montblanc 146: I used these the longest because of overall quality, a bit scratchier feedback than PPP/Clairefontaine and less dense than I think would be my ideal, so it's a bit too transparent for my liking but if I had to use one for the rest of my life I'd go with this. My main issue is the price/value ratio, we know 2/3 of the €80 price is the little button on the front cover. The 163 (nearer B5 size) isn't available with plain paper though. If anyone knows a manufacturer with the same/similar paper please let me know regardless of formats!

Based on what I've tried so far, my ideal notebook would be as follows (possible compromises in brackets):

Around 500 pages but no thicker than 5cm overall (400 or even 300 is great if every other criteria is met)
B5 size (A5 is more realistic for all other criteria to be met, I know)
80 to 100 gsm cream coloured, blank, rather smooth low feedback paper (anything acceptable from odd/dirty white to not too yellow ivory, as long as it's blank and has little to no feedback/resistance. Would even go lighter to a 52/68gsm Tomoe River paper if only that is available.)
Soft leather cover (doesn't matter much, could even be hard or thin cardboard for all that matters, I'll be getting a custom leather cover for protection and added feel/heft.)
Milled/produced/crafted in EU, certified, organic chemicals free all the jazz. (I understand there are very few mills and presses left in the EU, but I'd prefer local manufacture on principle. Exception is Japanese paper or even North Am. made if it's widely distributed in the EU.)

Finally, here's what I've found so far and am actually considering:

Leuchtturm1917 B5 notebook - I'm considering this because it's an established brand so they'll likely be around in case I fall in love. If it had more pages I'd have already ordered this guy.
This 300 page B5 Tomoe River notebook off Etsy - I only heard good things about TR paper, I'm willing to try it even if not my conceived ideal, plus bound in Italy, yay! He also makes 400 page A5 notebooks so I might go with that, though that's about 20% less net writing area.
Odyssey notebook, A5 but with 500 pages, 68gsm Tomoe River paper - would be the first contender but it ships from USA (slow and costly), does anyone know of a EU distributor?
I briefly looked at blank books such as this one but printing paper and fountain pens don't mix and I couldn't reach this particular seller for info. Also based on ads there seems to be a host of these, even in my preferred specs, on Temu, Ali etc, but I'm not even clicking on sites like that, I hope that goes without explanation.
At this point I'm even considering going the full distance and binding my own frickin notebook if I could find (and try / read reviews of) the absolute most ideal paper as described above - which I cannot seem to. Any idea where to even look for blank paper in the EU?

So my sincere question to the community is, do you know of any other notebook brands that could suit my needs, what's your experience with them if any, and where I might order for quick EU delivery? Any comments or help is much appreciated, and thank you for reading!

Hello friends of the Steam Deck.
I really want to have a good controller to play my games on the big TV with the Steam Deck docked.
Must have (for me): - wireless - SteamOS compatible - correct A, B, X, Y layout (Xbox style) - Gyro / Motion Controll working and configurable over Steam Controller-settings - asymmetric Joystick layout - analog Triggers (no digital like the switch Pro) - hall effect Joysticks
Nice-to- have (but not must have) - colored face buttons (green A, red B, etc...) - hall effect Triggers - 4 way digipad (instead this 8 way ring style) - some LED/RGB stuff - programmable backbuttons - some other stuff like turbo, macros, etc...
absolutely NO-GO: - Switch layout face buttons - Playstation layout face buttons - laggy or buggy gyro / motion which will only work in a switch mode - cheap plastik feeling like stuff from a Kids-menue
I don't need a mode for nintendo switch, because i will use it 99% on Steamdeck or maybe sometimes on Windows. I don't need a Android mode too as I don't use any controller on my phone or tablets.
If the controller supports all of that I do not have a problem, as long as I can play flawless on the Steamdeck with all of the points listed under "Must-have"
I tried so far - original xBox Controller, but there is no gyro or motion - 8bitdo Ultimate Bluetooth, but it is mainly for switch with wrong button layout and only working gyro in switch mode (and then the triggers turns digital) - 8bitdo 2.4GHz for Windows or Android, but there is no gyro / motion control - GuliKit KK3, but the gyro / motion is real pain in the ass and works only when u push L button and you can't configure it in the settings.
I also tried - EasySMX X15 - EasySMX X10 - BIGBIG WON C2 pro - BIGBIG WON Rainbow 2 Pro - some cheaper all-in-one things
but all of them are not what I want or have some disadvantages, gyro problems, etc...
I found out, that the DS Controller for the playstation works really well. They have nice gyro / motion, I can adjust some LED and the Steamdeck recognise them really well. BUT I don't wand symmetric Joysticks or some squares, triangles or circles in my face buttons. Even if I would put some A, B, X, Y stickers on this buttons (lol, dirty workaround), the menus show the symbols of the playstation controller (squares, triangles, etc...). Beside that, the DS controllers don't have hall effect Joysticks. I don't like that!! I love the Xbox style, asymmetric joysticks and nice green A, red B, blue X and yello Y buttons!!
Please help me out of this freaky jungle and tell me what controller fits to my "Must-have" list!!

So I did most of the quest, including raiding that caravan, but when I go to talk to Galmar he hands me 600 gold and doesn't say anything. It just goes to his standard two lines instead. Ulfric doesn't say anything either, and the quest marker stays over Galmar. I've tried reloading saves but I still run into the same issue. I haven't had any issues like this up to this point.
I try not to ask questions on here without doing my research, but I haven't seen anyone else have this bug. I'm just looking for a console command or something that will let me proceed.

Warning that lives are at stake, the House co-chair of a commission focused on postpartum depression called on her colleagues Tuesday to advance legislation to stem the tide of worsening maternal health outcomes that are disproportionately impacting people of color.
Certified professional midwives (CPMs) would gain a pathway to become licensed in Massachusetts under a redrafted maternal health care package (H 4566) that was reported out favorably by the Committee on Public Health and the Health Care Financing Committee, which last week shipped it to the House Ways and Means Committee.
The bill would allow midwifery care to be covered by MassHealth, remove regulatory and staffing barriers to operate birth centers in Massachusetts, require postpartum depression screenings for new parents during visits to pediatrician offices, and expand access to lactation support, among other policies.
A similar bill (S 2734) cleared the Public Health Committee in April and was sent to the Senate Ways and Means Committee, and the bills could be poised to emerge in either branch if they are able to get through their respective Ways and Means panels.
“I truly believe that the more eyes we have on someone who has given birth, the better,” Rep. Brandy Fluker Oakley, House co-chair of the Ellen Story Commission on Postpartum Depression, said during a maternal health forum at the UMass Club hosted by the Massachusetts Association of Health Plans.
“Research has shown that when Black women in particular have a midwife as part of the birthing process, it improves their life outcomes, it improves the outcomes for the child that they give birth to,” said Fluker Oakley, who sponsored midwifery legislation folded into the maternal health omnibus bill.
The rate of severe maternal morbidity in Massachusetts nearly doubled from 2011 to 2020, with Black women experiencing the highest rate of complications. Sen. Julian Cyr, co-chair of the Public Health Committee, said the panel advanced the omnibus package “in response to the crisis we see in maternal morbidity and mortality.”
“A number of members in both the House and the Senate view taking action on maternal health as critical,” Cyr told the News Service. “As we come to the final months of the legislative session, the recipe for success in getting bills over the finish line is often having policies that are well vetted by the committee, demonstrated need for action, and a broad coalition of support, both among senators and representatives, and advocates outside of the State House. Maternal health legislation seems to have all three of those ingredients.”
A spokesman for House Ways and Committee Chair Aaron Michlewitz, asked whether the Boston Democrat supports the legislation and when it could potentially reach the floor for debate, said the panel is “still in the process of reviewing that bill.”
Rep. Marjorie Decker, co-chair of the Public Health Committee, said there’s a “lot of strong interest” in Speaker Ron Mariano’s office to advance the House bill.
“Ultimately, this is a really exciting opportunity to advance a number of maternal health bills that have been vetted for some time and with the support of the speaker, who’s really been a strong supporter for me over the last year and a half in developing this omnibus bill,” Decker told the News Service. “I think what’s before us is the opportunity to advance some really good legislative work that’s been uplifted by my colleagues.”
A spokesman for Senate Ways and Means Chair Michael Rodrigues, asked about a potential timeline to tackle the Senate bill, referenced the chamber’s support for maternal health initiatives in the budget slated for debate next week.
“The bill was just reported out of the Joint Committee on Public Health late last month and is currently being reviewed by Senate Ways and Means staff,” Rodrigues spokesman Sean Fitzgerald said in a statement. “The Senate has historically made a strong commitment towards maternal health, and in the FY25 budget, we dedicate over $30 million in funding on an array of programs, including reproductive health access, WIC nutrition, and family and adolescent health.”
CPMs, who provide clinical care for low-risk pregnancies, are eligible to be licensed in 38 states – but not in Massachusetts, according to the Bay State Birth Coalition. Midwifery care is linked to fewer maternal deaths, infant deaths, unnecessary C-sections, and postpartum complications, among other benefits, the coalition said.
CPMs, should they gain licensure here, are eyed as a key workforce for birth centers, an alternative care setting beyond hospitals for low-risk pregnancies. Recent closures have left the state with only one open birth center, located in Northampton. Midwives, who are accredited by a national organization, are also trained to provide care for at-home births.
“I think it’s hugely significant that these bills have made it to Ways and Means. It’s definitely some recognition from the Legislature, I think, of really a critical juncture we’re facing,” said Claire Teylouni, director of governmental affairs at Reproductive Equity Now, as she invoked the worsening maternal health crisis and a growing volume of home births.
“The reason why this bill is so important is that licensing them (CPMs) allows us to formally integrate them into the entirety of our maternal health care infrastructure, allowing them to prescribe certain medications, allowing them to work first and foremost at freestanding birth centers,” Teylouni told the News Service. “We are really excited to see the Legislature showing interest in leading on this because there’s tremendous amounts of research showing these are policies that will tangibly improve birthing experiences and birthing outcomes.”
In May 2022, the Special Commission on Racial Inequities in Maternal Health endorsed legislation to expand access to midwifery care, among a raft of other recommendations such as growing the doula workforce, modernizing birth center regulations, and creating a “birthing justice” omnibus bill that invests in social determinants of health like housing, transportation and education.
As part of Gov. Maura Healey’s review of maternal health services, the Department of Public Health this fall said officials “will continue to explore ways to support CPMs and develop pathways to expand the settings in which they can be part of pregnancy and birthing care and coverage for that care in Massachusetts.” DPH said there were 48 CPMs in the commonwealth in 2022.
DPH also said officials would update birth center regulations, including concerns raised over physician supervision and staffing requirements.
Fluker Oakley said the special commission’s report is not intended to sit on a shelf.
“Literally, lives are at stake. Even here in Massachusetts, Black women are twice as likely to die in the childbirth process – that’s here,” Fluker Oakley said. “For us to have this report that has so many wonderful ideas, which I also think are low-hanging fruit to deal with this issue, and for us just to kind of say, ‘Oh, we’ll get to it when we get to it,’ it really feels like a slap in the face. It’s like, oh, so you don’t care if we live or die.”
At the forum, Fluker Oakley also promoted the “Moms Matter Act” that she filed with Sen. Liz Miranda, co-chair of the postpartum depression commission, to grow the perinatal mental health workforce and provide grants to community organizations. The bills (H 1984 / S 1261) were reported out favorably by the Mental Health, Substance Use and Recovery Committee, and the Health Care Financing Committee has until July 3 to take action on the proposals.
Fluker Oakley, asked whether the Legislature has time to tackle the workforce bill in addition to the maternal health omnibus bill, told the News Service, “I have to remain hopeful and optimistic. Otherwise, what am I doing here?”
“So yes, I will say I think there’s time, and we’ll see what happens by the end of session,” Fluker Oakley said. “I’ve had several positive conversations with House leadership throughout the course of the legislative session.”
During the forum, perinatal mental health experts discussed the importance of boosting the use of postpartum depression screenings for new parents and connecting them with treatment.
Maternal mental health disorders are the most common complication seen during pregnancy and birth, affecting one in five women, said Lora Pellegrini, CEO of MAHP. Among Black mothers, 40% experience perinatal mood and anxiety disorders (PMADs), though Pellegrini said “few” end up being screened for anxiety or depression.
Research shows that 13 to 19% of new birth parents experience postpartum depression (PPD), a number that’s likely too low, said Beth Buxton, lead of perinatal mental health initiatives at the DPH. She said that range would indicate between 8,897 to 13,134 birthing parents in Massachusetts were affected by PPD in 2021, making the condition more prevalent than the combined cases of preeclampsia and gestational diabetes.
Buxton said only about 16% of eligible pregnant and postpartum people are being screened for PPD, based on regulations that require health plans and providers to report their screening to DPH annually. While Black and Asian individuals have higher rates of experiencing symptoms “often or always,” Buxton said they are far less likely to be screened than white women.
“And when you compare screening rates, the Hispanic birthing population is far less likely to be screened – that’s a new data point,” Buxton said, as she referenced information from the Massachusetts Pregnancy Risk Assessment Monitoring System. “One of the things we’re really asking providers to think about in the community is promoting universal postpartum depression screening. That is so important in multiple settings – not only within a provider setting, but a hospital setting, home visiting programs, a pediatric setting, so the gamut for universal screening.”
