Gyni exam

Growing up, for whatever reason, I have always wanted a flat stomach. I would be so unkind to myself for the sole reason of not having this one thing.
Last week, I went to the gynaecologist for a pelvic exam. And the gynie tells me I have a retroverted uterus. Basically, my uterus is tipped and the top pushes into my stomach. Because of this I can never have a flat stomach. I feel free, released, and relieved. I am finally accepting my stomach and even loving it. I'm not sad or upset, Im ecstatic. I love my stomach for what it is and Im not angry at for being something it cant. I dont know if this makes sense, I just needed to share it.

Hello, I’m a British Expat living in Spain (Valencia Region). In January I had to be taken to A&E by my parents, I was nauseous, cold and sweaty, shaky and vomiting up everything I ate or drank.
This was of particular concern to my parents because at 5” (152cm) I weighed between 5-6 something stone (70lbs-84lbs), I’ve struggled my whole life (since the age of about 5 months) with anorexia and weight gain, which is paired with a naturally (but also stunted) small frame and fast metabolism. Thus, throwing up in large quantities is a real worry for my parents.
We rushed to the hospital and my mum stayed with me the entire time, as I mentioned, we’re expat’s and I don’t know the language but my mother possesses enough to get us by. She checked us in and within 5 minutes we were being seen by a triage nurse who’d ranked me on a level of concern that they work with in that department. We didn’t know what level but by the wait we were there we were FAR down the list.
This is despite there hardly being any patients in the waiting rooms and the people that we did see get called in when we were expecting my turn were all people who’d entered much later than us and looked (physically) fine. It’s pretty widely know that Spaniards will go to hospitals for practically no reason at all, they like the social event in the waiting room and we saw this with a woman who would wheel herself around in a wheelchair talking to people before GETTING UP AND WALKING AROUND JUST FINE.
That disgusted me and my mother, frankly.
Now I’m not ignorant or unsympathetic, my mother and aunt are Heath care workers, I know how prioritization works and in my short time here so far I have had enough serious hospital visits for various conditions/ accidents that I know you’re not there unless something has really gone wrong.
But before even seeing the triage nurse my mum had had to rush round asking staff for a bag for me to be sick in as we were far from the bathrooms and I was too physically weak and in too much pain to even think about walking.
We were there for a total of 9 hours with me alternating between being extremely hot and needing to strip down to my t-shirt to being so cold I’d need to quickly put my hoodie and teddy bear coat on as well as a pair of gloves, then I’d be physically convulsing in pain before throwing up nothing but acidic bile into a plastic bag in a room full of strangers with medical staff that didn’t bat an eye.
Even during the times where I’d pass out in my mum’s lap from the sheer exhaustion other patients would ask her worriedly how long we’d been waiting with me in the obvious state I was in.
My mum tried to bring it up with a nurse some 6 hours into our wait and we were rudely told to “sit back down”.
The real nightmare only continued when we were finally called to be seen by a physician.
From the moment she saw us for the second time she was rude beyond belief. Our first encounter was just with her arm through the curtain as she passed us medical grade masks to wear over our material ones. We put them on and then a few minutes later she came back, berating us for putting them on wrong.
We were taken aback already but it only continued. She argued with my mum in front of me, and loud enough that I’m sure other patients heard, about my lack of being able to speak the language.
My mum tried to explain why I don’t (my depression was started by my Spanish speaking exams in high school so learning and using the language is a huge anxiety trigger for me). She cut my mum off in the middle of her attempt to explain my mental health issues, telling her she’d read my file already, before in her words she said : “you shouldn’t live here or seek medical attention if you do now speak the language... I’m not from here and I speak the language.”
Never in my life have I been made to feel this way by a doctor, I’ve had bad experiences here in Spain with my mental health experts but not like this.
So after verbally attacking both of us she told me to lay down and she felt my stomach, I told her where it hurt and then without anything she headed out of the room, mumbling something to us.
I asked my mum if she heard what she’d said (I naturally assumed she said it in Spanish after all that abuse we’d just received) to be told my mum that IN ENGLISH she said that I needed blood tests.
That was horrible, I hate needles so I cried through the whole experience and then when my IV was inserted. Thankfully, the young spanish nurse sent in for the task was extremely kind and gentle, she even answered my mum’s questions (where it is the physicians job to keep the patient informed) and eventually she helped me move to a patient room with a few other people.
Time for the real legal concern, I’m sorry it’s been long winded but I felt it was important to give insight to the rest of our experience there and with the physician so you had a real sense of how I was treated.
I’m skipping over some more rudeness and hypocrisy on her end because while we were waiting for my results my doctor came back after a few hours.
She stomped in, looking mighty pleased with herself before taking a stance in front of me with her hands on her hips and then announcing to the room of 4 (me, my mum, an English speaking patient and her partner), “Weed, you are sick because you smoke weed.”
Data Protection Laws, who needs ‘em?
My jaw dropped and I don’t think my eyebrows will ever go that high again. I couldn’t believe what I was hearing (or where I was hearing it) and neither could my mum. I was just looking between them both saying, “No, that’s not the reason.”
The physician was already gone now, clearly she didn’t want to stick around to answer any questions, more like she wanted to throw me under the bus and then dodge the bullet.
My mum knows I smoke, I smoke because of my mental conditions and eating disorder. It helps me regulate my mood, sleep better, eat better. She’s okay with it as she has seen herself how it can help me but my physician didn’t know that.
What’s really scary is that had my mum not been so open and understanding, I could have been thrown out onto the streets for it for all the doctor knew. And she didn’t care one bit.
She didn’t come back until my mum had continently left to use the bathroom, she was coming in to give me my release papers but I wasn’t about to let her get away again. I told her to stop as she was already racing out of the room after handing the papers over and begrudgingly she did.
I argued that I did not believe her diagnosis, she had yet to ask me a single question about my eating habits or if I’d even taken the drug willingly. I hadn’t smoked an ounce that day as I had an eye test that morning for some new glasses and it was when we got home that I started to feel sick.
She again wasn’t interested, told me she sees it every week and then when my mum came back in the room she told me the nurse would take out my IV and I could go.
As someone who already suffers from dilapidating anxiety disorders I now feel even more anxious over the idea of ever having to go back to any hospital. I almost refused to go to see the gyny a month later because I was so scared of the same treatment in an appointment where I’d be alone. Aside from that appointment that was already booked, I won’t be going back so long as I can help it.
In Spain, health care and emergency services are required to have an certain level of the English language as there is a large expat community here, this I know because my mum and sister are both certified English teachers.
I have everything typed up in detail, I wrote down the events as soon as we’d gotten home that night so I wouldn’t forget a thing, ready to send off the complaint to the hospital as directed by someone I emailed a few months ago in regard to what I could do about the matter.
Problem is, due to more than one unforeseen medical and family emergencies and a huge dip in my depression I haven’t sent the complaint off.
Have I missed my chance? This happened on Jan 5th/6th 2021. Do I even have a valid complaint? I felt absolutely attacked by her and then she broke data protection laws just to get me into trouble with my mum.
Any advice would be really appreciated, it’s been a couple of hard months but my mental health is so far on the incline and I’d like to pursue this if I can.

3rd year junior doc here.
This is actually a repost but since its getting the thumbs up, thought it would be beneficial for all, especially thinking about interview question "why do you want to go to medical school?" I reckon it's helpful to get some clarifications. Note that these are still mostly superficial aspects so you are welcome to ask more specifics and Ill try to address them.
2nd and 3rd years are pretty much lectures and labs - highlight for me was human anatomy labs. It's really an experience to be able to learn from donated bodies.
From 2nd sem 3rd year you start learning basic assessment of patients, taking a history and doing basic exams.
4th year onwards - clinical years - you go to hospital and learn from patients and bedside tutorials. Learning is mostly independent.
You start to put it together during 4th year with core hospital placements. General surgery, general medicine, orthopaedics, GP, geriatrics and more. Learning to think like a doctor using the basic science knowledge you have. There is a big clinical exam at the end of 4th year, which you must pass, otherwise you may be tagged or have to repeat the exam in 5th year (covering more content)
5th year is similar to 4th year, but different placements (Obs gyny, paeds ,psych, etc) There is also a selective placement of 6 weeks where you choose a placement you want to do. Some cool simulations with pharmacy and nursing students.
6th (Trainee Intern) year is similar, but you do core placements again. The main focus is learning to be a safe doctor and to be able to do the things expected of a 1st year graduate. You will get paid about 26000 throughout the year. Highlight is the elective placement - 8 weeks of a placement of your choosing of your chosen destination. Important exams include: medicine long case, Obs Gyny OSCE.
Remember from 2nd year you will be doing the progress test as well - same test done by everybody at every year level 3 times a year. If you do bad you get tagged or have to repeat the year if you consistently do bad. The expectation is that you do similar to your peers, that you are not falling behind, regardless of whether you get 10% or 60%. And the idea is you get more right as you go from year to year.

Thursday (my yesterday) I had a D&C and hysteroscope (sp?). I'm nearly 60, and far from my first rodeo.
Last night I was in bed with a hot water bottle, reading reddit on my phone.
And I have spent some time recently on health issues with my "lady bits" and not for the first time.
There is not a whole lot of fun in tracking issues with your health, but this shit saves lives.
Mortality rates for childbirth were higher than now.
Phil Donahue saved my mother's life in the seventies by encouraging women to have pap smears. She had to push her doctor a little, and she did have cervical cancer.
Sooner rather than later always gives us better choices.
Had my first pap with abnormal cells at 22. But cervical cancer is really slow. My first coloscopy was my first cold speculum, but alas, not my last. But abnormal cells were minimal enough to just scrape and follow up, for the next 35 years of pap smears.
Felt like I spent 35 to 37 in stirrups, as I went through "early menopause". To be fair, it's not really common for women that age, but it is far from rare. A few weeks ago my gyne asked my about my "premature ovarian failure". Told him it I was in my forties and online that I learned that term.
Today is my daughter's 34th birthday. Stirrups were involved.
I am glad that in Canada that enemas are no longer the norm for births. Yes, you have to clean feces off the table, and you had to before, just not quite as much.
I got a little worried when so many of my recent tests were fasttracked, but felt relieved when the latest uterine biopsy was negative. But the intern that did the exam was way more painful than the gp that did one a year earlier. I have more issues than my gynecological health, a muscle spasm from holding my legs caused a cramp. She tried to persist, but my gyne intervened a little to let me have a moment to deal with it.
I lived to tell the tale. And just after surgery, I was surprised my gynie had been worried about cancer still. I had not been since the first biopsy.
I did feel pretty violated a few weeks ago with the resident or intern that did not stop a moment, and I do thank her teacher for giving me a few moments to compose myself in regards to the cramp.
I hardly ever leave my home other than medical appointments, but I did my best to be cheerful and make people laugh. ,That interaction was good for me, and I really hope it was for staff involved.
Hopefully, the polyp they excised was the cause of the post-menopausal bleeding I have been having for the past 18 months.
I always hated pads, and it seemed so unfair to be buying them at nearly 60.
I'm out of the woods until the next time.

I'd like to share my story and I'd thought there are plenty of mamas who is in the same situation and thought that vaginal varicose veins were just something they'd have to live with forever...
So I developed varicose veins from my first pregnancy. As I stated the obvious, it is located in the most unflattering place...that's right...I'm talking about my nether regions. I just woke up one morning, only to discover, "Holy shit my vag is swollen!" I developed it late in pregnancy, but my OB at the time said it was a normal and common part of pregnancy and to basically stay off my feet when I can. He assured me that once the baby is born, it typically goes away quickly. It was quite uncomfortable and as one might expect, it made me completely self-conscious, especially during sex. After I delivered, just as my OB predicted, the varicose vein went away, thank goodness I thought to myself...or so I thought.
Subsequent pregnancies: I had two more pregnancies and long story short, the varicose veins made its appearance again.
Pregnancy #2, very early on, I asked my OB if this would happen again. She basically said, if you had it the first time, it is very likely to happen again. She was right, it returned with a vengeance as soon as I started my second trimester. Unfortunately, in this pregnancy, it ended at 18 weeks. Addressing this was the last thing on my mind.
Pregnancy #3 happened not long after our loss and the varicose vein again, raged on. It improved somewhat from the previous pregnancy, but not completely. Unsurprised, I just tolerated it, expecting it to pass as soon as baby was born. Our beautiful daughter was born in August 2016...and I still have it. When I had my 6-week follow-up visit, I brought this issue up and she urged that I wait up to a year for it to resolve on its own. She also mentioned that it isn't recommended that I go to the vascular department because treatment (sclerotherapy) isn't studied well enough for the region affected. Besides the obvious swelling, I consistently had this dull, achey pain whenever it flared up, especially when I've been on my feet for quite some time, during sex, or during that time of the month. During the flare-up, I would often shift positions when sitting, I kept thinking my undies were snagged, but no, it was my lady bits, which often felt pinched!
Additional symptoms: yes, you read this right. The varicose vein is only a damn symptom! I will list several biggies.
Dyspareunia (medical term for pain during intercourse): I've had this issue since the beginning of my first pregnancy. I frequently complained of this issue throughout this pregnancy. I was often tested for yeast, bacterial, and UTI infections as well as the STI/STD panel and consistently came back clean. At one point, one of the male OBs on rotation dismissed my symptoms and said it was all in my head. Right then and there, I started hysterically bawling. I was in so much pain, to the point where it was destroying not only my sex life, but activities in my daily life. Even wearing underwear irritated me. Thankfully, the NP on staff was on my case and pointed out that I clearly had irritation (she apparently could see redness in a localized spot) and referred me to a specialized gynecologist. Yeah, apparently there is an extra special gynie for those with mysterious vaginal afflictions! So off I went. He did all the standard testing, which included spraying me with vinegar (holy fuck, it was extremely intense). Apparently vinegar will result in pain in only the affected/abnormal area. So from there, they took a biopsy. They did this when I was about 30ish weeks pregnant. When they took the biopsy, I couldn't stop bleeding, it was ridiculous, but that's how it is when you're pumping twice the blood volume.
Anyway, results came back confirming that I had HPV. Which is apparently really common, a majority of men carry it, but there is no preventative measure and based on my history of sexual partners, it was likely my hubs gave it to me. Regardless, at 33 weeks pregnant (with baby #1), I was treated for it via outpatient laser surgery. The gynecologist assured me that I will be able to give birth vaginally and that it would not affect the baby in any way. At 39 weeks, I gave birth and within a matter of weeks, the varicose veins left quickly as it came.
However, as I mentioned earlier, with the varicose veins, dyspareunia also returned during pregnancies #2 and #3 and it was extremely intense. I also noticed that it mellowed out between those two pregnancies and in the following months postpartum after that third pregnancy. Regardless, the pain remains. It feels like this horrible burning pain, each time my husband was in me, the friction was unbearable, especially in a certain area. I felt horrible that I couldn't please my husband and I was also extremely frustrated. My OB (different one) was at a loss and did his best to come to a diagnosis, but to no avail. After recent birth, at the 6-week follow up, he gave me a referral for pelvic floor therapy to help improve this condition. There were no improvements in this area of pain, buuuuut they sure did strengthen my pelvic floor.
Anyway, as I was routed back in the care of my gynecologist, I continued to press about this issue. Again, she did all sorts of testing, including two more biopsies. Came back with normal results! I even tried to see a skin specialist that specifically addresses vaginal issues but she was booked for over a year. Thinking maybe I have vaginitis or vulvodynia. ...so I just gave up and accepted the fact that I had to live with this, because I was just out of options.
My sex life is not the same. I won't say we have a dead bedroom, but it isn't the same as it was pre-baby, not even close. Not only do I experience this particular pain, I also experience increased sensitivity. I am so ridiculously sensitive that even lightly touching myself is just too much! I can't even tolerate being on the edge of a climax. So as you can imagine, these symptoms has really dampened our sex lives.
Kidney infection/bladder infection/UTIs: during pregnancy #3, I've suffered from a kidney infection when I was about 26 weeks pregnant. It was a horrible experience, I truly thought I was going into preterm labor. It is a long dreadful story, but I'll give you the short version, the OB at the time assumed that because I didn't have a UTI or a bladder infection, the growing baby must have pressed the ureter, resulting in a kidney infection. I've never experienced any type of urinary infection of any type prior to this. Since having the baby, I've had reoccurring kidney infections and UTIs, despite preventive measures. I've grown paranoid, if I felt the urge to urinate, I'd go immediately...and often. I would drink water as if I were a camel. I took my cranberry tablets religiously. If I experienced a a symptom for a UTI, I would go immediately go to an urgent care. Being on the lookout for this issue is truly exhausting. I've had 6 UTIs and two kidney infections. I was hospitalized both instances of my kidney infection and even scanned for issues with my kidneys (and it came back clear, no stones, nothing, nada). I have been on antibiotics more than I'd like to and my gynecologist even prescribed for me to keep them on hand! I have instructions that after every intercourse, I take an antibiotic. Who wants to live like this?! I can't even fuck my own husband without weighing the benefits, "Is having sex with my husband worth another damn kidney infection?" I kissed spontaneous sex good bye after that prescription.
Anyway, I'm sharing these different symptoms because it all falls under one damn umbrella. They are all symptoms related to my varicose veins.
It all adds up to this: pelvic congestion syndrome. Basically, it is another form of varicose veins, only in this case, they're like the super varicose veins located in your hips. The blood vessels in my hips is causing the vaginal varicose veins, the frequent kidney/UTI infections, and even the painful sex. It is often overlooked in the gynecological community (especially during exams in the supine position, because it isn't acting up!) or doctors would only be treating the symptoms, but not the underlying cause. The veins have a flap that opens and closes and often during pregnancy, your vessels dilate due to increased blood flow for two people, but the flaps can't compensate. However, during/after a pregnancy, it is apparently common for the flap to be abnormal, resulting in blood pooling (via gravity) in your pelvic region (hello swollen vag!). So in short, gravity is just not your friend during this time. As my doctor has explained, it is extremely common after multiple pregnancies, especially if it is close to each other. He also listed other symptoms and without knowing my medical history, he pointed out how PCS was a big factor in dyspareunia, I cried happy tears.
Had I been diagnosed earlier, I could have avoided multiple visits with different doctors and saved a ton of money. Let's not also forget the emotional and psychological stress that came with it! These issues also came close to doing some real damage on my marriage. It all could have been avoided if this condition were widespread knowledge in the obstetrics/gynecology community. Unfortunately this isn't the case...so I thought I'd do this post. If this is you, I just want to tell you, that you're not crazy and it isn't all in your head.
So there is treatment! A simple outpatient surgery (I will be scheduling mine this month) using different methods of sclerotherapy (basically saline embolization). First I had to get a CT scan (or for some people, an MRI) for the diagnosis and as he expected, I have two vessels (some people it would just be the one) that are extremely dilated. According to my doc, normal blood vessels in the hip are approximately 2-4mm...mine is 11 and 12mm, and apparently he has seen worse (sure that makes me feel so much better doc!).
So I am looking forward to this treatment because I just cannot let this affect my quality of life! So if you're one of these ladies, I encourage you to look into this as it is often missed! Most women including myself, accept their fate of having these symptoms as something to live with, when they really don't have to!!
TLDR; pelvic congestion syndrome, one of the big symptoms is swollen vag (vaginal varicose veins) and there are a few other big symptoms. It basically means varicose veins in the hip, which can only be diagnosed via CT/MRI scan. If you still have this issue, you might have PCS and there is treatment!
Sorry this is long, but I hope it gave you some insight and helps anyone who is having similar issues.

A few months ago I posted on here, very frustrated about being dismissed by a doctor (who I recently found out has a history of being unhelpful) and was met with so many lovely women encouraging me to keep going. Shortly after this appointment I put in an informal complaint and made a point to get myself put on the waiting list yet again. The original waiting time I was given to expect to wait was around 6-8 months! Sometimes having free healthcare isn't all that great. But I kept myself medicated and continued my studying to distract from the massive pain I was in daily. After my recent exam, I couldn't take it anymore and called the department asking if there was anything I could do to be seen sooner.
The lady at the front desk was incredibly helpful and put me on the "cancellation list", or I've heard some people call it the "emergency appointment list". She told me that this meant if anyone cancelled their appointment I would be on a list of people to be offered the spot. What do you know, a week and a half later I was offered an appointment just days away.
Admitting that you're scared is something that's very hard to do as someone with a chronic pain condition, because if I did it every tie I was scared, it would be a catchphrase. But scared would be an understatement for how I was feeling going into this app. Luckily, I managed to get the most helpful gyny I've ever met, who immediately apologised and listened to my symptoms: agreeing it was certainly Endo. Then he offered me either a laparoscopy (I was nearly in tears at this point) or a decapeptyl injection, or both if I wanted. I accepted both and now I'm waiting for a date to come through for my laparoscopy, which they mentioned is usually in a 12 week time period. Right now I'm on the Decapeptyl injection and was also looking for advice from any ladies who've tried this one. (Has anyone else experienced depressive symptoms as a result?)
My advice for women who're going through the NHS is the way I've moved through the system:

• Never ever accept a dismissive gyny. Legally, you have a right to a second opinion and don't feel anxious about being faced with adversity because they have to keep their reputation up.
• If you've been on the waiting list for an appointment for longer than a few months, call the desk and request to be put on the cancellation list. Tell them just how much pain you're in to help your chances. Keep calling. It's your human right to live with a decent quality of life. *If a doctor was incredibly dismissive and you feel like you are being ignored by the department, the CAB can help you figure out your next step.

It's been a long 3 years and I'm so grateful to be getting a diagnosis this early compared to many women's stories I hear. Thank you for reading!
