2024.05.19 19:55 World-Explorer53 Seeking Advice on Appeal Options for Insurance Denial of Medically Necessary Inpatient Stay

Hey,
I recently had a bike accident and broke my tibia. It was a grade I open tibia fracture. I have Anthem BlueCross/BlueShield insurance, and the hospital where I had my surgery is an in-network provider. A few days after my discharge, I received a letter from the insurance company stating that my inpatient stay post-surgery wasn’t deemed medically necessary.
I spoke with my provider about it, and they mentioned that this is a common issue with insurance companies and not to worry. The provider's office called and advised me to appeal the decision, explaining that I was on IV antibiotics due to the grade I open fracture to prevent infection at the wound site. I followed their advice and appealed, but I'm still concerned that the claim might be rejected.
The two-day inpatient stay cost around $90k, an amount I can't afford, even with a three-year installment plan without interest, as I am an international student. Can anyone advise on potential options if the insurance company rejects my appeal?
Location: Bloomington, IN. Thank you.

2024.05.19 19:37 by_poeticghost 05.19.24 He didn’t leave without a bang

So I’ve been single for 6 months now, and I felt like everything was going good until just last week. My mother received a letter in the mail stating that my ex was involved in a car accident when we were still together. My car is under her name and the insurance as well so as you can imagine he wasn’t covered. No damage was done to my car so I never noticed anything happened. I called up my ex and he told me it was a basic fender bender, I told him that the lady who’s car he hit got an attorney and were going after my mom to pay for the damages and to get her license suspended. As I knew he didn’t give a fuck, he told me it was a scare gimmick and that they wouldn’t do anything. Regardless I’m going to be the one responsible to pay the $1,670, I don’t think my mom should be held liable, I already set up a payment plan with the office. My ex took my car at the time without my consent, a coworker of mine was taking me to work that week. He somehow got the car to turn on and was on his way to Autozone when he rear ended the lady. I have so much late towards him because he never wanted to take responsibility for anything. But mark my words KARMA IS A BITCH…..

2024.05.19 18:56 Batman_In_Peacetime Update and request: Cheque Fraud: Rs 180 -> Rs 75,000 cashed out. Original text was overwritten. Please read the details and help.

I made a post 3 weeks ago about a cheque fraud (Linked here) that happened to my neighbor. Kind folks here commented and asked for details. Here they are.
Some of this may sound silly and stupid, but I am stating facts as they are. Please read and help.
  1. Scamster posed as Metlife Insurance employee who needed to collect "final installment" of Rs 180 from neighbor. He had accurate details of insurance policy. He visited neighbor's home to collect, but didn't accept cash. Payment could only be made by cashier's cheque.
  2. Neighbor wrote a cashier's cheque. He clearly wrote the name, amount (text and number), date and signature in ball point pen.
  3. The words on cheque were overwritten. Name, amount, date are changed. Signature is untouched. The cheque has some indication that words are overwritten. I mean, it is like how pencil is erased but some letters are somewhat visible.
  4. Neighbor did not have positive pay facility. He received no call from the bank when cheque was cleared.
  5. Complaint and escalations - Bank has taken no responsibility and shifted the blame completely on neighbor.
  6. FIR not registered - Police has taken a complaint, but refused to register FIR.
I am attaching masked cheque here. Please take a look for yourselves and suggest possible measures. https://postimg.cc/CB0ykmW4. Note that the cheque might look torn, but it is in one peace.
I am willing to escalate to Nodal, Principal Nodal desk and RBI Ombudsman. Also, I will push neighbor for FIR being registered - what are the grounds for FIR here?

2024.05.19 17:30 GrandPsychological How much leeway is there to be uninsured if my car is taxed.

Recently had to sorn my car as I didn’t realise the law around continuous insurance for taxed vehicles. (I’m a young driver and a student so I wanted to be able insure my car on and off) took them 8 months of me doing this (completely unknowingly) and then I got a letter in the post threatening fines / criminal record if I didn’t insure/sorn my car within 28 days or if they see my car is taxed but uninsured again.
Anyway I’m back for summer and my MOT is due so I want to get that done. However because I’m a young driver the insurance policies available to me are extremely limited in the first place and my go-to best value insurance will not cover an untaxed vehicle full stop.
Can’t tax my vehicle (or take it to MOT) without an insurance policy so I need to purchase extortionate temporary insurance to do that all for a day. But then I have to wait the standard 2-5 working days for my car to show as taxed on the online database, else my normal insurance provider will probably reject my application.
TLDR My question is this: can I get away with leaving my car functionally taxed but uninsured for 2-5 days whilst I wait for my car to show as taxed online? (obviously I wouldn’t be driving it) or do I have to suck it up and pay hundreds (literally) for a whole week of temporary insurance to cover me for that processing period.
Bit stuck over what to do about this because I don’t want to fork out hundreds for no reason but I also don’t want to get fined. Surely there must be some leeway because people have to switch their insurance policies all the time right?

2024.05.19 16:11 siherbie Help with Issues with Death Claim Settlement & filing deceased ITR

So I am 37(M) and my mother (73) before she passed away on 24/12 /23. The thing that happened was that my mom ended up, having health scares one after another last year. So essentially it was first when she was told to get minor cataract surgeries, this time of month last year and both her and me were busy with her tests and later surgeries in June, July. Since she had Care insurance, she applied for her cataract surgies claim - earlier both got approved for 30k each for both surgies costing almost 50k each but then suddenly they backtracked & said one claim was rejected as place where we got surgies done, was blacklisted later. Now the single 30k claim should have been deposited, right. But nope, it still stuck & since my mom later got diagnosed with stage-4 kidney cancer in September which really shocked both me and my mom as my mom's tests have always been decent, we couldn't follow up more than couple of attempts & instead got focused with dealing with the cancer situation. Irrespectively, I got her on keytruda immunotherapy asap as surgeon couldn't operate till it was reduced and my mom was indeed reacting very positively to the treatment despite some issues with side-effects. However despite my best efforts, my mother suffered a brain stroke and I had to take the difficult decision of taking her off life support while dealing with another care insurance tantrum where they rejected her icu charges when she was admitted as I rushed her to hospital while having a brain stroke. In short, I ended up paying almost 1lakh out of my pocket and still had to pay additional 2.5lakh for further processes which was further complicated by my narcissistic sister who was only around to get her inheritance & making my life hell so I had almost no time to arrange the amount as I had only 40k left. Thus I unfortunately withdrew funds from my mom's accounts (I am Nominee & had her credentials though I also refunded them once I had a more stable mindset & recovered from fevemy learning disability med issues throughout January). Either way, I initiated the Maharashtra govt ppo closure (I had refunded the amts taken from pension accounts prior to initiating paperwork) by Jan end & also approached local cbi home branch for also closing my deceased father's family pension to my mom. The branch manager directed me to this problematic deputy branch manager to whom I did admit that I had to withdraw funds from my mother's pension accounts but I have refunded them once I gathered resources(basically my sibling & family members are useless besides the fact that my sister has indulged in some criminal offenses like harassing me & my mom both before & after her death, etc while said family members did nothing besides ignoring my complicated health issues as I could have died the day my mom passed away). Since my mother also took 3 online FDs earlier in 2023, the deputy branch manager wanted me to break them & told me that individual account won't be closed & only her CIF would be directly deactivated. Also it was only few months for FDs to maturity & state govt pension closure was yet to occur - I told her that I will file for death claim settlement once state govt pension is closed & excess pension is recovered by them as deactivating the CIF will make this troublesome & a legal case. So she agreed to wait till state govt pension is closed and I gather all necessary documents in the meantime. Luckily state govt treasury did close the ppo & withdrew excess pension by 19th March while I also managed to transfer most of the mutual funds where I am Nominee. However since state govt treasury office (it's in another city from where I live & reasonably far), didn't give me letter of closure/confirmation - I was asked to wait for 1 month by local treasury officer (as financial year closing was approaching) which I informed to said deputy branch manager again. During this time, I also had to deal with legal paperwork as the lawyer my mom knew, didn't inform her or me to register the will, my mom made & also wasted my time in March. So I had to look for other ways to inherit one immovable property (my sibling holds 16.66% share and I have 83.34% as per my mom's wish to inherit the property as per her will), one problematic mf scheme transfer which I accidentally botched by editing Nominee details (still not sure as my mental health was seriously impaired in Jan/Feb & I also have same scheme, so not sure how it happened as there's no email confirmation except for an otp I found though I am not sure why I would update nominee details when I am already nominee there & even mentioned in will). Since UTI rejected both of my transmission claims so I am waiting for my current lawyer to complete the gift deed & apply for heirship certificate at municipality for filing my deceased mom's ITR. Also please note that despite timely updates to deputy branch manager, she let the FDs renew without my permission & has been acting dubious besides acting very rudely with me. So situation is,
  1. I have filed compliant with cbi bank (or rather couple of them as again, I wasn't informed properly nor received proper feedback from bank plus despite calling customer care for almost 7 times, most aren't clear in their instructions as some mentioned different email-ids to complain to one asking to file a complaint with branch itself but I am not sure as branch manager said nothing despite my confrontation with deputy branch manager happening in front of him). Now I will be collecting the FDs physical certificates from branch & wait for resolution as said deputy branch manager is very suspicious(like asking me to file false FIR for missing online FDs physical certificates which I never had & whose online receipts I already have to also blaming me that I didn't tell her they were online while she wasted time & made me do a Rs500 stamp paper indemnity bond with additional documents like death certificate, AadhaPan, etc or saying there will be penalty for breaking autorenewed FDs that strangely got renewed on maturity dates without any prior indication & some things not adding up with new scheme plus the whole confusing me with different instructions & false claims all the time). Though luckily I have recorded the conversation with her first blaming me then backtracking once her fault was found. Also I atleast managed to travel to the distant city for closing state govt pension bank account & it was transferred to my savings account (it's at same problematic home branch) on 3rd May & de-activated my mom's CIF. So as bank resolution will take additional 15days to come(customer care first said 48hrs & now saying different), should I approach branch with resolution & LHC to minimize further problems despite being Nominee.(there's an error in relationship in cbi's savings account where my name & details are there but relationship is daughter instead of son but I already made Rs.100 self-affidavit as per dubious deputy branch manager instructions & all other documents including death claim form).

  1. Applying for my mom's final ITR is necessary & I have already submitted most financial statements to my CA to compute including the TDS deducted on the FDs since I have been given July-end deadline. Plus as my mother & me spent more than 10lakh+ for her cancer treatment, I am hoping that I will get some deductible relief on her final itr besides tds. The issue is that I am still lacking any legal heirship document that will allow me to register as assessee for filing my mother's ITR. I did ask another bank where I am also nominee for mom's account, for issuing bank letter confirming Nominee details but they refused. However hopefully as I am going to execute a registered gift deed with help of my new lawyer that will mention my mom's unregistered will, I am hoping it allows me to register for the ITR. Otherwise I will have to approach local municipality office for issuing surviving family membeheirship certificate once gift deed allows me to initiate transfer property to my name.

  1. There are still some untransferred assets such as a SBG linked to my mom's account besides the UTI scheme that's linked to same account(for uti, I already have most paperwork ready except that they asked for legal document that confirms relationship between me and my mother so while I can submit my passport copy though feeling it's better if will gets registered during gift deed process as it's mentioned there). Then there's the care insurance claim associated with same account & taken by bank officials of same bank yet it came to my attention that Care apparently messed my mom's name multiple times while issuing the policy & the claim hasn't processed as policy name & bank account name don't match (it's only a difference in middlename and could have been conveyed to my mom when she approached them multiple times). I did ask my lawyer regarding this & as amount is only 30k, he has asked me not to think about claim as for now as Care doesn't seem sincere as they haven't given it in writing that they will issue claim if I submit LHC/Registered Will with other documents. So while I will get SBG once I close the account & also uti mf scheme (hopefully), I am not sure if I should file a consumer case against Care (as an employee even misbehaved with my mom & the claim situation feels really fraudulent) or just hope that Care would honor the claim once I submit the documents.(they also only mentioned sending documents over email so again felt weird)

Overall all these incidents have seriously affected my health and frankly I need a break for myself as I am still struggling with my grieving (I saw my mom die 3 times & the family drama still triggers my cptsd though I am not suicidal or having med issues unlike in Jan besides still unable to take some personal time off due to these commitments). I also apologize if this post ended up too long or having details all over the place as I am still trying to piece together as much information I have as I do have written journal records of the paperwork & events that occurred though I forgot to list down some details here & there due to trying to manage everything by my own.

Never Give Up! Appeal Won!
NEVER GIVE UP!
For those that were denied Mounjaro by their insurance company because they did not meet the Type 2 diagnosis please do not give up!!!! I’ve been fighting this since Jan 2024 and nearly gave up after my 2nd appeal I was so exhausted. Find a doctor that will help you in writing your appeal. I also have to shout out mochi because I had given up and was going to buy the telehealth version but they’re prior auth team sent in a prior auth for Mounjaro I gave them the accurate ICD codes to use (VERY IMPORTANT) and I was approved! I never got a denial letter from the 2nd appeal I submitted so I believe that did the trick.
I have insulin resistant PCOS, BMI 48, pre diabetes A1C 6.2 and family history of diabetes. This medicine is worth the fight keep going!

2024.05.19 15:37 therealmocha Can’t get an apartment, previous apartment says we owe $10k (IA)

Hello, if this is the wrong place for this lmk, I’m kind of stressing tf out right now. My fiancée and I are trying to get an apartment by the end of the month, since our current lease is up. Everywhere we’ve been applying keeps denying us, despite us making 3x rent and having decent enough credit.
One of the places sent me a copy of my report along with my denial letter, claiming unpaid rent as the reason I got denied. However, there was nothing showing in collections. When I called, they mentioned an apartment complex we lived in 2 years ago that popped up while doing background, that said we owed $10k?
I can get into those details if needed, but basically we came to the end of our lease there, were month to month for a bit, then moved out without letting the complex know. I understand now that that was stupid, since they ended up charging us rent for the next several months, which amounted to the $10k.
After calling the complex I talked to a guy who basically said “yea, you’ll just have to take it to court there’s not much I can do I’m sorry”. I really don’t have any idea how to do that, and still don’t have a place lined up.

2024.05.19 15:36 Ok_Poet4112 Income limit

Hello there I'm in Michigan and I keep getting denial letters and getting sent a chart with the income limit on it. It says that for a 2 person household you need to make 27,000 and I only make 25,000 annually on my disability. When I call (I get different answers everytime you talk to someone) one lady had said that the income limit is different for those on disability. Is this true? Also was told that I have 3 different cases open for some reason. Can someone please explain because when I try to Google for income limit on disability I don't find anything at all and also why are they sending me a chart that doesn't apply to me if this is the case?
Edit: I've been on medicaid and medicare for at least the past 3 or (maybe 4) years. My renewal was in January and I still was approved then in April got a sudden change of Healthcare out of the blue saying I don't qualify for full anymore and now I'm on something called the "first step plan" or something like that.
My main reason for this post is to see if anyone can confirm or deny if limits are different for people that are on disability, and if it's confirmed can you please tell me what the limit is for a 2 person household?

2024.05.19 14:53 talkaboutluck Husband attacked and assaulted at work and then fired?

My husband was attacked and assaulted at work by a coworker. He was punched in the face, jumped from behind, choked out, and stomped on. He was lying in the floor for who knows how long before regaining consciousness and reporting to HR. He filed a police report and sought medical attention. The company did an internal investigation and fired both my husband and his attacker. The attacker was later arrested and has felony assault and battery charges.
This incident occured at the tail end of March and we received a letter from their insurance company today that makes me pause. My husband has called countless lawyers and many of them say that he has a case for a civil suit. But we can't get anyone to call us back to go forward with filing. He also filed for workman's compensation, but was told it would probably be denied because our state doesn't cover assault. When we were told this, we were given the number for my husband's previous employer's insurance company, so he called them and they did not have much information. They only knew that there was an incident. My husband let them know what happened, including the fact that this man had been hired and fired from the company before for trying to attack someone with a hook before getting rehired and attacking my husband. The lady we spoke to wanted to take a recorded statement from my husband and had me send over the injury photos, police report, and medical bills. When I sent them, they weren't able to open the Google Drive links I sent and I had to re-send everything, so she wasn't able to review them before the scheduled call to take his statement. She still hasn't taken it. Yesterday we received a letter from the insurance company with some forms to fill out and I'm wondering if we should even be talking with these people since they cover the company. The lady we spoke to on the phone even said she couldn't speak with my husband if he had retained a lawyer, which he hasn't yet because we can't get a call back.
We are at a standstill with this right now. I am a stay at home mom to two little boys and my husband is still unemployed and healing from injuries sustained in the attack. We have had to use every resource in our state just to get by and let me tell you, it's not much. We are multiple payments behind on our mortgage and car and we have received disconnect notices from utilities. We have been utilizing food banks. This is not how we want to live. My husband is a hard working man who just wants to make an honest living and provide for his family, but we have no idea what we need to do or if he needs to wait for a workman's compensation decision before trying to get a job, a job he can't even really work because again, he was injured and has not fully healed yet.
Does anyone have advice for how to navigate this? I have applied for mortgage assistance and utility assistance, am in the process of applying for food stamps, and calling our auto insurance company and bank for the car loan to try and see if we can be given a grace period. Nothing has been working out well so far, including getting a lawyer to get back with us. It's a big waiting game and we are really struggling. What in the world do we do? Located in Arkansas.

2024.05.19 14:33 dpj2001 Boomer has finally gotten delayed karma

I’ve been sitting on this for a while as its inconclusiveness wasn’t really worth sharing, but life has gone full circle and my boomer coworker has gotten some ironic karma!
2 years ago I was driving into work and got into a very minor accident. I was following another car inside a roundabout (rotary, traffic circle, whatever you call it in your own area) when they suddenly slammed on their breaks in an attempt to yield for cars coming into the circle… Not the first time I’ve dealt with people violating the laws of the road in that way, but it was the first (and hopefully last) time I hit them as a result. I braked fast enough that the bump was super small. We checked and there was no visible damage and the young inexperienced RAV4 driver and I agreed to just let bygones be bygones. I was still a little agitated because it never should have happened and now I was late for work, but at least it was a civil exchange.
I get to work and I’m in a bad mood and when Boomy the coworker asked what happened I vented a little just explaining the story and how stupid the other driver’s action was. I didn’t want a yes-man or a, “oh you poor pained soul,” speech, but just a little sympathy between coworkers would’ve been nice. Just to lift my spirits a little? Nope. Instead I get a spiel about how technically I’m at fault and I shouldn’t have been so close and I should’ve filed a report and blah blah blah. Look. I’m willing to accept my own responsibilities. It’s not like I was tailgating, but clearly I was too close or not paying enough attention. By the letter of the law I as the rear ender am guilty, but it’s easy to ignore the nuance of the other person committing a dumber act that lead to it in the first place when you’re on the sideline. Just wait until it happens to you… and that’s exactly what has happened now.
Boomy drives a Porsche from the 60s. Very expensive car. She treats it better than her own son (a whole other story). Well, this morning I find out from her that the other day she rear ended someone who stopped their car when they weren’t supposed to. You guessed it; in a roundabout! This one was more damaging and she has to have the front bumper replaced. Of course she droned on and on about how stupid the other driver was and how they’re completely at fault and how she gave the guy a piece of her mind. Another older coworker who’s listening suggests it was intentional and an attempt at an insurance scam. All the while she’s completely forgotten what she said to me when I was in a similar position - if she even remembers it happened at all. I just sat in silence smiling to myself.
TLDR: I get into an accident and Boomer coworker berates me saying it’s my fault, then 2 years later gets into the same exact accident and whines that it’s not her fault.

2024.05.19 13:12 Charlie_London747 Disputing Insurance

I found out I was pregnant in October 2022, and in July 2023 I’ve delivered my little girl the whole time I had my insurance from my employer that I had had for years. My husband had picked up insurance on me as a secondary insurance. I had no problems with billing the entire time I was pregnant. I had to see specialist have extra scans, bloodwork, etc. anytime I had anything to pay out of pocket. I did so in office and then after my daughter was born, I went for my check up. I was told I had an outstanding balance, which didn’t make sense because I always paid so I followed up and come to find out my primary insurance that had paid all my bills had unpaid everything. I had never heard of this so I I tried to get a hold of the insurance company, but I had already terminated the insurance as I did not go back to work after I gave birth. They did not want to speak with me because I no longer remember and give them my information. Finally I found my member number. I was able to talk to someone explain the situation to them. They argued with me. I told them that this was my primary it always has been. Why would they pay my bills? I never got any answer they told me they would put it down as a dispute And a request for repayment, but I was never sent a letter or informed them paying any bills. They’ve never told me if they are going to cover any of the bill. I’ve tried to call back and talk to someone but I just had to keep telling the same story over and I never get anywhere. I’m getting bills and collection notices for other doctors offices. Come to find out they have unpaid every bill. They have paid from the date that I took a pregnancy test at the doctor, but I was never informed of anything. Just bills and collection notices. How should I resolve this? Should I reach out to a lawyer or should I write them a letter? If so, would anyone have any sample letters? I could try to follow my credit score is in the 805 have worked very hard to get it there and I don’t want the years worth of medical bills piling up. Also, my daughter is 10 months old now I’m worried that my insurance is going to tell me. I’ve waited too long to dispute this, but I’ve tried to contact them over the phone several times and I get nowhere with them.

2024.05.19 12:45 padoodles Hospital sent me another bill after I was already paid up

My son was in the hospital October 2022 for rsv. I got the bill for this in November and set up a payment plan for the remaining amount after my insurance took care of most of it. I had about $3800 to take care of. I finished paying this in May of 2023. On April 29th of this year I received a request to pay an additional amount from this bill. It was $3323.90. I freaked out and set up a payment plan right away so it wouldn't go to collections. I called my insurance and they couldn't see anything on their end as to why I was being charged again. I called the hospital and they said the insurance took money back and tough cookies I needed to pay this. Called the insurance again and they said they would get back to me. They called me back stating there was an audit and that they still showed the amount I already paid was the amount I owed to the hospital. I talked to the hospital again and told them to talk to the insurance. I get a message from the hospital that says my payment plan has been cancelled and I am all paid up on May 6th. I'm thinking cool they worked it out. Today (5/18/24) I received a letter in the mail that says my bill has gone to a debt collector.
What am I supposed to do about this? This doesn't seem fair.

2024.05.19 11:17 sparklinggambino UPDATE: Driving without insurance, because my insurance company failed to tell me my policy wouldn’t be renewed, after telling me it would auto renew

hi everyone! here’s the link to the OG post (https://www.reddit.com/LegalAdviceUK/s/rJKAuDrTej) incase you missed it. It’s been over a year now, i wish i had a bit more of an update but this is all.
the day after i presented all my “evidence” to the station & the officer sent my copies off with the ticket issued on the night, i sent an email to the camera & tickets office with a timeline of what happened and all of the paperwork. someone replied and said once they receive the ticket in office they will review. a week later, the insurance company finally sent me an indemnity letter, the conversations i had with them were honestly like pulling teeth - will never deal with them again, but i got it, sent it straight over, same stock reply “sure once we get the ticket in office…” (you get it).
3 weeks later im emailing again - “any update?” because i did NOT want this coming back on me or them claiming they sent me post and i didn’t respond etc etc. still the same. 6 weeks later, still the same.
and a whole year later still the same! no one has responded to me, no mail, no email reply, no points on my licence, nothing.
i suppose i do have one question - how much time before they can’t charge me with it anymore? like for example can they send me a letter in 2 years time stating they found i was liable?
also don’t you think it’s a bit cheeky how shit they can make you feel about these kind of situations and then just leave you in the lurch? i don’t expect a sorry, but some peace of mind would be nice!

2024.05.19 10:06 vegas_ken69 [NV]Seeing a job posting show up a few weeks later…

So when i see the same job post show up a few laters doesn’t that usually mean the candidate they hired didn’t work out during the standard 90 day probationary period?
Would be rude to mention that in the cover letter if i want to reapply for that job?
Or does that come across as a passive aggressive IE rubbing it in that they should have given me the chance the first time????
also if all these jobs are being created in Nevada why was the job fair last week full with at 1,000+ job seeker???? and i keep seeing job posting unfilled???
is this a company’s way of padding the numbers with the Labor Board for the state and evening trying to negotiate better rates with health plans saying they have all these employees they need to cover with health insurance???

2024.05.19 09:20 baobao1325 Worth it to appeal an insurance pre-authorization decision?

I was recently diagnosed with a hip labral tear, but my pain in my hip is very mild. I actually initially thought it was a femoral stress fracture, but that has since cleared up as well. My sports doctor prescribed me physical therapy, but my insurance (LifeWise AC, Washington) requires pre-authorization for physical therapy visits. I had my physical therapist submit the forms and medical records for this. Unfortunately, my insurance denied me visits because they deemed physical therapy as not medically necessary. At this point, I would like to know if it is worth it to appeal the decision and bring in a letter of medical necessity from my sports doctor? Or if this is just a waste of time? Or if there are any other suggestions for winning an appeal? I only ever get soreness in my hip occassionally, but I am an active athlete so I don’t want to wait around for my hip to get worse. Thank you!

2024.05.19 08:08 bt_dubz Responded twice to a letter of demand but haven't heard

Earlier this year I was in a minor accident where I was clearly at fault. I ran into the back of a motorcycle at a tricky intersection after he stalled taking off. This was very minor and obviously not at any speed close to which injury could occur. There was minor damage so we exchanged information and decided we could sort it out without insurance. He then messaged me the next day saying he had lodged an insurance claim because it turned out the damage was more extensive than first thought. Of course I understood and expected to hear from his insurance or mine as I had given him all my information. I never heard from them. A few months later I received a letter from his insurance demanding the total costs of the repair in which they had worked out to be $5000 somehow and I was to pay within 14 days or they may take legal action. Alternatively I could provide them with my insurance details and they would contact them directly and sort it out. I responded to the letter via the email address provided admitting fault and providing my insurance details as requested however I never heard back. I even sent a follow up email a month or so later but again never heard from either them or my own insurance.
My question is what do I do now? Do I keep trying to contact them? Do I hold out until something else may or may not happen? This was in February

2024.05.19 08:03 hypikachu Tyrion's Paternity: An open question? [Spoilers Extended]

Tyrion's Paternity: An open question? [Spoilers Extended]
Listen. I know "Tyrion Targaryen" is a divisive topic. I don't think there's any theory that gets more vehement criticism. I understand why it gets the ire it does, and don't even wholly disagree.
Buuuut, if I can play devil's dragonlion's advocate here: The counterarguments to the theory are all...kinda shaky.
1. "It hurts the story"
I'll admit it. I (kinda) agree with this assessment. A "Tyrion's a secret Targ" reveal threatens to undermine other big elements of the story. One secret orphaned prince is a tragedy, but a dozen is a farce.
It also arguably hurts the Tyrion-Tywin relationship. I don't think it'd be ruinous as many of the critics do. But even if it were, that still wouldn't affect whether or not it's canon. This argument really doesn't address "what is or is not," but rather "what should or shouldn't be."
But writers do stuff they arguably shouldn't all the time. Fans of Star Wars will gladly tell you that midi-chlorians undermine the Force. But they're still canon, bc George said so. Martin is no more infallible than Lucas. I love these books, and I think GRRM's the bee's knees. But can you tell me with a straight face that "Joffrey sent the catspaw" was perfectly executed storytelling? Whether something's good or not is a subjective matter for the audience. But whether something's canon is wholly at the whims of the author. And it definitely seems like GRRM's whims are pointing towards A+J.
2. "GRRM was setting it up in the early books, but abandoned the idea at some point."
Yes, George is a gardener and doesn't necessarily stick to a rigid story plan. He's removed or changed big elements of the story, like the 5 year gap or the Brightflame cloth dragons. It's definitely possible that Tyrion Targaryen might have been a similarly scrapped plan.
(Small Tangent: I'm even pretty open to the idea that it was scrapped in the show. It's totally the kind of thing D&D would hate. But you can ask Griff & Stoneheart if show canon = book canon.)
The problem is, there's no clear point where GRRM stopped dropping hints that align with A+J. It still seems front and center in ASOS (2000), when Tywin makes his last on-page appearance. He explicitly calls Tyrion's paternity (and the fidelity of his beloved cousinwife) into question twice in that book, down to his last breath. Bookending Tyrion's first speaking appearance (AGOT Jon I). The very first thing Tyrion says about Tywin is "he thinks of me as a bastard." The idea of Tyrion's paternity being in doubt hangs over the Tyrion-Tywin relationship from the first moments to the last.
The relevant characters' most recent book appearance was the worldbook in 2014. (The same year GRRM gave his now famous explanation of why abandoning setup makes for bad storytelling.) Even then, GRRM was obliquely pointing to the possibility of A+J=T with relentless determination. Every single mention of Joanna is attached directly to a note about how much Aerys pursued her. Tyrion's birth is one paragraph after the tourney of 272. Where the only notable event was Aerys lusting after Joanna, deepening the rift with Tywin.
Which moves us nicely from the meta-textual arguments into the in-universe "evidence."
3. "[Pycelle said] Tywin wouldn't have married Joanna if she'd been with Aerys"
Pycelle sure did say that. Pycelle is wrong. That's the point. How can we tell? GRRM's choice of wording in Pycelle's rebuttal.
As Pycelle insists in his letters, Tywin Lannister would scarce have taken his cousin to wife if that had been true, “for he was ever a proud man and not one accustomed to feasting upon another man’s leavings.”
Pictured: Pycelle's wrongness.
The 2014 audience already knows Tywin absolutely would do that. The climax of his conflict with Tyrion was him bedding Shae. "Feasting on another man's leavings" is already a defining part of Tywin's relationship with Tyrion.
GRRM wrote Pycelle huffing copium. Conspicuously. The fanboy maester's denial depends on a claim the audience explicitly knows is false. It's just basic dramatic irony: the audience knows something the characters don't. If Pycelle's claims rests on false evidence, what is the author saying about the claim?
4. "If Tywin knew/suspected, why didn't he do anything more than try to resign?"
I'll be honest, this one blows my mind. The man sacked King's Landing and killed every Targaryen he could find. Tywin's big defining pre-stories action was brutally overthrowing Aerys' whole family. Sure he didn't do it immediately. But when circumstances permitted, Tywin took extreme vengeance.
5. "Tyrion's dragon dreams aren't Dragon Dreams"
Why the hell not? Symbolically heavy. Seemingly prophetic. Showing a destiny of magical conflict, with stakes as intimate as family identity, and as broad as globe-spanning war. Tyrion’s dreams check all the boxes for what makes up a Dragon Dream. (Or should I say, “They meet any cry-Tyrion?”)
When Tyrion first mentions dreaming of dragons, he’s telling Jon “I know your secret. You dream the same kind of dreams.” Again, dramatic irony time. Tyrion’s saying it as “I know you secretly feel alienation.” But a reader who knows Jon’s lineage knows the real secret is why Jon’s magic dreams fixate on family alienation. Because they’re Dragon Dreams. The very first thing GRRM tells us about Tyrion’s dragon dreams is that they’re comparable to Jon’s Dragon Dreams. And Tyrion’s have actual dragons in them.
Oh, and very non-coincidentally, this scene happens only 20ish pages after Dany has the first confirmed on-page Dragon Dream. Which hits all the same elements. Prophesymbolic vision of a buried dragon identity. Which emerges through the crucible of sibling struggle.
GRRM returns focus to Tyrion’s dragon dreams in ADWD. He has two such dreams during his journey east from Illyrio’s manse as part of a plot to marry Dany to a guy with a big fighting force behind him. Eagle eyed observers will note that this is exact same setup Dany herself had in AGOT when her Dragon Dreams started.
In both the earliest and latest books in the series, GRRM draws immediate parallels between Tyrion’s dragon dreams and Targaryen Dragon Dreams. Just from an economy of storytelling perspective, it would be weird to have Tyrion’s special important dreams-w/dragons-in-them that just happen to be totally unrelated to Jon & Dany’s Special Important Magic Dragon Dreams™️.
Caveat: Schrodinger’s Targaryen
Despite all of this, I do not think GRRM’s endgame is as simple as “And then we learn Tyrion is 100% definitely Aerys’ son and not Tywin’s.” My strongest hunch is that the plan is for the story to end without a definitive answer, but a pointedly open question. Compare it to other “unresolved Targaryen/dragonrider ancestry mysteries” like Nettles and Daeron T vs Daemon B. GRRM loves this “the mystery is more valuable than the answer” approach to storytelling.
In AGOT and ASOS we’re told “Tywin thinks of Tyrion as not his.” In TWOIAF we see maesters publicly speculating about Aerys & Joanna’s relations. I think the in-universe uncertainty is the plotline here. The speculation already exists in Tyrion’s plot, which will come to a fever pitch when (not if) he saddles Viserion.
I don’t think there’ll be any raunchy Bran-vision or tearstained secret letter from Joanna that definitively confirms Tyrion’s parentage one way or the other. Tyrion seizes Casterly Rock and there’s a hubbub about legality. Is he a golden trueborn lion, Tywin’s legal heir? Is he the red of a Targaryen dragon and/or a color-inverted Lannister bastard? Who the hell knows? What does it matter? All the truth Tyrion knows is his mother was a lioness, making him a cat regardless of coat. That, plus he has a dragon, with sharp long claws. The dragon reins are all he needs to reign from Castam Casterly Rock.
This deliberate open-endedness leaves room for a lot of options. I’m very open to chimera theory. Nerd Tangent: In myth, the chimera is literally a fire-breathing lion-serpent hybrid. All Tyrion needs is some goat imagery and he’s got the whole animal. Plus GRRM keeps making the lady of Casterly Rock mother twins at every point in the timeline. Joanna’s were even fraternal. GRRM even wrote an unpublished conversation whereTyrion talks about Maelys the Monstrous (to whom Tyrion repeatedly compares himself) absorbing his twin in-utero, and imagines the same thing happening in his own mother’s womb. George is doing everything a writer setting up a “genetic chimera” twist reveal would do.
Separately, I really like the idea of Tywin misinterpreting prophecy and dooming himself to the fate he was trying to avoid. In perfect parallel to Cersei’s experience with Maggy. Tywin gets some kind of cryptic warning about Aerys’ bloodline displacing his own. Just like Cersei’s valonqar, he jumps to a misplaced suspicion of Tyrion, when he should be examining Jaime and/or Cersei. When TWOIAF bundles the tourney of 272, Tyrion’s birth/Joanna’s death in 273, and Tywin’s role in the Targaryen downfall together, it’s entirely possible that the point is the same as AFFC Cersei constantly telling us “valonqar = Tyrion.” A red herring; there to prompt the audience into thinking about the question. But preserving mystery by laying the false answer on thick while the true answer is surreptitiously sprinkled in.
Maybe there’s even in-universe discussion about how an AJT reveal makes a farce of RLJ? “Diluting the reveal by flooding the spot with something similar but even more outlandish” was Tyrion’s own in-universe strategy for dealing with the reveal of Cersei’s royal bastards. This could be GRRM going full circle. “Oh, Ned Stark’s other closely guarded secret about royal bastardy just came out? Well, this counterstory from Tyrion about royal bastards has juicy stuff like clowns and sex with a crazy guy and kids w/physical abnormalities. Once this story spreads, no one’ll know what to believe!”
It could even go the same direction the show went for Theon’s identity dualism. Tangent: (Theon is kinda directly connected to Tyrion already, having inherited the “burn Winterfell, torn between Starks & birth family” plotline originally meant for Tyrion.) You can be both a furry apex predator on 4 legs and a mythical beast with long wriggly appendages. Lizard & lion at once.
GRRM might even be highlighting this Schrodinger-esque superstate of “both one and the other simultaneously” with Tyrion’s ADWD intro. The first time we see the cat-man after he kills Tywin, he’s drunk himself half to death in a box while on his way to Illyrio. It's the moment when he’s most in limbo– after killing the lion Tywin, but before joining sides with Aerys’ dragonspawn – he’s a half-alive half-dead cat in a box.
All I'm saying is that I think, for George, the point is the duality. The uncertainty. The multiplicity of options. Tbh, I’m not arguing that “Aerys is the father” = The Answer™️. I’m just arguing that the story is designed to set up the question.
From Tyrion’s first lines to Tywin’s last, GRRM insistently raises the notion of Tyrion not being Tywin’s son. The most recently published account of A, J, T, & T deals heavily with the contentious love triangle. I’m not saying you have to like it. I’m just saying you can’t pretend it’s not there.

2024.05.19 06:30 vyerkxon Frustrated and Heartbroken: Schengen Visa Denied Despite Strong Application

Hello Reddit,
I’m reaching out to share my recent experience with the Schengen visa application process, and to seek advice and support from this community. My wife and I recently applied for a Schengen visa through the Swedish Embassy for a trip to celebrate our 15th year together (not our marriage anniversary). Despite meticulous preparation, our application was denied, leaving us frustrated and disheartened. Here are the details:
Our Background:
Documents We Submitted:
  1. Completed and signed visa application form.
  2. Proof of identity and marital status (photocopy of passport, national identity, marriage certificate).
  3. Round-trip flight confirmation.
  4. Hotel reservations
  5. Financial stability and proof of income (payslips, salary certificate, bank account statement, solvency certificate, TIN certificate, income tax certificates, and saving certificates).
  6. Health and travel insurance policy covering the entire stay.
  7. Detailed itinerary.
Reason for Denial: The refusal letter cited two reasons:
  1. "The information communicated to justify the purpose and conditions of the planned stay are not reliable."
  2. "There are reasonable doubts as to your intention to leave the territory of the member states before the expiry of the visa."
Our Feelings: Despite presenting a strong case with genuine and meticulously prepared documents, we were denied. This feels like a severe injustice, especially considering our stable backgrounds, education, and financial ability to support the trip, only for the people like us. It’s hard not to feel that this scrutiny is disproportionately harsh. I understand there are a lot of political neusance going on, but with strong background and education, and ability to spare, I guess this is just us!
Seeking Advice: Has anyone else faced a similar situation? How did you handle it, and were you successful on a subsequent application? Any advice on how we can strengthen our application further or address the reasons for denial would be greatly appreciated.
Thank you for reading and for any help you can provide.
TL;DR: Applied for a Schengen visa through the Swedish Embassy with my wife to celebrate our 15th year together. Despite a thorough and genuine application, we were denied for reasons that seem unfounded. Seeking advice and support from those who have been in a similar situation.

2024.05.19 06:10 letsjam88 Still have medicaid coverage, despite being over the income limit (NC)

I don't understand how I still have Medicaid coverage despite making over the income limit. I live in NC and never spoke to anyone about recertification. I've been receiving letters saying I have coverage, a doctor has been chosen for me, and a plan has been selected.
I qualified for Medicaid last year but never used it because I got a job used my job's health insurance plan.
I thought they went by my income, which they have, I filed taxes each year, and I made over the monthly income the entire time during the recertification period.
NC has this new EPASS account that I can't access because my account is broken. When I've called the number to get help, either no one picks up or they give me different phone numbers that circle back around to people who can't help me.
My initial case worker was not helpful and didn't answer my questions or provide guidance. The NC website isn't helpful, many pages I try to go do are either broken or have no information. And there's rarely a number that works or leads to anyone I can speak to. I was the phone once for 2 hours trying to get answers but go the run around.
My concern is if I'll be penalized for some reason. I never used the insurance once while I was on it. I didn't register a card or an account. The only thing I did once getting coverage was logging into the marketplace to choose one of the insurance companies they listed.
