Why did my testosterone go up on Spironolactone?

2024.05.17 05:13 livvvjh Why did my testosterone go up on Spironolactone?

Dutch Test
Female 27 aprox 125lbs 5'2, Medications: 15mg adderall, 1.5mg naltrexone (for pain), conditions: Ehlers-Danlos hypermobility type, PCOS, POTs, and Craniocervical Instability
I have been struggling with my PCOS for the past 3 years. My PCOS symptoms include polycystic ovaries, hair loss, Acne (face and body), weight gain, a little more than normal body hair, rapid body hair growth, brain fog, fatigue, and severe depression. Might be worth it to mention that I had none of these symptoms, apart from depression, before 3 years ago, I never even got a single zit before the age of 21. I had a progesterone IUD from 20-25 if that helps. I have been treated with Spironolactone in the past. It was honestly life changing. My depression vanished, I lost 30lbs in 2 months, and all of my symptoms either went away or got significantly better. Only down side was that I was constantly bleeding on Spironolactone, nothing too major but I need a panty liner everyday.
After 3 months on 100mg of Spironolactone, I noticed my fatigue started coming back and my mood started getting a little worse. My dose was increased to 200mg and immediately felt better.
Another 3 months and the same thing started. They tested my free testosterone and it had literally doubled while taking it.
Cut to January of this year, my doctor wants to discontinue the Spironolactone because of the chronic bleeding. She wants to use this opportunity to get a clearer picture of my hormones and their metabolites. So I wait an excruciating 4 months and take the Dutch hormone test. I am a little confused by my results.
My 5a-Reductase Activity is pretty low, but all research I've looked into shows that it is usually high in PCOS patients. My DHEA-S is actually on the low end and my testosterone is high. I'm having trouble interpreting these results. I'm wondering if this is an expected result for someone with PCOS and hyperandrogenism. Should I be asking my doctors to look into anything else? Does anything here explain why my Spironolactone would stop working/not lower my testosterone? Dutch Test

2024.05.16 23:17 Qalicja Medication recommendations/tips on what I should do?

I’m looking for input from people who specifically struggle/have struggled with BOTH ADHD and anxiety (but also if you have depression that “helps”). I have ADHD (combined type), generalized anxiety disorder (and possibly social anxiety), and have struggled on and off with severe depression and s*icidal ideation for years (although I’m kinda at a point where the depression is the least of my concerns and I’d argue it’s mostly caused by capitalism).
Do any of you take a medication, have a medication combo that has worked wonders for you??? What can I research into and talk to my doctor about that might actually help with both the anxiety and the ADHD?
Thank you in advance!!!
I’m inquiring because I currently don’t have access to a psychiatrist and my primary doctor is my current prescriber. Psychiatry is not his speciality and although he regularly prescribes psychiatric meds to his patients, he’s not familiar enough with all the different medications and combinations off the top of his head to be able to provide the help that I need with my combination of disorders. However, he’s always willing to hear my opinion and inquiries, and if I were to come to him with research and potential medication suggestions, he’d take them into consideration.
The medication combos I’ve been on before are: Adderall XR, Adderall IR, Prozac, and Xanax (only for panic attacks, as needed); Adderall XR and Adderall IR; Adderall XR, Ritalin, and Prozac; just Strattera
More detailed explanation of my situation below if it might help:
My biggest issue is that because I have both ADHD and terrible anxiety, stimulants make me more anxious if I’m already having a high-anxiety day, but they’re the only thing that has every actually helped my ADHD symptoms. Adderall 100% helped me get through uni (albeit I had an absolutely trash GPA), it helped me focus, get work started, got me sitting in one place finishing my work for hours, it helped me nap, helped me communicate better, and even improved my mood. But I also hated taking it every day (and often forgot to take it/pushed it off) bc it often made me more anxious, it decreased my appetite, made me feel less like myself, and gave me headaches when it was wearing off. I’ve also always had a fast metabolism and the Adderall XR only ever helped for 5 hours at a time and the IR only 2-3 hours.
I’ve been on Adderall XR (15mg and 20mg at varying times) and Adderall IR (7.5mg and 10mg at varying points) before, but since the shortage and being out of school I’ve been off of it (due to my anxiety I HATE dealing with doctors and pharmacists and fighting to have Adderall).
I recently started Strattera for my ADHD and anxiety. I’m 5-6 weeks into taking it (currently at 40mg) and I feel almost no positive benefits. It hasn’t yet at all lessened my symptoms or helped me manage the ADHD and anxiety. At first, I experienced being able to wake up earlier, but that doesn’t seem to be the case anymore and I’m also dealing with some insomnia. I’m also starting to feel a bit more irritable and angry, as well as constipated. I’d be willing to deal with all that if I was actually seeing benefits. I don’t know if I should ask to continue the strattera when I see my doctor next, have the dosage upped or ask to try something else.
I’ve tried Ritalin in the past as well and it made me feel awful and only increased my focus for 30 minutes or so, and I stopped taking it after a week. And for my anxiety and depression I’ve been on Zoloft and Prozac in the past. Zoloft was awful for me. And Prozac was actually okay, I didn’t experience many negative side effects, except maybe some increased s*icide ideation and headaches.
I’ve never been well medicated. And a huge reason for that is that I still have never seen a psychiatrist or psychologist. At uni, I was being prescribed meds by nurse practitioners at the school’s health services and from a big therapy/psychiatrist office where my old therapist worked. And then my primary care doctor took over and he’s been the one prescribing me meds for the past couple of years, but he’s not at all a specialist in psychiatric medicine so he’s not knowledgeable on all the medications and on how to help me. He and I want me to see a psychiatrist but a psychiatrist isn’t accessible to me at this time.
When my parents were still responsible for all my health bills, they either always rejected or were apprehensive about me seeing an actually competent prescriber bc of the cost. And I’d always beg my mom to help me research psychiatrists who specialize in ADHD and who are in-network but she never did. (I feel a lot of resentment.)
And now that I’m out of uni, they (understandably) expect me to pay, but I’m unemployed (for many reasons, main one being that my ADHD and anxiety are completely out of control to the point where they’re debilitating) and I can’t afford it. I have my parents insurance still but a lot of the psychiatrists and psychologists in my areas don’t take any insurance, some take insurance but not mine, and the rest are either completely booked, don’t specialize in ADHD, or don’t seem trustworthy. And I’ve also learned that since I haven’t met my deductible, I’d be still paying at least $100-300 for an appointment with a psychiatrist.
I feel stuck in a hole that’s only getting deeper.

2024.05.16 19:07 justanotherho_ 5mg that much of a difference?

Hi everyone, 30F here. I was diagnosed a few years ago and was put on Adderall ER, started with 5 mg, then 10mg, then went to 15. Last month I started 20 and this feeling is definitely more intense, not like it was when I have increased previously. Could 5mg be making that much of a difference? I would never say that I “felt anything” really in the previous doses.

2024.05.16 14:56 KaleidoscopeOk3232 What do you do when you "come down" from your meds and feel normal again in the morning/night?

EDIT: I appreciate the comments recommending me other meds I can try but I'm very much looking for anyone who deals with this and how they do it, how I can change my routine, etc
I'm on low dose (5mg) short release Adderall and it's really improved my life in a short time, and I havent build tolerance up yet, so everything's going well. I take it as it wears off every few hours. However, in the mornings and right at bedtime, I am the worst.
There is no side-effect happening, though. It's just... me!!!! My brain gets full again, I can't have a clear thought, and I can't continue my routine. It's a mess and I don't know what to do. I can't get a morning routine down so I'm still fumbling through my day and I stall to take Adderall so it makes it worse. Last night I had a meltdown because I was so overwhelmed from my thoughts being ten million different things on and off again that I couldn't get up and get ONE thing I needed for like 10 minutes at least. Since I'm also autistic it's not only paralyzing but emotionally overwhelming so I have been breaking down more often at night.
Basically, I can't function without it and I didn't realize how bad it was until now even with just a tiny amount of Adderall (I'm not perfectly focused but the euphoria is overwhelming so I'm taking it slow). Is there anything I can do to help? I feel like this is jeopardizing my days by having a bad start in the morning and tossing at night, not because I have insomnia (yet at least, thankfully) but because my head is having a party I don't want to be invited to.
Thanks for any advice you can give. I'm planning on asking my doctor but my patient portal is broken and the phone line is always busy.

2024.05.16 10:28 CoffeeAware Tolerance Build Up (HELP)

Heyooo,
I’ve been having issues with tolerance build up and dosages and such and can’t figure out WTF to do. See below for information.
So I’ve only recently been diagnosed a couple months ago, by an awesome Psychiatrist in Sydney NSW.
To start he put me on Ritalin, that was horrible. So we made the switch to Dexamphetamine or Dexidrine, I started on the smallest dose of 5mg, I had amazing results for a week, the next week I didn’t feel much, then i upped my dosage and was told to titrate up in dosage every 4 days to a week or so.
Fast forward a couple weeks, I got up to about 20mg of dexidrine. I asked if I could try Vyvanse, as I didn’t like the ups and downs of the IR medication (they weren’t bad, but not ideal).
We started on 40mg, that worked amazingly well, I had total coverage from 9am-5pm, my memory was amazing, my ability to do things were amazing, I got so much stuff done for work during that week and for my music studies as well.
The medication has ceased to work properly, I take it in the morning, around 8-9 then return fairly close to baseline around the 3-6 hour mark. And even the peak of the medication is no where near what it was when I first tried it. So my solution was to add some boosters of Dexidrine, I did so, it helped, but now they have a minor effect.
Now my psychiatrist wants me to try 60mg Vyvanse, I’m going to try it tomorrow and see how I go, but I think I’m going to end up on a higher dose and build tolerance to that as well and then just have to continue to increase until at max dosage and then that’ll become less effective due to tolerance.
What the f*ck do I do? I’ll give it a go and hope for the best, of course. But I am a bit skeptical and slightly annoyed, as the medication is bloody awesome when it works well. But I just hate how my tolerance keeps ramping up regardless.
Now here is some insight into my daily life, nutrition, health, etc.
Now I predict someone will say “the meds seem to be working due to the structure of the post” as this is something I’ve seen on others posts, but I am writing this as I’ve taken my other booster and gotten a good 10 hours of sleep. If it was yesterday when I had 7hrs, I’d be dead in bed by now XD
This post isn’t to say my medication is completely ineffective, but that over time, the effects are becoming less and less prominent and the duration is becoming shorter and shorter. In fact I’m writing this even as I really should be continuing my music study, but have gotten distracted by the issues of my medication.
Thanks in advance and hope someone can offer some insight or suggestions.
Bless.

2024.05.16 04:31 Plane_Vegetable790 Dog story

Dog story
My girlfriend brought me to the humane society of Springfield one day just to “look” at puppies because she knew I wanted one but was unsure of myself since I lived in a one bedroom apartment and I was a college student. I told the shelter employee I was looking for a German shepherd or German shepherd mix. They just so happened to have a dog named Alma. After doing the meet and greet I was not putting that puppy back in the cage. We took her home.
I named her Olive and Olive was not a chill puppy. It’s taken years of training and dedication to turn that chaotic puppy into the wonderful family / protection dog you see in the photos. She has this habit of eating the drywall next to where I sleep on my bed. This is not her first encounter with drywall either.
When we first got the house we left her uncrated in the basement with snacks, water and toys and a bed for when I went to work. I came home and she ripped a German shepherd sized hole in the wall and got into the other half of the basement. At this point I realized she didn’t like the basement. Weirdly enough she loves crate time and we never use it as punishment. It’s more like happy treat and nap time more than anything else since it’s 4-5 hours or like today I didn’t even go to work.
She turned into an amazing dog but when she turned 4 she almost died. My girlfriend noticed something wrong with her like she was having a seizure. We took her to the fanciest vet we could find and spend thousands saving her life. We found she has Addison disease and now takes 5mg steroids per day. Needs a shot once a month but she should lead a normal life expectancy as long as we give her those shots.
I’ve been dealing with some mental health issues and olive is really helping me. Nothing cuter than a puppy imho so it’s a real mood booster. She’s extremely sweet. Also a little bit tricky, she’s been caught on several occasions tricking my mom’s dogs to get their food when she visits.
Anyways that’s my dog and she’s currently squeaking her toy at me

2024.05.16 04:20 Busybee_unbusybody Tapering off, other options?

Hey everyone, so I’ve been tapering off of Lexie for the past (almost) month. I was at 10mg and now I’m at 5 mg. In a few days, I go down to 2.5mg. I can’t say that it hasn’t affected my mood. I’ve noticed a significant increase in my depression, but thankfully I’ve been in therapy for the past 2 years and can handle it for now. I exercise (a wonderful habit I picked up while on lexapro) and I eat relatively healthy. I want to incorporate other things into my routine to help me get past this curve. What are some other serotonin boosters I can add to my routine to kick depression’s ass? Some things that worked for you? And if you say meditation, can you tell me how it worked and how long it took to notice a difference? (Sorry but I know that might be a common response)

Today marks day one of taking Adderall, and I feel no difference. I am honestly so desperate at this point to feel any effect, my doctor originally prescribed me with Bupropion 300 and felt no positive change in my mental well being. My doc now has me on Zoloft 50 mg, and Adderall 5mg, im hoping this this helps. Nobody really talks about the the discouragement throughout the process of finding the best medication. Any advice/ experiences ? Thank you.

2024.05.15 22:19 Good-Fill-8850 Afternoon crash makes me question if meds are truly worth it...

31/M, been taking stimulants for about 6 months. I have tried quite a few combos. Vyvanse 30mg only, Vyvanse 30mg + Adderall IR 5/10mg booster, Vyvanse + Dex 5/10mg IR booster, Dex 10mg ER + 5/10mg IR booster and 2x Dex 10mg ER per day. I have even experimented with only IR Dex and didn't find it to be much better.
Vyvanse works best, but I only get about 5-6 hours from it and then crash in the afternoon. This is on 30mg. I've tried a second 30mg once before and then it is too much and makes me extremely irritable and depressed later in the day. I noticed this depression and irritability with the ER plus booster.
It seems that once a day Vyvanse is best side effects wise but the zap in energy and slight depression at 3pm makes it difficult to want to continue. Not to mention the cost of my Psychiatry visit and then the cost of Vyvanse. Just curious of anyone has decided meds weren't worth it for them or that the side effects were bad enough to continue?
I was able to function decently before meds, but when the meds are effective it seems to lift my mood, keep me out of my head and relieves some anxiety. Just looking to vent a little and see who else may have dealt with a similar dilemma. Thanks!

2024.05.15 19:57 Lucklessm0nster Medication and irritability (intolerance of interruptions)

I take 50mg Vyvanse and a 20mg Adderall booster dose in the afternoon.
When I started taking Vyvanse, it was a godsend. Compared to Adderall, I experienced next to no physical stimulation — negligible increase in heart rate or blood pressure — and I barely noticed when it came on. I just noticed it was easier to focus and perform tasks.
Lately, though, I've noticed myself unable to tolerate being interrupted when I'm on task. I get the Big Scaries, where everything starts to feel like I'll never actually be able to catch up, let alone get ahead. It almost feels like I'm racing against the clock and every time I'm interrupted, it gets less and less possible for me to do anything but 'fail.'
I feel like I can rarely get into the flow state necessary to finish a project. I am only able to focus long enough to make tweaks, because I'm aware that in 30 minutes I need to make another meal, or someone is going to need me to answer their message to maintain a friendship, or something else.
I get irritable and even my family, who I love dearly, feel like an interruption. I can handle the cats crying to play or be brushed once or twice, and a phone call or text message or two, my partner pointing something out to me, etc, but after a few of these interruptions, I start to feel like I am going to break down.
My partner and I both work from home in a one-bedroom apartment. I am rarely alone and thus am always 'on' to a degree. It helps if there isn't anybody else in my line of sight. My partner is wonderful and he set up a second desk in our bedroom, which has a door, and this helps a bit — until our older cat can't tolerate me being away from him anymore (he is nearly 17 and very bossy) and I have to go in the other room, at which point the cat in the bedroom starts crying to play, and the cycle begins again.
Does anybody have this problem? What do you do?

2024.05.15 18:39 kayisbadatstuff How to stop feeling like I’m going to puke when I eat on my stimulants

Hey y’all! So recently started my medication journey. I was taking adderall 5mg 3x daily but that gave me so much anxiety I felt like a different person. Have now switched to Focalin XR 10mg 1/day and have been feeling unable to eat when I’m on it. I can only take a few bites before I feel like I’m going to puke if I eat anything more. Does anyone have any tips for this?? If I don’t eat my anxiety gets worse so I need to figure something out 🙃 Thanks in advance y’all!!

2024.05.15 15:58 tinyglittertitties Am I loosing weight too quickly? Is semiglutide fixing something else?

*losing - my b
TLDR: losing weight surprisingly fast on a low dosage, curious if it’s because of a different issue that is being fixed with ozempic other than hunger suppression (like giving me normal hormone levels or something)
I started semaglutide one month ago. My dosage is .25mg, but the dr told me to start taking .5mg today.
In the past month, I have lost 15 pounds (190 to 175, I am 5’8 & pretty muscular) and I’m concerned I’m losing too much too quickly. It’s kinda crazy to see, because I really feel like I haven’t changed my lifestyle habits that much (or maybe I’m not noticing it). For example, I still have some nights where I binge eat, but it’s a slightly less amount than I used to without the medication. I am unable to workout due to an injury but I do walk at least a mile a day.
Anyways, I’m wondering if this has to do with something that’s wrong with my body? What I mean is that before taking this medication and my injury, I would workout 5-6X a week and eat pretty healthy but still gain weight. Nothing I did would help and I’m even on adderall which meant I didn’t eat much during the week. I got tests done for PCOS & other blood tests for my thyroid/hormone levels, and the doc said everything was normal (other than a ton of follicles in my ovaries lol). My mom has PCOS and thyroid issues which is why I thought I might. This was super frustrating because there had to be something wrong…has anyone been in a similar position? I will say the hormone test were very basic and my general doctor is questionable 😂
Grateful for any advice!!

2024.05.15 14:40 bullymaguire25 If you think my mom is bad, it gets much worse

Growing up your whole life, you never expect things to happen. You dont think thins could get as bad as they do. But they do. Nothing could prepare me for Freshman year of high school. Long story short i got tested for adhd freshman year of high school, and yeah it was adhd. Went couple years just taking adderall(15 mg just for exams). Was vyvanse at first but shortness of breath so stopped that. Concerta but it hardly did anything. Then adderall.
Junior year, 15mg became 20. 20 mg became 30.
Then, came around exam day. THEN and ONLY THEN. My mom suggested me to take 60 mg.
Some weeks later the dr had said okay 60mg it is but thats as far as we go. And i couldn't agree more.
A couple- few months pass and for the small anxiety i had i wanted see to get something done about it. At the time I was peaking.
Some weeks pass by and I was able to see not my PCP, But a different Dr.(vandy).
At that time he prescribed zoloft. I only took 2.5mg for 4 days.
That was a bad BAD move. Years and years of torment(severe depression, severe anxiety, severe panic attacks, paranoia, delusions, hardly eating/working out if at all) for YEARS.
Thats hard enough already. Unbearable almost. But to make it all the more suffocating was that during this, my mom would always yell, blame, and shame me yelling "ITS THAT DAMN ADDERALL!" Ill tell ya right now, it wasnt.
But several years later I have redeemed myself.
To this day anytime shes the one who creates the problem outve nothing for what reasons? Shes a narcissist.
She will create conflict and then when she has crossed the line and makes it personal, she will use my brothers death as an excuse(he died 4 years ago). She was still a terrible person 7 years ago. Unacceptable for her to be like that.

2024.05.14 20:35 AggressiveDuck6739 ADHD and lightheaded

Hey everyone,
I've been experiencing some lightheadedness when I take my Adderall (15mg in the morning and 10mg in the afternoon), buspirone (5mg three times daily), and finasteride (5mg daily). In the morning, I also usually have a cup of coffee and sometimes a pre-workout before my workouts.
Despite drinking plenty of water, I still feel pretty lightheaded, especially when I'm driving. Has anyone else experienced this? Any suggestions or tips on how to manage it would be really helpful.
Thanks!

2024.05.14 17:54 hankscorpiox Advise on losing fat while keeping muscle

My most recent diet I’m down 8lbs in 6 weeks but according to the eufy scale (which I know it isn’t accurate, but it seems fairly consistent) my loss has come from 4.9 fat and 3.1 muscle. My body fat % is still at 22%. I can gain weight fairly easily, both muscle and fat go up, and then both muscle and fat go down. Im smaller overall, clothes looser, a bit visibly leaner, but definitely have lost muscle size in six weeks.
My TDEE is ~2600, I’ve been eating for 6 weeks around 2100 with 170g protein. I’m male, 39, currently 166lbs. Lifting weights with progressive overload 4 days a week, all my lifts have gone up reps or weight during the cut. 1 day of boxing, then some light cardio 2-3 days a week. 8-10k steps a day. Sleep 8hrs a night. 1gal water a day. I’ve tracked all my food; my numbers are exactly where they’re “supposed to be”. No cheat days, always hit my protein targets. My diet is high protein vegetarian. Lots of eggs, yogurt, kefir, cottage cheese, whey and casein shakes, fake meats. Supplements are creatine, fish oil, D, zinc. Medications are wellbutrin and low dose 5mg adderall.
Wondering if I’m missing something easy I’m not thinking of, or something medical to think about. Know there isn’t an easy answer, but wondering if anyone has had similar issues and some ideas of things to look into. Trying to get a lower BF% but really struggling to lose fat while keeping muscle. Thanks.

2024.05.14 15:16 papipendejo intense fear of the dentist

Personal Info: M / 33 / 5'11" / 170lbs / Diagnoses: GAD, MDD, PTSD and ADHD. Current Med-list: Adderall 40mg XR, Adderall 20mg IR (as needed in afternoon), Cymbalta 90mg, Propranolol (as needed for anxiety), Buspirone 15mg and Prazosin 2mg (PTSD nightmares, not for blood pressure)
I have a very intense fear of the dentist. This has resulted in me putting my dental health on the back burner and now it has gotten to where I cannot ignore it. I have to go but my fear is so bad I cannot sit still without tremors and feeling like I can't breathe/hyperventilating. Currently I take propranolol as needed for anxiety but it depends on how intense the anxiety is. Would it be acceptable to ask my psychiatrist for a prescription for a single dose of .5mg Clonazepam? I will not be driving there or driving back. I just need something to get me through the appointment without losing my shit.

2024.05.14 13:45 catface156 When will I stop needing to increase my dose?

I was diagnosed with ADHD in my 30s, about 5 years ago. I tried a few medications and couldn’t sleep well (doctor and I realized I clear medicines EXTREMELY slow so moved to instant release and it was much better) but soon after that I was trying to get pregnant at the time so I stopped taking meds.
Fast forward to a few months ago, my kid is 3 and I was having a really hard time with focus. I decided to try medication again.
I began with 5mg IR adderall (I split the dose into 2) and even on the first day it was completely life changing. My brain stopped doing a million things at once. Sleep was hard at first but that side effect passed.
I also have terrible IBS which is better and my mood felt more even and balanced.
However, 5mg stopped working pretty quickly and didn’t last long enough. So I went to 10mg.
Now all the above positive effects and I was sleeping better and it was lasting most of the work day if I divided up my doses right.
But then I felt like 10mg wasn’t quite working the same and I am now on 15mg
I’m just feeling worried I’m developing tolerance but maybe this is normal? I began medication early March of this year.
It’s been so life changing in so many positive ways I really want this to keep working.

2024.05.14 05:39 aevigata Adderall… for anxiety???

My psychiatrist wants to put me on Adderall 5mg for anxiety. She says the anxiety is rooted in ADHD. I’m already on Abilify 10mg (i’m a small human), Mirtazipine, Lamotrigine, and Prazosin. I’m hesitant to add another chemical to my body, is this a real thing? I have debilitating anhedonia and anxiety. I really need something to help me. The mirtazipine was supposed to help but it doesn’t actually work “all the way” it only works like 50%, and 50% is still too much. Anyone else been prescribed a stimulant for anxiety or anhedonia?

2024.05.13 19:32 Ok-Painter6658 Adderall Stopped Working?

Hi everyone,
A bit of context, I recently starting taking 5mg of Adderall XR and at first it was really helping me. After about 5 days, I noticed the effects were tapering off with each day. I was told after a week that I could move up to 10mg but after trying 10mg I got a lot of terrible side effects. Headache, really fast heart rate (almost 140) and the focus wasn't really there either. I moved back down to 5mg and feel almost nothing. Not sure if this means I need to switch medications or if I should stick it out with the 10mg to see if my body adjusts. I'm wondering if anyone has had a similar experience and could lend some wisdom. Thank you!
