Hi everyone,
I'm [20FtM] a relatively new student of piano (I've had lessons for the past two semesters at my College), and I've recently been experiencing muscle soreness in my forearms that I think is related to my piano-playing. I believe I've identified the muscle as the extensor carpi radialis brevis. The soreness is present in both forearms, but more prominent in my left forearm. Does anyone know if this is a common injury or if it would be a posture/tension issue that I need to work on? I have a few clips of me playing the piano on my profile if that helps.
I would ask my piano instructor, but I've only just recently noticed it as a problem, and the semester has already ended.

My 35 female, just received 15yr old female daughters, MRI results.
Just asking if someone wouldn't mind explaining this to me, as I am waiting for the doctor to call about this but would like be somewhat informed when he calls and be prepared if I have questions.
Background my daughter has had wrist pain, we dont know exactly what hurts, joints, blood vessels, ligaments, etc,(don't know what else we could be looking for) for over 8 years and multiple doctor visits and exams but nothing.
Is there anything in this that would attribute to daily constant pain for her? Or anything I am missing or the doctors are missing that I can ask?
HISTORY: Bilateral wrist pain, chronic. The patient also has ankle pain. As discussed with Dr. Berger and with the mother, it was decided to proceed first with the MRI arthrogram of the most symptomatic wrist which happens to be the dominant side (right side). The patient has the MRI arthrogram of the contralateral side is scheduled in 2 weeks.
TECHNIQUE: A multisequence, multiplanar MRI of the right wrist was obtained. CONTRAST: 3 mL of a mixture of 15 cc of NS, 5 cc of Optiray 320 and 0.1 ml of Gadavist. SEDATION: Versed was administered orally for anxiolysis. COMPARISON: radiographs of the wrist from September 29, 2023 and prior
FINDINGS: Skeletal maturity: Skeletally mature Marrow pattern: Normal physiologic distribution
Bones and bone marrow: There is a branching, T2 hyperintense area in the body of the capitate that may represent intraosseous vasculature. There is mild surrounding bone marrow edema. Otherwise, the bones are intact without evidence of erosions.
Joints: Mild dorsal decentering of the distal ulna at the radioulnar joint is identified of approximately 3 mm. There is preservation of the joint space. Articular cartilage: Preserved.
Effusion: There is adequate distention of the distal radio- and ulnocarpal joints by the injected fluid.
Ganglion/synovial cysts: There is a small synovial cyst arising from the volar aspect of the distal radioulnar joint extending proximally. The cyst measures approximately 1.1 cm in caudocranial dimension x1 cm in transverse diameter x0.3 cm in AP dimension Intrinsic ligaments. Scapholunate ligament: Intact. Lunotriquetral ligament: Intact. Triangular fibrocartilage complex: Intact. Extrinsic ligaments: Not defined but there is no edema in or under the volapalmar or dorsal capsular region. Extensor tendons: EXT carpi radialis longus and brevis. Lister tubercle: There is mild fluid in the tendon sheath of the extensor carpi radialis longus in keeping with tenosynovitis. EXT pollicis longus: Intact. EXT pollicis brevis and abductor pollicis longus: intact EXT digitorum, EXT indicis: Intact. EXT digiti minimi: Intact. EXT carpi ulnaris: There is subluxation of the extensor carpi ulnaris with a heterogeneous, intermediate increased signal of the tendon distal to the joint line. No definite fiber interruption is identified. In addition, there is mild thickening and edema of the tendon sheath at that level in keeping with mild tenosynovitis. Flexor retinaculum: Defined. Flexor tendons and median nerve: Defined, no abnormal signal. Carpal tunnel at level of hamate hook: No abnormal signal. Pisiform level Guyon canal, ulnar artery and nerve: Defined without abnormal signal. Muscles: Pronator quadratus, intrinsic muscles: Intact. Subcutaneous soft tissues: Infiltration site is identified at the dorsum of the wrist
CONCLUSION: Slightly heterogeneous intermediate increased intrasubstance signal of the distal aspect of the extensor carpi ulnaris distal to the joint line in keeping with tendinosis. In addition, there is subluxation of the tendon and mild tenosynovitis. These findings suggest chronic repetitive stress. Mild incongruency of the distal radioulnar joint. Mild tenosynovitis of the extensor carpi radialis longus. Small ganglion cyst arising from the volar aspect of the distal radio-carpal joint.

Why is this exclusively prevalent among ethnic groups like ours, and some others like perhaps individuals of African descent? You don't see this among white/caucasian ppl. Most of them have evenly toned elbows & knees, and even those that do have some discoloration, have red elbows & knees instead of dark brown like we do. This seems to occur even among fair-skinned Indians. I'm asking this purely outta curiosity. I looked up the scientific literature to find an answer; this condition seems to be called FADES (Frictional asymptomatic darkening of the extensor surfaces). But if this is the case, then why do white individuals not get this skin thickening/hyperkeratosis on the joints as commonly as we do? And even when they do, the discoloration is red, unlike the darkening we get.

I have ultrasound on my right wrist and here are the results
Indications:
Pain at the ulnar edge of the carpus in an athlete.
Results:
Increase in volume of the extensor carpi ulnaris tendon. This presents a regular fibrillar echostructure without signs of cracking. It has a nodular appearance.
There is fluid effusion in its synovial sheath, 2 mm thick and extending over 13 mm. Normal appearance of the other carpal extensor and flexor tendons.
No synovial cyst. No joint effusion.
Conclusion:
Tenosynovitis of the extensor carpi ulnaris.
I don't know if I also have tendonitis but the pain is not that painful now compared to the first week, I have the injury because a 25kg slide on my hand a sudden pressure.

The rest of the components of the TFCC are intact.

A couple of weeks ago, I slipped and fell, and ending up slamming my elbow into a washing machine so hard it cracked the metal and folded the corner in.
It hurt really bad at first, but I've had a broken arm before and at least thought there was no way it was fractured. Haven't lost any range of motion; I go to the gym regularly and haven't had any lifts impeded; including those that put a lot of strain on the elbows including heavy bench press and triceps pushdowns.
However- in the affected arm, it seems like there's something wrong with the muscles in the forearm. The extensor carpi always feel overworked, in fact dramatically overworded, compared to the unaffected arm. This started immediately following the elbow injury.
Any thoughts? Feel free to ask any questions.
Of note: Male, 25, non smoker, mild drinker, very active, 6'6, 210lbs, no pertinent medical history, no prior injuries to the affected arm.

Rereading Best Served Cold. Here are my very scattered and long winded thoughts on how to adapt the story into a feature film script. I wonder if Joe’s screenplay shares any of the following sentiments…
STORY
Cut out Gobba and Mauthis characters/ sequences.
Cut out Shenkt
First setpiece is killing Aario and Foscar.
Next is visserine and Duke Salier plot (midpoint).
Then Puranti / fight with Faithful Carpi along with betrayal of Morveer as pre third act low point.
Then finale with Rogont coronation and then showdown with Orso.
-Cut out war scenes in Ospria, Shenkt and Sajaam, stuff wih Yuri.
Would have to significantly cut down character plots for it to fit in one movie
Monza and Shivers relationship would thus be sole B storyline.
Flashbacks of Monza and Benna’s time as mercenaries would remain as narrative framing. Would be juxtaposed against Monza’s relationship with Shivers as crucial character building.
Morveer would remain tertiary antagonist / final act villain. His big moment would be poisoning the crown as his scene at bank would be cut.
Scenes with Orso and Shenkt plotting following death of his sons would be replaced by Orso instead asking either Faithful Carpi or Ganmark to handle Monza.

Main roles/billing would be:

Aario, foscar, ganmark, faithful carpi would feature in opening scene and their respective deaths.
If Rebecca Ferguson is now too busy (due to newfound stardom following dune), My choice for Monza would be Daniella Melchior who imo is perfect age, look and would likely be available. She has never lead a film but I believe she could pull it off.
A popular fan cast for Morveer is Anton Lesser (Qyburn in got). While he would indeed be perfect, my hot take is someone a bit younger, a bit more ‘weasel like’ should be given the role.

Male 31. I have had recurring wrist pain, and carpal tunnel like symptoms. However, no test has been conclusive about CT. I am wondering if either tendinosis or subluxation could be the cause of my pain.
This is the result of my right wrist MRI:
Osseous structures/joint spaces: Bone marrow signal intensity is within normal limits. No fracture or dislocation is identified. No focal cartilage defect is visualized.
Ligaments: The scapholunate and lunotriquetral ligament complexes are intact. The triangular fibrocartilage complex is unremarkable.
Tendons: Medial subluxation of the extensor carpi ulnaris tendon is present at the level of the ulnar styloid process. Mild extensor carpi ulnaris tendinosis is also seen. The flexor tendons and remaining extensor tendons are otherwise of normal course, caliber, and morphology.
Muscles/subcutaneous tissues: There is no disproportionate muscle atrophy. Subcutaneous tissues are unremarkable.
Neurovascular: No abnormality is identified within the carpal tunnel. The contents of Guyon's canal are intact.
Could the subluxation or tendinosis be the cause of pain?

Is this the greatest mouse grip of all time? Donk, M0nesy, Zywoo, Twistzz all use a form of this grip. Let’s break this down from a biomechanical and performance perspective
This has become a more popular grip over the years but seems to be the most popular across the tac FPS community.
There is a reason for this as tactical FPS games, especially at the highest level, are primarily about crosshair placement & angle clearing (slicing the pie) vs. edge-locking. This limits the overall mouse activity needed.
Tactical FPS mouse movement is also horizontally dominant in nature as evident through these mouse movement maps. Being able to increase control of the mouse for these relatively smaller angles is important and this grip can help with that.

Here are the biomechanics of why it can increase control
This grip can be considered a hover palm/claw hybrid. With this grip the finger contact points are shifted forward which effectively moves the center of mass forward. But what is more interesting about this grip is that it reduces the distance between the wrist joint (primary point of control) and the OPTICAL sensor of the mouse. With the joint actually closer to the optical sensor, it creates increased overall sense of control. You can test this for yourself and feel the difference in smoothness when you try the actual position yourself within the smaller ranges of motion.

Other things to consider
Other variables to consider with regards to this grip are the mouse, mousepad and hand size
Depending on the mouse skates & mousepad there may be a certain level of friction which can affect the sensitivity and overall control. For example using an artisan hien fx soft vs. MP510.

This is especially relevant to this grip because of the downward pressure associated with this style. The downward pressure can also affect the friction and feel of the overall grip. More downward pressure will create more overall friction and slower mouse movement.
I’m sure the individual selection of mousepad / mouse combinations are reflective of the desired friction or feel from each of the pros who use this grip. Or they can just modify the pressure downward to get their desired feel.

The hand to mouse size ratio is also important to consider. Larger hands (when the mouse size is the same) can also potentially shorten the distance between the joint (body’s point of control) and the optical sensor (peripheral point of control).
Again just some variables to consider and can explain why certain individuals might choose specific mice considerate of their own individual anatomy

Using this grip can increase the amount of muscle use on the PALM-sided muscles. Again the misconception is often that it can increase the use of the extensor muscles. This is only true when the FOREARM is floating.
Depending on the amount of downward pressure utilized, it may also involve the triceps but the downward pressure is mainly applied through the forearm flexors. This means anyone who is using this group should be strengthening / building endurance of these muscles and stretching them regularly between games.
Here are a few exercises you can do to target this specific region

So should you use this grip?
This is probably what everyone wants to know. My answer is.. Use what is comfortable for you and stick with it.
Everyone is different and as you can tell there are MANY variables that can affect mouse control (grip, DPI, sens, mouse, downward pressure, pad friction etc.). Yes you can optimize your mechanical skill for a specific esports title… but what are the actual performance returns?
Performance is multi-factorial in esports and there is an over fascination with optimizing mechanical skill performance in the space. While it DOES matter, performance is affected by MANY things and arguably at the highest level the mental game plays a larger factor.
When pros are in flow, the difference in HS% or mechanical skill won’t be too different. But the mental toughness and performance psychology needed to succeed over multiple tournaments, lengthy competitions and unique team environments is what often sets aside the Goats from the great.
So Try it if you want, but whatever you end up doing, stick with it and don’t focus too much on it.
