I have sinusitis with polyps and a deviated septum the ent doc said that I would need a general anaesthetic
I have never had a general before and one of the things I am worried about is I have a history of vasovagal syncope. I have fainted 9 times since the age of 10. The triggers are usually pain but 2 were unexplained. Some examples:
It was my first day of work in a factory when I was 17 and after 10 mins from my start time I started thinking that I’m not going to last my 12 hour shift and other negative things. I sort of felt freaked out and thought to myself I hope I don’t faint. The next thing I’m feeling sick, dizzy and woke up with people around me. I basically thought myself into unconsciousness.
My most recent time was on a ferry I sprained my ankle and shortly afterwards blacked out. When I woke up one onlooker told me that during the episode my lips had turned blue and I had been making a snorting/gasping noise. However I did manage to continue walking the rest of the day with no concerns. I had an ecg and tests when I got home all were normal. This episode was followed by months of acute health anxiety I kept worrying about passing out again.
I’m just thinking that as I’m so weird and hypersensitive it’s probably better to have poorer quality of life than to ‘risk myself’ under a general anaesthetic. I would probably just get there and change my mind last minute. I’d be a nightmare for them : /

I suffer from eczema, and right now is allergy season and my lips are just destroyed from sneezing and wiping my nose. I'm also currently on antibiotics overcoming perioral dermatitis rash around nose from nasal steroid spray use.
Nothing was working, straight up Vaseline and aquaphor were just easily being wiped off and I was literally reapplying them 10 to 12 times a day, with little relief.
Somebody out of the blue recommended nipple cream! Which is basically just lanolin ointment which comes from sheep's wool, more specifically they're glands.
Now there has been a lot of controversy with lanolin as it was recently declared "allergin of the year"... But I was so desperate so I slathered the stuff all over my lips and I have been for the past week! And it has been a lifesaver!
This stuff is thick! I'm talking like dried sap from a tree. It does tug a little bit on my lips when applying it but not so badly once it warms up to your body temperature. It's also very difficult to squeeze from the tube, you definitely have to use some elbow grease and warm up the tube.
But once this stuff is on there it isn't going anywhere! I now only need to apply it three times a day; once in the morning once in the afternoon and once before bed. I've even been using it around my nostrils where it is super sore and the skin is compromised from perioral dermititis and tissue use.
So what is lanolin?
Lanolin is the wool grease secreted by the sheep sebaceous glands, and represents a complex mixture of high molecular mass lipids, including fatty acids and alcohols, sterols, hydroxyacids, diols, and aliphatic and steryl esters
Now let's get to the controversy. Is this really a common allergen? Well I looked at the studies. And they are all inconclusive. And they just state that more studies need to be conducted.

Abstract

Background: Irritable bowel syndrome (IBS) is a chronic abdominal pain disorder that affects women twice as often as men. While luminal mediators of both host and bacterial origin have been implicated in modulating abdominal pain in IBS patients, gonadal hormones have also been shown to influence pain signaling. Estrogen has been identified as a pronociceptive mediator that can modulate central and peripheral neural pathways. Given this, we hypothesized that the estrous cycle modulates sensory neuronal excitability, thereby altering the sensitivity to luminal mediators and this contributes to the female predominance of IBS.
Aim: Identify the impact of the estrous cycle on nociceptive signaling and compare the effects of fecal supernatants (FS) from male and female IBS patients on abdominal pain pathways.
Methods: Current clamp recordings measured rheobase and voltage clamp measured voltage-gated Na⁺ current in thoracolumbar dorsal root ganglia (DRG) neurons. FS from male and female IBS patients reporting high levels of abdominal pain were used. FS were perfused through male, female, and ovariectomized murine colonic preparations while performing extracellular colonic afferent nerve recordings to measure changes in action potential frequency during spontaneous firing and in response to colonic distension. Phase of estrous cycle in female mice was determined through analysis of vaginal swabs. Ovariectomies were performed 4 weeks prior to assays.
Results: Current clamp recordings revealed an increase in excitability due to a 20% reduction in rheobase in DRG neurons taken from proestrus/estrus female mice compared to males, metestrus/diestrus females and ovariectomized females (p < 0.05). Voltage-gated Na⁺ current density was increased by 40% in neurons from proestrus/estrus mice compared to metestrus/diestrus female and male mice (p < 0.01). Extracellular afferent nerve recordings revealed that FS from female IBS patients reporting high abdominal pain (N=3) increased afferent nerve discharge (p < 0.05) in proestrus/estrus female mice by 70%. Single unit analysis of nociceptive axons showed that their activation was increased over 50% following FS perfusion. H owever, this excitatory effect was abolished in ovariectomized mice. Interestingly, FS from male IBS patients reporting high abdominal pain (N=4) had no effect in male mice while female IBS patient FS (N = 6) increased afferent nerve discharge (p< 0.05).
Conclusion: This work suggests that the estrous cycle impacts abdominal pain signaling, which may contribute to the female predominance of IBS.

So I'm on my second playthrough, I did all supressions already and my goal is to get 100% codex and defeat wn and ab. And you probably guessed it, rn I'm staring at abno selection screen with pd on it. There is one problem though: I'm on day 12 rn 💀. So, here are my options: 1) Ignore pd and hope I'll get him later 2) Take pd, cheese wn with panicking heretic, memory repo 3) Take pd and save him for later, but I won't be able to get rid of him
Basically it comes down to whether wn is good or bad for my facility and the chances I have of getting him later. Paradise lost benefits a lot from wn's presence and healing is nice, but him being train on steroids is very problematic. And chances of getting pd later are pretty good. I really want to know about your experience with managing wn on days 46-50. I might have all the wiki knowledge, but I don't think I can make a decision just based on that.

i visited a pulmonologist today, i got my ige and ace tests done , CT scan, they were completely normal , but my cough is not getting better ( no chest tightness, no chest pain, no shortness of breath , wheezing when exhaled strongly which doc noticed today ) JUST DRY COUGH chest vibrating cough with little phlegm ( like a flare up ) that gets triggered when i expose to dust , pollution, allergens . Iam a 22y male, non smoker ,non-alcoholic , i live in india,hyderabad . the doc i consulted today prescribed me aerospin rotahaler with respicaps ( glycopyrrolate 25mcg, budesonide 400 mcg and formoterol fumarate 12mcg and ab-flo tabs ,cerafex-am tabs , ivepred 8mg. I consulted pulmonologists for 5 times now, every time i visit them , they prescribe me the same kind of medicine ( budesonide mdi inhalers,formoflo inhaler, trimium inhaler ,all MDI inhalers , i used them all with a spacer from the past 6 months , anti-histamines ( ebastine , odimont fx , montelukast ),steroids (prednisone , defcort , ivepred )etc...) and they insist to prescribe me a nebulizer even though i ask them to do so , they say they don't usually prescribe nebulizers for adults . I don't understand what to do , i've been on inhalers from the past 6 months and i beleive that using a nebulizer with better respules would treat my dry cough , any suggestion would really be helpful .

LOGICALLY it makes no sense to keep it. but my heart hurts thinking i’m letting this little life down and being selfish for the sake of my own future even though the mistake has already been made. we talked ab it and even though logically it makes sense, the idea of getting to raise a child with the love of my life sounds wonderful. we have been planning on getting married soon anyways and i know he would support us as a father, but there are a billion factors we would have to figure out. i’m scared aborting it would scar me forever and give me religious fear that i would go to hell and i’m scared keeping it would send me in a depression in losing the life i have worked so hard to achieve and i have no clue what i would even begin to do to figure out my future career or changing it to something i would complete quicker. since he’s leaning towards an abortion but not trying to sway my decision, he explained how he fears that i would hold it against him forever if i didn’t keep it. i love him and want to make the right decision but i’m scared i will have some animosity towards him for not wanting to keep it as bad as me. i know this is only our decision to make but i really need help determining if i’m crazy and what would be the best option mentally and life-wise

hello, i am a 19 year old female who has been dating my boyfriend (19 years old) for 5 years. we are best friends and our relationship has never been more stable. he works blue collar making 22 an hour and i am in school to become a doctor. i was valedictorian, have done years of college credit hours through dual enrollment, and have just completed my freshman year at a large university a couple hours away from him and our hometown. i am now living with him for the summer. i just found out i’m pregnant and have had the hardest time figuring out if i should keep it or not. we both feel that we are not ready financially, mentally, or in consideration of our futures. he told me he will support me and be there for me no matter what decision i make but i know part of him wants me to abort it. i come from a muslim family who is extremely strict about this type of thing and i fear how i would even approach them about this situation and how they would react. i have had past incidents on a smaller scale where they found out and were extremely angry. i own my car and don’t live with them anymore but i still love them and value them in raising me. it would absolutely break their hearts that my future dreams of becoming a doctor would most likely go down the drain. i have immense guilt about considering an abortion and i know either decision i make will impact me majorly. i’m scared i’m going to go to hell if i don’t keep it and i’m scared i’m going to lose my family and my future if i do. i love babies so much and i know i would love my child, but me and him rent from a friend and would have to move if we kept it, do not own a house, have barely any money saved up, he is trying to get on his feet and figure out his future, and i still have 6 years until i finish medical school. Any existing relevant medical issues: I do have a “lazy” eye that I’ve had for my entire life that’s never affected me. I just can’t move my right eye fully to the right otherwise everything is normal and you wouldn’t know. I also used to get migraines in middle and high school often but I only get them once or twice a year.
Current medications: Montokulast, Beyaz, vit D (prescribed once a week), 0.05% tret and Xanax as needed.
Hi everyone, I’m posting this cause I don’t know what to do at this point. I’m 34/F with a moderately healthy lifestyle. I work from home and I’m a gamer so I do have quite a bit of screen time. I don’t smoke, I drink occasionally and I’m somewhat bad a drinking water but I do try often.
I finish work one evening, took a shower and was getting dressed to hop online and play some games. That’s when it started. I thought I was getting a migraine, the sensitivity to light and a patch of blurred brightness straight in the center of my right eye. Normally this last 20 mins and then it’ll go away and the headaches and nausea would follow, but that never happened. Just the vision blur and in just my right eye.
The rest of the evening came and went, my right eye remained blurry. This was a Thursday, by Monday I went to the optometrist who said my eye was perfectly healthy and I have 20/20 vision but I should see an ophthalmologist to see if they can help cause he didn’t see anything that could be causing this.
By Wednesday, I was able to get in with an ophthalmologist who did a pretty extensive exam and again could not find anything wrong with my eyes. Both eye doctors did dilation, the first was photos and the second did dilation.
At this point, I make an appointment the next day to see my PCP. She looks at the notes and examines me, until she notices that my ears are completely filled with fluid. Normally I do get ear aches where it feels like some stabbed me in the ear with a knife. But it normally goes away and sometimes it’ll feel like when you dive into a pool and you get water in your ear. So my doctor suggests that I take 6day steroids and Afrin because she thinks my sinuses cause be inflamed and putting pressure on my eye. She also ordered up bloodwork to check for any infections.
Labwork came up normal, I finished the steroids and nothing changed or improved. So the next step was an MRI of my head, with and without contrast. I’ll note, I’m still waiting on her feedback after this, but the note stated:
IMPRESSION: 1. A focus of abnormal FLAIT2 signal and contrast enhancement, is present, within the deep white matter, of the right temporal lobe, measuring approximately 5 x 5 x 4 mm. The finding is somewhat nonspecific. It may be related to a focus of demyelination. However, no additional lesions are identified. Clinical correlation is recommended. A follow-up pre and postcontrast MR examination of the brain, in 8-10 weeks, is recommended, for additional assessment.
I do not know what this means but I’m not sure if this has any correlation from what I googled/check reddit.
I’m thinking I should maybe see an ENT next and have them double check my ears. Has anyone heard of this or have any clue where the next appointment should be.
One thing to note. I noticed weird flashes of light in my right eye one evening before this started. Then it happened once last night. It looks like a flash of starlight in my peripheral. This blurriness can sometimes improve slightly but it never goes away. It’s like having a spot of Vaseline in one spot of my eye but with aight brightness in the fuzziness. Every now and then I get headaches, but only once was it super bad. My eye does feel strained and it sometimes can feel like I’m cross eyed when I’m not.

In December of 2023, I developed nerve pain in my left hip. Initially, I thought is was piriformis syndrome because I had been doing a lot of pelvic floor stretches and was had been doing desk job from a hard wooden chair.
I had X-rays of my hip, back, neck, and shoulders. Everything looked fine. I did a round of steroids which were not beneficial.
Eventually, nerve pain spread into my right hip as well, which made my question the piriformis diagnosis. Shortly after, I developed nerve pain in both shoulders. I decided to do PT, which I did for about three months. PT seemed to help some, but my symptoms plateaued at a certain point.
During PT, I tested positive for Lyme. I think 6 igg bands and 1 igm band were abnormal. My Lyme ab screen index was 7.5. My PCP treated me for Lyme arthritis, the standard CDC 28-day doxy course. My symptoms did not seem to improve during this time.
I've since met with a Lyme doc, who has been skeptical that my symptoms are all due to Lyme, but is testing me for co-infections, among other things. So far, all of my tests have been normal, aside from Lyme.
I don't really know what to think at this point. I've read of cases where Lyme seems to contribute to issue like sciatica, but it doesn't seem super common.
Has any here had similar symptoms, and did you have any improvement by treating the Lyme?
I've had some other symptoms in recent years. Honestly, since I first got COVID in 2021, I've had chronic iritis, chronic sinusitis, chronic epididymitis, chronic neck pain (which I did PT for right before my nerve pain started). I have had wrist pain over a few years in my right wrist, but have developed similar wrist pain on my left side since my nerve began.
