around my mount/nostrils and eyebrows are the spots that are the most bothersome. it has a bad smell and sometimes yellowish spots
tacrolimus ointment had some positive impact but not ideal for a long term solution
started noticing flakes in my eyebrows around 5-6 years ago and it has gotten progressively worse. there are patches of flakey oily skin covering most of my face I can only remove by scrubbing. the flakey buildup has spread to different parts of my body including inside my ears, belly button, groin and butt crack area
zinc shampoo doesn’t seem to help
any suggestions would be appreciated. thanks and hopefully I am not including too many photos but I wanted to show the flare ups at different stages

It started as an anal gland abscess in dec. She had an infusion into the gland of antibiotics and steriods. Got better then the abscess returned in full force in February.
In mid-march she had been doing good for 2 weeks and then bam! blood & pus constantly leaking out so the vet decided to do weekly anal gland infusions.
which worked along with the Tacrolimus ointment twice a day. things were going well, still doing weekly check ups but no infusions to the anal glands since Apr 25
today the perianal fistula decided to start leaking blood and pus again. so i guess my question is for anyone who’s gsd has had this problem…
how did you handle it? were there setbacks and was there an obvious amount of time that passed where you were like, okay a setback will hit any day.
seems like 2 weeks without the infusion is when we get kick backed to square one.
any advice or help would be much appreciated. i have such a healthy and happy gsd that this really breaks my heart for her. she’s so good about the at home care and vet visits. but i know she has to be in pain. that breaks my heart.

Severely chapped lips for 4 months
I started with what I assumed was normal chapped lips in mid January. Treated as I normally would but worsened. They burn and itch, and swell some but no cracking or flaking or the skin. It’s mainly the lip border and surrounding tissue. When they really flare up I’ll will have tiny pin head sized white head in three main areas that are the worst. I’ve seen a dentist, my primary twice and my dermatologist. My dermatologist said it was eczematous cheilitis and rx tacrolimus ointment as I had already been using Dr Dans Cortibalm. But that doesn’t really seem to be working. The area does get better with a round of oral prednisone but comes right back within a few days once I’m off if it.
I’m going to try and get another opinion by a different derm but that may take weeks to months.
Please help! I’ve been dealing with this now for 4 months and it’s so miserable (and embarrassing). Also I have no history of eczema and I’m 41.

I am looking for suggestions for facial cleanser and moisturizer that is compatible with SD. I have seen people mention MCT oil and will probably start with that. I am 40f and haven't had a regular facial care routine for years (since having my son 4 years ago- just splash water on my face and use a cleanser when I shower every couple days). Looking for cleansers/creams that may support my current situation. Only thing I have used recently is CeraVae but would like something else to try. Read on if you want more context.... Thanks!
In the last few winters I developed cracking and flaking on one ear, but it always cleared up as weather improved (it was nonexistent May-November). This past winter, I had dramatically worsening symptoms - my whole right ear (outside especially, like a red ring around it) and now my left ear has a little bit in the ear canal. My eyelids started to have inflamed patches, also worse on the right. I have minimal signs of SD on my scalp, sometimes light flaking.
I saw a derm a few weeks ago and was prescribed tacrolimus for my eyelids and a steroid for my ear. After two applications of tacrolimus, my eyelids totally cleared (aside from some dry skin). It just flared again 10 days later, I'll reapply tonight. My ear did not improve after a week of steroid cream, but after 1 application of tacrolimus last night, it is vastly improved. I was not given any antifungal ointments.

I have my first rheumatologist appointment on Monday after having to wait four months for an available appointment. It all started because I had a horrible rash that lasted 8 months total, at the time I saw my first dermatologist I had it for 6, it had started as a small patch under my armpit and over time had grown and was covering my arms and even my legs but not my torso. That dermatologist did a biopsy (while I was taking steroids prescribed from an urgent care) and said it was inconclusive so likely eczema, but I felt like he was wrong. It’s one thing to develop eczema at 34 but majority of my body? So I went to my primary care (backwards I know) and he ran blood work.
As soon as the blood work came back he told me I needed to see a rheumatologist and gave me the referral. He also gave me a referral to a different dermatologist. He tried several medications including one that I had to sign a liability form stating I was aware it can cause cancer. NONE of the medications topical, oral, steroids, anti-fungal, ever helped they all seemed to make it worse or cause it to spread. The only thing that helped was when I was prescribed a medication to help control the itch, it’s for anxiety and blood pressure but off label can help with itchiness.
I have positive ANA and SJ. Negative rheumatoid factor, but as we all know a pos ANA SJ doesn’t tell us anything and just stresses me out. Between that and the fact that I have had four months to ignore it or spiral (no in between) I am so ready to start the process of figuring this out.
I realized I don’t know what is a symptom and what is just getting older. I am lethargic 3-5 days a week, like fall asleep as soon as I get home from work until I have to go back in the morning. I have been losing weight (even though I purposefully eat 3k+ calories a day, which has slowed the weight loss but not stopped it). My lymph nodes are swollen, (and even painful under my arms) my hands and feet swell up to the point I cannot wear rings anymore. My back hurts, my joints hurt, I thought my arms/hands hurting was due to my carpal tunnel (even though I haven’t done anything really to aggravate it) I get headaches that are debilitating at least 2x a week, my pinky and ring fingers are almost always numb, my big toes are constantly numb, brain fog a couple times a week, acne even though I’m mid 30s. I think that’s the majority of it.
Thank you all for letting me vent, any insight would be appreciated.
EDIT: update answering questions and clarifying a bit…
I think the derm was just tryna get me out of his hair, he had so many patients and I took up too much of his time (🙄) the fact that he didn’t even think about the fact I was on steroids when he did the biopsy (skews results because they reduce inflammation)…he should have known that.
Primary did bloodwork and sent me to specialists, which I get, and maybe he would have done more testing if I went back in to see him but 1. he said he wanted to follow up about an entirely different test result that I have known about for years and wasn’t worried about because 2. I was too busy spiraling about this new issue.
My job is (semi) seasonal, meaning we don’t have a lot of work in the summer (live in AZ) and plenty of OT in the winter so I just dove into work so I could save for this summer. Then when the rheum. Told me it was 4 months I said well, that works I guess and didn’t push it. I know I should have been more proactive about all of this, but in my mind I was either going to freak out or convince myself I have lived like this for this long a few more months shouldn’t hurt.
I could probably find another doc but this one is apparently highly rated and takes my insurance (which is half the battle sometimes).
The anti itch medication that actually helped is hydroxyzine
The medication I had to sign for that made a little bit of a difference but I was only allowed to use it one week on then two weeks off due to excessive thinning of the skin and other risky side effects is called tacrolimus ointment. I was able to do that twice before my follow up and had to stop in case he did a biopsy. As I said above it helped a little bit, but I couldn’t keep using it, kind of like with all the oral steroids they would give me (so much prednisone like sooo sooo much) it helped a little bit when I was taking it but as soon as I stopped it came back with a vengeance.
The other topical medications I used basically chased the rash around/made it spread. Eventually my legs from my hips down were covered in the rash, scabbed from itching, and bruised, along from the bottom of my bra line/under armpits and even on my fingers.

(M20) Hello everyone, I am very happy to say that I am relieving all of my inflammation and hypopigmentation by using tacrolimus for the past month or so. It has worked better than any other medication like steroids that I’ve tried (gives me rashes and makes skin more susceptible to irritation). Only problem is that I’m worried that the ointment may be working in the short run but not the long. Anybody who has used it to treat their LS, is it unsafe to use for this long and have you found success like I have?

Hello everybody! How are you. First of all, english is not my first language, so excuse any mistakes...
I've been struggling with SD for a year now, but just got diagnosed. My derm put me on Ketoconazol shampoo, and Tacrolimus ointment 0,1%
I have a few questions... I'm not sure how to follow my skincare routine, since my derm told me to use the ointment 2 times a day. And i'm also not sure why i should wash my hair with the shampoo, since I only have SD on my face.
I usually wash my hair with the shampoo (i used to co wash, and follow the CG Method, but i'm a bit lost now), then rub it on my face. After the shower i put the ointment on my face. I read that I should wait two hours before putting on moisturizer? I'm currently using the cerave in a tub that i read was safe. Stings a bit though. I Used to use Vit C + Niacinamide + Ha + Sa Toner on different days (all the ones i have are safe according to Sezia.) But now i'm not sure whether to wait for the ointment to work for a while (hopefully a few weeks) and then re start my routine, or if it's safe to use the ointment and then proceed with the routine as always?
The products I use (I'm based in Argentina):
So, to make it short:
  1. Why should i wash my hair with the shampoo if I only have SD on my face? Can I use it as a facewash of sorts, so I could still do the CG method ? My hair is starting to be straight and i've only used it for 4 days...

The sinful woman understood what Judas did not: Christianity is about healing. Not just temporary healing, but from man’s biggest problems: egotism, sin and death. The great paradigm of Orthodoxy is not that of righteous/sinful, but of healthy/sick. All of us need healing. This is our primary Christian aim, and is evidenced by the fact that even Jesus implies it is of greater importance than giving our riches to the poor and needy. The social gospel had taken such a hold of the disciples of Jesus that they forgot what is even more essential. The sinful woman shows her great love by her humble offering, and not only received healing by the Physician of our souls, but has become an example for all time for what the Lord truly seeks from His disciples.

2024.04.28 16:44 rari_845 Advice: doctor prescribed Protopic 0.03% (tacrolimus monohydrate) for melasma

My mother has had melasma for over 20 years. She recently went to the GP who prescribed her Protopic ointment 0.03% (tacrolimus monohydrate). She said when collecting from the pharmacy, the pharmacist was quite surprised and advised her to use it with caution as it's an extremely strong ointment.
After researching, I found that it's used to treat severe eczema, with no mention anywhere of hyperpigmentation/melasma or any of that sort. There is some mention of Protopic being used for psoriasis and vitiligo, neither of which she has.
The main ingredient of Protopic is an anti-inflammatory, so I really question if this cream is at all effective on melasma and if it will cause unnecessary adverse reactions on her skin :(
I was also looking to get her 0.03% tretinoin + hydroquinone as I've heard this is the "gold standard" for melasma and is researched. Just not sure about this cream and if the GP has prescribed her the right medicine.
Would appreciate any advice!

2024.04.27 17:57 nddjjsjsnsnfndndnd Constant vulvar burning and itching for 4 years. Any experience with this?

Most of my itching and burning is in between the crease of the inner and outer labia (interlabial sulci). A bit on the anal area but much less.
No skin changes, no bacteria, no STD, no yeast, pelvic floor therapy doesn't help, etc.
Never had a biopsy because no skin to biopsy from. Have seen many specialists and inconclusive diagnosis. Some suggested LS, some dermatitis, some aren't sure. But again, I have 0 visual symptoms.
Responsive to: Tacrolimus ointment. Currently flaring because stopped Tacrolimus and now will take a long time of daily use to get better. I was in maintenance mode and had no symptoms for a while but had to use Tacrolimus once a week. I went two weeks without it and am now experiencing the worst flare. I've had to apply it daily for almost 2 months, often two times a day, and I'm just anxiously waiting for it to improve so I can taper down the usage. But so far, it's working more slowly than before, and I'm scared it won't work anymore. I did 2x daily for 6 weeks, there might have been a slight improvement, and now I'm doing once daily but feeling the burning more. I get immediate relief when I put it on.
If I don't put it on for a few hours extra, my burning will worsen drastically.
I've used clobetasol before for a long time but was never able to control the initial flare 4 years ago so switched to tac. Clobe also gave me some relief but temporary. Indicates a skin issue.
