Is this more than hormonal / cystic acne?

TLDR: Went to the derm. Forgot to ask a question... Could my "cystic acne" be me getting transferred skin bacteria from others?

I went to the dermatologist a couple of days ago and she prescribed me dapsone and trentinoin. But the problem is I forgot to mention one of the possible causes...
I was wondering if someone who has near perfect skin her entire life minus some acne here and there during puberty and never had to use specific skincare products. Why all of a sudden is my skin breaking out with painful acne / pimples I've never had before? Yes I am now 42 at the end of the month but this rarely has happened in my family's history.
The pimples come out of nowhere for a day. Will be really painful and then scar up the next day. As soon as I think it's over another one will pop up and it seems to be happening in a line across my chin?
I've been wearing mask more.often as I'm immunocompromised and started working in the public but I take breaks from my mask and wear a clean disposable every day.
It's weird that it happens on the ONE side my boyfriend sleeps on / kisses me / Eskimo kisses my face. He's had acne and cycstoc acne issues in the past. Is it possible he's passing it off to me ? I know acne isn't supposed to be contagious but if the derm is giving me a cream to combat bacteria is it that far off of a theory ?
I recently found out I'm allergic to dogs and cats. He loves to snuggle with his pets... One of them has constant yeasty ear infections... And does face to furry face Eskimo kisses. I've asked him to refrain but of course he doesn't or forgets. Am I crazy for thinking he's transferring bacteria to my face and it's breaking out cause of it ? I sleep on satin pillow cases and have been washing my face specifically before bed too. Is it just acne or could it be more ?
Also I've taken OTC supplements that could possibly give me break out such as probiotics but ive been past the break out stage and have been taking probiotics for years. I stopped them just in case it was that and all other supplements. Still the same issue. Please help! Thanks 🙏🏾

My mild cystic acne has been back for ~8 months. I have been using winlevi and dapsone. I feel like I’ve run out of options but figured it’s worth one last shot.
My skin was so clear on spironolactone, but it significantly decreases my libido. I also have lower BP & take propranolol as needed so I don’t think I’d be able to go back on it if I wanted to.
Birth control also helped my skin, but had the same effect on libido. If I wasn’t in a relationship I wouldn’t care about this side effect. I also take Lamotrigine and heard that they interact.
Accutane was a no when I asked but I think I’d have to go on birth control anyway so it would be pointless. Is there anything else I could do? Thanks in advance.

Hi, I started getting acne on my chin after I started stims, figured it would sort itself out after taking hormones, but it's been two months since my egg retrieval and it still hasn’t gone away!
I’ve been on Spiro rx and a dapsone/niaciminade rx cream for YEARS and it’s always kept acne at bay. I used to struggle with hormonal acne around my chin, which is why I was on these medications long before egg freezing, and my skincare routine hasn’t changed:
  1. Wash face with gentle cleanser or Bioderma micellar water
  2. Paula’s Choice BHA Exfoliator
  3. Dapsone/niaciminade rx cream
  4. Dieux Skin Air Angel Gel Cream moisturizer & eye cream
  5. Just started using Differin 2 weeks ago but no changes
I'm going to the dermatologist on Friday but I am just curious if anyone else has experience this and if anything worked to get rid of it?

I have officially been on Slynd for one month. I am also taking 50mg of spironolactone. Before Slynd I was on no birth control for over a year, and had acne cysts on my chin and jawline the two weeks leading up to my period.
My first month on Slynd had been as follows: First week: Terrible breakout, large cysts on forehead and chin.
Next two weeks: Skin clears up, no new cysts. However, I have noticed that my cheeks are covered in closed comedones. I don't recall having this issue before, as my skincare routine has always keep those away.
Final week: All hell breaks loose, skin is worse than it had ever been before. 15 very large cysts on my face; chin, jawline, and forehead. It's embarrassing to go to work.
Today is the first day of the four sugar pills in the pack. I guess I just want to know if this sounds similar to anyone else's experience on Slynd. Does it get better? I just want to know if I should start the new pack after my withdrawal bleed, or stop this birth control before I do irreparable damage to my skin.
I'm confused why this would happen on this birth control, if it's supposedly antiandrogenic and equates to about 25mg of spironolactone itself. In the past spironolactone has cleared my skin, but it seems like on this birth control its not touching it.
Please don't tell me to ask my gynecologist; I am in communication with her. However acne is not really her priority like it is mine. Doctors kind of give a standard answer of "wait three months" to give something a try, but I believe many of us know from experience that 1 month is enough to know if something isn't working.
My skincare routine, just because I already have prescription products and they are doing nothing:
AM: Cerave moisturizing wash Dapsone 7.5% Vanicream moisturizer Sunscreen
PM: Cerave SA wash Tazarotene 0.1% (retinoid) Vanicream moisturizer Aquaphor around nose/mouth

I have hormonal acne. I started taking BC and spiro around the same time, but my acne got worse. I’m fairly confident the BC is what made my acne worse, though, not spiro, as I had a whole host of other negative BC-related side effects on top of just worsened acne.
I saw a new derm recently and was prescribed amoxicillin, tretinoin, and dapsone gel. I was wondering if adding back spiro would be okay with this new routine?

2024.05.06 22:28 kthreerry good products for hyperpigmentation while on Spiro?

I’ve been on 100mg of Spiro for a few days short of a month now. I wouldn’t say I have been purging necessarily, but I am still getting about the same amount of breakouts I would expect to be getting if I wasn’t taking Spiro at all.
I have pretty bad hyperpigmentation from ~9 months of crazy hormonal acne all around my chin and jaw before I got prescribed Spiro, so I think it might be skewing my perception of any progress I may have made so far.
Are there any good products to use for helping with hyperpigmentation that don’t react negatively with Spiro? I’m not sure if that’s something I should be worried about. I also got prescribed Dapsone topical gel. I have the Ordinary glycolic acid toner, but I stopped using it a few weeks before my derm appointment because I wanted her see my skin with as few products as possible in my routine.
any advice or recommendations would be appreciated!!

I was diagnosed with HS early 20s nearing on 30 now, I've had my fair share of meds from harsh acne meds isotretinion to clear up what was bad acne when I was around 16/17 which worked wonders.
Fast forward to HS diagnosis I cannot remember in which order and because it's been years, I vaguely remember names of antibiotics I was put on. I believe I was on Clindamycin,Dapsone and Lymecycline. Some of these not remember which worked best for me there may have been other prescribed over the years but I haven't been on any antibiotics in almost 2 years now.
The flare ups were minimal and was fairly controlled I had some painful one in the groin and armpits which I had the cut out in 2022 and I didn't get many flare ups after that it did really help. However the past 3 or so months I've been getting really bad reoccurring lumps in the same places, which go down then inflate like a golf ball very painful and I believe it's making me feel unwell.
I'm not sure what's brought these on so suddenly possibly stress? Though I like to think I don't get stressed but I could be just unaware perhaps? I also met someone who I really like in a ldr and planning to see them it would be my first time travelling to another country. I'm very worried about what they would think so maybe I am stressed and that's made me flare up. I have told them about it and I don't think they care about it although they said they wasn't sure what it is but they like me so that's given me some relief from that.
In regards to feeling unwell within the past month and a half I've felt so tired and drained waking up with muscle and joint aches and from all I can describe is brain fog, I am unsure on what to do where these flare ups have caused some inflammation I have a bad immune system so I don't know.
I have also tried epson salt baths hibiclens which I don't think hibiclens helps at all also tea tree oil. I've seen people mention vicks here I've been using that the past 4 days yet to see any results.
My dermatologist kind of took me off the books since I had surgery so I'd have to go through my GP to get referred again. I'm at a loss and really don't know what to do part of me wants to see if I could go to out of hours to try see if they could prescribed something to help with the flare up, I just want to feel better and comfortable especially when I plan to travel within less than 2 months.
Sorry for the long post any advice or help is greatly appreciated.

SKINCARE ROUTINE: COSRX + BUBBLE
Just making a quick post to say that I have the most stubborn, oily, hormonal, clogged pore prone skin. I’ve been on and off topical and oral medication’s for over 10 years (clindamycin, doxycycline, minocycline, tret, adapelene, azealic acid, spironolactone, dapsone). And any store brought product was sure to break me out, irritate my skin, or cause new acne.
I just recently found the COS RX whitehead power liquid and bubble fresh start gel cleanser.
Both amazing products.
For moisturizer if I need it I will use a basic hyaluronic acid serum, however this tends to make me feel and look very oily and shiny.
For sunscreen I use NIVEA SUN protect super water gel, ive heard reviews that the SPF isnt as effective as it states, however this is the only SPF that doesnt break me out or clog my pores and seems to do the job.
Lastly, Mixsoon vegan snail mucin is a very gentle exfoliator if you feel that you have some roughness and/or texture and is nice to use in lieu of the cosrx product if my skin is feeling tight or dry.
I do use winlevi (clascoterone cream) on my chin/jawline area one week out of the month to prevent cystic acne that I get from my period. I havent used adapelene since listing all of the products I mentioned.
Overall, listen to your skin. The vehicle the product is in matters! That is, cream products will certainly clog my pores. Ive noticed gel based products work very well with my skin type. Everyone is different. Don’t over do it, be gentle and patient. Eat clean, workout consistently and find what your skin likes.

Hi all! Since I changed my birth control method over a year ago I’ve been battling hormonal acne like never before. I have a dermatologist and so far everything we’ve tried has barely helped, if at all. I’m trying to stay topical rather than go the spironolactone route. Also just found out my new insurance won’t cover one of my Rx (dapsone) and it’s expensive without the coverage. I was doing some reading in here about azelaic acid helping a lot of people with their hormonal acne.
Just wondering what you all suggest in terms of brands and also where to add it into my routine? Anything else to look into adding as well or instead of? I have oily but also sensitive skin so I always introduce things slowly and stay hydrated.
Morning:
1) cleanse with Cetaphil Gentle Cleanser 2) Good Molecules Brightening Serum (just added this a day or two ago) 3) Good Molecules Super Peptide Serum 4) Rx Winlevi 5% cream 5) spot treat with Rx Dapsone 5% gel 6) moisturize with Cetaphil moisturizer 7) mineral sunscreen
Night: 1) cleanse with Cetaphil Daily Facial Cleanser 2) Good Molecules Super Peptide Serum 3) Adapalene 0.3% with 2.5% benzoyl peroxide 4) moisturize with CeraVe Moisturizing Cream
Once a week I will skip #2 and 3 at night and add in a 5% lactic acid peel before moisturizing.

So I never had acne until I quit what I didn’t know what was an androgenic birth control pill in 2014 at age 21. Normal hormones and endo told me my androgen receptors are just very sensitive to my normal levels of androgens. Not a candidate for Spiro due to hypotension (I have POTS so I’m even on meds to RAISE my BP). I tried yaz for a few years but still broke out - I don’t think it was anti androgenic enough for me. I was clear for a while on curology (tret/clinda/Azelaic acid) with dapsone gel and yaz BCP but then dapsone was no longer covered by my insurance and yaz was making me lose hair (I already developed androgenic alopecia after quitting birth control along w the adult acne so I couldn’t take that).
So I stopped yaz and kept using my curology formula of 0.09 tret/clindamycin/Azelaic acid. I’d use chemical exfoliants and even BP daily too..I had no idea what barrier care was lol. Anyways I kept breaking out in premenstrual chin cysts (which only dapsone had ever helped with) and sometimes even on my cheekbones which was new.
So in december - Jan I switched to taz .1 cream besides focusing on my barrier which has done wonders for my skin! The bottom half of my face barely breaks out anymore and I find taz way gentler and never get cystic spots anymore. However about 2 months ago I started breaking out on my forehead (blackheads and red bumps). Now I always have a couple of pimples there and my forehead used to always be pristine since I had the classic lower face hormonal acne. I’ve always applied meds to my forehead anyways bc anti aging. Remembering how well it worked, I got dapsone gel from skinorac and have been applying that twice daily like i used to since 4/1/2024 but I have new forehead pimples daily. The only lifestyle changes I’ve made is eating HEALTHIER 😑
according to Chinese face mapping forehead is GI - I have always struggled with IBS C and gas but never this forehead acne. Anyways I started taking more fermented foods besides my daily probiotic and it actually worsened! So I backed off on those but the pimples are relentless. Not painful and never come to a head. Just a bunch of red bumps and blackheads mostly in the center. No I don’t have bangs or use hair products. I’d wonder if it was fungal but the blackheads make me think not. I just don’t get how if it is acne it’s breaking through taz and dapsone (although I did only start the dapsone 1 month ago but still).
Current routine
AM
Vanicream cleanser
Dapsone 5% gel
Aveeno restore serum
Vanicream daily facial moisturizer
PM
Same but add taz between dapsone and serum
Double cleanse w mineral oil then vanicream cleanser if wearing sunscreen (I’ve used food grade mineral oil for cleansing for years w no issues)
If it was was one of my products it wouldn’t be just my forehead breaking out? So I don’t think it’s that. Also no new products were added before forehead acne appeared. Alsooo my acne usually only occurs during ovulation and before my period but this forehead acne is daily. It’s just ridiculous bc how does my face get better then WHOOP HERES SOME NEW ACNE YOUVE NEVER DEALT WITH

About 2 months ago, I visited a dermatologist for my acne for the first time, who prescribed me dapsone 5% and recommended OTC adapalene 0.1%. At the time I was just using Thayers Blemish clearing salicylic acid toner in the PM as a treatment, which helped with smaller breakouts and enlarged pores, but wasn’t helping with deeper breakouts.
I cut the salicylic out completely, and my acne has improved quite a bit with the dapsone and adapalene instead. However, the enlarged pores have returned on my T-zone, so I’m considering adding salicylic back into my routine.
The dermatologist also recommended using a benzoyl peroxide face wash, but I decided against that, because I used Proactiv in the past and my face got way too sensitive and dry.
Would you recommend trying Benzoyl peroxide again? If yes, what product? Or should I add salicylic back in? Where would I put a new product in this routine? I’m afraid salicylic would react with my other actives no matter where I put it.
AM routine:
CeraVe hydrating facial cleanser Dapsone 5% gel TO Hyaluronic acid 2% + B5 serum FAB Vitamin C 10% serum CeraVe Daily moisturizing lotion Hyalu-Cica water-fit sun serum SPF 50+ PA++++
PM routine:

My skin is otherwise doing well, I am prone to dryness and irritation but that is under control. ALSO — I AM ALLERGIC TO NIACINAMIDE AND FRAGRANCE!

I have no clue what is going on with my skin right now. I am in the midst of a terrible breakout that has been getting worse for months. The last time my skin got like this, I was prescribed accutane. Nothing seems to help and it's exhausting. I've gone to the derm and was prescribed dapsone for hormonal acne. I'm trying to simplify my routine to help my skin get back to "normal," which was very imperfect.
One thing that I think I've noticed is that products with shea make my skin react. Also, the one product that I've been using regularly - while I'm trying to simplify my routine - is Cetaphil Moisturizing Cream. Based on my research about hormonal acne, the B vitamins in Cetaphil Moisturizing Cream could make my acne worse. The Cetaphil Moisturizing also has silicones.
My next thought was to try vanicream. Vanicream has simethicone in it. (It's the 7th ingredient on the list. Not sure if that matters.)
I would love some recommendations for a thick moisturizer without Shea and without silicones. My skin is very dry when it feels like it, oily when it feels like it.

Has anyone had experience with acne treatment using both dapsone and aczone (not at the same time but at different points in time). I ask because 1 1/2 years ago my skin was doing great on Aczone then I stopped for pregnancy. I am now on dapsone, the generic version, and it’s not helping at all, my face keeps breaking out.
Besides the 7.5% dapsone gel, some of the surrounding ingredients differ

Went back for a follow up and now being treated as hormonal acne with dapsone again , spirolactone , sulfur soap & retinol . I haven’t started the spiro yet . Any pointers ?

As stated in the title I’m a 25 year old female and I have been dealing with acne since I was about 21, it was generally mild with some more moderate cysts on my forehead, but last year it started getting worse and spreading to my cheeks and jawline. I tried changing my skincare routine and diet and was only getting worse so I went to a top rated dermatologist in my area for 6 months and only continued to get worse. I was on tretinoin, a topical antibiotic, and topical dapsone. I tried oral dapsone but it makes my blood pressure drop and gives me constant nausea so I can’t take it. It was to the point that my dermatologist said that accutane may be the only solution, but for some reason my liver enzymes are elevated so I can’t even take it (no I don’t drink or do any drugs). My skin was so irritated I backed down to just cleansing (double cleansing), a hydrating toner, and a moisturizer, and it has stayed the same (pretty bad). I’m just at such a loss and I really hate having to leave the house looking like this. Any suggestions please?
