Nursing diagnosis for transportation

I need someone to hear me please.
I feel like I’ve failed my parents. I feel like a failure. I barely went to school this year, completed the bare minimum to graduate. And yet here I am. Preparing to stand next to everyone else who tried. Who deserves their diploma.
My dad died in December of 2023. From those of you who have been on this subreddit long enough you’ll recognize my story. Shorty after, my mom was diagnosed with terminal brain cancer, and has been in a nursing home slowly declining since February. I see her often. But everytime it’s like a stab to the gut. Seeing my mom in this much pain. So out of it, barely speaking a word. Only 47. It breaks my heart. Nobody deserves cancer. But my mom especially. Because she is the most selfless, brave, kind, loving woman I have ever met. She gave up so much in her life and never complained. Went through cancer 3 times. While my father made it all about himself. Worked her butt off for years, being basically the only income in my household while my dad spiraled into depression and alcoholism.
Don’t get it twisted. I’d do anything for my father to come back. I miss him everyday and love him dearly. But that doesn’t excuse the pain he caused me and my family when he was drunk.
Anyways I’m getting off topic.
I was always the prodigy of the family. Taking college classes in 8th grade. All A’s etc*. When my dad died, and my mom got sick. I didn’t go back to school for months. So stuck in grief, and PTSD, and Depression. I didn’t leave my room for weeks, only to eat and go to the bathroom. Eventually, I had to go back. And I almost didn’t graduate. As I rightfully shouldn’t have. But, I had done all my work while gone. My GPA is a 3.4. So the school, thankfully, is letting me walk.
Despite this. I know I don’t deserve it. I’m not the success story they wanted. “Girl graduates after dad’s death and mom’s cancer diagnosis with honors”.
I’m not a success at all.
I don’t have any plans for my future. My dad’s gone, my mom’s dying. I haven’t told anyone the date I graduate. Haven’t set up a party. I don’t deserve their support. I don’t deserve anything.
I’m so lonely. I miss my dad. I wish my mom could see me walk. I wish they were the ones planning my party. I wish they were the ones sending out announcements. I’m not proud of myself. And I feel like they shouldn’t be either.
I’m usually good with words. But when it comes to this. this ugly feeling. I don’t know how to speak it.
Here’s something I wrote about grief. Or how I’ve experienced it everyday these past months. Maybe someone will understand it too.

A gasp of sudden breath, to make sure you're not drowning in it. In reality, it's not the water you should be dreading. That's the easy part. The shock.

There's no sunscreen for grief. No way to stop it, to make it hurt less. You can run from it sure, enjoy the day.
But in the end, you're still sunburnt.”

I posted a week ago about the difficult situation with our father and my sibling and I not knowing what to do. From this subreddit and the dementia subreddit, I received a lot of really helpful guidance.
My sibling and I contacted his PCP and they advised us to call 911 for an ambulance and tell them that he’s no longer safe living with us. We received similar guidance in the subreddits and it was probably the most difficult act of our adult lives thus far. I was in tears on the phone with his PCP’s staff, I was in tears when calling 911, and I could barely get the words out when explaining things to the paramedics.
The two paramedics who responded were absolutely amazing. I explained the situation but became very emotional and one of them proceeded to share that they were going through a similar situation with their parent as well. The other paramedic kept our dad calm and engaged him in talking about art. It was really wonderful to see. They transported him to a closer hospital, which has been significantly better for our father and us. The staff is much kinder, the rooms are nicer and cleaner, and everyone has just been so kind and attentive. He was assigned a case manager the following morning and she’s been helping us since.
As of right now, dad’s still in the hospital and the case worker is trying to place him in a rehabilitation home. A MoCA screening was conducted over the weekend and we’d assumed he’d score somewhere around 15-17, but he scored a 10 so now we’re trying to figure out the next steps. Because of his unusual immigration status (his birth country has a weird treaty with the US), he qualifies for Medicaid but not Medicaid long-term care and we can’t afford the $10k/month for a memory care facility.
We also spoke with his hospital case manager to reiterate how our mother has neglected and been verbally/emotionally abusive of our father. His Medicaid/Medicare applications are in and he’s covered by her insurance through the month of May. So at least we know that all isn’t lost if she has one of her typical knee-jerk reactions and tries to boot him from her insurance. Also, his DPOAs are now on file. No AHCD, mainly because our parents don’t have assets. Our mother hasn’t called to check on him in over a week now, and we don’t expect that she will until after we start notifying his family.
Things are happening so quickly, it’s been hard to process anything or really adjust. We’ve been visiting him twice a day, every day, for the past week. The day visits are rough, but the night visits have been the most difficult. Last night, he was extremely agitated and we had to ask the nurse on duty how to best talk to our father when he’s in that state.
He’s always been such a mellow, quiet, thoughtful man who never swears and is so concerned about the state of the world. But that dad is worlds apart from the person we’ve been visiting and it’s hard to accept that we may never get to have conversations with him like we used to…
So that’s where things are for us now. Everything is scary, sad, and heartbreaking. We have to start notifying his family and I spent a lot of the weekend rehearsing these conversations in my head. They all end with everyone more scared, more sad, and heartbroken. And I know that’s the reality of the situation, but everything just feels so damn bleak.

This is going to be a long one. I just need to write it out so I don’t end up talking to anybody about it IRL (I don’t want many people to know - because I can’t handle anybody saying “but she is your mother” at this point), I just need to write this all down to get it out of my system.
And yup, definitely mommy issues here. I grew up with only her, only child, and she isolated us from everybody else because of her hoarding (and her personality). I feel like I have been living my life like an orphan trying to raise myself, combined with the strange mix of being a child who never felt like a child, who was trying to be the parent for my parent.
I’ve had a lot of… unfortunate events take place in my life, things that when I have told people 5% of it they would say it sounds horrible, that no person should experience things like that (in a variety of categories). But… I am dead set that the one thing that has affected me the most in my life is my mother’s neglect.
She did her best, it wasn’t good enough.
It’s been almost 6 months since I went no contact (well technically very low contact since I have exchanged maybe 5 - 6 text messages in the past half year) with my mother, and it got me thinking what pushed me over the edge to go no contact this time around - considering my mother hasn’t changed that much at her core. Sure, she has gotten older and even more rigid, but she is still my mother, with the same personality traits she always had.
I went to visit her three times last fall. I tried to help her. Be her “parent”. Take her to various doctors after she has neglected her own health for years. Arrange for home help. Clean up the worst of the hoard (like the literal rotting trash). Get the bathroom and kitchen back into a functional state so she can continue to live in her home. Try to clean up the worst parts of how she has neglected herself and her surroundings (her apartment).
That in itself didn’t make me go no contact.
It wasn’t that I came to a hoarded, dirty house that made me cut her out.
It wasn’t that she blatantly lied and said she had cleaned the apartment to bait me into coming visiting in the first place that made me go no contact.
It wasn’t that she refused to accept help from anybody but me until I put my foot down and pretty much forced it through.
It wasn’t even that she was so rude to both nurses and me, always either wanted to have power over someone or being the victim.
It was wasn’t even that she has so little interest in me or anything about me that she has no idea about what is going on in my life.
All of those things are sad, devastating even, but those things didn’t make me say “enough”. I’ve been living with her as my mother for close to 40 years. Of course, all of these things were some of the underlying factors, but I what drove me over the edge was a ridiculous, tiiiiiiiny little thing, tbh. And to explain that tiny little thing, I need to give some back story.
When I moved back from another country I had brought back a blanket from that country that I used on my bed. I used it when I lived with my previous partner in this other country, I used it when I lived alone in that country, and I used it when I moved back to this country and stayed over in the tiny room in her apartment I cleaned out so I had a base to help her with the rest of the apartment.
Because of Covid (and my reluctance to go back into her hoarded apartment), I had not been in that apartment for 4 years (2019 - 2023). In that apartment there was a tiny 6 square meter room that I cleaned up.
When I say “clean up”, it is an understatement. That room had been completely been blocked off because of her hoard from top to bottom from when I was younger. To the point where the door did not open and it was impossible to enter the room. I literally had to pry the door open, and try to pull out item by item until there was enough space to actually open the door.
But I cleaned the entire 6 square meters over the years! I bought a little foldable bed, a pillow and a duvet, some furniture, sorted my stuff in there, and my mother promised that this could be “my room” in her apartment. This was because she really wanted me to come visiting (and she needed my help), and the rest of the apartment is… unsanitary. I had two specific requests for that room - that she would not put any of her hoard in there, and not let the cats in there - since I am allergic. (I can take an allergy tablet and be fine around cats, but I can’t live in an environment with a ton of cat hair.)
When I returned for the first time in 4 years in 2023, the little room I’ve kept clean years prior was covered in cat hair and cat puke. And she has started hoarding there again as well. So she broke that promise - to keep that ONE tiny room cat- and clutter-free. (Technically she also flat out lied and said she has cleaned her apartment so I would come visit in the first place, but I thought that it sounded so far fetched I didn’t believe it in the first place. But the ONE tiny room though, I trusted she could keep that tiny space in her house - and her heart - for me.)
I got both sad and angry when I saw the state of the room. I had to buy a new pillow, and a new duvet, new sheets - which wasn’t covered in… cat piss, puke and hair. But my blanket, my dear blanket… I said that the least she could do was getting it dry cleaned. That was on visit number one last fall.
I even found a place she could hand the blanket in, and we even went there together and made sure they could clean the blanket for like 50 dollars (not a terrible price in this country for that type of blanket).
The blanket was completely covered in cat hair and what I presume to be cat puke (despite her swearing that the cats had never been in that room EVER or on that blanket on the bed EVER. She really took delulu is the new solulu to another level. I really think she believes her own lies). I cleaned off as much haipuke as I could from my blanket. Then I packed it up in a sealed bag, so it was ready to go to the dry cleaners.
Came back a second time, about a month later. She hadn’t taken the blanket to the dry cleaner yet. My itinerary was packed, following her to doctors appointments, setting up home help, the whole shabang.
Came back for the third time last fall, about a month after that again. She still had not taken the blanket to the dry cleaning. It was right where I left it, gathering dust.
So that is what broke me. That god damn blanket is the catalyst to why I went no contact. I haven’t been there since November 2023. (She lives a 7 hr train ride / 45 min flight away).
I will contact her at the end of May, and ask her if the blanket has been dry cleaned.
I am willing to bet a substantial amount of money that it has not. Despite me finding the place she can hand it in to be dry cleaned. Despite me packing the blanket up in a bag for easy transport.
I honestly don’t know where to go from here when I will (most likely) confirm that she hasn’t gotten the blanket dry cleaned. The place to hand it in is literally 5 min away with car, 7 by bus, 25 min to walk. And if she hasn’t been able to do that for me in 6 months - it is baffling to me if she doesn’t understand why I have to limit contact with her?
I know this is such a tiny thing. It’s just a blanket. But this is how every little tiny - and big - thing is with her.
I chose the flair that says “support through advice”, and I guess what I am asking is not how I can change her or her habits - I have lost most hopes she will change - but if someone can tell me if there are other solutions than no contact here? I feel so guilty. She doesn’t have many people in her life. I am an only child. She has no partner. Very little contact with friends. But at the same time I felt enraged whenever I had to communicate with her last fall before I went no contact. I thought maybe the no contact would have her reflect on her behaviors, but clearly this is a moot point. So where do I go from here? I don’t think I can ever forgive myself if she were to pass away and I would have been no contact. But I also feel so bitter and angry just thinking about picking up any contact with her again.

Hi! Not a doc or EMS, but this sub popped up and I figured it was a great place to ask this question.
My husband was transported via ambulance to an ER yesterday for what turned out to be a heart attack and blockage in an artery (had a stent placed, doing fine).
How would I best go about getting the names of the EMTs, nurses, docs, techs, etc who helped him (and me)? That kind of thing doesn't seem to be a thing in his electronic records. We would really like to write a couple thank you notes. He is alive with a great prognosis because of what they did.

I (49m) had my first trip to the urologist a couple of weeks ago, and I'm left with feelings of doubt as to her diagnosis.
I was referred to this Urologist by my GP after I found a lump on my testicle. The GP ordered an ultrasound and referred me to the urologist. Ultrasound found that the lump was a cyst and the pain in my other testicle was from a swollen, likely infected, epididymis.
The urologist visit was about a week later. The nurse took my vitals, asked if I could provide a urine sample, and I couldn't on the spot, so she said we would just get one before I left. The attending Dr asked a few questions and told me that it's common for men my age to have enlarged prostates, so he was going to prescribe flomax. Likely the swollen epididymis was from incomplete voiding due to the enlarged prostate. I asked if he was going to run any tests, and he said no. I asked if we were going to try to find out why it was enlarged, since it's only really been in the last 8 months that I've had the issue. Again, no; men's prostates just enlarge around this age and so, we just treat the symptom. I pressed again about testing and he said that the only really effective test was one that had a catheter in one end and a balloon in the other and it was rather extreme, since we could simply resolve the issue with the flomax. He did indicate that men should start getting PSA tests at 50, and so I will get one of those in 9 months when I turn 50.
He left, then came back with urologist that I booked the appointment with, and she said that she agreed with his diagnosis and felt nothing further was needed. When she directed me to the clerk for checkout I let her know that I wasn't able to provide a urine sample when I arrived, and still needed to do that. She said there was no need, so we would be doing any of that.
Am I wrong in feeling like maybe a root cause should be searched for? It just feels like I've been diagnosed with a case of being old and shitty.

This is just me venting so bear with me here.
I have a few work conferences coming up and I'm very nervous/unhappy with the idea of spending a couple nights away from my baby. He'll be 10 months old, and he's still exclusively breastfed (I pump while at work and he nurses 5 pm to 5 am). I'm also a first time mom and the type of mom that loves to be around her baby. I get that some moms like the time away every now and then, but I genuinely prefer spending time with him over anything else (working or being alone or being with other adults). FWIW I have nights out and I see friends, and I go in to work and whatnot, but if I had a choice, I wouldn't spend this much time away from him (working full time, traveling for conferences).
With the upcoming conferences I have been telling people how I'm nervous, not very excited about being away etc. and I have a child free friend and a family member that keep telling me it's "good for his attachment" for me not to be "always around" so I don't become a "helicopter mom". Wtf? First of all, I'm pretty damn sure it is not "good for his attachment". Second of all, while maybe they think they're being supportive, I think they are just projecting their own feelings on the matter, complelety ignoring what I'm saying, and it's just annoying.
Mind you, the family members sends her kid off for 3 months to spend time with their grandparents and she has all summer "child free". She was like oh I'd love that break if I were you! I'm like sure okay but I already told you I don't want that "break", plus what kind of a break is it when I have to managing pumping and storing milk (tbh I'm probably dumping it instead of stressing about freezing and transport) etc. Not to mention that my baby still wakes up 5-8 times at night to comfort nurse and this is going to be the first time we're separated. I feel like very very few people understand my situation (basically just two new mom friends that I have) and everyone else either wasn't a working mom, didn't breastfeed, or just doesn't at all feel like I do regarding their babies/young kids.
Anyway, rant over. I just wanted to vent without being mean to my friends/family lol.

CONTEXT: I (17F) work in a nursing home as a waitress. I missed two shifts in a row due to a POTS/Cyclic Vomitting flare. I texted my manager to let her know the situation and to make sure she didn't think I was just skipping work. I told about my diagnosis, how they flare without warning, and that I can provide documentation if need be.
The issue is when the May schedule came out a week ago, I wasn't on it. I know I put in my availability on time but I wasn't scheduled for any shifts. I texted her about it, assuming there was a mistake, but she told me she took me off the schedule because of my disability. She said: "I figured that would be easiest for both of us given the issues you have to deal with."
Does this violate ADA?

Patient Info: Male, Caucasian, Mid 60s, Unknown Weight (but probably overweight), Cannabis use, Diabetes (I think), previous spine surgery at base of spine with hardware that has cracked, other medications unknown.
Backstory to current situation: He began having balance issues a couple months ago, and started falling more and more frequently. He then showed occasional confusion, like he didnt quite understand what you said to him but still responded in a related way. The confusion would clear up and he would be fine the majority of the time. Zero neck or arm pain, his lower back where his previous surgery was hurt him a lot due to a broken screw and his knees hurt him (previous knee replacement surgery). Went to the ER one night when family made him due to him being very disoriented and confused. Diagnosed at ER with some sort of issue at the base of his skull that was causing these balance and confusion issues. It was relayed to me as some sort of bone spur or restriction that needed intervention ASAP or the next fall could cause serious damage (if his neck is jolted). Cervical fusion surgery was scheduled pretty quickly fusing 4 vertebrae in his neck.
Current Situation: My Family Member (Male, mid 60s) had a cervical fusion surgery at c3-7 (4 vertebrae) about 1 week ago. First day he woke just fine, seemed happy, wanted to sit up and was doing fine. He is discharged, and over the next days he complains of worsening pain between his shoulders either at the very top of his back/base of neck (second hand info). The pain worsens each day for 3 or so days until he is in such a state that he is grunting, groaning, and banging his fists on things because of the pain. This past Friday he started getting almost violent and seemed incredibly disoriented, delirious, and terrified. He could not talk and did not recognize anyone or seem to understand words spoken to him. He was taken by ambulance to the hospital. Once there, he attacked staff (not like him at all) and seemed like a feral animal or a rabid animal. It took 4 people to get him restrained: and they gave him drugs to paralyze him to keep staff safe, restrained him, and sedated him to the point of being on a ventilator. Blood tests came back with only mild THC as anything out of the ordinary. He likely took his entire 30 day pain killer rx from the surgery (bottle was found empty) within a few days HOWEVER the prescription was weak enough that the doctor remarked that the 'drugs he is currently on is stronger than the whole bottle' so unlikely to be an issue. MRI came back fine, all tests they have run have come back fine (aside from blood tests, MRI, and 24hr seizure monitoring test, I do not know if other tests were preformed or what they would be.)
Each day they try to let the sedation wear off and remove the vent to gauge his reaction, and each day it has been the same nearly immediate intense aggression putting staff at risk. He broke restraints. I was told his eyes rolled back into his head where only white showed during one of these wake up attempts, and that when he does gain alertness he looks absolutely terrified and behaves like a cornered prey. So they just sedate/paralyze/intubate again. At this point the plan is to try waking him each day until he stops being aggressive due to tests not showing a diagnosis. He has been given a feeding tube today.
His surgeon's nurse had been contacted today and she said she does not believe this is related to the surgery and she has never heard of a reaction like this. His current doctors (not affiliated with the surgery) have told his mom and sister that 'they have seen this before' but also 'they don't know what this is', which is odd.
Is there anything else that could be going on, and tests that should be preformed, any idea on why this is happening? I cannot stand the idea of him being in terror over and over waking up to being pulled off a vent then put back on, indefinitely.

I'm a 35 year old woman living in England and have struggled with my MH all of my adult life. I do have a diagnosis of depression, anxiety and PTSD and I have long suspected I have ADHD also.
I've been back and forth to the GP more times than I can count and I was always just told I had depression, anxiety and more recently PTSD. So the last time I saw my GP she told me to make an appointment with the practice Mental Health Nurse which I did but before our appointment she sent me an Autism AQ test and an ADHD ASRSv1.1 screening test. At our appointment we went through my answers in more detail and she carried out a full assessment of my mood and we talked a lot about my childhood linking with my current MH situation.
The outcome of the Autism screening was what I expected in that there aren't any obvious traits right now so I scored low risk, however the ADHD one I scored 5/6 points on part A and 6/12 on part B which she said indicates that I "probably" do have ADHD but that the waiting list on the NHS for a formal diagnosis is around 10 years. So she basically said if I wanted the diagnosis in writing and on my health records to push for it or if I was coping okay and managing then she suggested to just keep going as I am but know myself that it's probably ADHD.
I just didn't think one screening test would be enough to diagnose? Am I wrong?

Hi nurses,
I’m a nurse of 8 yrs and I love my job, but rent in my area is off the charts and the cost of living is just too much. From what I gather cost of living in Boston is less and so is rent. I eventually want to live in a home, but have a reasonable commute.
So tell me the good, bad and the ugly. What hospitals do you recommend working for? What are standard pay and benefits like? What’s your commute like? How is public transportation? I’m coming from New York City so I really have no idea what to expect and I’m scared to do the move without more info.

I am a frustrated nursing instructor who hates the way health assessment is taught in our program. Tons of focus on PRECISE technique without teaching when and why an assessment actually matters. For example, second year students can perfectly demonstrate how to assess pupil response but none can tell you when it makes sense to do this or what an abnormal response even means. Heart sounds are also a big deal with a lot of time spent on landmarking for those. Are full heart sounds something done routinely (like Q shift) in your practice? If yes- what type of unit is it? I worked acute internal medicine and it just was not something we did routinely. Of course we often assessed apical Hpulse deficit [esp in pts with a fib]. I can easily rationalize when doing an apical makes sense vs not to students.
I reviewed an older post on this sub related to the topic of heart sounds but it seemed like people were mixing together the laying of a stethoscope on a chest for ANY reason with actually assessing for heart murmurs/extra sounds. There is a big difference between assessing for rhythm and rate and assessing for SOUNDS. So, in what situations would a new grad be expected to do full heart sounds every shift? What changes are you looking for from shift to shift? Is hearing an S3 really more sensitive for fluid excess in a pt with HF than daily wt/BNP or other assessments? If we already know a pt has a valve disorder like regurg or stenosis...are we able to pick up an acute change to that diagnosis that would require acute intervention? I want to be able to explain when taking the time to assess full heart sounds matters and when it doesn't in real life for a new grad nurse. HELP!

Just want to discuss this, I won't judge your opinion and I'm not trying to start an argument, I just want to see what other people think to help me decide what I should do. Surely I can't be the only one concerned about vaccines?
I'm hopefully going on holiday in September with my parents (so they can look after me). Very relaxed and should be able to get public transport, hire drivers etc. GP surgery has recommended I get 2 vaccines - hepatitis A and typhoid. I know these illnesses can be bad, but hepatitis isn't the end of the world and typhoid can be easily treated with antibiotics + very unlikely to become severely ill once receiving prompt treatment. I haven't had any vaccines since getting ME/CFS. I understand that with ME/CFS, T-cells don't work properly, and I know vaccines activate the T-cells which is the main reason I'm concerned. I know healthcare professionals rarely stay up to date and don't consider these things, they just think "you're not immunocompromised as per blood tests so you must be completely healthy so you should definitely get the vaccines". I also have 4 other linked health conditions. I'm unsure whether it's safe for me to get them at all, whether I get both or not and whether I should space them out. I think I'll get hepatitis A, not so sure about typhoid. I had bad experiences with my previous vaccines for COVID, I felt like I was forced into it but I wasn't comfortable with the risks, they made me feel terrible and they didn't stop me from getting long COVID so they were a waste of time and suffering. Not keen on more vaccines especially because I can't trust what healthcare professionals say and they've done so many unnecessary things that have just made me suffer and don't help at all
Parents think I'm anti-vax just because I'm concerned about the impact on my health and because I'm skeptical of a few vaccines so I can't ask them, they just laugh in my face. I believe in looking at vaccines without bias and I know they often aren't as safe as the NHS tells people. I'm worried these vaccines will make me feel much more unwell long term, and I'll have to spend weeks recovering from each one. I know the typhoid vaccine is only 50% effective and won't protect at all against paratyphoid. I I know the NHS doesn't care about the harm it causes so if something happens I'll be left to suffer alone. I'll ask the nurse when I go to an appointment (not sure when, not booked yet) but I suspect they haven't even considered this and I haven't been officially diagnosed with ME/CFS yet (everyone thinks I have it but won't diagnose 🤔). I need to decide what I want before I go to the appointment as it takes me a long time to make decisions due to my brain fog and I am absolutely not going to let them bully me into doing something I'm not comfortable with. I also know that they get paid for every vaccine they give so it's in their best interests to give as many cost effective vaccines as they can.

Hi, just wondering if anyone’s had experience with something similar.
I didn’t apply for DSA last year or this year because I didn’t think my anxiety was serious enough for it. However, I saw a post on here by someone discussing getting taxi or uber costs paid for instead of using public transport.
A couple of months ago I had quite a serious panic attack which I hadn’t experienced before on the underground. it was rush hour and extremely crowded. Despite the weather being freezing I really suddenly felt myself burning up and thought I was going to faint or die. There were no seats available so I ended up pushing my head against the window of the train to cool dow. I was really terrified, and my heart rate increased a lot really suddenly. The person I was travelling with was so shocked and worried about me
I don’t have an official diagnosis of anxiety, but my GP believes I have it and offered me medication which I chose to decline. I’m just wondering if this would make me eligible for this DSA?
