I (21M) recently broke my nose playing basketball and needed surgery to fix it after doctors claimed it was too late to be reset manually. I had always had a small bump in my nose and a deviated septum, but never bothered to do anything about it. Ever since my nose bone and septum were even further disrupted from the basketball hit, it was much more obviously crooked.
My parents helped me find a really good plastic surgeon in the area, and luckily we were able to afford the surgery. He first met with me to take pictures of my nose from different angles, and to show me a virtual simulation of what I would look like with revisions. All he was doing was re-straightening my nose and septum, but a 3D rendition really showed how different I would look. He sent me home with some drugs and I began to prep for my surgery in 7 days.

Prep

Preparing was very easy, with only a few dietary restrictions. 5 days prior to the surgery, I had to start applying ointment inside my nose, and start taking homeopathic tablets. All was fine.
Wasn't allowed to eat before midnight prior, like any surgery, and on the day of the surgery I came in at 8 AM, very thirsty. I changed into my gown and took a nausea pill and another pill they gave me. Wheeled me into the OR and the anesthesiologist started to administer an IV. Fell asleep shortly after.

Immediate Post Surgery

When I woke up, I felt a bad head pain. I later realized I had my contact lenses in during the surgery, and that was most likely why I woke up in pain. Please don't do the same. The surgery took about 3 hours and I was sleeping for another hour or so. I was very woozy and now had a cast and bandages all over my nose. You're not allowed to mess with any of those bandages, except for the one that catches your nosebleeds, which you change out regularly.
For context, they put about 4 stitches on my septum (columella) and 5 on each outer side of my nostrils. These were the ones they ended up removing 8 days post-op. Usually they're taken out after six to eight days. They also put in around 10 total "dissolvable stitches", as they put it, which are meant to simply dissolve or absorb after anywhere from 2 weeks to 2 months. More on this later.
At 12:30 I was brought home. They gave me Hydrocodone-acet 5-325 for the pain. Even though the nurse encouraged me to try Tylenol as an alternative to avoid nausea and constipation, I already have IBS and the Tylenol wasn't cutting it, so I quickly turned to the opioids. To put it bluntly, the opioids made everything manageable. Even my IBS pains disappeared. The bottle said take 2, twice a day; I took 1, twice a day, and hardly felt any pain while under the effects. As soon as 6 hours passed, though, I could tell the effects were wearing off. By the third night, I barely needed them anymore. Ended up with 4 extra pills. MAKE SURE YOU TAKE OPIOIDS WITH WATER & FOOD!!

Early Recovery

Like I mentioned, the first two days post surgery were very bloody. I was getting up to change my blood-soaked gauze every 30 minutes. I also had no appetite and could not really open my mouth fully to eat. I stuck with soup until Day 3 or 4, when I started to gain an appetite again. The tip of my nose was also completely numb, as well as my upper lip area.

Sleeping

Sleeping is a big issue for some, although my quality of sleep was personally very good.
MAKE SURE YOU HAVE A HUMIDIFIER! I slept well, but I did not wake up well. Before I bought a humidifier (Day 3), I woke up both mornings with a completely crusted shut nose. Despite cleaning it properly (more on this later), the bleeding that happened during my sleep built up and dried up completely. This could be inevitable, but it stopped happening when I started using a humidifier, so I think cleaning it before bed and using the humidifier could be the difference.
Also, I am a side sleeper big time. You CAN NOT side-sleep no matter what during recovery, and you NEED TO HAVE A GOOD NECK PILLOW to assist with this. For me, it helped to use two pillows + the neck pillow so my body knew not to roll around. Keeping your head elevated is the only way to stop incessant bleeding during sleep.

Cleaning Nose

From the day after surgery (Day 2) until the day of cast & stitch removal (Day 8), basically all you do is CLEAN and REST. I sat in bed for hours waiting until I could clean my nose again, since they only said I should do it two times a day. You use a saline rinse to liquefy some of the dried blood, and then use peroxide to quite literally burn it away. The peroxide is a life saver and will break down big chunks of blood. YOU WILL FEEL LIKE THERE IS CRUSTED BLOOD IN YOUR UPPER NOSE. This is how I felt the whole time, and I received many emails from my OR nurse telling me that I should not put my Q-tip further up since there are internal stitches. I learned the hard way by reaching inside with tweezers and tugging at a stitch. Just clear out the inner crest of the nose and use the nasal rinse to try to enable breathing. If there's one thing I learned, it's that you can't expect to properly breathe until the cast comes off. Worth the wait.
Also, using the peroxide will make your skin raw and dry. You must immediately follow it up by running a thin layer of Mupirocin (they will provide) on it. This will not only stop the pain and protect the skin, but it also stops itchiness of stitches and stops cold air from hurting the nostrils.
Cleaning the stitches properly with peroxide will prevent the stitch removal from being too painful. You will know they're clean if all you can see is the little knot of the stitches. Also use the Mupirocin for the stitches.

Icing

This whole time you should also be icing your nose. I failed to do that the first two entire days, and all that resulted was a little more bruising and swelling. It can help to distract from the pain. My doc's strategy was using a rubber glove filled with frozen peas, which can be reused plenty. I just rested the palm on my forehead and let the fingers sorta touch my cheekbones.

Bathing

My doctor didn't let me take a bath until the day after surgery (Day 2), and basically the whole time I was paranoid about getting the cast wet. I took baths frequently, and washed my hair separately by leaning backwards over the bathtub. It's impossible to shower without messing something up. After the cast removal my first shower was super relaxing.

Later Recovery

By Day 6 and 7 the only symptoms are a little bit of pain (I didn't need opioids after Day 3) and a lot of itchiness. Itchiness is of course a good sign, since it means you're healing in those places, but it happens more and more prior to the cast removal. First the stitches started to tingle and itch, and then by Day 7 the bridge of my nose was itching too. I found ice to be SUPER helpful in distracting from the itchiness.
Even at this point, going out into cold air was super painful and my body was definitely not a fan of being outside. I stayed in mostly until Day 8. If you're in a moist, hot place, definitely go outside. Cold and dry is not very pleasant.

Cast & Stitches Removal

On Day 8, I got to the doctor's office and he applied some numbing cream along my stitches. He removed the cast with a bit of pressure on my nose, but nothing terrible. He let me keep the cast. After a bit, he started to cut and remove the stitches. He told me it wouldn't hurt too much since I did a good job cleaning the stitches, but it definitely hurt. Again, definitely worth it given how much trouble they were giving me. After an annoying minute or two, he finally finished. He then clamped my nostrils open and used a suction device to suck out all that gunk in the back of my nose, the stuff I was saying not to touch. It was amazing. It didn't hurt and it felt amazing to breathe at that exact moment. I had never breathed quite properly so it was an instant memory.
All that was left after this was the many (internal) dissolvable stitches that remained, and the complete lack of feeling in the tip of my nose. I asked my doctor if this feeling would go away soon thinking he'd say yes, in a couple days, but he responded saying it would probably take 1 or 2 months. This really bummed me out since the sensation is so uncomfortable, but I'm trying to take it in stride. I also have not been able to smile properly (currently at Day 9) which I am assuming will go away soon. There is also a single dissolvable stitch in the upper bridge of my nose, which I assume was there to fasten the cast to my skin? Not sure.
On the bright side, my nose looks great, and I should gain a good amount of feeling back in it after a couple of weeks. I hear horror stories about having a numb nose for years, but no need to worry about that. The tip of my nose is obviously still pointing upwards, but in a couple of weeks that will also hopefully change.
One thing that bothers me is the fact that I still have at least 6 stitches inside my nose, which I am not sure will disappear anytime soon. The time range on a dissolvable stitch is pretty wide, and it's very difficult to have little spiky things in your nose constantly. As a note, the numbness has seemed to get better after a couple of days, and the stitches are less restrictive. It really helps to make different faces and try to re-adjust your face to scrunching and whatnot. I think this allows the nose to feel back to normal, even if it still has stitches inside.

2-3 Weeks Post-Op

The week following the cast removal was much, much better, although still difficult. My main symptoms through this period were a buildup of nose gunk and obviously numbness. The numbness is very easily ignorable, but breathing gets hard at times when (I assume) stuff latches onto the internal stitches and you need to clean it. I found I can minimize the stuffy nose by using the saline rinse every couple of hours. Also, several of my dissolvable stitches came out less than a week after cast removal, including the stitch on the outside of my nose on the bridge. That being said, it seems a bunch of them are still inside. I am still hesitant to pull on anything or to dig inside it 20 days post-op.
I went out clubbing (pretty stupid) around 2 weeks post-op and got knocked in the nose probably 4 or 5 times during the night. It hurt a good amount, since the nose stays very sensitive until about a month into recovery, but I don't think it did any damage. I would suggest not risking this until around 3 or 4 weeks at earliest.
I also have not been elevating my head properly during sleep, and at times I sleep on my side even though I have heard I am not supposed to. It is also not worth risking.
There is only one real worry I have besides waiting for the numbness to slowly dissipate is that of a nasal valve collapse (thanks to one of the commenters LOL). I have felt like one of my nostrils is smaller, specifically that the upper rim is hanging a bit too low. It is not noticeable at all from outside, and only a little bit annoying. I will be logical and wait until a couple months pass before I make any judgements, and I also trust my surgeon way more than enough to know I am probably just freaking myself out.

Overall Experience

Overall, I would say the surgery was worth it, given the outcome. The prep was nothing, the first 3 days of recovery were hell, but the last days were just a waiting period, and after the cast comes off, you feel much better and can return to anything basically. My surgery was on January 4th and it is now the 13th and I am completely back to normal. Besides the stitches in the nose, which bother me less and less each day.
You will have no issue (and not much pain) at all with this surgery if you are willing to:
- take opioids for 3 days post-op (I did fine with just half dosage)
- have a numb tip of the nose for up to weeks/months after (gets better with time)
- deal with a bit of annoying poking stitches in your nose for a couple weeks after (no pain)

Since I'm only a week post-surgery, I'll keep updating this where it's needed. I'm only going in for a follow up next month, so I'll probably only add updates about the stitches or numbness.

rash for 2 months
I am going bonkers with this rash on both sides of my body. This came out of no where in mid August. THE MOST itchy and burning I’ve ever had ever.
My PCP gave me a oral and topical fungicide and said it is chaffing/yeast. The cream is very sticky, gooey and of no help.
My derm echoed the PCP.
Hydrogen peroxide wipes offers very short-term help and are nice and cool. I accompany HP with roll-on Cortaid (hydrocodone) along with a spray on anti-itch (diphenhydramine and zinc acetate) but would like to get to the root of the matter rather than this bandaid approach.
Cannot bear to wear clothing over it which severely limits my life.
No history of dermatology anything so this is very perplexing.

My wife is having a dental issue. Her dentist sent two rx's (antibiotic + hydrocodone/acet) to our local CVS. Yesterday, she was able to pickup the antibiotic, but was told that the pain medication was on order. Today, CVS' online system today showed that they probably would not be able to obtain the drug. She called the pharmacy and spoke with the pharmacist who confirmed they had not been able to obtain the drug and probably would not be be able to. They suggested we call around to local pharmacies to see who might have the drug in stock.
I was surprised at this. I am a retired pharmacist. Checked the ASHP and FDA drug shortage lists. Hydrorcodone/Acet didn't appear on either. I called the pharmacy. Spoke with the pharmacist (PIC) who told me that was no mechanism for them to order in the drug. Said that they had not been able to obtain the drug for a while. Told me that CVS' systems would only order the drug after the Rx was input and that there was no mechanism for them to order in specific drugs.
I am concerned about this on a few different levels: 1. My wife is pain and there is no good solution for her. The idea that we should call other local pharmacies to see if they have the drug is idiotic(No pharmacy is going to give an honest answer re this). She tried to call the dentist's office for them to send the rx to another pharmacy, but they were gone for the weekend. 2. As a husband, I am wondering why we should be patients with an organization that doesn't care to the point that they can't provide basic care and have common drugs available 3. As a pharmacist, this situation surprises that the heck out of me. The PIC telling me that they can't order drugs to meets patients needs is amazing.
I am going to push this further. Can anyone provide any insight into CVS' ordering system for CS? Also, any other thoughts are perspectives would be appreciated.

My mom has abused pain killers and alcohol for a lot of my life. Last year, she overdosed on tramodol, and my grandfather called the fire department to check on her. In her confusion she fell and suffered a severe spinal cord injury. Fast forward, and she is stable, in an assisted living facility and on prescribed pain medications and muscle relaxers.
This is precarious for me because she is always a little loaded and acts in triggering ways to me. But the reason I post is because she recently told me she wants to self-administer her medications. She says that the nurses bring her the wrong meds, too much, too little, and at the wrong times. She also says that the pharmacy chooses weird times for her to take them and their pharmacy is always messing up the orders.
Now, I know that the medical system is pretty effed and all of that very well could be true. I also know that I’ve heard about “pharmacies messing up prescriptions” my whole life from her. It’s hard to figure out how to handle this situation because she probably does need pain medication, and there probably is some degree of incompetence with the staff, and there is also most likely some degree of addictive behavior going on with my mom.
I asked for her medication list and these are the prescriptions she has. I have no understanding I’d this is a little, a lot, etc. the fact that there are doubles of medication from different doctors is a red flag.

Currently on hydrocodone acet 10 325 6 x day and duration is ridiculous. One does have to sleep you know
Was thinking fentanyl bc I have small fiber neuropathy and digestive system is affected. So thinking bypassing that bc that may be part of problem why meds suck
Is this likely to make me really woozy? Does one usually have break thru meds for day 3?
How do you mentally feel taking it?

My husband, a healthy 24 yo male, became very ill all of a sudden 11 days ago. He works in pool plastering and was in a pool when he became overly hot and faint. Got out of the pool and immediately began chugging cold (not ice cold) water. Later that night, he began to feel cold/chills/slight muscle aches/mild headache, didn’t think anything of it. Mind you, we live in Texas and it gets to 100+ here by mid-afternoon.
The next day was a Friday and he was notably worse. Fever, headache, severe chills, EXCESSIVE sweating, shivering so bad his teeth were chattering. This started about Friday night. He said he felt like he was going to die. Things continued to get worse through Monday. I began rotating him on Tylenol cold/flu and ibuprofen and I figured he’d come down with a cold. Nothing helped break his fevehis headache.
He went to see his PCP on Monday and she chalked it up to dehydration. Negative COVID/flu (rapid + PCR). She called in antibiotics. We started him on Gatorade/pedialyte/coconut water and continued the medical rotation mentioned above. By Tuesday, fever was worse and he didn’t feel that he could withstand going to an urgent care, so I did an urgent care at home kind of thing. They were worried about COVID and said antibiotics wouldn’t do anything, that he was at the end of whatever “this” is. We explained his negative results and the doc had stated that even though he doesn’t have all of the symptoms of COVID, he probably had false negative results and to not be worried, that he sees people have 7-9 false negatives and then a positive.
Wednesday was bad. This man never gets sick. In the 6 years we’ve been together, he’s been sick MAYBE twice, and one of those times was in 2020 when we both had the OG COVID. I knew it wasn’t good when I got home from work and he asked me to take him to the hospital because he felt like he was dying. I took him to a BSW ER and they did a full work up + another COVID/flu test. Everything came back negative, again. His BP when I first brought him in was 155/100. No one was concerned. They started him on IV fluids, IV antibiotics, 1800 mg Tylenol/Ibuprofen. His headache became so bad while we were there, they pushed 25 mcg of Fentanyl and it did nothing. At some point, his nose began GUSHING blood (he experiences nose bleeds at least once a day, but figures it’s because of his line of work). After an hour, they pushed morphine (unsure of dosage) and Zofran, and headache went away. PA came back to tell us everything looked fine and that something was wrong, he just didn’t know what. Though, mind you, when I was able to see all of his blood results the next day (I’m a nurse in primary care), everything was far from fine LOL. PA said we were ready to be discharged and to just go home and continue Tylenol, that they’ll do blood cultures and call back if they find anything.
My husband, after about 30-45 mins, asked the RN if he could get a scan of his brain. He said he knew something was off and it would make him feel better. She went to ask the PA, he came back in, very annoyed and was reluctant to order it. Asked my husband what he thought he’d gain from it and assured him they wouldn’t find anything and it’s unnecessary, but he’d order one anyhow.
After the results came back, the PA asked my husband if he’d ever had a CT of his head before. Answer was no. The PA said this was “purely incidental and unrelated to his symptoms in any form”, but he has a middle cranial fossa arachnoid cyst measuring 2.8 x 3.5 cm - that he doesn’t know much about them so I needed to “Google it”, my husband was fine and could live w it, and to go home and, again, take Tylenol. They’d get us a referral to neuro (never did) and a hydrocodone before he left, along with some prescriptions.
The next day, his PCP was outraged by the lack of urgency provided by the PA at BSW due to his blood work, and urged me to take him to the county hospital (JPS in FTWTX) to hopefully see someone in neuro. At this point, his headache was the only thing bothering him so badly. I took him to JPS and they were concerned at first and called this a complex case. There was a lot of back and forth in the 8 hours we were there. A cancelled CT amongst other things, but finally, neuro wanted an MRI at the least. The MRI showed the same finding, but a 4.6 cm cyst. I’m going to assume this means it is a 2.8 x 3.5 x 4.6 cm arachnoid cyst.
The DO came back and said that it’ll go away on its own, not to worry, and that it wasn’t even touching the brain (???? So it’s just floating? Lol ok). Said we need to schedule an apt with their neurologist. We’re discharged and are desperately trying to get scheduled with someone in the neuro department, but I understand it’s county and it’s a process.
I spoke with BSW nurse on Friday night (day 9) who stated it’s great we haven’t heard anything about the blood cultures, and that it seems like both ERs know what the issue is but they won’t say anything as 1) it’s not life-threatening and 2) they’re not specialists. To just wait and see the neurologist, but if the headaches come back while he’s on this pill cocktail (but-acet-caf), then he needs to be taken back to the ER.
Hopefully I will get through to someone today for the neuro appointment, but I’m worried they’ll write this off too, as did the others. I’ve read a lot about these cysts in medical journals other the last week. 3.0+ cm is dangerous, 1 in 73/1.4% of the population, not much is known, usually congenital and asymptomatic.
As of today, he’s feeling a little better and has gone to work. He took his meds (amoxicillin + headache cocktail), but had another gushing nose bleed this morning. Called me on the drive to work stating he feels unsafe driving. That he feels heavily lethargic and almost drunk, cannot focus. That it’s too much for his brain to think about. My question is, does any of this sound like it could be coming from this cyst? Isn’t this cyst considered large/dangerous? I’m at a loss and so is he. Unsure of how long it’ll take to see a neurologist and he only has preventative insurance, so we can’t go to just anyone. I’d also like to add that his balance is off and he’s not making sense at times. Asking the same questions over and over, simple questions. Or thinking he asked something when he only did so in his head. Amongst other things that aren’t logical.
What do we do if the county neuro writes this off as nothing, if it is in fact, something? Any input will help. TIA.
TLDR: my husband has a middle cranial fossa arachnoid cyst measuring 2.8 x 3.5 x 4.6 cm. He’s very ill, no one thinks it’s of any relevance. Please help.
Update: Scheduled for July 15 at county’s neuro facility at 7:45 am for 15 mins as a “follow-up” appointment with a surgical APRN. Unsure how to feel about this.

I need to create a way to look up patients by medication. The data (much simplified) has 2 columns with column A being the patient ID number (1-20 in this example) and column B being the semicolon delimited list of medications that the patient is taking.
Medication descriptions sometimes contain commas. Medication dosages need not be included.
What would be the best way to help an end user look up patients by medication? (There are approximately 14,000 rows of patients in the spreadsheet that I am working with.)

Healthy diet and regular exercise. Normal vitals. Happy and relatively normal. Negative for Lyme's. No preexisting conditions.
Meds: Naproxen 500mg 3x/day, Hydrocodone/acet 10/325 prn., cyclobenzaprine 10mg prn.,

TL;DR:
Double vision and pain in left eye and slight loss of vision making driving and even walking nearly impossible unless I close my left eye. Vision normal upon closing left eye. After several months, symptoms resolved and since then I get intermittent double vision and pain in the head and left eye.
10 months ago I noticed I had lost feeling in the toes of both feet. This has not improved and has gotten worse on left foot.
Back pain and spasticity in legs, butt and back lasting for months, now. Abnormal reflexes.

Symptoms: Numbness in toes both sides. Pins and needles episodically in left foot and ankle. Double vision and pain in left eye. Double vision gone if I close one eye. Intractable back pain and spasticity in buttocks and upper thighs and sacrum area and lower abdominals. One episode of dramatic spasms in abdominal musculature, and thighs, lurching me forward.
I want to be as detailed as possible because my family and I are at the end of our ropes.
OCT 15(ish) 2019: back pain caused by spasticity, mild and constant,
NOV11: back pain and spasticity becomes overwhelming and nearly paralyzing. Sciatica in both legs. Spasticity in abdominal musculature. Barely able to get to ED by being mostly dragged by my wonderful wife. Admitted for 4 days, lumbar MRI done. ED doctor says that MRI shows nothing that would cause this level of pain. Discharged with many narcotics and steroids and a NSAID. Since have stopped taking most of them.
NOV20: Began physical therapy. Continued until FEB28 with very little progress.
NOV 23: Osteopath performs epidural steroid injection at L4/L5. This specialist repeatedly declares the MRI as being "unimpressive" and really only targets this area because there might be a mild issue, possibly fluid leaking but nothing definitive. Procedure provided some relief.
DEC15: begin feeling much better but not well.
DEC 20: second epidural steroid injection at L4/L5. minimal relief.
FEB04: seeking second opinion at a highly regarded spine specialist. Spine and pelvis xrays performed. His message was that there is absolutely nothing structurally wrong with me. My MRI looks better than most people my age. He is the first doctor to think that my numb toes (present for nearly a year) and my intermittent double vision(present since 2016) are related and says with comfidence that he thinks these things are related and indicative of a systemic issue-autoimmune, infection, etc etc. Rheumatologist scheduled, neurologist scheduled.
FEB20: dramatic downturn in condition. spasticity in back and butt and abs return. Double vision returns. This is when the episode of dramatic spasms in abdominals happens and lasts for about 12 days.
FEB22: On a whim, physical therapist checks reflexes: abnomal results indicitive of upper motor neuron damage: clonus in left ankle, positive babinski, possibly positive lehmerties(?), unable to walk on heals (left foot toe slap), hyperreflexia in right knee, no relfexes in left knee.
FEB28: discharged from physical therapy, little progress made.
MAR08: begin improving slowly for no apparent reason at all.
MAR10: visit with rheumatologist. He suggests that I might have a disk issue. We (wife and I) explain history with spine specialists and that structural issues seem to be ruled out, that in fact, I was referred to him by a spine specialist. Notes reflex abnormalities, thinks eye issues are unrelated, orders blood work and pelvic xrays. I leave there yesterday with a recommendation to take naproxen instead of Ibuprfen and to make an appointment with a spine specialist. Needless to say, this was a very frustrating appointment.
I am still dealing with debilitating pain, I am taking NSAIDs every day, the flexeril side effects and the opioid dangers are horrendous. I am unable to stand for longer than 45 minutes or so, spasticity in abs cause me to hunch forward slightly.
We need suggestions. I am improving little by little over the past few days and have no explanation.

I leave there yesterday with a recommendation to take naproxen instead of Ibuprfen and to make an appointment with a spine specialist. Needless to say, this was a very frustrating appointment.
I am still dealing with debilitating pain, I am taking NSAIDs every day, the flexeril side effects and the opiod dangers are horrendous. I am unable to stand for longer than 45 minutes or so, spasticity in abs cause me to hunch forward slightly.

We need suggestions. I am improving little by little over the past few days and have no explanation.
DOES ANYONE HAVE ANY SUGGESTIONS ABOUT CONDITIONS TO RESEARCH OR LOOK INTO?
