Explant and lift of 400 mL Mentor silicone. Today has been rough, and I have been having sneezing fits lol, which are very painful!! Zyrtec has not helped.
I honestly have a lot of mixed feelings so far. It took me 6 years to choose a surgeon and I traveled out of state. I had an overall good experience, and my nurses and the support staff was all wonderful, however, I wish my surgeon had taken more time to explain things to me as far as how things will look after and what she will do. I felt like everything was so rushed and I was given so much great pre and post op info, but not actual info about my procedure.
I have a list of questions to ask my surgeon on Monday. I haven't seen my breasts yet and am all wrapped up and I can't help but worry. I noticed that her markings were very high nipples on the OR and I asked about this and she said she would make them a little lower as I had stretch marks on the upper part of my breasts and I didn't want the areola to be in the stretch marks...I just feel like that stuff should have been talked about first. Not as I was about to get put to sleep! It just moved way too fast.
I'm grateful it's over, and just hoping that that is just her style and she has such beautiful before and after photos and many happy patients on the support groups so I'm trying to let go and say what's done is done and trust that she used her expertise to do the best job on me as possible and that I will be happy with the outcome.
I'm draining a lot on my first day and the pain and pressure is a lot. I know the first few days are usually the worst and I'm only 32 hours out from surgery. Happy to share any info and wishing health and fast healing to all the ladies going through this with me!!

In your opinion, which U.S. surgeons have the best record on corrective gynecomastia surgery for grade 3 and grade 4 gynecomastia cases. Often, the bigger those tatas get, the more challenging the case. It seems grade 3 cases are more borderline (meaning some of them receive the gland/tissue removal + lipo and some require the full mastectomy, skin tightening and nipple grafting.) When I see a surgeon's before & after photos are almost entirely grade 1 and 2 cases, I have to wonder how they do with the bigger ones. I would define 'best' record as...

• the most transformative before/afters
• with the best-case scenario on scarring (and, yes, I know, the patient has a role in ensuring their scarring heals as best as possible -- not to mention age, quality and elasticity of their skin are all factors here.)
• The least amount of 'problematic' results (ex: remaining gynecomastia, concave/caved in chests or areolas, that "balloon knot" appearances around the areolas or places of 'relocated' / tightened skin)

On the evening's 30/4/24- 1/5/24, I was absolutely horrified and left in disgust at the behaviour of one of your creators on tiktok live. Elphaba Orion Doherty, who also uses other accounts to side step account bans, displayed extremely horrifying behaviour that should never be witnessed on the app.
Thousands of people, potentially even many more thousands after the initial incident, were subjected to dangerous suicide baiting behaviour and violent content by Elphaba Orion Doherty. She held a knife to her throat and threatened to kill herself to thousands upon thousands of viewers, whilst appearing to be very unstable after an incident the previous evening of claiming she had self harmed. On the day of the 1/5/24, she was even showing her viewers, who again were in the thousands her self harm "injuries" on her wrist and arm, saying that she would "show them" to her viewers so she could manipulate her audience for more gifts.
Elphaba Orion Doherty was allowed, despite thousands and thousands and thousands of reports for online safety due to the content of brandishing a knife to her own throat, threatening harm to herself and saying she "wants to die" These reports were all received with no violations and the live was allowed to continue for over twenty minutes.
Children under the age of sixteen years of age have self harmed as a result of this. Adults who experience mental health battles have harmed themselves as a result of this. Families and friends of those who have committed suicide have been affected by this.
This creator has a track record of inflammatory behaviour on the app, which again has been reported on numerous occasions because of the very real risk to children who do have access to the app. Not only just children and impressionable young people, but also the vupnerable adults and everhone else who is subjected to this behaviour. The content of this creator suicide baiting, threatening to kill themselves, actively abusing alcohol and narcotics whilst on live is completely unacceptable. If she isn't displaying the above-mentioned behaviour, then she is demanding gifts and money from her viewers, literally demanding.
Due to Elphaba being a trans creator and also young (20 years of age) She is very attractive to the younger generations, the target audience being children as a large percentage of her gifters and viewers are children. She already has an extremely negative influence on these children, with her regular intoxicated ramblings, narcotic induced tantrums, and now suicide baiting and self harming while on live, is a psychological and emotional minefield for children. Elphaba needs IP bans and enforced bans from making other accounts for the safety and well-being of so many people who use the app and platform for welcomed, creative purposes.
On a daily basis, Elphaba breaks the platforms own policies. Please see below. And I am using general terms in referring to youths 13+ who are legally allowed to have accounts and access to the platform.

• Threatening behaviour with an offensive weapon - This can be prosecuted under The Offensive Weapons Act 2019, Threatening with an offensive weapon etc in a private place – Section 52.
•Threatening behaviour to other creators - This can be prosecuted under the Malicious Communication Act 1988 and the Communications Act 2003. Online threats could take many forms including threats to kill, harm or to commit an offence against a person, group of people or organisation
• Talking sexually to Minors - This can be prosecuted under Section 67 of the Serious Crime Act 2015, Section 15A. Furthermore, it can be prosecuted under Protection of Children Act 1978 if "sexting" occurs towards a minor. To also add that if the offender is under the age of 16, the following legislation can be pursued Protection of Children Act 1978.
• Obtaining money through tiktok gifts and Go fund me under the representation it for transitioning surgery - This can be prosecuted under The Fraud Act 2006, a,b and C, being false representation and misinformation. Other charges under the same bracket can be made due to the nature of the offences.
• Refund fraud, ie ordering food and returning it for monetary gain and at the loss of another - The above act also covers this.
• Fraud committed through the use of computer or mobile, technology devices including using the above mentioned methods to further commit crimes - This is prosecuted under Computer Misuse Act 1990.
• Blackmailing or threatening particular actions or behaviour that is to force someone to do a specific thing - This is prosecuted under The criminal offence of blackmail under the section 21 of the Theft Act 1968.

Hi SCAddicts! My wedding is in October and I’m trying to improve hydration and texture before then. Currently ISO a serum to use after cleansing but before moisturizing prior to the rest of my bridal makeup.
Skin History: Three years ago I moved back to the US and developed a random eye irritation that made my lids puffy and itchy. I had some small dry itchy patches also pop up near my lip line, hand, inner elbow, areola, and ear (??) and had several derms do patch testing for allergies only to write it off as eczema. My current derm has classed it at some type of rosacea. I’m on 50mg/day of doxycycline, which eliminated it. However, I’m finding I often have mild irritation in response to a lot of products. It’s often not a blatant allergic reaction, but develops after using a product for 2-3 days. It’s been hard for me to pinpoint exactly what ingredient(s) are causing it. I basically stopped all skincare when this issue started and have been slowly starting to work back into a routine.
Skin Type: I’m in my mid 30s with pretty clear, albeit sensitive skin. I have some fine lines, especially in my under eye area when makeup is applied, and err more dry than oily. Generally speaking I’d say I have combination but decently balanced skin. Photo here.

I’ve tried the Kiehl’s Avodaco Undereye Cream and The Ordinary’s caffeine serum and multi-peptide eye serum and was allergic to something in them. Besides the normal “I hate their size or look” I just hate the sensation of their existence. I can’t control them like a limb but I have extra nerves there I guess bc it’s so sensitive all the time! I only have a B cup right now (I lost a lot of weight and they seem deflated to me but I’ve always struggled with body dysmorphia anyway) but they feel like the weight is always flinging around. I hate wearing bras or bralettes most of the time but also can’t stand my breasts anymore.
Since I was young and they were growing I had a weird ass fantasy of cutting them off. On tumblr I saved a lot of artsy photos of women who had mastectomies or had like the nipples removed but cool tattoos on them. I just became obsessed with not having nipples or areolas or even boobs.
Sometimes though I really feel as if the media and societal standards have truly conditioned me because I also find myself heartbroken that I don’t have perky, smooth, tiny pink nippled, big breasts. I just don’t feel womanly enough with my current breasts, but don’t think I could ever go through with changing them.
I’ve been trying to teach myself body positivity through exercise and positive self talk. I’ve always been insecure but never been too crazy about my appearance in the sense that I’d go through great lengths to change it. I’ve started learning how to use makeup and the first time I walked out with some lipstick on (I did it right and watched hella tutorials) pretty much everyone had a negative remark for me.
I’m sure a lot of y’all relate. Does anyone have tips for how to deal with these confusing feelings around my breasts and body as a woman in general? I’m 25 and thought these feelings would go away after puberty or once my frontal cortex developed 😭 but they aren’t. All advice and anecdotes are appreciated!

I’m trying to decide on an areola size and am having trouble finding photos which indicate actual sizes, like 35mm, 38mm, 40mm etc. Actual pictures of the breasts so I can see proportions. Searched this sub and found some but looking for more. Any ideas?

Hey guys! My first post here, great to be a part of this community. I have a question regarding lighting the subject from behind. I took a photo of a Young girl, and I lit her with classic loop and also put a light behind her (and towards her) to make her pop more out of the photo and separate her from the background. Now, personally I'm satisfied with the results, but my friends keep pointing out to me that the light behind her makes some of her hair lit up in a way that it forms "unfinished" areola around her head. They're saying this looks weird and ruins the photo...
My question is: is this an objective mistake? I keep thinking about it and cannot really make up my mind. I'd upload a photo if I could, so this description will have suffice.

My wife wants to enhance her breasts (45yo) and we looked together at some photos. We are confused by some of the scars and sometimes the lack of them.

1. The scar that goes from the bottom of the nipple/areola downwards… what surgery causes that?
   
2. Sometimes there doesnt seem to be a small scar (1-2”) under the breast where we assume the implant goes in. Is that just photoshop or is there another surgery to make that happen?
   

If theres a “breast augmentation 101” video or something on this, we are glad to do the reading. We tried googling this stuff but i think we are using the wrong terms or something.

I wanted to get through the major stages of healing and all that before I posted my amazing experience with Dr. Nazarian in the Jefferson Health system. As far as I know, she is the only top surgeon in South Jersey. There was hardly any information about her on Reddit or the Facebook group, so I hope this helps you in your journey! I got surgery at 24 years old after being on T for over a year.
Jefferson has its own LGBTQ+ primary care office that has been a godsend. Dr. Nazarian is originally a breast cancer surgeon, but has since dedicated nearly all of her practice to trans and non-binary top surgeries. My first encounter with Dr. Nazarian was when she gave a webinaQ&A about top surgery as an LGBTQ+ community event at the end of 2022. She didn’t have any strict rules for obtaining surgery such as length of time on T (Sorry, but I don’t remember if she operates on minors). Coincidentally, she was one of six top surgeons covered by my insurance (AmeriHealth Administrators). To my surprise, she said that they could get people scheduled pretty quickly. I hadn’t formally dove into planning for surgery yet, so I scheduled a consultation to confirm what type of surgery I qualified for and feel her out. The soonest appointment was about 4-6 weeks later since she only does consultations on certain days of the week.
January 2023 rolled around and I prepared extensively for the appointment. My consultation and everything thereafter was at the Jefferson Cherry Hill Hospital in the Breast/Urology office. The staff members were extremely kind and always used my preferred name despite it not being my legal name. I’m cis-passing so on occasion I did get asked if I was there for Urology lol. Most of the patients in the waiting room were cis, but I saw a good share of trans folks.
I got called back to one of the exam rooms and somebody probably came in and asked me some questions, but I just remember sitting with Dr. Nazarian in the regular chairs. Dr. Nazarian is an extremely intelligent, very chill person. She is calm and cool all the time and pretty conversational. The first thing she asked me was about how I got here, and she listened to my journey of accepting my trans identity and how T has been. It was really nice of her to do that, and she made me feel smart and empowered. I mentioned how I would probably need double incision, so she asked if she could examine my chest. I sat up on the exam chair. It honestly took longer for her to ask for each stage of consent (is it okay to see me shirtless/make a few light touches) than it did for her to know I definitely needed double incision. I appreciated not having to have my shirt off for more than a couple of minutes.
Now that my procedure type was confirmed, we talked about what that looks like. She likes to sketch to help visualize the process. She carves around the pectoral muscle and up into the armpit. This helps prevent dog ears and accentuate the pecs. The scars would basically look like this on each side ﹂. She explained that it is statistically rare for free nipple grafts to fail and that she hasn’t had it happen to any patients yet. She explained that a “cookie cutter” is used to size the nipples. They are then set aside in saline. They are stretched to reduce the projection of the areola and then added to a “cookie cut” spot on the chest. Bolsters are sewed on to help them “set.” When I asked, she said that my scars would not need to connect in the middle because my breasts did not connect to make cleavage.
I made a list of questions and brought photographs of some trans and cis chests to better explain what I liked and didn’t like. I am very short and husky, so I tried to find bodies that matched mine to stay realistic about what top surgery could do for my body. I was overly cautious and suggested that maybe I should leave some fat to ensure my body was proportional. She said that wouldn’t be necessary at all; we can easily go all the way flat (she was right - if I hadn’t gone completely flat, I would’ve had breasts). She said that I could send her photos leading up to the surgery to help show what I want and don’t want. She said that she would prescribe me a scopolamine patch to put behind my ear the night before surgery since I was in a demographic that tends to get nauseous after getting anesthesia.
She answered all of my questions and dispelled many of my aesthetic fears when it came to surgery. However, she did not give advice on scar care. She felt that it was all too variable to be able to recommend anything, but she mentioned that silicone and staying out of the sun have the most evidence of fading scars. She does not keep a portfolio of her work because she feels it’s more impactful to talk to previous patients. She connected me with eight previous patients and they were all happy to answer my questions and send photos of themselves. My only criticism of this was that none of them were over one year post-op, and I wanted to see mostly-healed results.
Dr. Nazarian was an amazing professional during this consultation and in all our interactions. She listened intently to every question and follow-up question I had and didn’t rush me for one second. She ended the appointment by encouraging me to take my time and look into other surgeons before scheduling anything.
The next day, I sat down and scoured Reddit for results from the six top surgeons my insurance deemed worthy of their funds. Only one surgeon in the RWJ system had results that I would want for myself. When I called to schedule a consultation, they were booking nine months out. FOR JUST A CONSULTATION. I scheduled it just in case things went to shit, but I was happy with Dr. Nazarian and went ahead with booking surgery. Jefferson scheduled my surgery date for one month later. What the FUCK? Absolutely insane, God was looking out for me for real. The woman who works with the insurance companies is a sweetheart. I emailed her my therapist’s WPATH letter (I saw this therapist for over a year and she drafted it up a few months before this just so that I had it) and she got to work. She said it would take up to two weeks to get a decision. In the meantime, I had a special telehealth call with one of the nurses and she walked me through the logistics/preparations necessary for the day of surgery and recovery process. I also had a special telehealth call with Dr. Nazarian to provide input for my aesthetic (mainly nipple) desires in extreme detail along with some questions. I also needed to get some bloodwork done.
Sure enough, I didn’t get my prior authorization decision as scheduled. I had already called and knew what codes were covered, but all I knew was that it was pending. I got the call that my surgery was fully covered and that my surgery in two weeks could go forward. That was an incredibly stressful time, and I had no idea how much I would be paying, so I was just socking money away. I paid $0 instead of the quoted $90,000. I hate this country.
I got surgery first thing in the morning as scheduled. I talked to the insurance people, went to the surgery wing, stripped to a gown, and hung out with my mom. The literal worst part was getting the IV inserted, and that took three people to successfully do it, but then we were smooth sailing! Dr. Nazarian came in and we chatted a bit and she answered a few lingering questions. She sketched on me a bit, and then I told her to please just make my chest look like it belongs on my body. Even if it looks weird at first, I’ll trust the process. Anesthesiologist came in and had me affirm that it would be okay if he knocked my teeth out.
I started to get some fluids flowing. It was really happening. I got wheeled through the surgery wing. I got to the OR and shimmied from my bed to the operating table. They put some compression socks on my legs and then said they were putting something in my IV to help me relax.
Then I groggily woke up in the recovery wing and proceeded to doze off and wake up ten more times. I looked down and my chest was wrapped in an Ace bandage. It happened!!! I said that I had to use the bathroom but the nurse said that I would have to use a bedpan. I said never mind and went back to sleep. Eventually, my family came in, and I got to nibble on some lemon cookies. My mom was very confused and said they definitely weren’t lemon (We got to take some home and they were in fact plain shortbread). My mom helped me to the bathroom and I felt like the most fragile flower. I was able to wipe but it was all very tricky with the IV pole. When I was getting ready to be discharged, a nurse explained how to keep track of the amount of fluids coming out of my BD drains and how to empty them. I was not prescribed narcotics, but if I truly needed them, they would give them to me. Pretty hilarious that I got oxy at 16 for my wisdom teeth but not when I literally got cut in half. I was on a rotation of ibuprofen and extra-strength Tylenol every four hours. I was then wheelchaired out and on my way!
Once home (after a quick pit stop to pee at a diner cuz Jersey), I then set up shop in bed. It took some time to figure out how to climb into my tall bed without my arms. I needed to have a step stool next to my bed and then “walk” with my knees, then bring my knees out from under me and slowly collapse backwards onto my backrest pillow. Besides a backrest pillow, you need a mastectomy pillow. I used mine for three months. It truly elevates your ability to just exist comfortably, especially when the drains are still in. I also had a water cup with a straw and cough drops on a tray next to me. The first day I was slightly nauseous but not bad. I watched TV all day and would get up to eat meals in the dining room. I would also freak out and worry about my results and if I was messing them up. The hospital called to check on me.
A week later, I went back to the hospital to get my drains out! Thank god. Not painful, just unpleasant and fickle. Getting them out was not painful for me. I didn’t feel anything except a mild pull. I was shocked that it was as quick as it was. My drains were lower on my ribs and not in my incision and these spots didn’t scar. Rewinding a bit, they obviously had to unwrap my bandage to do this. Once the bandage came off, I felt like I didn’t have enough air in my chest. I wasn’t at risk of passing out, but they laid me back anyway. Dr. Nazarian cut the stitches off my nipple bolsters, and my nipples were still alive!! It’s the little things. She taught me how to put bacitracin on them and bandage them with a non-stick pad and paper tape (for the uninitiated: a wet-heal). I did this for about six weeks until they stopped getting crusty.
First-impressions! So flat and oh my God, the pec contour was insane! Expertly done, really happy with the scar shape. I had little dog ears in the center just from how I healed. I didn’t have any swelling or bruising! It was insane that I didn’t have to continue to wear the bandage/binder after the first week! My nipples looked way more centered and larger than I expected, especially when I stressed that I wanted them to look small and not too centered. I had to reckon with this, but they healed great. Overall, it was shocking to see them because they were so dark. If I had done my own surgery, I probably would’ve moved them further off to the sides, but I do think she listened to my deep desire to have a chest that matched my body. I’m husky and this is how husky looks. When I look down, they are perfectly placed. I guess, as with any body, there are some angles that seem a bit off.
And then the long road to healing began - with a shower! After another week of doing nothing, I started to regain my independence. I was unemployed at the time, so I was able to take my time with my body. At my two week follow-up appointment, I got the mesh taken off my incisions. That sensation of rubbing the glue off that numb area was truly terrible. I was probably cleared to do the exercise bike at this point. I got a ton of itty bitty scabs along my incisions, and at my 2.5 month follow-up appointment, Dr. Nazarian was just able to brush them off to leave a beautiful, in-tact scar. With this, I was able to start scar care! I massaged daily with silicone lube until I hit one year post-op, missing a few days here and there in the last three months of that year. I wore silicone scar tape 24/7 except when massaging and showering. I was cleared for basic exercise too! I chose not to lift my arms over my head for three months, but the hospital encouraged I start raising them slowly. I had one more follow-up appointment after five months post-op where I was cleared for all activity.
It’s getting really late now so I gotta wrap this up. I am so grateful to feel so full and free in my body. I’ll recap my healing over the last year: the mini dog ears in the middle didn’t go away, and I could technically get it revised, but they’re not crazy or abnormal-looking and kind of add to my pecs. I had a little tail on one of my nipples and it faded to nothing! Really happy about that. In terms of my nipple size, it definitely got easier to love them once they stopped being an angry, dark red color and softened to a light pink. They are undeniably male-sized. My one nipple stretched upwards a little bit, probably because that’s my dominant side, but that’s just how I healed. It’s perfectly normal looking if you don’t analyze it. My girlfriend’s nipples are ever so slightly different sizes and she’s au natural. I’m especially happy that the borders of my areolas pretty much faded into my skin instead of looking like they were pasted on top. I’m also glad that I don’t have a lot of projection. I lacked sensation to begin with so this wasn’t a priority for me, but I was surprised that both my nipple nub and areolas react to cold! That was great. Looking back, it’s actually crazy to see how dark and deep purple my scars were. Today, they are practically white. I feel very blessed. When I look down, I don’t even see scars because my pecs cover them. It’s incredible. My scars stretched the most under my armpit, but the thickest part is probably a quarter-inch if that.
When I first started looking into top surgery, I didn’t want to go to a breast cancer surgeon because I thought their experience would inhibit the masculinization I was looking for. Dr. Nazarian is truly an artist and professional who really cares about our community. I’m very grateful that she lends her expertise to providing these life-giving surgeries. I feel like I hit the jackpot with my results and lack of delays. Happy to answer any questions, but I can’t imagine there are any left after all this lol.

Recent symptoms- sudden breast enlargement in left breast. It grew 2x its size over night. I have a photo if needed. The nipple on my left breast areola also grew inside and my nipple is pointing more upwards then before. I’m having dull aching pains constantly in my left breast significantly more on my lower portion of my breast. When I touch or wear a bra I’ll get constant shooting sharp pains in my breast. My breast also feels more firm then usual and the skin is more textured. I went to a walk in doctor (mines currently away until mid March) and was prescribed antibiotics which I’ve been taking still with no changes in breast size, pain or texture. (Been taking now for 7 days). I had an ultra sound done and this is my results.
Female 27 . Weight 135lb . No smoking/ drinking/ drug use or vaping . History In 2015 I had a suspicious lump biopsy and removed in my right breast and armpit. But came back benign.
Here is what the ultra sound report says
DIAGNOSTIC LEFT BREAST ULTRASOUND HISTORY: Palpable abnormality around the 7-8 o'clock position. FINDINGS: Scanning through the area of concern shows a 2.5 x 1.4 x 1.0 cm isoechoic area of glandular tissue. This does not have the appearance of a focal mass. There is no concerning internal color flow. No adverse changes are detected in the area when compared to April 26, 2022. At the 10 o'clock 3B position there is a 1.2 x 1.0 x 0.6 cm hypochoic mass. This is smoothly marginated and wider than it is tall. This has no internal color flow. This probabiv represents a ribroadenoma IMPRESSION: Presumed fibroadenoma at the 10 o'clock position of the left breast. Prominent glandular tissue at the 7-8 o'clock position in the area of patients reported concern. No suspicious abnormalities detected. Follow up in 6 months is recommended to ensure ongoing stability in the shon-term. This is a BI-RADS category 3, probable benign findings.
