Ds 3053 fillable

Technical problem with ecrba

2024.04.21 07:03 Comfortable_Note_651 Technical problem with ecrba

I'm having trouble with the ecrba application.
I tried uploading a ds-5507 because I'm not going to be there. But my partner will. We're both American.
Can I just do a paper application and get it notarized?
Will the us consulate accept foreign notaries for ds5507 and Ds 3053? This is a western country in Europe.
Also we aren't married. Do I need to sign the section for us citizens out of wedlock births?

2024.04.11 12:55 FlamingoAlert7032 Signing my daughters DS-11 before submitting at CRBA interview?

I will not attend my infant daughters CRBA embassy interview as I'm still in the states (USA). I will mail my notarized DS-3053 and a copy of the DS-11 to my wife along with a ton of other paperwork for the CRBA.
Instructions note to only sign the DS-11 in front of an official such as a consular officer. But I won't be there so I will have to sign it, right? Worst that can happen I guess is I send one form signed and another unsigned but I'd like to know so my wife will be prepared.

2024.04.09 23:51 Wooden-Ad-7576 Non US Citizen online notary

Can my husband use an online notary to fill out a ds-3053 consent form for minor US passport for our newborn son? My husband doesn't have an ssn have not never been to the USA. I see some online notaries but most of them require a ssn. Can anyone point me to reputable online notaries that can be used for our situation. Thank you.

2024.03.27 10:21 Henchbish Does photo ID photocopy submitted with notarized Form DS-3053 also need itself to be notarized?

A 12-year-old child has an appointment to apply for a US passport at a US consulate overseas several days from now. One non-cooperative parent returned to the US, leaving the child and other parent behind. This parent was persuaded to provide DS-3053 and ID photocopies and send it by an express international mailing service.
Just a few days before the appointment, ACS sent an instruction email for the appointment, which mentions "[The absent parent] must also provide a notarized copy of the front and back side of the photo ID that was used during notarization." This appears to differ from the form DS-3053 instructions, which state: "You must submit a clear photocopy of the front and back of the identification you presented to the notary" (nothing about the photocopies themselves needing notarization).
Can anyone confirm that the photocopies of the ID need their own notarization?

2024.03.24 04:35 Vistandsforvicious Passport application question, please help!

Passport application question, please help!
So I've had a passport issued under the age of 16 about 15 years ago. I've lost this expired passport and I'm not sure how to fill out this section of the DS-11 form. What should I put for passport book numbeissue date. And which status box do I need to check because its not just lost, but it's lost and expired!
Thank you in advance for anyone that can help!

Hello Have an appointment scheduled for my child to do passport but her mom is unable to attend. I got the DS-3053 completed and notarized, but do I need to have a photo ID of her mom when we go to USPS?

2024.03.08 21:00 Heart-New DS-3053

Any ideas on how my husband properly fills out his address on the DS-3053 form for our daughters passport, when he lives outside of the US? (no states, zip codes, different country ect) is it okay to leave blank or is there something he's supposed to put there?

2024.03.06 16:46 Rude-Glove6976 What does it mean when DS- forms expire? How can I get the latest form before the current ones expire?

If you look on the top right of the DS-5507 and DS-3053 there is an expiration date of 3/31/2024
The problem is I have to fill out these forms and then mail them overseas. But it's possible that by the time I have the necessary documentation to mail them, they may expire within a week. I may also be in another country by then and I'm stuck. Then what???
Is there a way to get an "updated" version of these forms with a later expiration date without having to wait for these ones to expire first?

2024.03.06 01:05 Zyr47 [BX ADnD OSE][Online][Saturdays 530pm CST Weekly][Dungeoncrawl Emergent World Focused]

I'm running a game of Old School Essentials, which if you have familiarity with any classic edition of D&D then you have it close enough. For those unfamiliar, the game skews more survival and wits based than feats and face-tank heroics, though tonally I have nothing against antics and welcome the improvisation inherent in a ttrpg group.
The focus of the game will be on dungeon crawling and I have been inspired by Delicious In Dungeon, an anime that brings pleasant vibes along with the more grounded fantasy of early D&D's survival/survival-horroSorcery roots. I want to learn about the world as we build it together over time, placing it in no particular setting but building around facts we establish through play.
I request that you have a decent mic. This means no heavy static, echoing players voices back into the chat, or parents watching loud tv that breaks into players' turns to talk. This last one can of course be managed by the push-to-talk function on Discord, which we will be using. Semi-automated sheets will be provided through Roll20, but you may also simply use a form fillable PDF.

2024.02.20 22:26 Shadow-Prophet Question about Texas notary fees (additional signature?)

The specific wording in Texas State law is "$10 for the first signature, $1 each additional signature." I understood this to mean additional signatures on a single notarial certificate, like when 2 affiants sign a certificate that I'm only signing and stamping once, I would charge $11 for that notary, but if they brought in two separate documents each with their own certificate (like, say, two PoA's), then I could charge $20 for that transaction, as I signed and stamped two different certificates with one signature each.
Is this a correct understanding? Or am I putting myself at risk if I charge someone $20 to notarize two separate DS-3053 forms for someone's two different kids? I wish the wording of our laws was much clearer!

2024.02.19 04:40 jiminibble Passport for Newborn with only one parent present

Hello everyone, I am going to be applying for a passport for my soon to be newborn daughter. ( I am 39 weeks pregnant so any day now). The requirements of getting a passport for a child under 16 state that either both parents be present or that one parent sign an original notarized DS-3053 in case only on parent can go with the child. My dilemma is that my husband is not in the USA. We are still waiting on an interview date for his spouse visa to be approved. On the DS-3053 form it also requires the missing parent to provide their US address, which my husband currently does not have. He cannot be here for the passport application for our daughter, nor can he sign a DS-3053 form. What would the next course of action be for me to successfully apply for the passport?

2024.02.17 22:40 Brave-Reflection40 acceptable notary for passport

My ex-wife is in Japan and I need a notarized signed form from her to get passports for my kids to show that she consents. The US Embassy in Japan website says "In Japan, a DS-3053 notarized by a Japanese notary public is acceptable." So, does this mean if she mails the form to me, then it might not be acceptable in the US? Is it only acceptable for getting US passports in Japan? Or is it acceptable since it was signed in Japan with a Japanese notary? How am i supposed to do this? It is very hard to get appointments at the embassies and consulates.

2024.01.17 01:10 jaehc DS-3053 and appointment question

got the DS-3053 notarized today but the notary put a stamp instead of a seal. Does this matter? also they wrote the date with hyphen between the dates instead of dashes. Is that gonna be a problem?
secondly, I have a 3pm appointment at the NYC agency for my daughter passport application. How long should I expect to be there?
thank you

2024.01.15 13:51 Ok-Boysenberry-1512 Would RON work for CRBA and passport applications?

For the US embassy of the country I live in, DoS says that you can use many online remote notaries located in the US for any "notorial acts." Can you do the same for DoS forms such as the Ds-2029 and Ds-3053?

2024.01.01 06:04 Foreign-Pack-4135 How to find a notary for ds-5507?

I'm located inside the USA and looking to notarize ds-3053 and ds-5507 so my wife can get a us passport for our kid.
I've checked 3 local notaries, and they refuse to notarize the ds-5057 because it says
PLEASE STOP HERE! Part II of this document must be signed before a Consular Officer, Passport Specialist, or Designated ConsulaAcceptance Agent at the time the oath is sworn.
So now I'm thinking of using ds-2029 because it says specifically it can be filled out in front of notary public.
Or should I just create a notarized affidavit on plain piece of paper and send it with my transcripts?
I guess I need to sign the forms because our kid was born before we were married. So I need to sign the extra section for out of wedlock kids.
Or should I just sign the notarized ds-3053 and send in my transcripts and use the marriage certificate as proof of financial support?
