Hello hello, and thanks for taking the time to read this.
I have a 71 yo male presenting with stiffness and discomfort (but no pain) that he finds debilitating, his main issue/goal being that he can’t walk as much as he’d like/ ?feet and toes become so stiff it makes it difficult to walk.
Would just love to hear anything at all from anyone who might have any advice/ ideas/ suggestions. He’s been with me (as an outpatient) for a while now and while is experiencing some relief he’s frustrated to the point that he wants to quit physio.
Meds and injections haven’t given him (“ANY” he says) relief.
Motivated but disappointed with his care so far by me and by docto consulting etc.

I feel so dumb. I’ve literally been stressing about this since I got my prescription and finally took my first injection today. I read the instructions over and over and after I injected 25 units did I realize I was only supposed to inject 10!! Anybody done anything similar? Should I take any next week? I don’t have a fear of needles or anything and it didn’t hurt going in but about 30 seconds after my injection I immediately started feeling dizzy. I went and laid on my bed and felt like I was going to pass out and immediately started profusely sweating. I was okay after just a minute but I do have a dull headache already. Please advice would be appreciated on whether I should continue with an injection next week. I’m sure I’m gonna feel like hell for a few days

I've been working on a mod called "The Matter Manipulator," inspired by Starbound and Subnautica. It's designed to be a small necklace powered by singularity-class technology.
The purpose is simple: to invalidate all tools in the game. Once acquired, the player would never need another tool, workstation, cook-station, etc.
However, I've been having some problems implementing it. Everything reliant on qualities seemed straightforward, and the items under the use_action category were easy enough.
But then came the cooking and welding recipes. I couldn't get many recipes that required the oxy torch or a cooking facility to work. From my research, I know you can inject your item ID into the JSON/requirements section of the code, which makes it show up as an option in the recipe list. However, this makes the matter manipulator require charges, which detracts from what I'm going for.
Any help or guidance in the right direction would be greatly appreciated. Here's the code I have so far: ———————

I’ve been on for one week at 0.25mg and am hoping to lose slow and steady so I can focus on diet and lifestyle change. I’d like to dose less frequent than once a week, say every 8-10 days. Has anyone done that!

Taylor & Francis Online For nearly 80 years, nano aluminum (Al) salts have been utilized as vaccination adjuvants. Nevertheless, there is a risk of adverse effects associated with the amount of nanoaluminum used in various national pediatric immunization regimens. This study aimed to investigate the possible genotoxic effects of nanoaluminum incorporated in human vaccines on the brains of newborn albino rats and whether nanocurcumin has a potential protective effect against this toxicity.
Fifty newborn albino rats were randomly assigned to 5 groups, with 10 in each group. Groups 1 and 2 received “high” and “low” Al injections corresponding to either the American or Scandinavian pediatric immunization schedules, respectively, as opposed to the control rats (group 5) that received saline injections. Groups 3 and 4 received the same regimens as groups 1 and 2 in addition to oral nanocurcumin.
The expression of both the cell breakdown gene tumor protein (P53) and the cell stress gene uncoupling protein 2 (UCP2) was significantly greater in groups 1 and 2 than in group 5. Groups 1 and 2 exhibited severe DNA fragmentation, which was observed as DNA laddering. Nanocurcumin significantly reduced the expression of the P53 and UCP2 genes in groups 3 and 4, with very low or undetectable DNA laddering in both groups.
Vaccination with nanoaluminum adjuvants can cause genotoxic effects, which can be mediated by the inflammatory response and oxidative stress, and nanocurcumin can protect against these toxic effects through the modulation of oxidative stress regulators and gene expression.

I do not get very good results from nerve blocks. It takes about a week or so to experience the relief. But as soon as that relief wears off (after about 7-10 days) my skin reacts horribly. Terrible facial flushing, big painful acne cysts all over my face (around pressure points notably), and skin gets dry and flaky. And it's like this for MONTHS after the fact. My insurance doesn't even cover this so I really don't think it's worth my time/money/appearance.
I read a study where people with ON got Botox injected directly into the occipital nerves and got great results. Outside of the medical literature I haven't heard of people getting this treatment though and neither has my pain management doctor. Has anyone here done this, and can share their experience?

My ring and little finger are always numb for ~10 months. The doctors say my ulnar nerve is entrapped in elbow/arm and neck (double crush). We've tried OT and PT but no real results. Next I'm going for a NJX interlaminar cervical or thoracic (CRV/THR) injection... has anyone had good results with that injection or has anything to share about it?

Just spent about an hour brainstorming how far I can take this concept. This is the final outcome. Let us begin, shall we.
Glyph of warding specifies that it can be engraved on a surface no greater than 10' in size, but it never gives a lower limit.
Consider a cubic virus, 20nm in width, length, and height. This virus will have a volume of 8x10^-24m^3.
A bag of holding can hold 64ft^3, or 1.81228m^3.
1.81228/8x10^-24 gives 6.25x10^25 viruses inside the bag of holding (I haven't checked if this weighs more than 500lbs, but I am almost certain it does not).
Now, biology.
We genetically engineer 6.25x10^25 strands of DNA to curl up into the shape of a glyph of warding, and hence enchant the glyph. Make sure to do this within the bag of holding to prevent breaking this glyph by moving 10'.
Painstakingly inject a single DNA strand into each virus. This will likely take more time than the universe has life, but you got it, champ!
Now we have a Very Large number of bombs.
Each virus deals 5d8 damage, meaning that, so long as the creature isn't further than 10' from the bag when you turn it inside out, it'll have to make 6.25x10^25 dexterity saving throws, taking 312500000000000000000000000d8 damage, or half on a success.
Assuming you have a +5 int mod, +6 PB mod, and a +3 arcane grimoire, your spell save DC is 22.
If you target a creature with a dex save of less than +2, taking the average roll of a d8 to be 4.5 and discounting critical successes, this deals an average of 1406250000000000000000000000 (1.4 octillion) damage.
Feel free to double-check my maths if you can be bothered lol. I very well may be wrong, as maths definitely isn't my strongest field, but the calculations are fairly simple that I doubt even I could mess em up lol. Also, if I missed a rule or a wording which messes this up, please do let me know.

Grandfather Program: I am looking to build a decent reputation before fully releasing the client, so I'd like to take between 5-10 people to be "grandfathers." As a grandfather, you will get a temporary license to test and review the client for a set amount of days. The client also has automatic updates, so whenever I release a new script, which will be pretty often going forward, you will receive a notification as you launch the client and it will allow you to update to the latest version right away. I will also take requests/recommendations from the grandfathers as to what scripts to add in the coming weeks.
Additionally, I am in the process of obtaining a code signing certificate for my program so that it can be verified and validated as legitimate software.
Since the grandfather seats are very limited, I will take applications and decide on a certain number of people to vet in. Me (30m) and my wife (27f) recently moved to the largest city in our state about 2 years ago, which unfortunately caused us to have to leave our friend groups behind as we had lived where we were at for almost a decade. Since moving here, I haven't really made any new friends because I'm kind of an introvert and don't really like going places/doing things without her. She has made a great group of core friends through a dance studio she started taking classes at about a year ago and has been made part of the studio's official dance team.
Now before we got together and in the initial part of our relationship, my favorite activity outside the house was going to Magic: The Gathering events and open play nights. I kind of got out of it after we started dating because most of my time (and money, Magic ain't cheap to have as a hobby) went to hanging out with her and doing things together as she is not into Magic or any of my personal hobbies. Now that we have moved to a large city and she has started dancing multiple times a week, I started to get back into it since I had time where she was busy dancing anyway, I have more money now because of my new job, and I figured it would be a great way for me to go out and meet new people doing something I'm comfortable with. Typically my nights to do this are Wednesday and Thursday evenings.
Now we get to the current scenario I'm asking AITAH for: she started going to a fancy gym called LifeTime Fitness, and it's minimum 25 minutes away, more like 45 minutes (or longer) with traffic between 4-6pm. Her doctor's office she goes to to get an injectible medication that has to be refrigerated is near this gym, only a few minutes away. Today being Wednesday means I usually go play Magic from about 5-8pm in downtown, which is only about 10 minutes from our home. Important to note is that I have not played magic on either Wednesdays or Thursdays for about 3-4 weeks now due to various things. Some of it has been me personally (I had a business trip for my job one week) and others have been because she wanted to do something together on Wednesday because she doesn't dance every Wednesday.
Yesterday she asked me if I would be able to come with her to her doctor's appointment and then take her to the gym, and then take her medication back home to be refrigerated. I assumed that meant she would be getting a ride home from her friend that goes to this gym with her, and lives right around the corner from where I play Magic. I figured her friend would bring her there and then my wife would come home with me when I'm done, or her friend would take her directly home. Important to note is this medication is not something REQUIRED for her health. She will be just fine without it and it is completely unnecessary for her, but it helps with some things that don't need to be discussed here. Today I found out that I misunderstood her, and that she will be driving herself to her appointment, I will be following her in my car, she will hand me the medication to take home, and then she goes to the gym and she can drive home.
The problem is when you go to play Magic, the game shops have limited seating, so I like to get there as soon as open play starts otherwise I may end up not getting to play because there are no open spots. She is asking me to do this simply so she isn't inconvenienced by having to drive to her appointment, drive home to refrigerate the medicine, then drive back to the gym. The problem is this completely ruins my plans for the evening as there will be heavy traffic coming back from her appointment, which is at like 4:30pm. I probably won't be getting back home until 5:30pm, then I come in drop off the medicine and then drive back into town myself if I want to still play magic at that point. Additionally while there isn't traffic to get to the card shop when I leave home at 4:30, there definitely will be if I leave at 5:30pm, so it will turn my 10-15 minute drive I to a 30 minute drive, so I won't be arriving to the gameshop until 6pm or later. At that point I may as well not even go because I'd probably have to end up waiting 30 minutes to an hour to even get into a game since all the slots will be filled by then.
I've expressed to my wife countless times how important these Magic nights are to me because it's basically the only personal time I have where we aren't hanging out or I'm at home alone while she is out at dance class/rehearsal and that it's basically the only way for me to meet people IRL that share common interests. It's also basically the only time I get to actually talk to and hangout with other men my age because I only ever hangout with her and her friends when they come to our house or at her dance team events.
Now I told her no, I don't want to do that and how it would affect my plans and how it would ruin my magic night (that I haven't gotten to do in at least 3 weeks). She reacted with "Are you serious? You won't help me with this? I'll remember this next time you want help with something like when I had to bring you over pamphlets for your work event that I bet no body even took". She has essentially guilt tripped me into doing this and saying how she'll take me out to my favorite Mexican food restaurant to make up for it.
My thing is just that it's unnecessary, she could do it herself but would rather inconvenience me than herself, and I just don't feel like my time is being valued for what I want to do but she flat out refuses to ever miss dance. So this I ask the internet: AITAH for saying no and that she can just drive it back home and go back to the gym herself?
TL;DR: Wife wants me to miss my plans that are important to me that I haven't done in weeks just so she's not inconvenienced in something that is 100% unnecessary and voluntary of her to do, that she can do herself, and it makes me feel like my time isn't as important as hers. AITAH for saying no?

Going on 10 months with back problems/sciatica

Been through months of physical therapy Taken many drugs from steroids, NSAIDS, nerve blockers, muscle relaxers, to opioids.
Got injections a few months back but they only lasted a few weeks.
MD was 1 month ago this Friday and unfortunately I’m regressing. I can’t stand or walk for more than 5 minutes.

some notes: - I have had six facial hair laser sessions and plan to do electrolysis after another two. - I am 5'10" and was assigned male at birth - I thought I might be a trans woman at some point but realized I feel more comfortable with androgny (and they/them pronouns). I do tend to be more female presenting and whether I pass as a woman or not is really random. - I am unsure of further surgery but am considering a tracheal shave (to get rid of my Adam's apple) or potentially an orchiectomy. - Facial hair was my biggest trigger pre-HRT and with laser and HRT it's gotten better. - Currently I am on estradiol valerate (I inject it each week) and spironolactone (testerone suppressant, pills I take each day.) I want to get on progesterone soon which will help to shape my boobs. - I am attracted to only men (pre and as of currently.) I have always been only attracted to men which hasn't changed at all through my journey of self discovery.

I 24f recently got my diagnosis after two years of being bounced between drs, having every test done they could think of, trying every medication they could try, every homeopathic remedy, physical therapy, massage, chiropractics, various types of acupuncture, injections, muscle scrapping, dbt/cbt and psychiatric approaches, etc and have found no relief other than in a combination of robaxin, ibuprofen, and hydrocodone (used sparingly for flares). Unfortunately, due to my age they will no longer prescribe this combination and switched me to tizanidine (because the robaxin didnt work alone) which has not helped even daily pain. Im curious what people have found works for them in situations where the pain gets unmanageable. I find my self waking up (if i sleep) every day with a 6/7 out of 10 pain and on my worst days im in the ER getting a migraine cocktail to get one or two symptoms to a manageable level. I feel like i spend all of my time trying to symptom manage and i hate living like this. What works for you? What have you found triggers symptoms or flares? Thank you!!!!

• Types of providers seen/tried: Chiropractor, Physical Therapist, Massage Therapist, Internal Medicine, Family Medicine, Pain Management, Neurosurgery, Neurology, Rheumatologist.
• Spinal Stenosis, Degenerative Disk Disease, PTSD (In therapy and medicated 8 years), sleep apnea (on CPAP 5 years), and high cholesterol (On a statin for 5 years).
• Exercise: Stretching and core exercises 3+ times a week. Recently able to walk daily with a cane and a slow/moderate pace for 30+ minutes. Have gone to the gym for years, typically 3x a week for weightlifting and cardio.
• Recent non-drinker, smoked in my 20's but quit. Occasional THC gummy for pain.

Bone marrow: No marrow edema
Vertebral bodies: Multilevel degenerative changes of the lumbar spine characterized by scattered disc desiccation, osteophyte formation, disc bulges. Level by level discussion of the spinal canal and neural foramina as follows:
L1-L2: No significant neural foraminal or spinal canal stenosis.
L2-L3: No significant neural foraminal or spinal canal stenosis.
L3-L4: Disc bulge. No significant neural foraminal stenosis
L4-L5: Disc bulge with superimposed central disc protrusion. Facet arthrosis. Severe spinal canal stenosis, increased from prior.
L5-S1: Disc bulge and facet arthrosis. Mass effect on traversing nerve roots.
Spinal Canal: Congenitally narrowing of the spinal canal. Conus terminates at the L1 level
Paraspinal Soft Tissues: Atrophy of the paraspinous musculature
XRAY FINDINGS:
There is some persistent very mild levorotatory curvature of the lumbar spine noted on the AP view. On the lateral views, there is some persistent very mild grade 1 retrolisthesis of L4 on L5, which increases slightly with extension and reduces completely with flexion. Lumbar spinal alignment otherwise remains intact, with no fractures or other subluxations evident. There are relatively stable mild degenerative spondylitic type changes noted, again most pronounced at the L4 level. Vertebral body heights and intervertebral disc spaces appear well-maintained. There are no pars defects identified.

I have not been in any accidents. My back has always been sensitive. Starting at age 19, I've had bouts of intense lower back pain that have become almost constant. I was pretty active at the time and about 60 pounds lighter than I am now. About 5 ago (age 31) I was diagnosed with spinal stenosis and have received multiple epidural injections. They stopped working, so I was sent to a new Neurosurgeon for a second opinion. He recommended facet joint injections. If needed, he recommended the path forward to potentially be back flexion x-rays for a stability check, nerve ablation, and then back surgery to either decompress or stabilize the spine.
I did the facet joint injections almost a month ago. The facet joint injections worked wonderfully for about a week but the following week, pain built to the point that I was sent to the ED by my Neurosurgeon's nurse. I had severe pain in my lower back and outer-facing portion of the left leg and I'm not sure how I was even walking. I also had some tingling and numbness in my left leg, plus some possible incontinence (#1 and #2. It was just a little bit each time, so I didn't know for sure). I was in the ED for 10 hours for an MRI and a Norco, and was sent home because the spine specialist wouldn't answer his/her pages.
Another nurse from the neurosurgeon called a few days later and recommended I use a cane until I can see the MD or PA for a follow-up. They also ordered spine/back flexion xrays, which only showed minor abnormal movement of the vertebrae. The cane does really help! But unfortunately, if I go without it for even 15 minutes, the pain flares back up again. I don't always need to put my weight on it either. It seems to help me balance.
I just saw the PA yesterday and she did a lengthy physical exam to find out what hurts, check strength, et cetera. She was miffed at my use of a cane and didn't believe the stenosis had anything to do with the pain because "the pain *should* be near my tailbone". The worst pain is in fact from bellybutton-down on my back, then my left buttcheek down to my knee. She prescribed gabapentin and physical therapy even though I've done both before. A lot of us with chronic pain recognize the gabapentin/PT combo late down the line as the professional's kiss-off. She didn't mention any plan of honoring the MD's course of action. I left her office in tears because I didn't feel heard or seen, and hobbled back to my car.

I'm at a loss with how to move forward. I would like some reasonable relief from the pain so that I can go on outings with my family comfortably, get back in the gym (I was going regularly before this most recent incident)or go for a walk without a cane. I would also like to keep up with the spinal stenosis so I don't become paralyzed or incontinent. These are the paths forward I am considering, and would really appreciate some input:

1. Stop going to the doctor. Try to manage with the cane and OTC pain relief and THC gummies. Either there's nothing more they can do for me, my symptoms aren't "severe" enough because I can still walk, I'm going to the wrong kind of doctor, or they think I'm a loon.
   
2. Go to PT and take the gabapentin. Whether it works or not, I'll have something to report back to the PA.
   
3. Schedule an appointment with the Neurosurgeon to straighten things out, possibly without seeing the PA again.
   
4. Return to my PCP and get another referral, possibly to a different specialty at his suggestion.

I've had a hormone panel tested twice and have had a testosterone of 3 both times. I've been doing everything I can to increase my T naturally, but it remains low. Below are the rest of my hormone values if applicable.
Total T: 3 ng/dL Free T: 0.6 pg/mL Estradiol: 32.8 pg/mL LH: 6.4 mIU/mL FSH 10.0 mIU/mL TSH: 1.530 uIU/mL Prolactin: 12.4 ng/mL Progesterone: 0.1 ng/mL T3: 2.6 pg/mL T4: 1.212 ng/dL Thyroid Peroxidase Ab: <9 0-34, IU/mL AMH: 1.81 ng/mL
I exercise daily, lift 3-4x/week. My diet is balanced, a variety of meats and seafood every day. Lots of vegetables, 1 serving of carbs like potato or oatmeal per day. Greek yogurt and whole milk daily.
I've never had normal or regular periods except when on a combo bc pill (years ago). They're infrequent and very light, often not requiring pads or tampons.
Unfortunately I'm seeing an OBGYN and unable to see a hormone specialist. There are none in my area accepting new patients. Does anyone have any advice for treatment ideas or tips to mention in my follow up appt tomorrow? The T injections appeal to me the most, as opposed to pellet or cream.
