For the sake of common ground, I am going to assume these types of cancers do not need to be elaborated on as we all likely have a basic understanding of what they are. The medical conditions treated by Tavalisse and Rezlidhia I felt needed a more in-depth explanation because they are not common. I will elaborate on RET+ a little later in this writing.
~What is Gavreto?~
GAVRETO is an oral once daily prescription medicine used to treat certain cancers caused by abnormal rearranged during transfection ~(RET+)~ genes in:
Adults with non-small cell lung cancer (NSCLC) that has spread

~What is Rearranged During Transfection Positive (RET+)?~
RET-positive cancer is caused by a mutation or abnormal re-arrangement of the RET gene. It occurs most commonly in lung cancer and several types of inherited and sporadic thyroid cancers. RET alterations also occur in an estimated 1-2% of multiple other cancers, including ovarian, pancreatic, salivary, breast, and colorectal cancers.

~IRAK/4 – Clinical Trials~
Rigel’s investigational candidate, R289, is an oral, potent and selective inhibitor of interleukin receptor-associated kinases 1 and 4 (IRAK1/4).
Toll like receptors (TLRs) and the interleukin 1 receptor family (IL-1Rs) play a critical role in the innate immune response and dysregulation of these pathways can lead to a variety of inflammatory conditions such as psoriasis, rheumatoid arthritis, and inflammatory bowel disease. Chronic stimulation of both receptor systems has also been implicated in causing a pro-inflammatory bone marrow environment leading to persistent cytopenias in lower-risk myelodysplastic syndrome (LR-MDS) patients1.
R835 is a selective dual inhibitor of IRAK1/4 that blocks TLR4 and IL-1R-dependent systemic cytokine release. In preclinical studies, R835 demonstrated activity in multiple animal models of inflammatory disease2,3 and showed that dual inhibition of IRAK1 and IRAK4 provided more complete suppression of inflammatory cytokines when compared to an IRAK4-selective inhibitor4.
Development of R289:
In a Phase 1 clinical trial, R835 was well tolerated and inhibited LPS-induced inflammatory cytokine production in healthy volunteers, demonstrating proof-of-mechanism.5 Phase 1 clinical studies of R289 (an oral prodrug that is rapidly converted to R835 in the gut) are also complete.

RIPK1 inhibition has therapeutic potential in treating autoimmune, inflammatory, and neurodegenerative disorders.
Rigel’s RIPK1 inhibitor program includes R552, a systemic molecule being developed for the treatment of autoimmune and inflammatory disorders, and brain penetrating RIPK1 inhibitors for central nervous system (CNS) diseases. In preclinical studies, R552 demonstrated prevention of joint and skin inflammation in a RIPK1-mediated murine model of inflammation and tissue damage.

Rigel has a long-standing collaboration with Daiichi-Sankyo for developing murine double minute 2 (MDM2) protein inhibitors in cancer, which were discovered in Rigel’s laboratories.
Preliminary safety and efficacy data from an early Phase 1 study of milademetan (formerly DS-3032), an oral selective MDM2 inhibitor, in hematological malignancies suggests that it may be a promising potential treatment for oncology indications.

~Summary and Prediction~
The current share price of sub $1 does not feel justified. I would anticipate financial breakeven by the end of 2024 or potentially in Q1 or Q2 of 2025. The robust pipeline, progress, and expected revenue growth are enough to justify a much higher valuation. The debt load is manageable, but the potential for S is concerning. I believe that the S is not necessary and revenue growth and progress should speak for itself. I am not as bullish as the analysts at HC Wainright for a $15 PT, but the valuation should be at least 3x to 5x from the current value. This thesis does not highlight the patents surrounding their drugs either which some extend into 2035 and beyond. Perhaps what Wall Street is discounting is the fact that most of the drugs are very niche. However, the currently available drugs have an addressable market, albeit less universal than some, but you should value it in the sense of multiple facets (a 1000 headed snake is the phrase I wanted to use). I believe the company should be valued with specialty drugs in mind which would command a higher PE ratio. At the current day and time of writing, the value should be at least $1.50 to $1.75 ~at a minimum~ with a 12 month price target of $3 to $5+. I will be looking for continued revenue growth in each quarter this year and realization of revenue from Gavreto in Q2 or Q3 this year. The partnerships should not be discounted either and the current share price if it lingers here perhaps may attract a merger or acquisition. I initially began the research thinking that perhaps the drugs were too niche, but given the multiple drugs they are working with, I believe their revenue sources will continue to grow if you do not focus on one particular drug as the main performer. With the most recent inflation report being cooler than expected, I would suspect larger funds and institutions will be circling back to riskier assets.

"Using a feature phone in the USA: a brief guide to carrier activation and MVNOs"

Introduction
Unlike many other places in the world, the USA uses a completely different set of LTE bands for cellular communications, additionally, American carriers impose device restrictions, which may adversely impact uses of feature phones, as they often are heavily locked down to the carriers from which they came originally, or have to be imported. This guide simply outlines my experiences dealing with the three major carriers (AT&T, T-Mobile, and Verizon) and general assumptions about regional carriers (such as U.S. Cellular and Viaero Wireless).
AT&T
AT&T is potentially the worst provider for feature phone users, given that AT&T enforces a whitelist of devices which may be used on their network (PDF: read the whitelist on KONAT.xyz, mirroring the original on att.com), which impairs the use of feature phones as only a handful of models (mainly AT&T/Cricket branded [in-house] models, US-market Nokias, and rugged/work-oriented devices from manufacturers such as Kyocera). From experience, AT&T's enforcement of the whitelist is generally extremely strict, to the extent that your account may be suspended for using non-whitelisted devices.
Even if a device works on AT&T's network and is whitelisted, it may not have additional features (such as WiFi calling) functioning on the network. Additionally, carrier variants of devices (i.e. the variant sold by T-Mobile) may not be whitelisted. Below is a non-exhaustive list of MNOs/MVNOs which do and do not enforce the whitelist; this list may become outdated at any time.
AT&T uses LTE bands 2, 4, 5, 12, 14, 17, 30, 46, and 66. In my experience, bands 12, 17 and 2 are the most useful to have in urban areas. Band 14 is used for the First Responder Network Authority (commonly referred to as FirstNet). I would not advise using a device without complete band compatibility outside of urban areas.
ENFORCES WHITELIST: AT&T post-paid, AT&T pre-paid, Cricket Wireless (the prior three are all owned by AT&T). The vast majority of MVNOs can also be assumed to be enforcing the whitelist.

T-Mobile
In general, T-Mobile can be argued to be the best provider for feature phone users, as it does not enforce a whitelist, and generally allows any device with the LTE bands used in a particular area to function on the network and utilize VoLTE, WiFi calling, or other carrier functions. To further simplify this, this means T-Mobile will allow the use of imported devices which have the band(s) required to function in a particular area, as well as devices from other carriers, in addition to US-market unlocked devices and devices sold specifically for T-Mobile's network.
T-Mobile also operates an EDGE/2G network and small slivers of HSPA/3G. The EDGE network is moribund at best (and hasn't been expanded in well over a decade, nor does it appear to be a going concern maintenance-wise), and the remaining slivers of HSPA are officially meant to be offline. T-Mobile's legacy networks should not be depended upon for reliable communication.
T-Mobile operates on the following network bands relevant to feature phone users: EDGE/2G: 1900 MHz (PCS) LTE: 2, 4, 5 (only utilized in Myrtle Beach, South Carolina), 12, 66, and 71.
From experience, devices lacking any band other than B2 will work in urban areas, but B12 is generally needed for indoors signal. B71 is needed for good coverage in rural areas. As a resource for figuring out which bands are in use in particular areas, take a look at the band rollout map.
For reference, MVNOs which operate on T-Mobile include Mint Mobile, Ultra Mobile, MetroPCS (the prior three all are owned by T-Mobile), Tello, and US Mobile.
Verizon Wireless
I would personally rank Verizon as second-best for feature phone users, as Verizon generally allows any device with LTE band 13 to utilize their network. Verizon does restrict new activations, however, this can be bypassed by activating a new SIM card in a cell phone that is known to work with Verizon (such as an Apple iPhone or similar known-working device) and moving the SIM card to the B13-having feature phone (or other non-accepted B13-having) device after confirming that the activation went through successfully.
Devices without B13 may work in urban areas, but may not have voice or SMS functionality (limited to data only), thus, it is a necessity to use a device with B13. Additionally, some MVNOs on Verizon's network (the only one I know of currently is TracFone and associated brands [Simple Mobile, Total Wireless/by Verizon, Straight Talk, et. al.) may bind the SIM card to the device it was activated in, and in this case will likely suspend accounts for moving the SIM, even into another 100% functional device, thus making it impossible to use devices that require the above activation method.
Verizon uses the following LTE bands: 2, 4, 5, 13, 46, 48 and 66
MVNOs that operate on Verizon's network include TracFone et. al., Visible Wireless (the prior two are owned by Verizon), US Mobile, and Xfinity Mobile.

My car was recently totaled and it’s like the second I solved that more problems came up. To sum up this "response", McLeland doesn't really respond to anything brought forward in the Defense's 4th Franks Motion. He sidestepped every issue.

• He sidesteps the 4:33 am ping.
• He sidesteps or misconstrues what Blocher's statement means in terms of that 4:33 am ping. (He makes two contradictory claims about this.)
• And he adds a lot of nonsensical phone data information into the mix as he did the last time he addressed this issue.
• There is no need for him to remind anyone that when a phone company "pings" a phone of a missing person by way of tower data, this won't provide an exact location. The best the cellular company can expect to find out by doing this, is if the phone is in that general vicinity and in working order. This is common knowledge. People have gone missing and even when their phones were within coverage and working, had not been found for this reason.

And this wasn't the issue that the defense brought up in their motion, anyway. Defense motion was focused on the 4:33 am ping on February 14, not based on the location of Libby's phone, but how a phone that isn't responding to these pings, suddenly does--which again McLeland never addresses.
The location of that phone, if the State's theory is correct, would have to have been under Abby for that entire time. But if the two reasons a phone doesn't respond to this battery of pings from the Cellular Data Provider are that it is either not working or out of the area of coverage of the tower being used to ping, then there has to be something wrong with the State's theory that the girls were murdered by 3:30 on the 13th, and never left the spot where they were eventually found on the 14th.
That 4th Franks Motion was really all about the 4:33 am connection between an antenna on the Cell Tower and Libby's phone, which remains a mystery.
And interestingly, McLeland also does not mention the Magic Bullet, as was pointed out to me by someone in the comments!! Thank you Smart_Brunette. I had completely forgotten about that bullet--and apparently McLeland forgot about it, as well.
McLeland opens with:

That, as in the previous three Franks’ motions filed by the Defense, this fourth motion illustrates a complete misunderstanding of the ruling in Franks v. Delaware, 438 U.S. 154 (1978) and the purpose of a Franks’ challenge to a search warrant.

But McLeland only states what he means by this later in the motion. Just as a refresher, here is the key part of the opinion given in Franks v. Delaware

Held: Where the defendant makes a substantial preliminary showing that a false statement knowingly and intentionally, or with reckless disregard for the truth, was included by the affiant in the warrant affidavit, and if the allegedly false statement is necessary to the finding of probable cause, the Fourth Amendment, as incorporated in the Fourteenth Amendment, requires that a hearing be held at the defendant's request. The trial court here therefore erred in refusing to examine the adequacy of petitioner's proffer of misrepresentation in the warrant affidavit.

[Seems to me the defense has met this standard. Above and beyond the standard, even.]
Next:

The new topic that the Defense focuses on in the fourth Franks’ motion is the AT&T “pings”. The Defense includes in this motion information about pings generated by AT&T on February 13th, 2017 through February 14th, 2017. The Defense also includes statements made by Sergeant Blocher when he was consulted by Steve Mullins about the “pings”.
The information in the motion show that the Defense does not understand the information contained in the “pings” and then exacerbates the problems by mischaracterizing the statements by Sergeant Blocher.

[McLeland includes a lot of irrelevant information, or what I feel is an attempt to obfuscate, following this claim, but he does give some new information we haven't seen before.]

In our current case, when the girls went missing, law enforcement contacted AT&T and asked AT&T to “ping” the cell phone of Liberty German. This was done in an effort to locate the girls. The pings came in from AT&T every 15 minutes. That means that AT&T would send an email to law enforcement every 15 minutes showing the “ping” results of “pinging” Liberty German’s phone. Each “ping” will give a longitude and latitude of the device, with a radius estimate and the source of that data....
...If the tower cannot make cellular contact with the device then the “ping” data will show a historical data location.

[What exactly McLeland means by this isn't entirely clear. Is he saying that If for 10 & 1/2 hours the tower cannot make contact with the device, then it just keeps referencing historical data locations? Are these always the same historical data locations? Or do they differ? And what about the 4:33 am connection on 2/14 ? Was their historical data that came with that?]
(Also it is the antennas on a tower that do this-the tower itself is not sending signal, antennas are sending and receiving signal.)
McLeland continues on this theme:

A tower can lose contact with a device because the device runs out of power or because the device loses cellular service.

How is this different than what Blocher said? Here's Blocher's statement--

Sgt. Blocher advised that his interpretation of the information which we
were receiving from AT&T indicated that the cell phone was no longer in
the area, or no longer in working condition. He advised that since there
had been no change in the every 15 minutes update we were receiving and
the last known contact time had not changed since 17:44 hours.”

[Losing cellular service means the phone is no longer in the area.]
And then McLeland completely contradicts this here:

Sergeant Edwards later says “with AT&T, part of the reason why it says historical here is that if they say, all right, we’re going to ping that phone but they can’t get to the phone, well then they’ll use the last known location from the historical records.
So, that is different than the live ping; it’s going back to the historical records saying we can’t find the phone right now, but here’s where we know it was last.” The “ping” emails collected all show historical data. That means that the tower could not find the phone at the moment the “ping” was initiated, so it just gave the service provider the historical data location. That doesn’t mean the phone wasn’t in the area or that the phone wasn’t in working condition. It means that the tower could not contact the phone. That historical data continues through all the “pings”.

(If the two reasons a tower antenna can't connect to a phone are because it is either out of service or out of range, then one of those two factors had to be true about Libby's phone for 10 1/2 hours. And again, McLeland never addresses the 4:33 am call in this. Was that a live ping or not?)
McLeland goes on to write:

Sergeant Blocher reports that from his evaluation, the last contact that the phone had with the cellular tower was at 5:44 P.M. on February 13th, 2017.

[OK. so the 5:44 pm connection, or ping, was live! Good to know. But what about the the 4:33 am call?]
But then McLeland appears to contradict himself again:

AT&T show that there has been no contact with the tower since then. From that information, he determined that the phone was no longer in the area, or no longer in working condition. The Defense wants this to mean that the phone is not in the area from the evening of February 13th , 2017 until the morning of February 14th, 2017. That is not what it means. It means that since the “pings” were only showing the historical data location, the phone was not having contact with the tower. Sergeant Blocher explained that could be because the phone was not in the area, but it could also mean the phone was not working. This is consistent with the “pings” collected from February 13th, 2017 through February 14th, 2017. That the “pings” are giving an estimated longitude and latitude based on historical data location information not live “pings”.

Still no mention of the 4:33 am call on the 14th, and he's just repeating information he already gave.
And more non-information, information from McLeland:

This information is consistent with the information in the probable cause. The information that we have is that the girls were dropped off at the trails around 1:30 on February 13th, 2017 and that at 2:13 P.M. their kidnapper ordered them “down the hill”. There is no evidence that the girls were ever taken from the area or that they were brought back to the area later to be murdered. Every indication is that they were ordered down the hill and then murdered soon after. The historical location data from the AT&T “pings” does not mean the phone was not in the area during the evening of February 13th, 2017 until the morning of February 14th , 2017.

McLeland finally gets to what would warrant a Franks Hearing:

The State has outlined the test used in Franks in its prior responses. In order to have a Franks Hearing, the Defense’s attack on probable cause must be based on deliberate falsehood or acts of reckless disregard for the truth and those allegations must be accompanied by an offer of proof. The claim should point out specifically the portion of the warrant affidavit that is claimed to be false and should be accompanied by affidavits or sworn statements or reliable statements of witnesses. The Defense must show that there is inaccurate or omitted information, and that probable cause is lacking if the omitted information is included. Franks v. Delaware, 438 U.S. 154 (1978). Herein, the Defense claims omissions if included would negate probable cause for the Search Warrant of Richard Allen’s residence.
"In order to have a Franks Hearing, the Defense’s attack on probable cause must be based on deliberate falsehood or acts of reckless disregard for the truth and those allegations must be accompanied by an offer of proof."

[Right. And the defense has done just this.]
And last but not least:

Further, Richard Allen admits that he was wearing clothing matching the clothing that “bridge guy” was wearing that day. That along with other eyewitness testimony and the other evidence gathered establishes probable cause that Richard Allen is “bridge guy” and he is the one who kidnapped the girls.

And this gets us to the very information McLeland never mentions in any of his responses--that Liggett lied about the eyewitness testimony. BB saw a young man on the bridge, no resemblance to Allen or BG & SC saw a man with only mud on his pants, wearing a tan not a blue jacket.
Basically, I feel that this response proves all claims made by the defense. It proves that the State not only misrepresented the facts of this case in all their PCAs, but continue to misrepresent the facts of this case in current motions. Obfuscation is the tool the State likes best, it seems.

We’re suddenly weeks away from the earliest presidential debate in U.S. history. If that seems unusual, you’re right — in more ways than one.

• It's starting to feel like everything is beef these days. The mega-rap-star-showdown between Drake and Kendrick Lamar has already inspired one Biden-Harris ad, using Lamar’s diss track, “Euphoria.” On Wednesday, President Biden dared disgraced former President Trump to debate with a schoolyard taunt: “Make my day, pal.” Now the old boys are set for June 27, three months earlier than usual. Biden’s tight timeline leaves him all summer to recover if he flops, before round two on September 10. But it also feels like this rumble is coming on faster than anyone was ready for. So what’s with the rush, fellas?
  
• Just like the Drake-Kendrick beef, there’s plenty of speculation about each side’s motivations. Biden’s bravado may actually be a sign of concern. Recent New York Times polling shows him behind in five-out-of-six key swing states. He needs a way to shake up this race. Trump, equally excited to throw down, may be eager to redo his disastrous performance in 2020, when he yelled out of turn, interrupted the moderator, and shredded decorum to the point of incoherence. He may also be looking for a way to change the subject if a Manhattan jury finds him guilty in a few weeks.
  
• Biden jeers, Trump sneers. Are we doomed to suffer the tabloidization of politics? These guys are vying to be President of the United States, not climb the Billboard charts. Sure, lots of people find beef entertaining. But there are real stakes here. Reproductive rights, LGBTQ+ rights, and voting rights are being stripped away. Two major international wars have claimed tens of thousands of lives. One candidate stands accused of serious federal felonies. It would be downright refreshing to have a big-boy discussion about actual policies and issues that impact American lives.

The Department of Justice vowed to crack down on threats to election officials, the kind that made the 2020 election so fraught and could make 2024 dicey as well. Trump’s stolen 2020 election narrative helped prompt hundreds of such threats. Four-in-10 local election officials say they have experienced threats, harassment, or abuse just for doing their jobs.
But the DOJ’s efforts may not measure up to the sheer size of the problem, according to the latest episode of this newsletter’s sister show, the What A Day podcast. Garland’s Election Threats Task Force has only brought 13 convictions since its inception in 2021. It’s far from clear how the DOJ will trace each threat back to its source. And this time, they’ll need to grapple with the rise of Artificial Intelligence, which could supercharge both the spread of misinformation and the reach of the worst people.

The House Judiciary Committee advanced an effort to hold Attorney General Merrick Garland in contempt of Congress Thursday after the White House refused to give up an audio recording of President Biden’s interview with the special counsel about his handling of classified documents. The White House seems to think Republicans want to take the audio, chop it up, and distort it for political gain. And, of course, they do. But voting to hold Garland in contempt effectively means asking Garland to arrest himself. Good luck with that one, fellas!
The North Carolina Senate voted to ban anyone from wearing masks in public for health reasons… as if the state isn’t still reporting hundreds of respiratory virus cases weekly including COVID-19! Republicans want to crack down on protesters in masks, accusing them of abusing pandemic protection to hide their identities. We know this is really just part of the GOP’s freaky little crusade against masks that is now entering its fourth year.
Cardi B is doubling down on revoking her support of President Biden in a new Rolling Stone interview, and to our disappointment, making a public stance that she is not voting for either candidate this year! Never thought we would see the day we disagreed with the iconic rap star but she has to “Be Careful” with her words and her influence. Sorry Cardi, we don’t like it like that.
A far-right Missouri secretary of state candidate might end up getting sued by rapper Lupe Fiasco for using his music in a diabolical ad featuring homophobic slurs. The ad shows candidate Valentina Gomez randomly running with an armored vest on before she blurts out: “In America, you can be anything you want. So don’t be weak and gay. Stay hard.” She even adds an F-bomb in there for good measure. Not sure what wearing a bulletproof vest for no reason and calling people gay has to do with being Secretary of State but hey, we’re not the * freedom * experts.
Gov. Greg Abbott (R-TX) said he will pardon a former Army sergeant convicted of fatally shooting a Black Lives Matter protester, citing the state’s “Stand Your Ground” law. A review board recommended Daniel Perry’s release after he was originally sentenced to 25 years in prison last year. Court records revealed Perry regularly shared racist memes and threatening content on social media, and now he will be back out on the streets thanks to Abbott. Perry’s firearm rights were also restored. Another winning day in Texas government!

The Justice Department is moving ahead with its plans to reclassify marijuana as a less dangerous drug. Reclassification wouldn’t legalize weed outright, but would federally recognize its medical use and take it out of the category with drugs like heroin and LSD. High five to that?
The Biden administration announced it will end coal leasing on federal lands in the Powder River Basin, which produces nearly half of the country’s coal in a region spanning 13 million acres across Montana and Wyoming. I am at a loss. I bought a refurbished Apple Watch Series 3 off eBay. the watch works/looks great. Set up went smoothly.
However, when I ask Siri to create a calendar appointment, the conversation goes like this: “Hey siri, add something to my calendar”, “What is the date and time of your appointment?”, “X date at X time”. Then the appointment preview pops up, with “Confirm” or “Cancel” buttons. I press “confirm”. Siri replies, “Sorry, I’m afraid I can’t do that”.
I have tried resetting my watch. I have checked all the settings I can think of to check. I have tried to find answers online. I am in the U.S., Siri should have no problems for me. Siri works just fine on my iPhone and Macbook Pro.
I do not have cellular data on my watch. Should this matter? I can make calendar appointments with Siri on my Macbook so I don’t see why that would matter in this case.
I bought this watch basically for the sole reason of making it more convenient to create calendar entries, reminders, and alarms. If i can’t figure this problem out I am going to return the watch. Can I go to the Apple store for help or do they only help you if you have purchased from them?

Hello, everyone! It's been a while since I posted anything or even visited the sub. I do not visit the sub anymore as I collected all the information I needed long ago and staying on the sub only led to more thinking about flox. Focusing on other areas of life has been a great life hack for me! I have done a lot of positive things in the past half a year - I am starting my own business, been meeting new people and making a lot of new friends. Flox has changed me for the better.
I want to preface this by saying that I was probably the only person (or almost only as I've met maybe 1 or 2 other people on Reddit) who claimed flares from CoQ10. It actually flared me quite a lot — sometimes I could handle 100mg and sometimes even 30mg would lead to terrible pain. It was frightening to be one of the rarest cases in a pool of already rare cases, so, naturally, I tracked reactions to supplements extremely attentively (u/vadroqvertical won’t let me lie about that) and I have tried a lot (my cupboard is full of supplements — I spent around €3,500 on them in the span of 1.5 years). I will list reactions to supplements and the approximate timeline of when it happened:
— First of all, CoQ10/Ubiquinol flared me not so much 1 month out (tried 100mg ubiquinol multiple times) but it got worse as time went on to the point that April 2023 I could not even take 30mg without great pain. I tried it 1, 2, 3, 4, 5, 6, 8, 16 months out all without luck with varying doses flaring me to different extents. I will outline the reasons for it below;
— Vitamin E flared me a lot 2, 4, 6 and 8 months out. Never tried again. Tried 200-400 IU at a time. Due to poor GSH regeneration through Glutathione Reductase dependent upon B2 and NADPH;
— Benfothiamine flared me as well (doses 150mg-300mg/day). This is due to high sulphite and blockage of complex IV of the Electron Transport Chain in the mitochondria the reason for I will explain further. Thiamine is easily broken down by sulphite in the body and it is broken down into sulphite as well, which causes a negative loop reaction in people with high sulphite levels. Benfothiamine also caused me a severe allergic reaction (extreme anxiety and itching) that gladly did not require hospitalisation but was extremely scary and scarred me psychologically (likely high sulfocysteine activated NMDA receptors);
— Vitamin B6 increased my neuropathy when I got it. Likely due to poor B2 functional status. The problem I was also deficient in B6 and its supplementation led to great improvements in sleep quality once I could tolerate it. Note B6 is easily destroyed by sulphite just like B1;
— Riboflavin flared me (tried at 100mg, doses under 10mg never flared me). This is likely due to unmatched NADPH supply due to high sulphite load in the body (speculative);
— Astaxanthin greatly improved my physical health at 5-6 months out (proving that the core of my issues was solely ROS) but it caused reductive stress (NADH accumulation), which also caused pain, albeit the pain was a different kind and asta caused worsening neuropathy and visual snow. It accumulates in fat tissue, so stopping it was nice with ROS coming to a balance at about 10-12 days after discontinuation (after a loading dose of 36mg/daily for 3.5 weeks) but ROS then came back after it went out of the body further. I did not retry astaxanthin as I realised it caused me reductive stress and neurological issues;
— NAC helped me a damn lot. It was the best antioxidant for me. The problem is it depleted my molybdenum and copper and started giving me allergic reactions (low molybdenum + copper as well as blocked complex IV will lead to way higher sulphite generated from NAC);
— Did not feel much from vitamin D. I live in a very sunny country and tested at 51 (ref. Range 30+) without any supplements;
— Magnesium helped me a lot. #1 supplement;
— Calcium did not help me much in the beginning, actually, caused me heart palpitations. Was fine taking it after a few months;
— Potassium was a good supplement. I took 800mg/day for a while and it supported my muscle health;
— Important: vitamin B5 made me feel a lot better. It took my ROS down like crazy — I could feel normal muscles again, it removed my oxalate pain completely, too but for only a short while like 3-4h.
I have tried many more supplements that were phyto-supplements and such and none of them really helped me beside maybe some placebo effects. Some made me feel worse and were not worth it at all. I did not try anything mood-changing as I was not interested in it. To note, GABA supplement made me feel a little euphoric at first.

Now, to the real question: why did CoQ10 flare me even at high nutrient status (just after flox). I have to stress that flares from CoQ10 were much less at the beginning of flox likely due to better nutrient status (it went from extremely terrible to slightly more extremely terrible while 6 months out it went from ‘eh’ to terrible).

1. First, I have to say that NAC made me worse long-term. How? Over a long period of time I was taking it and was not watching my copper levels (NAC increases metallothionein and causes poor copper absorption) and molybdenum levels (NAC raises generation of sulfite and it needs molybdenum to be detoxified). Some NAC formulations have molybdenum in them but I was not lucky to get one of those and, due to lack of knowledge, did not supplement any molybdenum. The result was high sulphite and from that high ROS (with a combo of benfo which further increased sulphite it caused me peripheral neuropathy at 5 months). Sulphite causes Fenton reactions when complex IV gets blocked up. H2S (a signalling molecule and a vasodilator) also needs to be detoxified by a CoQ-10 dependent enzyme and turned later into sulphite and then sulphate by molybdenum and complex IV (dependent on copper) and if it is not detoxified, it causes a complex IV blockage and starts Fenton reactions as well as electron leakage during production of ATP, causing ROS. This causes a negative feedback loop that was described in the linked article as follows:

«This can be explained as follows:
1) hydrogen sulfide inhibition of complex IV generates superoxide in the respiratory chain, which becomes hydrogen peroxide,
2) hydrogen sulfide reduces ferric iron to ferrous iron, which makes it release from storage in ferritin,
3) this increases Fenton reactions between free iron and hydrogen peroxide, which generate more dangerous reactive oxygen species like the hydroxyl radical,
4) all of this deplete glutathione,
5) since a major purpose of the trans-sulfuration pathway is to provide enough cysteine to make glutathione, glutathione depletion hyperactivates the trans-sulfuration pathway, leading to more cysteine availability, the excess of which is catabolized to sulfite by alternative reactions that do not produce hydrogen sulfide and therefore do not require CoQ10.»

1. In the article linked below, you will see that CoQ-10 protects against reactive oxygen species mainly due to improving hydrogen sulphide clearance (H2S). Therefore, CoQ-10 deficiency did not cause much ROS in complexes I and II but mainly produced issues in Complex III (where sulphite detoxification starts) and complex IV (where the last electrons are delivered during the sulphite-sulphate reaction). Excerpt: «In human cells with CoQ10 synthesis defects from the same study, CoQ10 protected against reactive oxygen species, but suppressing the enzyme that uses CoQ10 to clear hydrogen sulfide abolished this effect. This shows that the reactive oxygen species were coming from poor hydrogen sulfide clearance.»

Considering this, and oh my god, finding this article was like god sent it to me: my CoQ10 flares were coming from poor hydrogen sulphide clearance. At that point there were multiple reasons this could be happening:

1. Cellular CoQ-10 deficiency;
   
2. Manganese toxicity;
   
3. Copper deficiency;
   
4. Molybdenum deficiency;
   
5. SUOX (enzyme which converts sulphite to sulphate) or another genetic impairment;
   
6. Blockage of complex IV by something else.
   

I checked my molybdenum and copper transporting genes, SUOX using DBSNP and my AncestryDNA.txt file, and they were all good (Yes, I know Ancestry does not do a full genomic profile but it still had the main SNPs for that.). I also checked my manganese transporter genes and seemed I was homozygous for an important one but fine with others. It is really hard to estimate how that might affect you IRL, perhaps that would require a real genetic counsellor (or lots of hours spent ruminating again). I also did not think I had any genetic issue since I was very very healthy all my life and had 0 pain or health issues before flox occurred (I have extremely healthy young looking parents that drink, smoke and do whatever they want and have 0 consequences to their health as well).
I took some tests, for example: Genova NutrEval at ~6 months out, full nutrient blood test panel at ~11 months out (abstained for 35 days from any supplements at all, even vitamins and tested literally everything, paid around €1,200) and my CoQ10 levels at both of those occurrences were at 1 & 1.07 in absence of supplementation with ref. Range 0.8-1.4, so it was definitely not low. That way I eliminated #1 and #5. While I was not entirely sure whether genetic issues had to do anything with it, I decided to pretend like they didn’t, since I had to try out other solutions before jumping to the most complex one. I took a lot of molybdenum, so molybdenum deficiency was not at the table for me. In this way I was left with #2, #3 and #6. In the full blood panel, my manganese was slightly high (20.1 with ref. Range <~18) and the SNP people were talking about that caused them manganese toxicity was homozygous for me, so I definitely considered it but manganese when supplemented made me a feel a lot better, actually (mentally, not physically), so I was also likely deficient in it. For now, I just avoid it in supplemental doses but I do not avoid foods containing it. Besides, I do not have iron overload genes that could contribute to manganese toxicity.
I could not take copper because it would lead to high ROS immediately (due to complex IV blockage the reasons for which I will outline further). Considering manganese was likely deficient and not superfluous, I discarded reason #2 and reason #3 could not be fixed by copper, so it was definitely not only copper deficiency but either another factor or another factor coupled with copper deficiency. I was stuck for a long time until I found another article from the same author about B12 and B9 helping to detoxify oxalate. As I said before all this explanation, I have been oxalate dumping throughout the whole process (already 4 months). I should note I was oxalate dumping even before I got floxed (I likely had oxalate overload to my appendix surgery — this is proven by inflamed mesenteric lymph nodes confirmed by 3 MRIs — Sally Norton has the same case of over-absorption in her book) and that is how I actually got the E. Coli they gave me Cipro for (oxalate crystals create a good environment for it in the urinary tract lol) and how I got floxed (I went full circle, lmao). When I was floxed, I was not oxalate dumping for at least a year likely because my body was not in the state to handle the dumping process but it was still affecting me as I will outline further. First of all, I want to say that biotin actually promoted dumping for me as said in the article and not relieved it like it is said in Sally Norton’s book (I am not sure if there is a genetic variation to this). The proposed mechanism of oxalate detoxification in the article is as follows:
«Recall my proposed two-step detoxification process:

1. Pyruvate carboxylase [biotin-dependent] converts oxalate to formate.
   
2. Formate is joined to tetrahydrofolate to enter the methylation cycle, be used for the synthesis of purines or DNA, or be converted to carbon dioxide and exhaled in the breath.»
   

This are also very important words: «There may be more regulation layered on top of this to prevent excessive formate accumulation. It would certainly be preferable to have oxalate crystals cause pain or disrupt the skin than to have formate accumulate beyond the capacity to clear it.» This is why I felt best when dumping. Could eat anything, drink beer, even smoked weed once without issue. Another time though I got too brave, smoked a lot of weed and got a very bad ‘relapse’ but recovered quickly from it. The next morning when using a towel after a shower I had the same pain I used to have 2.5 months out from Cipro (which was extremely bad and took me back 14 months in memories) while before I smoked weed that second time I had almost 0 tendon pain in my daily life apart from oxalate [Here I thought maybe I and DrHungry share similar issues then? He also had an extreme (same in intensity relatively to his flox journey) flare from weed and is also using a lot of sulphur-based antioxidants still. Could such weed flares be related to complex IV dysfunction and/or impaired sulphite clearance?]. In either case, I felt best when dumping, probably because my body was able to regulate formate accumulation and ROS production greatly reduced at those times.
I was sitting outside with my parents and their friends, researching my flox issue when I read these lines: «Formate accumulation is the principle mechanism of methanol toxicity. Part of its toxicity is driven by inhibiting cytochrome oxidase, complex IV of the mitochondrial respiratory chain, which would inhibit the clearance of sulfite and hydrogen sulfide and block the production of ATP.» It finally clicked. It was honestly one of the best moments in my life when I realised. I made the connection between great improvement from B5, formate accumulation, issues with copper supplementation, general ROS improvement and oxalate everything together. Suddenly, my whole flox journey became crystal clear to me.
B5 is mainly used in the body to create Coenzyme A. An intermediate molecule in the production of CoA is called 4’-phosphopantethine and is used in the enzyme 10-methyltetrahydrofolate dehydrogenase (high formate will pair with THF and form 10-MTHF in the attempt of the body to detoxify formate). This enzyme converts 10-MTHF back to THF and creates NADPH in the process which is used by Glutathione Reductase to regenerate Glutathione. Hence, high-dose B5 led to a lot of those reactions occurring and me feeling a big relief from ROS AND OXALATE, so oxalate is indeed detoxified into formate by biotin-dependent pyruvate carboxylase.
Okay, so theory is very interesting but what is theory if it has no proof? When I read it, I realised I finally cracked my flox but I had to get real proof.
Just a few weeks before this, I drank some wine and got nerve damage (likely from high sulphites in it, again, duh — while this was a terrible experience, it played a role in me getting closer to the solution of my issues). Beer caused me no issues, could drink 10 or more bottles in one sitting, eat a lot of rice with no issue. Before, I had only numb hands and top of feet. After the wine, I had burning up to the knee and burning in palms and behind my shoulders. I got fed up with this, I just decided to methylate the fuck out of my nerves and eat copper not in supplements but from calamari (very high in copper but low in vit A, so no toxicity risk like from liver). At that time, I was dumping and my ROS was not too high. I started consuming around 200g protein per day, eating a lot of copper 3-4mg/day and my nerves really healed a lot. To the point they even became normal after 3-4 days. My vision became brighter, it was absolutely crazy. I was also supplementing 150mg molybdenum/day. After a week of that, though, I started getting ROS back and it was very bad ROS, like almost a year ago when I had low molybdenum and copper from a lot of NAC use. That confirmed my suspicion that my issue was indeed sulphite. Eating almost anything caused ROS for me, dumping stopped since the body had no free reducing agents (NADPH) to support sulphate-producing enzymes (oxalate is transported on sulphate transporters, so it literally could not drive out of the cell because it had no car lol). As you understand, high ROS prevents a lot of enzymes from working and here it causes, as you have probably understood, a negative feedback loop.
So, back to the proof. Since I realised that my issue is probably formate, I just decided to take high-dose B5 again (did not add any high dose B2, B1 or other B vitamins, just took my usual B complex with food). It really helped me a lot, again. I felt almost normal. Then, it caused me some pain but I felt how I was getting better and the next day I took it in the day, then in the evening I ate around 80g carbs and took double the dose of B complex (my B complex has low doses: 10mg B1, 10mg B2, 25mg B3, 20mg B5, 5mg B6, 100mcg B7, 100mcg B9, 50mcg B12) instead of adding a lot of B5 and boom, no pain and oxalate dumping restarted quite more strongly than it even used to be before megadosing protein. So I was in pain for at least 2 weeks dying from ROS and then 2 days of B5 and suddenly I was normal again? It felt like paradise. The next day, I went out with my friends. I was a little nervous since we were going to eat out and we ordered 600g of carbonara (the portions here were huge there). I ate it all at once with 2x my light B complex and guess what happened? NO PAIN, just oxalate dumping. I finally realised that I was right and detoxified formate unloaded my complex IV, allowed sulphate transporters to be created, reduced ROS production from food and suddenly I felt like a normal human being (except the dumping part). I recently retried CoQ10 — no flare. Likely before formate got recreated a lot because I was dumping a lot (if you read my comment, you will understand).
I am not megadosing B5 right now but just stuck to 80-100mg B5 per day, so 4x my light B complex as my B6 tolerance improved a lot. Why I am not megadosing B5 is because oxalate likely blocks conversion of vitamin B2 into its active forms as I at ~11 months out when I did full-testing in the absence of supplementation 35 pre-testing had high molybdenum, iodine, (almost above the ref. Range (113 with ref. Range <120) selenium and very high B2 even though I was cellularly deficient according to Genova NutrEval (at 356 with ref. Range <295).
Hence, we can understand what happened to me from the beginning:

1. Oxalate overload led to formate overload as oxalate is converted to formate through the action of biotin-dependent pyruvate carboxylase;
   
2. Formate overload led to complex IV blockage, high ROS and high sulphite, which also leads to high ROS and also leads to complex IV blockage (negative feedback loop);
   
3. High sulphite destroys vitamins B1&B6 as said in the beginning, which caused endogenous production of oxalate to skyrocket (you can read about this if you google, this information is very available);
   
4. Hence sulphate transporters also got impaired, oxalate detoxification in the form of physical crystals also halted, which led to even higher overload;
   
5. This led to higher formate, this led to even more ROS.
   

Mega-dosing B vitamins and especially B5 and B9 led to formate detoxification and the ability of my body to detoxify oxalate. This improved me a lot and it definitely feels like it will inevitably lead to my recovery. I feel good now, I still have some remaining neuropathy but it’s minimal and I know what to avoid to not make it worse and how to improve it quickly if I need to. I have no OS from beer, coffee or food. Also, I am dumping a lot right now. You can ask me all kinds of questions that you want and I will try to answer them to my best ability since I know what it is like to be floxed and I will help anyone who is in the same situation. I am only 22 years old and this experience led to me rethinking my whole life. I plan to become an extremely rich person to be able to fund biochemical research in the future and will focus specifically on floxed individuals and I will help floxed people first. I will try to reach my goals as fast as possible, I promise.

My understanding is that Airsense devices can only connect to cellular network in the U.S.
Since Personal Therapy Assistant feature is using myAir app as the interface, I imagine that it works only in the U.S.
It's difficult to believe that Resmed created features that only works in the U.S., considering this machine is sold worldwide too.
Can anybody who owns Airsense 11 who live outside of the U.S. fill me in on this? How was your experience with it?

So long story short I bought a Tab S8+ 128GB 5G on Amazon renewed. I love the AMOLED screen but there have been issues with each order. The first time it had screen burn-in and the tablet was still tied to Verizon. I ordered a replacement and this time there was no burn-in, but it is still tied to Verizon because it probably wasn't paid off. I paid right at $400 for it. I am wondering if I should just deal with it at this point.
My alternate question in case if I really wanted to do this is what tablet would you guys recommend as an alternative? I use the tablet for running emulation like the switch or 3DS, also media consumption, general gaming, and note taking. I heard some good things about the Xiaomi Pad 6 Pro, but I'm not sure about longevity or how well it works in the U.S. the tablet doesn't need to be cellular, just would've been a plus at the price I paid.

Complete remission of depression and anxiety using a ketogenic diet: case series

Background: There is little data that describe the use of ketogenic metabolic therapy to achieve full remission of major depression and generalized anxiety disorder in clinical practice. We present a retrospective case series of three adults with major depression and generalized anxiety disorder with complex comorbidity, treated with personalized ketogenic metabolic therapy, who achieved complete remission of major depression and generalized anxiety disorder and improvements in flourishing, self-compassion, and metabolic health.
Methods: Three adults, ages 32–36, with major depression, generalized anxiety, other anxiety disorders, and comorbid psychiatric conditions were treated for 12–16 weeks with personalized whole food animal-based ketogenic metabolic therapy (1.5:1 ratio) in a specialized metabolic psychiatry practice. Interventions included twice-weekly visits with an experienced ketogenic registered dietitian; daily photo journaling and capillary blood BHB/glucose/GKI monitoring; virtual groups; family/friends support; nature walks and talks several times per week, and community building. Successful adoption of the ketogenic diet was defined as the achievement and maintenance of capillary BHB ≥ 0.8 mmol/L and GKI < 6. Remission was assessed by GAD-7 and PHQ-9, and quality of life was assessed subjectively and with validated scales for flourishing and self-compassion. Metabolic health was assessed by laboratories/biometric measures.
Results: Two patients achieved remission of major depression (PHQ-9 ≤ 4) and generalized anxiety (GAD-7 ≤ 4) within 7 weeks of therapeutic nutritional ketosis; one required 12 weeks. Anxiety responded and remitted more quickly than major depression. Flourishing and self-compassion increased steadily. Patients lost 10.9 to 14.8% of their initial body weight within 12 weeks and improved metabolically; one achieved optimal metabolic health.
Conclusion: Complete remission of major depression and generalized anxiety disorder occurred within 7–12 weeks of therapeutic nutritional ketosis during treatment with a personalized animal-based ketogenic diet (ratio 1.5:1) in adults with complex comorbid depression and anxiety engaged in a specialized metabolic psychiatry program.

Introduction

Emerging brain-based research in psychiatry and neurology has focused on identifying fundamental metabolic disturbances within neurons and throughout the body involving insulin resistance, inflammation, oxidative stress, and alterations of the gut microbiome (1). All four of these fundamental metabolic disturbances are present in major depression (2), and underlying anxiety disorders (3) and can be directly modulated through the use of ketogenic metabolic therapy (KMT) (4).
As psychiatric disorders have risen over the past several decades, the prevalence of metabolic syndrome has sharply increased, with only 12.2% of U.S. adults meeting the criteria for optimal metabolic health, leaving 87.8% metabolically compromised (5, 6).
Metabolic syndrome affects almost a third of individuals with major depression (7). It is a significant contributor to their morbidity and mortality (8) and is rooted in impaired glucose metabolism and utilization. Insulin resistance has been well described in many tissues, including the brain (9), where it is being investigated as a link between metabolic health and mental health conditions. Preclinical models demonstrate that glucose intolerance is directly associated with anxiety and that insulin resistance triggers depressive behaviors (9). In brain tissue, insulin resistance results in cerebral glucose hypometabolism and a vicious cycle of unmet energy needs (10). In human studies, cerebral glucose hypometabolism is a feature of major depression (11, 12) and generalized anxiety disorder (GAD) (13).
KMT, also known as the therapeutic ketogenic diet, or KD, is a low carbohydrate, moderate-protein, high-fat diet that supports a fundamental metabolic shift from glucose to ketone bodies as the primary fuel source (14). Classic KMTs are formulated with strict macronutrient ratios, most commonly 4:1 and 3:1 (fat: protein + carbohydrates), and have demonstrated efficacy in intractable epilepsy and genetic disorders. More recently, modified classic KMTs with lower macronutrient ratios of 2.5:1, 2:1, and 1.5:1 have been utilized in research and clinical practice (15, 16). These allow more variety in the diet, meet micronutrient needs except vitamin D (17), and are easier to sustain for extended periods of time.
KMT exploits the body’s natural ability to produce ketone bodies (d-beta-hydroxybutyrate (BHB), acetoacetate, and acetone) in the liver from fatty acids by keeping carbohydrate consumption very low. Acute and sustained production of ketone bodies produces a fundamental shift in fuel energetics within cells, particularly neurons, which can radically re-route and quickly rely on readily available BHB and acetoacetate for cellular energy (18). Ketone bodies also increase vascular density at the blood–brain barrier, which can strikingly increase the availability of ketone bodies for brain energy metabolism by 40-fold (19). Ketones are a preferred energy source in the CNS (20) and neurons will choose ketones over glucose when available.
Nutritional ketosis (10) using a KMT is a natural, not pathological, state (21) where the body’s energy and protein synthesis needs are met with a high-fat/moderate-protein/low-carbohydrate diet, resulting in sustained elevations of serum ketones and fatty acids and normal glucose without acidemia. In both acute and long-term nutritional ketosis, ketone bodies have a number of biological effects that directly change the brain’s cellular energy status (15), increase mitochondrial density (22), and improve mitochondrial morphology, which has been shown to be altered in mood disorders (23, 24). Mitochondrial abnormalities have also been postulated to be responsible for changes in synaptic function and neuroplasticity, potentially associated with symptoms of depression and anxiety (19).
Recent research shows that a ketogenic diet (KD) reduces neuronal firing rates, modulates ion channels and cell signaling cascades, and stimulates the biochemical synthesis and neurotransmission of GABA by inhibiting glutamate decarboxylase, a major inhibitory neurotransmitter involved in neuronal firing and anxiogenesis (25, 26). BHB activates the transcription of antioxidant-related genes by inhibiting histone deacetylases, triggering long-term adaptive changes in gene expression. In addition, at physiologic concentrations, ketone bodies reduce neuroinflammation through direct action at G-protein coupled receptors (25). KD also favorably alters the gut microbiome (27). Perhaps most importantly, KD directly increases NAD+, which reduces reactive oxygen species and increases mitochondrial ATP production. It is also utilized as a substrate for sirtuins and PARP enzymes associated with DNA repair and longevity. A sustained increase in NAD+ may underlie the pleiomorphic benefits of KMT across multiple neuropsychiatric conditions (28). In terms of the frequent abnormal alarms set off in the amygdala during anxiety, nutritional ketosis may provide an acute and long-term intervention to reduce generalized anxiety, panic attacks, obsessive doubt, and symptoms of post-traumatic stress disorder (PTSD). For apathy, anhedonia, amotivation, and abulia seen in major depression, therapeutic nutritional ketosis may provide higher and more sustained intraneuronal energy and repair (29, 30).
There is no published data that describes the implementation and use of personalized KMT for adults in real-world clinical practice who present with major depression comorbid with GAD and complex psychiatric comorbidity.
The aim of this case series is to examine the response to the treatment of major depression and generalized anxiety with whole-food animal-based personalized KMT in adults with complex psychiatric comorbidity and varied metabolic status. We conducted a retrospective review of three cases from our Metabolic Psychiatry Registry that demonstrate a consistent response and remission of major depression and generalized anxiety among patients who are psychiatrically and metabolically complex, despite differences in the initiation and adoption of KMT, and varied metabolic dysfunction. We describe the evaluation process and prescription of KMT, baseline metabolic workup and monitoring, elements that fostered treatment engagement and adherence, and challenges encountered during 12 weeks of KMT. We correlated capillary BHB/GKI with time to the remission of major depression and GAD and the achievement of metabolic health.

Discussion

Although KD was first shown to produce antidepressant effects and alleviate “behavioral despair” in preclinical studies more than 20 years ago (20), there is little clinical data regarding KD in major depression and anxiety disorders.
A retrospective analysis of 31 individuals with primary diagnoses of major depression (N = 7), bipolar II disorder (N = 13), and schizoaffective disorder (N = 12) who had failed to respond to conventional psychiatric care was treated with KD (75%–80% fat, 15%–20% protein, 5% carbohydrate) for 12 weeks in a psychiatric hospital (31). Of these patients, 22 were voluntarily admitted for the initiation of KD, and the remainder were offered KD during their inpatient hospital course. Change in depression was measured by HAM-D and MADRS in 6 of 7 patients with major depression and 12 of 13 patients with bipolar disorder. Notably, 100% of patients given the HAM-D showed statistically significant improvement in depressive symptoms (mean HAM-D decreased from 25.4 to 7.7; mean MADRS decreased from 29.6 to 10.1). However, serum ketones were not measured; urinary ketone measures were obtained once in 28 patients during the 12-week intervention; 18 patients (64%) showed positive urine ketones (31).
Bipolar depression was included in a recent randomized controlled pilot study assessing the safety and feasibility of KMT as adjunctive therapy, and reported safety and feasibility with excellent adherence and maintenance of ketosis (mean BHB 0.88 ± 0.99 mmol/L for 12 weeks) (32).
One case report utilized KD (65% fat, 25% protein, 10% carbs) with a time-restricted feeding window in a 65-year-old woman with major depression and type II diabetes and reported remission of depression (PHQ-9 17 to 0), normalization of HbA1c, decrease in estimated average glucose from 216 to 96 mg/dL, improvement in HOMA-IR from 9.4 to 2.3, and TG/HDL ratio from 4.7 to 1.2 over 12 weeks. The only measured serum BHB reported in the case was a mean of 1.5 mmol/L by week 12 (30).
A recent meta-analysis of low carbohydrate diets used in controlled trials that evaluated symptoms of depression and anxiety, not disorders, in varied metabolic and inflammatory conditions reported that the symptomatic response of these symptoms was inconclusive (33). The conclusions may not apply to KMTs; the meta-analysis was limited by grouping varied diets with higher carb intake and higher protein intake than usually associated with diets formulated to induce nutritional ketosis; serum or capillary BHB was not reported; and primary and secondary outcomes varied across studies.
In anxiety, preclinical research shows that exogenous ketone supplementation reduces anxiety behaviors (34). There is one case report of a self-administered Atkins Diet for weight loss in a woman with panic disorder (35) but no reports of KMT in panic disorder, OCD, or PTSD. Case reports of KD addressing anxiety describe two cases of decreased anxiety symptoms in a woman with women, one with bipolar I disorder and another with unspecified mood disorder, comorbid emotional dysregulation, body dysmorphic disorder, and eating disorder (36, 37), and one case report that describes the elimination of anxiety symptoms in a man with bipolar disorder (38). Anxiety and obsessive preoccupations improved in weight-restored anorexic women, in one case report describing complete remission of anorexia (39) and in a retrospective case series where animal-based KD was adopted without dietary prescription and monitoring (2); however, ketone measures were not reported. One small pilot trial where KD was followed by ketamine infusions reported a significant lessening of obsessive preoccupations in weight-recovered women with chronic anorexia; here, ketosis was measured by breath acetone (40).
Finally, all three patients had comorbid ADHD, which may be important. Approximately 65–89% of adults with ADHD experience one or more comorbid psychiatric conditions, and ADHD often occurs comorbid with anxiety and depression (41). Preclinical studies in murine models (42) of ADHD with hyperactivity suggest that KD may improve symptoms via alteration of the gut microbiome. Preclinical studies in dogs with epilepsy displaying ADHD-like behaviors treated with a medium-chain triglyceride KD have shown decreased pathological behaviors (43). Further research exploring the effect of KMT in humans with ADHD should be considered to understand the mechanisms of action and assess short- and long-term risks and benefits.
There is little research regarding the selection, implementation, and treatment course for KMT use as adjunctive or sole treatment in individual psychiatric conditions. Given the potential benefits of therapeutic nutritional ketosis and the restoration of metabolic health (44), there is a pressing need to identify the biological underpinnings of KD in psychiatric disorders and delineate factors associated with the successful adoption and adherence of KMT and responses in common psychiatric disorders such as depression and anxiety. In clinical practice and real-world settings, where patients often present with multiple comorbidities, consideration of KD can seem daunting to clinicians.
Despite that, this case series illustrates complete remission of both major depression and GAD in three adults with complex psychiatric comorbidity and previously unrecognized metabolic dysfunction using whole-food, animal-based personalized KMT. Anxiety responded first and time to remission occurred rapidly within 7–12 weeks, despite varied challenges, including preferences for time-restricted eating, slow adoption, inconsistent monitoring, and emergent fatigue during strenuous exercise, which occurred many weeks into KMT due to low serum carnitine and spontaneous reduction of protein intake rather than keto-adaptation or “keto-flu.” All patients improved metabolically, and one patient achieved optimal metabolic health (6).
These patients were representative of many adults in clinical psychiatric practice who present with persistent, serious symptoms interfering with several life domains. They each had five DSM-V psychiatric disorders: severe unipolar major depression, GAD, at least one other anxiety disorder (OCD, PTSD, and/or panic disorder), and ADHD, and two had binge eating disorder. They had all failed at least two previous adequate trials of medications and psychotherapy and were seeking relief. All had family histories of mood and anxiety disorders and documented metabolic disease in first-degree relatives. Extensive laboratory testing and bioimpedance evaluations were eye-opening because, although they were overweight, they were not aware of the extent to which they were already metabolically ill; we suspect it enhanced their motivation to adopt and maintain KMT.
This case series is limited by describing only three patients, which limits the generalizability of our results as well as the inherent selection bias, as they were interested in KMT after failing standard therapies. In addition, they were selected because their complex psychiatric comorbidity reflects the complexity seen in the majority of our outpatient psychiatric practice. This degree of complexity may limit the generalization of these findings, although it is important to note that outpatient clinical psychiatric practice as a whole has seen an increase in complex psychiatric comorbidity over the past two decades (44).
As a retrospective case series, there may be additional limiting and confounding factors, including the lack of a control group. Some rating scales and digital data are missing, which may impact the completeness of the analysis. Time to consistent nutritional ketosis and delays in obtaining necessary labs requiring intervention may have contributed to a longer time to response and remission; response and remission may occur earlier than reported here, and this deserves further research. Finally, it is not clear to what extent immersive treatment (see Supplementary material 1) and additional interventions during KMT, such as close digital monitoring, frequent clinical contact, group supports, and nature walks, contributed to the rapidity of response/remission of anxiety and depression, or to overall treatment success, independent of the biological effects of KMT, and which of these elements were most critical; more research is needed. Carefully designed prospective studies and randomized controlled trials providing higher levels of evidence are needed to examine the use of KMT and time to response and remission in individuals without comorbidity and determine the extent to which comorbidity may confound or alter these (8).
As metabolic psychiatry moves forward, we need both preclinical and larger, well-controlled clinical studies examining the pleiomorphic effects of ketone bodies in the brain and in the body to understand how we can best leverage whole foods to optimize brain energy, enhance genetic expression, reduce neuroinflammation, optimize metabolic health, and safeguard the promise of our future.
