Quick Question for the group. I am 3 months out from my NASM CPT certification expiring. I have .3 CEU and .1 CPR complete. Didn’t want to spend a lot of money. Just had a baby 6 months ago and am focusing on him 100% at the moment but don’t want my cert to expire because it cost a lot of money to get here.
I know I know. I waited last minute. So I bought a course on Exercise Etc. as it was mentioned a few times in this community. It fulfilled the requirement and checks out in the NASM CEU account view when I go to add it in my portal.
The kicker, it won’t let me access the program test without a login. Wtf. I received an email receipt with the e-book code. That all checks out. But the test, I can’t figure out a login or even where to create one. No answer yet from their customer service. Was I scammed?

I am not sure if anyone in this subreddit has experienced this, I am based in US. I went for an initial check up with a CRS, hr exams me, said I have fissure, put a red color ointment( I forgot the name ) on it, prescribed me diltiazem and that’s it. A month later I got a medical bill for over $1300, one of the service they billed me is curettage/cautery anal fissure(cpt 46940) which costs $1000. I called my doctor about when did he do that, he replied that the red ointment is chemical cautery.
  1. Is the doctor coded correctly? I googled the code but can’t find the details about the procedure, most of them said if the doctor scrapes or burns the fissure with electrosurgery
  2. If the doctor coded correctly, I think it’s not justified to charge me 1000 for just apply the ointment, how can I argue that?
Any advice will be helpful!

This was a question that I have come across a few times recently. Basically new coders (or people thinking of becoming a coder) are wondering-What happens if I make a mistake? Will I be get fired? Be in trouble? I could see why this would be a concern. Medical coders are held to a very high standard both by ourselves and the industry.
But let's think about this--what happens if a coder makes a mistake? Let's start by saying--no one is perfect. Every single person who is working now as a coder, has made mistakes at one point or another.
The thing that most companies look for is your average accuracy rate. I don't have an exact number of what all places are, but in my experience (which is radiology coding) a coder had to always have a 95% or above accuracy rate. Different places audited coders at different times. Sometimes it was monthly. Sometimes it's daily. It really depends how the company is set up. Another thing to keep in mind too--was it a CPT or ICD-10-CM mistake? Usually a CPT error is counted differently against a coder than an ICD-10-CM error. In other words--it's worse to make a CPT mistake than a ICD-10-CM mistake because there is a more likely chance the claim will be denied because of this. So what happens to the coder? It depends how often you're making mistakes. Everyone makes a mistake once in a while. So if this is a rare occurrence for you--in short--nothing happens. Maybe your manager will let you know about it, but that is it. I think what's important here is how you handle it. I think medical coders by nature are perfectionists. So if we hear we make a mistake...we don't like that and may get defensive about it.
If you're told you made a mistake and truly don't think it was a mistake, you can bring proof (an authoritative source) to your manager and talk about it. Just be careful here--all places are set up differently and managers are different. Sometimes it might be best not to say anything. But if it's really bothering you--yes, it's ok to say something. Sometimes auditors make mistakes too and I'm telling you this as an auditor--there have been times that I marked something as incorrect when it really wasn't. Does that happen often? No. But it has occasionally. I don't mind when coders tell me to recheck something. I've had coders come to me an explain their thought process and use sources. If I can see their point of view--there has absolutely been times when the coder proved me wrong. I don't think that makes me a bad auditor--all it means is that I'm human too. So in short--everyone in the industry will make mistakes sometimes.
If you're wondering what happens to the actual claim that was coded wrong--usually a corrected claim will be sent in if it denied because of the error. In my experience the person who originally coded the report is not the same person who does the corrected claim--but it may work different elsewhere.
My whole point in writing this really, is don't let being afraid of making mistakes stop you from pursuing medical coding! It is an awesome field to be in.

2024.05.15 19:16 Grand_Ad9783 FINALLY GOT APPROVED FOR TREATMENT! Would you say this is a good price per session? And any ways I can get it lower? (United HealthCare)

Long story short, I was diagnosed with Epilepsy a few years ago and after a long battle I got approval from my neurologist & psychiatrist to start the treatment!
I have United Healthcare and each session with cost about $200 (including my co-pay as noted below). Would you say this is a pretty decent price? And do you think I'd have a shot with calling UHC to try getting them to cover some of it?
I was sent all of my paperwork this morning, along with a sheet that goes over what to expect with the treatment, the cost per session, etc. I have been researching Ketamine Therapy for over a year and would have been curious to see this, so I've included it below for anyone who cares!
" We like to inform anyone interested in receiving treatments on what to expect and what is required. Please read over the information regarding cost, time, and requirements for receiving ketamine treatments. 1. It is required that ALL patients be observed in our office for a minimum of 1 hour for each treatment. The following treatment requirements for the first eight weeks are as follows." Weeks 1-4: twice weekly Weeks 5-8: once weekly * After week eight, treatments can continue weekly or bi-weekly; this will depend on how the patient responds to the treatment. 2. You MUST have a driver to pick you up and sign you out before you can be discharged from our clinic on treatment days. For those of you that are wondering if you can get insurance to cover an expensive wig, it is possible. I had a $4,700 piece covered. You just have to really talk to the right people. After about 60 days, my claim is FINALY done. I have UHC. Here are the steps I followed in my case. Of course everyone's case is different. Here's what I did in my case:
  1. Called insurance and asked a few times if they covered a wig. I kept getting the answer "wigs are only covered up to $350". My first problem was calling a "wig". Don't do that. Find out the CPT code for "Cranial Prosthesis".
  2. I did some research and found out my doctor can write me an Rx for a "cranial prosthesis" be sure the doctor doesn't call it a wig on the Rx. See below the info included on the Rx.
    DME: Cranial Prosthesis (not "wig") CPT: D5924 ( in my case) Diagnosis Code
  3. I submited the Rx to insurance to get a prior authorization. You can submit yourself for out-network providers. Most providers that do custom units will be out-of- network.
  4. I found the correct form for gap insurance approval and reached out to my insurance to process an approval for gap insurance on my cranial prosthesis. This helped insure the claim was processed in-network.
  5. I searched for providers that hold an NPI and create custom units. I used Heavenly Tresses and purchased a custom cranial prosthesis that is used for people with total hair loss. This part is very important because you'll want to purchase one that is very specific for people with total hair loss and have a medical need and cannot be used with the absence of a medical need.
**"Heavenly Tresses" has customizable options for people that have a medical need for this type of unit "cranial prosthesis".
  1. Once I purchased my cranial prosthesis, I reached out for a "medical bill of sale". This part is important because you'll want to use a provider that can provide a "medical bill of sale". I would recommend reaching out to the provider you choose and making sure they are able to write a medical bill of sale.
  2. I submitted the medical bill of sale to my insurance for reimbursement. I called my insurance and asked for a supervisor to help make sure the claim was processed correctly. I looked at the rubric for the CPT again yesterday and noticed my program didn’t exactly meet the requirements (ie. the algorithm having to be in the body of the procedure with the parameter)(having two calls to two different procedures). I know, I should’ve paid more attention.
Anyway, is it a big deal? I know I’ll get a point or so off, but if I answer the rest of the questions right according to my code, will it matter enough to completely ruin my score?

Hello all,
I’m currently contemplating a job offer for an orthopedic office. It would be sports med with a new surgeon. He’s not super busy yet, but it is a practice with a few extremely established surgeons so it stands to reason he will get pretty busy eventually. The salary is reasonable but the bonus structure is only 1% of total billing. I’ve heard of other PAs in the area getting something like 20-30% of total billing, once you cover your salary and benefits (roughly 1.5x your salary), and for a busy practice, that seems like a much better structure. I don’t want to seem greedy, but my previous practice worked us extremely hard, with no bonus or salary increase, so I just want to be sure I’m being compensated fairly for my work.
Any advice would be appreciated.
In addition, does anyone have any resources to see reimbursement rates for various CPT codes? I’ve tried googling but seems to get a pretty wide range.

I’m wondering if anyone has experienced this. We used an out of network genetic lab for PGT-M and PGT-A. We do have out of network benefits (40/60 coinsurance) and met our out of network deductible. When we submitted our largest claim, BCBS processed the claim for CPT 81228 (PGT-A), which was $1,100 and denied the claim for CPT 88299 (PGT-M), which was $6,700, stating that “it was an integral part of another service and therefore separate payment cannot be made.” We’ve tried to explain to them a million times via message and phone that they are two separate procedures. We sent them detailed CPT code descriptions from Cooper Genomics, but no one can explain to us why it is being treated as the same service.
Has anyone had experience with this? Should we just go straight to appealing it?
Medical billing and insurance is such a racket. After our out of network deductible and their BS “adjustment” for what they say their in network provider would charge, they have reimbursed us for $190 out of the $11,000 we paid to Cooper. I’m not expecting to get a huge amount back, but seriously? We have out of network benefits. It should get us more than $190.

2024.05.14 21:40 Epigrammic_Pastiche Does pediatric care automatically score higher in MDM levels?

I am trying to understand how an allergist's office billed a 10 minute visit with my 20 month old as level IV. I brought her in because of a concern about nausea/vomiting after eating oats. They weighed her, palpated her belly, and ran no tests. They did not prescribe medication, establish follow-up care, or make any referrals. I was basically told she had a sensitivity and to "wait and see." They said it wasn't anything to be concerned about but now I'm getting a $500 bill for what they deem a high complexity visit.
They used CPT code 99204 (New patient office or other outpatient visit, Level IV ) When I inquired about how they categorized it they said they reviewed the charges and "Where she is pediatric and a new patient, the visit was a little higher this first time to establish care."
I can't find anything in the AMA MDM guidelines that states pediatric care is classified as a higher level. Is this accurate or do I need to request my insurance audit this?

2024.05.14 10:30 MeasurementLow8404 i feel like i’m going insane

so i have two more exams left! i’m halfway there yay but i’m actually tweaking. i have csp tmr and bio the day after but I HAVE NOT LEARNED A SINGLE THING IN CSP, I DONT UNDERSTAND MY CODE AT ALL😭 i learned how to do basic coding in two nights and i forgot all of that for the cpt. i have no idea what the frqs will ask and it’s making me so nervous. then there’s bio, and i really want to get a 5 on it. my reviewing is going p slow though, only at unit 4 and that’s stressing me out sm. i’m so confused if i should be putting my time in to review bio or do csp- everyone has been saying csp is common sense but idk if that applies to someone who has little to no knowledge on the topic. what do i do.

Recently a few claims were denied (UHC). I have a surgery coming up, and even though the clinic said I don’t need a prior authorization, I decided to check and was told I do need it. I called back with the clinic and they checked again and said I don’t need. I called the insurance company again and the representative was super rude on the phone and not helpful .
I don’t get it: I get the surgery, I pay for insurance, and I might need to pay out of pocket. Why would the representative is so unclear and rude? I shared all the CPT codes that they gave me as he asked me.
Without any list or documents about it on the website, how can I possibly know what would I be required to pay?
I am frustrated and angry. Advise?

Can someone translate this into regular English please as to what I'll have to pay?
I need a bladder procedure with cpt code 51700. I asked my health insurance if it would be treated as a doctor's visit with a co-pay, since the procedure would be done in office by her or her assistants and the clinic & doctor are in network. The doctor's office told me the procedure costs between $280 and $350 each time. I got the following convoluted response from my health insurance company:
"Thank you for your recent inquiry regarding your benefits. The following information pertains to the general in and out-of-network benefits for such services when eligible under your benefit plan. Verification of benefits is not a guarantee of coverage or payment. All services are subject to medical necessity, contract exclusions, and eligibility at the time the service is rendered.
You have general benefits for both in and out of network services. When utilizing an in-network provider, the service would be considered subject to a $1,000.00 individual deductible per benefit period. Once the deductible is satisfied the services would be considered at 80% of the allowed amount, up to an $2,500.00 individual out-of-pocket per benefit period. After the out-of-pocket is satisfied, services will be considered at 100% of the allowed amount. The benefit period is based on a calendar year from January 1, 2024 to December 31, 2024.
When utilizing an out-of-network provider, the service would be subject to a $1,000 individual deductible per benefit period. Once the deductible is satisfied, services would be considered at 60% of the allowed amount, up to the individual up to an $2,500.00 individual out-of-pocket per benefit period. After the out-of-pocket is satisfied, services will be considered at 100% of the allowed amount. The out-of-pocket does include the deductible totals. The benefit period is based on a calendar year January 1, 2024 to December 31, 2024. An out-of-network provider can balance bill you.
We can provide you with a benefit quote regarding how the claim will process with the given codes. You may view the estimated cost of a procedure by accessing the provider search on our website and using the Care Cost Estimator. Please log in to the member portal and click on the Find A Doctor tab then click Medical you will be prompted to acknowledge that you are leaving our site to reach the provider search, click continue to proceed.
On the Sapphire provider search page click on Browse by category and select Care Cost Estimator from this menu you can select a procedure by type or click on All Procedures (A-Z) in order to search for a specific service. Reminder this is an estimate.

So I've been re-scaffolding a custom specific-project/specific-client plug-in to be something more general and helpful to the wider community. The plugin helps to decouple content authorship from user accounts by reassigning post authors to a "Team" custom post type.
