 The necropolis was gorgeous, for what it was. Its white outer walls hiding the darker Victorian Gothic interior. The tech that was hidden in the walls though was able to move bodies in their caskets from a designated place in the necropolis to the "viewing area" as the necropolians called it. This was where I waited for my, for lack of a better term, escort to take me to the mortuary. Since science has grown surprisingly fast our abilities for forensic sciences have also grown, and that's to whom I was headed. (S)He was an, unusual (wo)man to say the least. An expert in their field and about as learned as a doctor, if not multi-doctorate. If you ever asked them why they never pursued an actual doctorate, they would get angry and act all prissy while saying that going to school would've slowed them down and all they needed were the basic certificates for their work. The reality though, revealed to me during a drunken bout, they just never liked school and believed that it ultimately stunted a person's growth and ability to question the reality around them, that everything that you need to learn is already in books and in some form or another in digital content online. They were brilliant, if a little wacky. About five minutes after I had arrived and was sitting down in the viewing area, a little box rolled up to me making a couple of beeps to let me know to follow it and immediately started rolling towards the wall opposite of where it came from. When it looked like it was about to hit the wall, a hidden door opened up by the casket viewer, inside was a set of stairs leading down into the darkness. Stepping through the doorway I became acutely aware of sounds seemingly coming from all around me suddenly. It really is impressive, as though I just stepped from a tomb to a busy workshop, the sounds of gas escaping pistons, whirring, and clanking chains flooded my ears. I continued down the stairs following my helpful little box, which despite its size and shape would suggest was actually quite nimble on the stairs. It seemed to have wheels that would extend down to the next step as the edge rolled over it and once the back of the box was clear of the step it would drop back into its squat position, hiding its wheels as quickly as possible. It continued to do so the entire way. The box seemed to notice me watching it and made a kinda shrill whistle and its undercarriage light went from a comfortable yellow to a, is that... Peach? Is it blushing? My god I think it is! I let out a small chuckle and my little blushing box stopped dead in its tracks mid-step, its light suddenly going white, almost blinding me from behind and lighting up the hallway for a split second. Luckily both of my feet were solidly on a step so I didn't take a tumble or anything, but I couldn't help doing anything but laughing harder. 
After a second the little box crept up behind me and continued down, its status light continuing to show pinkish. I followed it slowly, the chuckle slowly dying in my throat as we reached Ceriths office. Well "office" was being nice. Morgue, mortuary, both of these fit just as well. Cerith was, for the most part, a recluse. We reached the door and the little robot continued through a little hole in the wall. I waited a second and knocked. "Enter!" Came the voice on the other side. I opened the door and stepped through. Along one wall set doors that normally housed the dead waiting to be processed. One out of dozens were open, its occupant missing from its silver slab. The middle of the room was brightly lit from a single overhead light. In the middle of the circle of light stood a figure, long Raven colored hair bound in a single braided ponytail, the rest of them bound in medical examination garb. They seemed to be engrossed in the corpse in front of them. The little robot rolled up next to Ceriths feet and made a little chiming noise. "Thank you Tabitha. That'll be all," said a voice that was neither male nor female from beneath the mask. Just sort of in the middle. "Tabitha? Never knew you to be sentimental," I said gently, the chuckle in my voice making itself clear. "I see you still find even the darkest things funny," Cerith quipped back. "My line of work Cer, you take the laughs where you get them. Look who's talking anyway, you're usually elbows deep inside someone 25/8. Even you have a seriously fucked up sense of humor." That got Cerith laughing, sounding like thunder and the whip crack of lightning at the same time. "You've got me there Julius," Cerith said after his laughter subsided. I think he suits him today. Which is both a good and bad sign. When Cerith is acting like a man, it usually means some grim news, but they are going to try to make it seem like not a big deal and laugh a lot. Plus they almost never call me Julius. Something was wrong. Very seriously wrong. As this realization hit me I got this odd tingle in the small of my back. Like someone had put several freezing needles under the skin and into my spine, something I'm familiar with from the anima-games from the cyber sphere. Halos: Divine Retribution If I remember right. Those Angels were sadistic bastards. I shuddered at both the memories from the game and the shockingly similar feeling I was experiencing. Dread, that feeling is dread my friend, the quiet part of my mind whispered to me. "Cer, what's wrong bud," I asked. He didn't say anything. For a long time. After a few minutes I was about to ask again, but then he spoke. And what came out will haunt me, quite possibly till the day I die. "This ones temporal lobes are gray matter. Nothing even close to being coherent. Just. Dead neurons. And he's not the first." Gone was the jovialness of the past ten minutes. This was Cerith the whisperer. In an almost dead tone they continued, "the others didn't fare nearly as well as this one. Most of the brain is intact here, which means that if they didn't deliver a massive shock or something similar to fully kill him he would have possibly lived as a vegetable with memory issues, but that's not what I'm looking for in this one here now. Now I'm trying to figure out what else the others had in common with him, and so far that's brought up all but naught. Well this one has a bit of liver damage. But that's about it. So Mr John was a drinker. Not much there." When Cerith is "whispering" the best thing to do is just let him be. But I couldn't help but prick my ears up at mentions of others with similar wounds, and the fact that this one had liver issues.... "Cer. You said... CERITH," I finally snapped out and caught his attention mid ramble. "Thank you. You said liver problems. But nothing similar to the others? No drugs? Alcohol? Not even a synth brain-pattern? You checked Everything?" "Well let's see, John here was a drinker that's for sure," Cerith said his hands never ceasing their work as he started to put 'John' back together seemingly satisfied that he found nothing else, " Mr Lombardo in chest 3 had cocaine mostly, and Mr Lei in chest 9 had opium. Although to tell you where it came from for both I'd have to do a molecular analysis and see what it compares to. Other than that, no. Absolutely nothing connecting any of them. As far as I can tell they are all unique cases completely separate from each other except for the damages to the brain. And I only found this by accident. During a routine scan I happened to look at the screen as it passed through the brain and noticed an odd density in his temporal lobes. Just slightly higher than normal. Hell to be honest with you it had the density of a fresh cutie, you know those little oranges?" I nodded, and he continued, "Right of course you do, who hasn't? Anyways it's just super dense compared to the surrounding tissues, and I take a sliver probe and drop it in like you do. And when I turn the damn thing on to look at the neurons the area all I see are dead cells packed on top of one another. Not natural decay death, but forced to die. Most of the cell walls were torn open like they had blown up from the INSIDE. That's when I called you." He finished up with 'John' putting the final few perfect stitches in place and sealing him up for good. Once he seemed happy with his work he called out to his seemingly empty morgue, "Grom I'm done! Can you put Mr John Doe here back in his room? Number 11 if you please." He turned away from the body on the table and removed the giant rubber gloves that went to his elbows. He walked into the dark calling out over his shoulder, "I'll be back in a sec I gotta scrub out, want a drink? I have beer, whiskey, vodka, I might have some Cognac somewhere, and bourbon. Your choice, just call out what you want and Tabitha will be there with it. Also have a seat! We have much to discuss." With that he disappeared from both sight and sound in the dark. It was a neat trick I have to admit, and it had something to do with how he had his morgue set up. Even the giant war machine that was Grom was absolutely quiet unless you managed to catch him through the gloom. I thought for the longest time the reason why I could never catch him sneaking around was from some sort of stealth program put into place, but when he goes up and down those stairs he's as loud as can be. So it was definitely not his program but the way the morgue was built. I'm confident in saying that because when I turned back to look at the table, or rather where it was, there was now a chair that looked like it had just grown out of the floor and the body was gone. Also the thought of something as big as a fridge just sneaking up on some poor combatants and snapping their necks as quietly as he walks in the morgue just gives me the heebies. As I sat in the chair a thought occurred to me. Considering how advanced the morgue seemed to be it would make sense that it had some sort of AI or integrated computer. "Computer?" I had been here a million times but I'd never had a chance to think about it nor try anything. But not even a second after I had said anything a response came. "Yes Detective Julius. My name is DANNA. Or Dynamically Actualized Neural Net AI. How can I be of service?" The voice seemed to come from everywhere and nowhere, slightly feminine and breathy, all service but no sex. Honestly I was just surprised that it worked. "DANNA, I was just wondering if I could take a look at the files that Cerith had mentioned? If it is as bad as they claim I think I might need to know anyway. Also if you can get those blood works done for me I'd appreciate it. Also something with whiskey or rum would be amazing." "Of course Detective. I will have Tabitha bring it shortly. And how would you like the information to be displayed? Desktop or dynamic?" That piqued my interest. "Dynamic please." No sooner than I had said a series of screens blinked into existence in front of me. It was some sort of Holographic display. I reached out and touched the display and was surprised that I got stopped by something. It was hard but surprisingly I found that I could push into the screen with my finger if I pushed hard enough. It kinda felt like... Oobleck. I also found that by pinching the corner I could pull the screens closer or further from me. I even found that I could grab individual pages of the reports off the screen and hold it. It felt like a thin sheet of plastic and responded like both a tablet and a singular document. If I switched pages the old one would appear back onto the screen and the next would pop onto it. This was about as slick a set up as I had ever seen and whistled my appreciation under my breath, I'm definitely going to have to ask Cerith about where they got DANNA from. "See something you like, big boy?" A very DEFINITELY female voice said in my ear from behind, soft and throaty, screaming come hither. I felt small dainty hands gently caress the tops of my shoulders before slipping down the front of my chest, pulling me back into the chair that I didn't realize I had been slouching in. "You know better than that, Jules. Your back is important and slouching will destroy the muscles and cause some to atrophy." The voice left no room for argument, and left me more than a little bit flushed. I closed my eyes and dropped my head back as far as it would go, the back of my head hitting something soft and warm, stretching my neck and back out. "Damnit Cer I thought you were scrubbing out, not completely changing." I hadn't realized it, but at least an hour had passed from when I started playing with the computer and working with the files if the clock on the computer was to be believed. "You looked like you were pretty into it so I decided not to disturb you. Plus you know how much fun it is for me to tease you like this. Especially after, well these..." One hand waved at the screens in front of me. The small hands' nails were painted the darkest black and almost made them blend into the void that existed outside of the screens. "I do Cer, and that's part of the problem, we both know that it's never going to happen. Least of all for you." She laughed a little, a clear beautiful sound and the body beneath my head bounced slightly telling me I was against her stomach. "Still I know you enjoy these little moments," she said, the pressure on the back of my head disappearing and was replaced by the voice right by my ear again as she whispered, "especially when we both know that's not at all true." At the last words she nibbled my ear gently. I couldn't help but roll my eyes at her, in spite of my baser instinct rising to meet her VERY juicy insinuations. But for as long as I've known Cerith and as many times as we have both been VERY drunk, they have NEVER cashed in. I just assumed that it was a quirk of theirs. "Anyways," she said standing back up, "what are you thinking so far about the files? Spooky, right? Like I said, nothing that I can see connects them." Her hands gestured in front of me in an approximation of a shrug. She then clasped them together, wringing the knuckles and effectively trapping me in the chair and back against her abdomen. I scrubbed my eyes with my fingertips acutely aware of the growing headache that suddenly made itself known. "Your right from the medical side. I can't see everything you can, of course. I don't have near the knowledge that you have," which is true being that Cerith is at least 200 years old. I never asked directly, the old adage still holding about women and their age. Still though her answers to certain questions would lead one to believe her being her first adult car was a Bing Cherry 2201 Firebird GT with white walled hover trim and chrome accents. From pictures that I could find it looked like a slick piece. Looking back to the screens I couldn't help but feel that itch again. I couldn't explain it. That prickly feeling of ice needles again, this time in the back of my skull. As much as I'd hate to admit it. I think Cerith is right. I sighed heavily before saying "send me everything. I'll open a new case file and have the team start working on it first thing." She made a happy noise and bounced slightly, clearly satisfied with my decision to take it on. I reached out and to my left and a glass was placed gently into my hand by Tabitha. I hadn't even realized she had come over while I was working and was now ready for that drink. Room temperature rum and cola. The drink went down smoothly enough considering I drained the glass in one gulp, during which time I finally got a good eyeful of Ceriths current form. Or rather the underside of part of it. From what I could tell she was wearing a black T-shirt. That was it. I put the glass back down, it's job done without moving my head and said, "What a lovely view Cerith. I'm guessing you chose this to try to get a rise out of me?" I couldn't lie though it was affecting me, but I couldn't let her know that. Not when she's like this. Otherwise she'll continue to tease me till she leaves me with the absolute worst case of blue balls this side of the City. Her hands came up and cupped my chin almost lovingly, and her voice said "Of course Detective. Do you not approve? Or would you rather I change back to my medical examination form? Or something else?" Her words dripped with implied sex. I groaned, loudly, and said, "This is fine. Jesus Cer." Before we could continue our most scintillating of conversations there was a sudden PING! And DANNA said, "I'm sorry to interrupt, but there's a message for you Cerith. It says 'If you can get to the department Cerith, do so. We need you to explain your paperwork. And if Detective Julius is still with you have him come in too.' signed the Chief. Would you like to reply?" 'Shit, I forgot the morgue kills all signals,' I thought to myself as I stood up gently (regretfully) prying myself from Ceriths grasp with a, "duty calls. Need a lift?" I stretched gently, the scales in between my shoulders clicking appreciatively for the stretch, and turned around to notice she was indeed, just wearing a black T-shirt that hugged her voluptuous figure closely. The scales in my back clicked shut in surprise. Cerith let out a small cute chuckle, "I see after all this time I can still surprise you," she said blowing a kiss my way, reminding me of a little Gothic pixy. I rolled my eyes away from her and willed my scales to relax. I grabbed my jacket off the back of the chair, slinging it on and clicking the neck clasp shut under the cord that connected my scales to the unit in my head. I was awarded the cybernetics upon completing my training and getting all my licenses to have them. The force had allowed me to customize it, I had chosen top of the line. A dual unit with custom built AI. The individual scales were ceracoated titanium microprocessors all running in both series and parallel, and could move to expel heat or react. The main unit was the same except it was one solid unit that replaced a chunk of skull. Once that was done I zipped up the front of the leathers and ran the scales through the racer setting. They clicked and flattened against the outside of the jacket, securing it to my back. I shrugged making sure it was comfortable. "I'll take the fact that you're only in a t-shirt you'll be along shortly?" "Certainly detective." Her voice was filled with dismissive submission... And sadness? I looked back at her and noticed her makeup was gone. Or had she had any on in the first place? I gave myself a mental shake. There's no way. This was Cerith, veritable goddess of the necropolis. I put the last few minutes away for review later. Chief called. I have to go. On an instinct I thought long dead, I reached out and squeezed her hand. I felt a slight squeeze back. And then she let go with a, "Go on, be a good detective. I'll be along shortly." I left with Tabitha as my guide. Before Cerith disappeared into the darkness I thought I heard her whisper, "please don't leave." My scales raised in a saddened response. I couldn't be sure I heard her right though. If I heard her at all. I reached back and stroked them, knowing my ai probably heard her, and knowing it could feel me touch the scales. After a few seconds the scales settled down. 'I know buddy,' I thought to the AI. It couldn't respond like usual AI. The force thought that was too dangerous. What if it went rogue? What if it tried to kill the host and take over? The list went on and eventually they decided the basics were ok. When I got my unit one of the first things I did was jack it into a diagnostic to see what kind of hardware I was dealing with exactly because manufacturer specs from real use are sometimes different with AI if the bits and bobs are in place. When I did, all I got on the screen was 'Hello?'

Hey guys took the exam on Monday, May 21 online. Was on pins and needles until I received the results today roughly 24 hours later. Super excited as I received Above Target in all 3 domains! I honestly wasn’t sure if I passed or not but I felt good about it! At first I wasn’t sure because I just received the “congrats for taking your exam, your results will be sent within the next 5 business days” message. I wasn’t sure if that was good or bad because when I took the CAPM online I did receive a provisional passing message at the end of the exam.
I took the CAPM back in March as my employer required it for a promotion. I passed the exam with T/AT/AT/BT, and that is honestly what gave me the confidence to go for the PMP. I figured I was already in the habit of studying daily and I might as well take the PMP as well while I still have the knowledge fresh in my mind. So I scheduled the PMP exam for May 20.
I primarily relied on SH, taking all the practice questions and exams and redoing the ones which I did poorly on. I did take the full mock exams 3 times. Averaging a 65% total which I thought was low and I kept doubting myself on whether or not I was ready. I found that I wasn’t taking my time to read questions or all answers thoroughly before answering. So come test day I committed to taking my time to read slowly. I also purchased AR’s PMP course which I only listened to while at the gym, mowing the grass or in the car.
The test went well, I used all the time available and used both breaks (2 10 min breaks at question 60 and 120). I flagged questions I was unsure of for each section and reviewed at the end to make final decision. I used the strike through tool as well to cross out answers I knew were incorrect. There were 2 or 3 matching questions which I thought were easy and I believe only 1 formula related question. Lots of agile practices questions. I honestly think the SH questions were slightly more difficult.
Advice - do SH daily, figure out what areas you’re scoring poorly in and focus on those. Don’t doubt yourself. Read questions and all answers thoroughly. Tell yourself “YOU GOT THIS”. Cheers. -R

I was thinking on also discussing Aetna and Humana within the model as well, however in the interest of time and effort, I may have to revisit those 2 creatures at a later time - not because they are particularly difficult, but that it would be a similar rehash of UNH and BHG. The minutiae of improper population model care and investment made by CVS/Aetna and Humana is in my personal belief the reason why their MCR exploded, and perhaps inadequate preparation for CMS V28 blend adjustment also played a role. Even though CVS/Aetna has an adequate service sector, Humana does not, and we may see further margin divergence of these 2 behemoths.
Without further digression, let us begin:
Vertical consolidation - what is currently happening
"Consolidation allows these vertical conglomerates to use capitated payment structures in ways that drive profits without creating value. As noted above, corporate consolidation in health care has existed for decades, largely driven by hospitals. The primary result: hospitals can negotiate higher prices in the commercial market, cut labor costs and demand more of clinicians, and exploit various loopholes in the fee-for-service Medicare system. In addition, private equity (PE) rollups in health care have risen dramatically over the decades, also exploiting fee-for-service reimbursement and threatening patient care."

The damage to physician morale is especially acute during this consolidation process.

The last one from Corvallis is a particularly spicy acquisition - the change healthcare cyberattack created a cash flow crunch within Corvallis, creating the impetus for Corvallis to then sell itself to Optum. If I were to use cruder terminology, the analogy is this: Optum/Change groomed Corvallis to be dependent on its "service", and once Corvallis is dependent on Optum/Change, Optum/Change then "convinced" Corvallis that Corvallis needs to sell its body (clinics) to Optum for the couple to stay together after Optum/Change failed in its basic "pimp" duty on protecting the groomed subject. Absolutely disgusting analogy, and I hope it brings the message across - what Optum did was absolutely abhorrent and should have paid Corvallis interest and damages, but instead gets to steal the Damsel-in-distress. Who said pimping doesn't pay?
"Today, as policy moves away from fee-for-service, analogous patterns of corporate consolidation are emerging under capitation-based financing. While vertical conglomerates promise clinical integration, familiar harms are emerging, such as the push toward “productivity” medicine and the replacement or supplementation of physicians with less expensive advanced practice providers."
Ok, I am not sure if that last part was absolutely necessary. Advanced Practice Providers (APP) are Physician Assistants (PA), Advanced Nurse Practitioners (APRN), or in the case of Anesthesia, CRNA (Certified Nurse Anesthetist) who work under a supervision of an overall Medical Doctor (MD) for the practice. In the current labor shortage, there are not enough physicians to make up for lack of physician help. I would rather work with our APP colleagues on creating a more equitable platform, and this paper is not helping the cause in this manner. Vertical integration can bring plenty of harm, but having APP being part of the Team is NOT a problem. In fact, I have worked with many APP who are wonderful healthcare colleagues and we all want the same thing: equitable treatment of our patients in the most humane, effective, and medically appropriate way possible.
I. Gaming capitated benchmarks:
"Vertical conglomerates in capitation-based models are keen on controlling primary care physicians, who are essential for inflating risk-adjustment payments and quality bonuses. In MA alone, gaming of risk adjustment is responsible for at least $23 billion in annual overpayments to insurers, while quality payments amount to another $10 billion in subsidization without demonstrated value. In addition to increasing costs, the preoccupation with risk-coding and box-checking wastes scarce time with patients and contributes to dissatisfaction among clinicians."

We on the field AND CMS knows this is happening. In fact, CMS is attempting to stop this practice by the rollout of CMS V28 AND the April 1 2024 Contract Year 2025 Medicare Advantage and Part D Final Rule (CMS-4205-F). What CMS is attempting to do is to decrease the capitated payments on risk adjustment and quality bonus payments to elongate the Medicare Trust Fund depletion runway, which hopefully also will force the MA companies to take care of patients. As was seen in our 24Q1 DD, companies will instead retrench, retreat, and cut benefits instead. this indicate a potential market failure of the capitated model payment. We should readdress this point in a future post.

With the Change Cyberattack in February 2024, UNH unleashed the biggest cybersecurity threat on our patients' medical history in this nation's history, all because someone forgot 2 factor authentication on a whole server worth of data. I do take exception to the samples argument. Look, we just got out of the VERY SHADY stuff happening in the 90s with MD offices taking a HUGE backhand deal when giving out samples, I don't really want to see that stuff back in vogue again. Do you know how frustrating it was for a patient to say they are on a med that is from their doctor's office, they got it for free, and they must have it as an inpatient, when they don't even have it on the medication fill list or on the electronic medical record database? "Specialty Pharmacies" are basically pharmacies who carry high dollar cost items that may have more stringent FDA counseling requirements, but because now the Marketplace is littered with PBM funnels, most independent pharmacies cannot "enter the foot in the door" of the PBM requirements. Guess who does though? Optum Rx!

III. PATIENT “FLIPPING” AND ENROLLMENT ARBITRAGE

CMS is acutely aware of this gaming, however their model hasn't exactly been able to pinpoint how to address this yet. This was discussed in the CMS Final rule comments section.
"More direct than marketing, provider acquisitions allow conglomerates to “flip” patients into MA plans or ACOs by coordinating efforts with sister provider subsidiaries. Conglomerates can initiate the flipping strategy by using some of the steering tactics discussed above. In another suit, UnitedHealth, with 50% of the MA and commercial insurance markets, allegedly attempted to force the sale of local practices to Optum. Hello all! I've been stalking this subreddit for the better part of two months trying to learn as much as possible. This has been so helpful and uplifting that I truly do feel like I'll feel better with time.
While I was only diagnosed a week ago I had suspected I had thyroid issues from the moment I started browsing here.
A few questions I still have though are regarding treatment.
Doctor prescribed levothyroxine 50mg. I have started taking it but as I do more research I'm starting to wonder if I should or should not take it at this time. Today marks the 4th day that I've taken it.
I've had blood work 3 times in the last 6 months.
In Nov TSH was 5.05 T4 was 1.2
In April TSH was 5.64 T4 was 1.0
A week ago TSH was 4.24 T4 not tested. TpoAb 442 Thyroglobulin 8
I understand the antibodies being present very likely indicate hashimotos. However my TSH levels were back within the "green" range on the mychart app.
Doctor wrote the prescription after the most recent panel.
Reviewing my current symptoms
I have a noticeable but not debilitating goiter. Seems to be getting worse lately. I felt cold today (it was 87 degrees outside but the AC inside was chilly). Then I started to feel super hot. Before taking the meds I used to have palpitations but they seem to have gone. I had overwhelming dread and anxiety but they seem to be gone too.
As I'm sure many of people are searching for answers I'm just trying to find out if these symptoms are common as I work towards balance or if I should be more concerned.
As with any autoimmune disease I want to make sure I'm not missing anything.

Hello Y'all! I am just reposting the update and adding some insights in case my last post was removed.
OLD POST << Hello, folks! Some people here must have seen my last post about my experience with folliculitis and how it wouldn't stop coming back no matter what I did ( even after a ton of antibiotics and other medication for 6 months ) and I think I found out why. After endless textbooks and science papers, I finally came across some interesting papers :

There's a parasite that is so small and easily caught ( they come from animals, mostly dogs and cats ) that has the same characteristics of bacterial folliculitis. In fact, in some of these papers, it is said that if the antibiotics and nothing else works, it's probably this thing. They don't appear on blood tests and sometimes you can't see them even with a biopsy.
( Doctors found out because of a guess, actually. )
One thing that I did not find on textbooks and was surprised to read is: bacterial folliculitis rarely itches. Itching is the main sympton of parasitic folliculitis, in fact, the itching is worse at night. Although it was studied in 1929 at first and they are found everywhere ( principally in tropical and subtropical countries) most doctors end up missing the clues because these little f**ks are not that well known and they don't show the usual characteristics that parasites show ( for example, at first I thought they were scabies, but mine are not contagious so I ruled it out ) but this kind of parasite can be non contagious too.
By the way, I must specify that other kinds of folliculitis CAN itch too. Here they are talking about the difference between bacterial and parasitic folliculitis.
So it explains those past symptoms that I previously talked about :
⦁ Comes back after medication even if weeks later.
⦁ Sugar can make it worse ( parasites like that )
⦁ Heat and sweat make it worse.
⦁ There are no other symptoms like fever or pain ( apparently, the larvae can't reach deeper skin, so it stays trapped on the outer layers of skin. )
⦁ The lesions appear some days after first contact and can take very few days to appear other lesions.
⦁ The lesions follow a TRAIL. They can be linear or just jump from one place to another. They are always close initially before going onto other places.
⦁ It started on my thighs, but besides the thigh it is also common to appear first on the skin of your feet or above the gluteous. And then it goes to the rest of your body.
Yeah, so Doctors can end up misdiagnosing because it can look like scabies or bacterial folliculitis. As one of the papers said, it can last years because the parasite can die but come back. A person can get them anywhere but it's most common to get them after having contact with water or soil contaminated, even stepping on it for a second can be enough.
Another interesting thing is that infections of the skin can look different on everyone, no matter the race, so sometimes they can be parasitic but look like bacterial, virus, fungus folliculitis. Most of them usually have a brown dot in the middle though.
So I ask you guys to read those papers, they may help you. They talk about the medication that was used and apparently it's rare for them to have side effects, and of course, be careful with self medication, if you can please contact a medically qualified professional. I know that choice is not available to everyone but if you can, a second opinion can be very helpful.
Today I went to check with my doctor and everyone was very happy to confirm that that was the actual problem. So I am happy to share this information with you guys.

It's working. As I answered someone else back there " The good news is that was exactly the problem. The bad news is that folliculitis is a very uncommon thing to get when it comes to parasites and it's the hardest to get rid of, unfortunately. They basically say " It's hard to get rid of, take the meds until it's gone" because that's something that doesn't happen every day and the doctors and scientists are kind of not totally sure what to do. So I'm taking 12mg ivermectin every monday ( two pills of 6mg ) because that's usually what people do and works most of the time. At the start I had them all over my body, but then months later I took ivermectin for three days and they kind of died, so I had to stop taking any kind of medication for 30 days to take another lab test, but surprise, the lesions did not grow like before ( every 3 days ) , for a whole month I only got two or three new ones and they would die almost immediately. It really depends on the person how many times you'll have to take the medication, in my case , my immune system and overall health is not the best so it makes a bit of sense that it's taking a while."
I'll add a part of Cutaneous larva migrans with folliculitis: report of seven cases and review of the literature here as for some reason the paper is not uploading :
[ We reviewed all cases that were seen in our department from April 1991 to April 2000 of CLM in which creeping eruption was associated with folliculitis. Patients were included if they presented with localized folliculitis and creeping eruption. Folliculitis was defined as pustular follicular cutaneous lesions. Creeping eruption was defined as one (or more) linear or serpiginous mobile cutaneous track(s). The following information was recorded: age, sex, recent travel history, country visited, time from return to clinical onset and presentation, clinical signs, treatment and outcome. All the patients received oral antihelminthic agents, plus topical treatment in one case. Treatment options for CLM in our department include a single dose of oral ivermectin 12 mg weekly until cure, and oral albendazole 400 mg twice daily for three consecutive days.
Five patients were cured with one to three courses of ivermectin (unit dose 12 mg), including one also treated with thiabendazole ointment. Two patients were cured with albendazole 400 mg twice daily for three consecutive days. Lesions of folliculitis took longer to clear than those of creeping eruption.
Folliculitis is an uncommon clinical form of CLM, as only five cases have been reported in the literature. However, it may be more frequent than previously thought. For example, in a 2-year prospective study, CLM was observed in 69 patients, two of whom also had hookworm folliculitis. Folliculitis in a patient returning from (or residing in) an endemic country should lead to consideration of infection by a hookworm (or other nematode), even though bacteria are the most common cause. All the cases of parasitic folliculitis in our series and most of those found in the literature were pruritic, in contrast to most forms due to other microorganisms. The diagnosis of parasitic folliculitis was based on clinical findings alone. Histological studies are unnecessary, but show nematode larvae trapped within the follicular canal, stratum corneum or dermis, together with an inflammatory eosinophilic infiltrate. In three cases, the larvae were identified as Pelodera strongyloides, a free-living nematode belonging to the order Rhabditoidae. In another case of folliculitis the larvae were identified as Ancylostoma caninum, a hookworm. The pathogenesis of parasitic folliculitis may be related to an allergic reaction to the presence of the nematode larva in the follicular canal.
The remaining patient's clinical signs subsided only more than 2 months after presentation. In one case, thiabendazole eradicated the creeping eruption but not the follicular lesions. Our experience is similar. The clinical response to a single 12-mg dose of oral ivermectin was less favourable in our patients with folliculitis that in most other patients with CLM. A single dose of ivermectin cured two of five patients in our series, whereas it usually gives cure rates of 81-100% in CLM. Treatment was repeated until the pruritus disappeared, this symptom being indicative of parasite viability. In conclusion, folliculitis should be added to the dermatological manifestations of CLM. The clinical diagnosis is facilitated by the pruritic nature of the lesions and their association with serpiginous tracks. Treatment is more difficult than in classical forms of CLM. ]

KHZMALYAN (European Party of Armenia): The referendum is our suggestion and demand. We presented it to Pashinyan and expect a response within a reasonable timeframe. We advised making a public statement soon. A referendum would also make Pashinyan's job easier by allowing the people's will to decide Armenia's direction.
REPORTER: How did Pashinyan respond?
KHZMALYAN: He suggested the meeting be held in a closed format so I can't reveal all details. We hope that our voice, our offer, our demand, was heard. If we do not receive a positive response, we will take measures by employing the same methods currently used by anti-European forces [the ongoing protests led by pro-Russian ex-regime]. We will take to the streets and demand a referendum for Eurointegration if necessary.
REPORTER: How long are you ready to wait before taking action?
KHZMALYAN: A few months. Georgia will hold parliamentary elections in October, and Moldova will hold a similar Eurointegration referendum - also in October. The U.S. will hold presidential elections in November which will be important for Armenia's future. Even if the worst scenario unfolds in all three countries, Armenia must stand firm and not take a step back. Armenia could become the leader in democracy in all of Eastern Europe if you consider the situation in Georgia, Hungary, and Slovakia [pro-Russia populist leader]. Armenia could receive an unprecedented level of assistance from Europe under those circumstances.
REPORTER: Did Pashinyan say he plans to leave CSTO?
KHZMALYAN: We did not demand specific dates but we urged him to complete all these processes this year, preferably by November.
REPORTER: Did Pashinyan agree?
KHZMALYAN: The participants of the meeting are under the impression that we were able to present convincing arguments but time will show what actions he takes. The government functions very poorly but nevertheless, they are doing something, they are taking steps.
REPORTER: On May 9, Bagrat Galstanyan gave Pashinyan 1 hour to resign but it's May 21 and he is still here. Why wasn't the opposition able to remove him?
KHZMALYAN: The 3 former presidents and their surrogate forces, backed by 30 years of power, were unable to bring forward a figurehead, a single ideology, that could win the hearts of the people. They used various political technologies, they switched between black and white robes, and they extended the 1 hour to 7 days, but none of that helped because the Armenian people hate them more than they hate Nikol Pashinyan.
REPORTER: You used to be a lot harsher on Pashinyan but something appears to have changed in your tone after your recent meeting with him. What is the reason?
KHZMALYAN: We were able to present our demands and the timelines to accomplish them. Unlike the archbishop and the former regimes surrounding him, we did not give "one hour" for Pashinyan to accept our demands; we did not make ultimatums. We believe the current government is carrying out and will carry out the will of the people to overcome the status of a Russian colony and join the ranks of free nations. We did and do see steps [taken by the government] towards this; we cannot ignore this. We saw the Strassbourg, but also Moscow. We saw the [April 5 meeting in] Brussels, but also [the recent meeting between FMs in] Almaty. We are told this is an attempt to "balance" and we understand where they are coming from, but our factions speak on behalf of over 60% of Armenians who view Russia's policies as deadly for Armenia. We expect concrete actions and we have formed a platform to continue this work; there will be petitions and other actions from us. We are ready to assist Pashinyan, who may be constrained due to ongoing negotiations, by being the public voice and saying the things he cannot say. We will continue to work, to pressure, to influence, to demand. Our meetings also continue abroad. We will press on until we reach our goal - the Eurointegration of Armenia. //

REPORTER: ... the "holy movement", by calling it a "pagan" movement. What is the ruling party's stance and do you discuss this internally?
ALEXANYAN: Of course we do, although I don't think Kocharyan's decision to name this movement "holy" is helping them. It's comical.
REPORTER: Do you have proof that Bagrat srbazan is [tied with Kocharyan]?
ALEXANYAN: Bagrat Galstanyan is surrounded by the entire political team of Robert Koharyan, he was a member of Kocharyan's 2022 protests and gave similar speeches he gives today, he received a praise from Russian propagandists, and he was named by a Russian propagandist Aram Gabrielyanov as a potential candidate for a new protest movement just a few months prior to these events, so it's beyond reasonable doubt that Bagrat Galstanyan is part of Kocharyan's team.
REPORTER: You were earlier accusing Galstanyan of being a Russian agent, and now Kocharyan's agent?
ALEXANYAN: Same thing.
REPORTER: The opposition gathered many thousands of supporters on May 9. What does the size of the crowd indicate?
ALEXANYAN: It was similar to Kocharyan's final rally before the 2021 elections. And?
REPORTER: The opposition created a video of "Pashinyan's lies" and broken promises. Will you respond?
ALEXANYAN: The clip itself is a lie and manipulation. For example, one of the supposed lies was Pashinyan's promise of an amnesty for overdue fines and penalties. You are a journalist, you tell me, was there an amnesty or not?
REPORTER: Pashinyan had also said that POWs could wait "just a few more months" and that he wouldn't "sacrifice Armenia's sovereignty" in exchange for bringing them back sooner. Why aren't they back?
ALEXANYAN: Dozens of POWs returned home in December, and efforts continue to bring back the rest. Literally 1-2 days before the POWs' return, the opposition claimed that the POWs were "forgotten" by the government. "Ոչ մի բան չեք անում:"
REPORTER: What about the Nagorno-Karabakh leadership?
ALEXANYAN: There are efforts to return them as well.
REPORTER: Why did the police form a wall to block entry to village Kirants [for nonresidents]?
ALEXANYAN: I think the NSS said they had to take action to prevent interference with the border work. From what I can tell, the protest leader himself went there yesterday and returned only 15 minutes later so there wasn't a big desire to stay there apparently.
REPORTER: They plan to hold a big rally on May 26 where they might nominate Bagrat srbazan as the PM's candidate to replace Pashinyan.
ALEXANYAN: Why not go straight for [alleged narco baron] Mihran Poghosyan [who publicly praised the protests]?
REPORTER: There appears to be a consensus around Bagrat srbazan after a meeting between several forces.
ALEXANYAN: Consensus minus one, where "minus one" is the people. Or to be more precise, "minus two", because the Constitution also prohibits Bagrat from serving as a prime minister. Congratulations on an initiative that's dead on arrival. It's absurd. They suggested replacing the Constitution to allow a dual citizen to serve as PM. Okay... To get Canadian citizenship you have to take an oath to serve the best interests of Canada. To allow dual citizens to serve as PM in Armenia we have to accept for the PM to have loyalty towards another state. The alternative is for the candidate [Bagrat] to admit that his word [oath to Canada] wasn't worth a dime. Երևի մատերը խաչ արած ա երդվել։
REPORTER: But don't you think you are manipulating because Galstanyan received Canadian citizenship just to be able to serve in the church in Canada? He didn't go there to live or conduct business.
ALEXANYAN: It doesn't matter why he went to Canada. He is a citizen of Canada and took an oath. Even religious leaders with dual citizenship cannot become prime ministers. I don't have an issue with him being a dual citizen, but it limits his office options. He still hasn't renounced his Canadian citizenship even after all these talks about a possible nomination.
REPORTER: Galstanyan said he is ready to renounce it as soon as there is a need for it.
ALEXANYAN: In other words, he seeks "guarantees" that he will be the candidate in order to renounce it?

A Serj-era judge is leaving the Constitutional Court after receiving a job offer elsewhere so one seat is vacant. It is the judiciary's turn to nominate a CC judge, so hundreds of judges recently held a meeting and voted for Judge Balayan as their candidate. Balayan's father was a CC judge during Kocharyan's tenure. He was accused in the Wikileaks report of having inappropriate deals with President Kocharyan. OPPOSITION MP: The Soros Foundation publicly listed him as a member so I don't understand why it's being hidden now. In your opinion, how will his work at the Soros Foundation affect his impartiality?
JUDICIARY REP: The majority of judges chose him and it's up to Parliament whether to confirm or reject.
RULING MP: Was the voting held in a closed session or can you see how each judge voted?
JUDICIARY REP: It's a closed format. My duty here is to present the candidate. [basically, leave me alone I'm just doing my job]
RULING MP: Why not make the court voting and internal discussions more transparent for the public to see so we will have fewer conspiracy theories on why one judge was preferred over the other?
JUDICIARY REP: I share the view that it needs to be as public as possible but the majority of judges voted to keep the session closed to the press.
RULING MP: Just because the discussions and the vote were behind closed doors that doesn't mean it was anti-democratic, but still, it's better to publicize the process. We hope to see a change in this approach.
JUDICIARY REP: The ballot casting will always be secret and I think that's the right approach in order to allow judges to express themselves without constraints, but I agree that the discussions and speeches should be public. Either way whatever we discuss behind closed doors always reaches the press so in practice there isn't really a major transparency issue there.
CANDIDATE KHACHATURYAN: First, every part of my biography has always been public and part of public discussions. Second, I have always voted in favor of making internal discussions open to the press; it's the best way to avoid incorrect assumptions. Full speech here.
OPPOSITION MP: Congratulations. The ruling party intends to approve your candidacy just as they voted for you in the past for other positions. In all of those instances, you didn't mention your work at the Сорос Foundation. Now, a very interesting coincidence [drumroll... are you guys ready for this bombshell?], during the same time while you worked at Сорос foundation, in 2006-2013, you were also a U.S. Embassy employee [got 'em]. //

As for my simultaneous work with the U.S. Justice Department, there was no conflict with my membership or presidency at the Board because the latter was fully a public activity and we were only given a symbolic sum to compensate for our expenses; we are talking about less than $100/mo. I will let you be the judge regarding an agent of influence. There is a lot that's missing from my biography that was read here [lists some boring constitutional research work, etc.].
OPPOSITION MP: You keep being "promoted" by the ruling party without having the chance to finish your terms. The longest job you've ever held was at the Сорос Foundation - 8 years. The brother of the head of the Anti-Corruption Committee, who is about to join the CC, cannot be impartial. Again, what guarantees can you present that you are not a [Western] agent?
CANDIDATE KHACHATURYAN: Your expectation for guarantees stems from the assumption that the [Open Society Foundations] were engaged in activities other than the ones they were actually engaged in. I won't present any "guarantees" because there is no "threat" to begin with. As I've said, that part of my biography has always been discussed publicly and the activities were public and covered by the press.
RULING MP: In your speech you spoke about the judiciary in the U.S. and brought examples. The U.S. doesn't have a Constitutional Court. Don't you think we should also merge the Constitutional and [Cassations Court] with the upcoming referendum to have one Supreme Court with corresponding chambers?
CANDIDATE KHACHATURYAN: I support having one unified body like in the U.S., or clarifying the duties to prevent each body from interfering with the other. Lengthy answer.
OPPOSITION MP: A person [in Kirants] received permission [from USSR] to build a house and live in it. Then the [Armenian] government told him there was a mistake [after the clarification of borders]. Some 50 years after this incident, what would your verdict be in this case?
CANDIDATE KHACHATURYAN: I understand the context. This issue is the task of the Prime Minister and Foreign Minister and we cannot interfere in each other's institutions. I assume there will be a peace agreement eventually that will be sent to the Constitutional Court for examination, therefore I will refrain from making prejudgments now in order to be allowed to take part in future processes.

About a month ago I decided to purchase a used 80d, 24mm ef-s stm, and 15-85mm. Money was a factor, i could have gone with a 6d and 40mm, which would likely cover the majority of my photography. I also have a Nikon D700 and a couple lenses for it that are nothing special but work ok. My thought with the 80d was to have a more compact camera than the D700, and with a more functional live view for those odd angled shots. Also my D700 is old and gets cranky sometimes and refuses to let the mirror back down if its cold.
Well...I think I f-cked up. Multiple reviews and comments led me to believe high iso performance would be sufficient for my comfort level at say 3200iso abd in a pinch maaaaaybe 6400. Welp, It's not great. I get past 2000iso and things start getting noisy quickly. P mode with auto ISO turned on is also weird, like it will crank the iso and drop the shutter speed, but leave the lens stopped down quite a bit. I've played with settings but I'm just setting iso manually since it will always max it out indoors.
This was all disappointing, but i got that sweet 15-85 so it's not all bad news... except the thing is like a heavier, more versatile mediocre kit zoom. The image quality, even stopped down a bit, is unimpressive. Not completely bonkers like there's something wrong, just not on par with my expectations for the cost of it, even used. The 24mm is ok, not spectacular but definitely works.
So now I'm thinking I should have gone with a 6d. The shit part is that if I trade all this in, I'm losing about $150. I know, I can sell on Craigslist or Facebook marketplace or something like that, but I have no interest in dealing with that crowd. I looked at eBay, which is barely better than the aforementioned local options, abd with fees and everything it's not worth the hassle.
So I'm thinking my options are:
1) trade the 80d and lenses in for a 6d and 40mm.

I scored 488 on a diagnostic and ended up with a 515 on the real thing. On top of normal advice everyone gives, I wanted to share a couple tips that worked for me.
  1. My CARS was pretty stagnant around 125-126 with minimal improvement since I started studying. About 3-4 weeks before my exam I essentially gave up on CARS and used those 1.5-2 hours each day to do more of the other three sections. I maybe did 1 passage per day, but not the 4ish per day that I thought I should do. As expected, I scored a 126 in cars on the real exam but my other sections made significant improvements. I by no means am recommending this to everyone, just sharing it was something that worked for me. I know that may be lower than ideal for some schools but I probably would’ve ended up with a lower overall score without doing this. I was able to get my other sections to 129/130 by focusing more on them. Also, I’m shooting for low/mid tier MD schools so there’s that.
  2. Don’t be afraid to take your time reviewing your FLs. Some of my FLs I ended up taking close to two days just reviewing everything, and I felt like I was wasting time but in hindsight that’s where I made lots of my progress as well.
Also, it’s possible!! Don’t get me wrong it will be A LOT of work if you started where I did, but I believe everyone can do it with the right approach. Happy to answer any questions.

I am worried because of my relatively low UW gpa and because most asian males who get admitted to t10 unis usually have 4.0s while I will have a 3.89 when applying. This is not a shitpost 😭
Demographics: Asian male, no hooks
Household income: 400k+, going to pay full tuition
UW/W gpa: 3.89/5.45 top 10% of graduating class (expected when applying to college)

Started nonprofit that increases access to research opportunities for hs students in my city: Matched 25 hs students to undergrads to learn about research skills and develop a research poster in their area of interest, held workshops, developed an app to facilitate collaboration between hs students, fundraising a couple thousand dollars as scholarship money for a cumulative symposium where these hs students can showcase their work.
  2. Student researcher at a surgical oncology lab at a top 5 med school (going to have done it for 4 years: Researched surgical techniques and will get co-authored on paper(s) along side surgical oncologists.
  3. Play Tabla (indian percussion instrument) (going to have done it for 4 years): Took classes since I was in 1st grade, taught a group of 5 students the basics, played at different temples.
  4. Volunteer Coordinator (and volunteer) for an Alzheimer's Care Home (going to have done it for 4 years): Increased number of volunteers by 300%, assisted in patient-care duties, helped feed patients, assisted patients through bingo games and art, set up an art gallery full of the art pieces produced by these patients over the course of 2 years with 40 attendees, learned the basics of Korean and Memoni languages to effectively communicate with certain patients.   5. Orthodontic Assistant (going to have done it for all 4 summers): Shadowed a dentist and helped with miscellaneous stuff around the clinic.
  6. Literature Review Lead at Cystic Fibrosis nonprofit (2 years): Led a team of 6 through weekly literature review meetings on prophylactic antibiotics in relation to cystic fibrosis. Was working on writing paper, facilitated the creation of 20 Cystic Fibrosis awareness posters, led 3 info sessions raising awareness for cystic fibrosis.

[Rules for combining types]
  1. The square brackets [ ] show the order for combining. For example, you first combine the types inside the smallest square bracket and then you start using the types outside of that bracket to change certain attributes of the types in the smallest square bracket. (Rule 12).
  2. The letter A (affector) is used for types that affect E or M types.Types in a double type combination can not be both affectors.
  3. The letter E (effector) is used for types that get affected by affector types. Types in a double type combination can be both Effectors. Being an effector type bring out the type's natural rules such as choose a number or choose wh- questions in double type combinations, but does not combine them. Two E types can be unusable if there is no A type next to them. Additionally, if every type in a combination is an E type, it can be a switch combination.
