WARNING! This scenario is designed to make you think! No reputable physician would ever prescribe this list of drugs for a patient, and some of the doses are wrong and some instructions that go with the doses are wrong. Have fun with this! You will encounter patients who have a bushel basket of drugs, many that are redundant. The idea here is to learn as much as you can about these drugs, why they are given, usual dose ranges (no need to try to remember them—that will come soon enough as you encounter the same prescriptions over and over), and try to spot danger signs from polypharmacy. Don’t worry about being wrong. If you fall flat, I’ll write explanations for you and let you pick yourself up and try again! But do try to figure it all out. This should be a fun experience, not one to fear or loathe. Trust me, you will see patients who are over-medicated. This exercise is designed to point out some of the common areas of drug duplications and over-kill. GG.
Scenario: Your 90-year-old nursing home patient, Aunt Maude, suffers from Alzheimers, hypertension, renal failure (Class III chronic kidney failure with an average glomerular filtration rate of about 45 and a creatinine of 1.3 mg/dL), chronic heart failure, angina pectoris, and cardiac dysrhythmias such as atrial fibrillation. She has anxiety and difficulty sleeping. She also suffers from depression and bipolar disorder. She weighs approximately 110 pounds. She has had a number of different physicians over the last several years as the nursing home’s attending physicians have changed. She has not actually been examined by the nursing home’s newest physician, who has been on board for about 8 months, nor had any trips to the ER over the last year. She currently has prescribed the following medications, many of them prescribed by different doctors; however, she is taking them all daily as administered by the nursing home staff. She does well over-all, but seems lethargic and tends to sleep a lot. She often does not want to get up and exercise or go to the dining room to eat. She suffers from “flat affect” and usually just either stays in bed or sits in her chair and nods off.
Fluoxetine (Prozac) 40 mg po qd: Can treat depression, or OCD
Haloperidol (Haldol Decanoate)10 mg po tid: Antipsychotic
Lisinopril (Prinivil) 10 mg po qd: ACE inhibitor It can treat high blood pressure and heart failure.
Atenolol (Tenormin) 25 mg po qd: Beta blocker, that can treat high blood pressure and chest pain.
Digoxin (Digox) 0.250 mg po qd : Antiarrhythmic and Blood pressure support
Prazocin (Minipress) 10 mcg po qHS: Treats high blood pressure and uninary retention
Metoprolol (Lopressor) 50 mg po q12 hours: High BP, Chest pain, Heart failure
Tylenol (Acetaminophen) 325 mg tabs take 2 tabs po q6 hours PRN for joint pain: Analgesic for minor pain, fever
Diltiazem (Cartia) 60 mg tabs take 1 po q6 hours for atrial fibrillation: Calcium channel blocker and Antihypertensive drug
Nifedipine (Procardia XL) 20 mg tabs take 1 po q8 hours for angina: Calcium channel blocker and Antihypertensive drug
Furosemide (Lasix) 80 mg tabs take 1 po bid: Diuretic for fluid retention
SlowK (potassium chloride) 8 mEq tabs 2 po bid: Electrolyte replenisher
Amitryiptyline 150 mg tabs 1 po qHS: Antidepressent nerve agent
Propranolol (Inderal LA) 80 mg 1 po qHS: Can treat high blood pressure, chest pain (angina), and uneven heartbeat (atrial fibrillation)
Phenelzine (Nardil) 15 mg 1 po bid with a glass of red wine: Antidepressant It can treat depression.
Nitroglycerine (Nitrolingual) tablets 0.4 mg place 1 tab under the tongue PRN for chest pain. May repeat 3 times in 15 minutes: Vasodiolaters It can treat and prevent chest pain (angina) in its oral form
Hydrocodone bitartrate and acetaminophen (Zohydro ER) 5/500 1 or 2 tabs po q 4 – 6 hrs PRN for pain:: treat severe pain of a prolonged duration, if other measures are not sufficient.
Alprazolam (Xanax XR )2 mg tabs take 1 po qid for anxiety: used to treat anxiety and panic disorders. It belongs to a class of medications called benzodiazepines which act on the brain and nerves
Motrin (Ibuprofen) 800 mg tabs take 1 – 2 qid PRN for pain: Nonsteroidal anti-inflammatory, It can treat fever and mild to severe pain.
Meperidine hydrochloride injection (Demerol) 50 mg IM PRN for joint pain: It can treat moderate to severe pain.
Hydrochlorothiazide (Microzide) (HCTZ) 50 mg 1 po bid: Diuretic- It can treat high blood pressure and fluid retention (edema).
Bisacodyl (Dulcolax) 2 tabs PRN for constipation: Laxative- It can treat constipation.
Olanzapine (Zyprexa) 10 mg. Dissolve one waver in saliva qHS: Antipsychotic It can treat mental disorders, including schizophrenia and bipolar disorder.
Ziprasidone ( 20 mg tabs take 1 bid: Antipsychotic: It can treat schizophrenia and bipolar disorder.
Lurasidone (Latuda) 20 mg PO qd : Antipsychotic It can treat schizophrenia.
Exelon® (rivastigmine) 3 mg tabs take ½ tab bid: Cognition-enhancing medication It can treat dementia.
Prednisone (Rayos) 30 mg tabs take 1 po qid: Steroid It can treat many diseases and conditions, especially those associated with inflammation.
Clopidogrel (Plavix) 75 mg po qd: Blood thinners-It can prevent stroke, heart attack, and other heart problems.
Aspirin (Bufferin) 325 mg 1 po qd: It can treat pain, fever, headache, and inflammation. It can also reduce the risk of heart attack
Esomeprazole (Nexium) 40 mg 1 po qd: Proton-pump inhibitorIt can treat gastroesophageal reflux disease (GERD)
Rivaroxaban (Xarelto) 20 mg 1 po qd ac eve. Blood thinners- It can treat and prevent blood clots.

The drugs listed above are almost all available and prescribed in generic form; therefore these are the names you will see on prescriptions. However, people still call them by their original brand names, so:

Understandably in the world of nuclear, risk management is focused on safety. Wider considerations of longer term strategic risks and short- to medium-term project risks can easily be neglected. A broader approach in which risk management is embedded across your organisation could be key to improving overall business performance, writes Val Jonas.
Ideally, risk management should be an integral part of every business process for organisations working in waste and nuclear. However, the nuclear industry is unique in some of the challenges facing it.
If we start with the extreme time-frames we work to, effectively our industry life-cycle spans decades. Already this complicates the decision-making process, and sets the scene around the scope of risk that organisations need to factor into their management approach. For example, horizon scanning becomes necessary, taking into account threats such as catastrophic terrorism, cyber-crime as well as those posed by the natural world like earthquakes. We're contending with such huge risks - climate change and how this affects rising sea levels - that risk takes on an enormous role.

The real gem though was this IEEE report, "Is Nuclear Power Globally Scalable", by Derek Abbott, in 2011. This is a detailed look at the costs and technological considerations of scaling nuclear power out to the 15,000 reactors which would be necessary to provide all global energy needs. Note that presently there are fewer than 1,000 reactors (commercial, research, and marine nuclear propulsion) in existence, and I suspect the total number ever constructed is below or close to 1000. In particular, plants have ~40 year life cycle (imposed as a consequence of nuclear chemistry and embrittlement), a 4-8 year construction process (noted above) and 20 year decommissioning process, meaning that total lifetime consideration approaches or exceeds 70 years, half of which is nonproductive.
