I've been wrestling with the best way to come out to my friend and would love some input from all the lovely people in this subreddit <3
So, for context, I am 21, have been tranisitioning medically for well over a year now, and have been generally passing quite well.
Now, I met my closest friend from being housemates with her last year, which was my second year of uni and her last year. She graduated and moved back to her country, but we still talk and play games for hours each night. She does not know that I'm trans, but knows that I love generally to do makeup and most of my clothes are from the women's side. She also knows that I have been lasering my face. She knows that I exist in online spaces as female (voice included) and actually goes along with it so well, has never misgendered me online or even messed up my name irl when i poliety gave her a new name to use for me, abeit a shortened, feminine nickname. We've done makeovers and photoshoots and just overall done "girly" things together with no issues. She did ask me if I was gay one time but I said that I am not because even though I am gay (lesbian) I know she meant am I a gay men.
I never came out to her before because honestly I never thought we would become so close and stay friends for so long to the point where she has actually brought a ticket and will be staying with me for two weeks during my graduation. The need to come out never really happened and I was happy with how things were so I never saw the point in telling her, like I can present completely femme, use a female voice and she uses the right name for me.
In the time that she left the country, I doubled my dose of E which is when the majority of changes, including me being able to pass as a cis woman have happened. I'm at the point where most people in my life know me as a woman, and I would like to come out to her.
The only reason I'm a bit nervous is that she may not actually view me as a woman. I know that she is completely fine with people being gay/trans or just queer in general but I need more than that, I need her to view me as a woman and use the correct pronouns and everything for me. She's said a few things before such as not accepting trans people as their correct gender until they actually start to tranisition medically which make me think that she may accept me and being trans but not view me as a woman.
Now, of course I don't want people in my life who cannot view me as a woman (in fact I recently cut off one of closests friends for this reason) but I really hate argueing and really enjoy her being my friend, It's almost like a sort of if it aint broke then dont fix it sorta thing, because I have pretty much everything I could want considering im not out to her but I also think being out would help explain some things such as boobs when she comes. Overall I'd just love for her to accept me fully as a woman, and to be honest not that much really needs to change in our friendship and I generally feel like she treats me like one anyway but for some of the reasons I mentioned earlier I'm just kinda nervous and think it may be smart to have some extra talking points for when I do tell her with how some of the things that she says can be problematic sometimes.
I really hope this made sense, my brain feels very scattered currently during the last week of my uni and honestly typing this out made me feel like I am 15 again, confused about identity and pretty much all things trans as well as how to come out to people in my life at the time.

I use Opus for neuroscience research. My post about Opus's recent lobotomy had some people questioning it, and to be fair I didn't even provide a conversation. Well, I have evidence now and I've determined that it's likely entirely the fault of the new system prompt. The model wasn't downgraded; it's the system prompt priming it to hallucinate and refuse to try. Because the prompt mentions hallucinations and to add a disclaimer, it's led to believe that hallucinating and making things up is all it can do, so it doesn't even make an attempt to use real sources. If you understand how LLMs work, this makes perfect sense.
Below is concrete proof. As you probably know, the Opus system prompt was changed a few days ago. Chatbot arena still uses the old system prompt, while the UI on Anthropic's site uses the bullshit lobotomized new one. Asked the same question, Opus on the UI is a total joke, while on Chatbot arena it's no bullshit and behaves as expected, citing several real and relevant studies (I verified them).
Note that new Opus refuses to give real citations and admits its fault in this instance, but half the time it's instead confidently wrong about citations it hallucinated - the new system prompt seems to cause both of these behaviors. Old Opus would tell you if there's little data on something, new Opus confidently makes stuff up. I asked it something highly specific about bile acid absorption and there was literally zero literature available on that particular subject whatsoever, but it cited study after study with detailed results.

I'm an AFAB enbie. I feel uncomfortable with certain parts of my body, but I would not want to have corrective surgery because the mere thought of being in hospital makes me panic . Experiencing periods has always been a massive issue for me. It worsens my dysphoria to the point I feel repulsed by my own body - and I'm also having debilitating physical effects (joint pain and joint instability). A couple of years ago, I didn't get any periods for months because of job-related stress. It was such a big difference and despite the stress, I felt better than ever and above all, finally somewhat comfortable in my skin.
I'm a fairly active person and like travelling. To ensure that I won't get periods when I want to go swimming or hiking all day, my doctor prescribed birth control pills to me. It worked to stop the periods, but the side effects were terrible, most likely because progesterone-based contraceptives worsen joint instability (according to my doctor!) and I had some weird brain fog and would get anxious very easily. I've tried different pills and some issues always occurred. I can accept those if it's just for two weeks or so, but long term, it's not a suitable option for me. On top of that - it might sound stupid, but as an asexual person, the thought of taking "birth control pills" kind of freaks me out. The thought that my body is the kind that could theoretically give birth... it makes me feel sick, to be honest. I've been told that some types of birth control can also increase chest growth and this would honestly be a nightmare for me. It was difficult enough to come to terms with what I have and to find effective enough binders to wear.
A friend of mine (trans man) was wondering if a somewhat low dose of testosterone would be an option for me. I know that periods usually don't stop on low doses, but each individual case is different. Do you think it would be worth trying?

Went to the walk-in clinic to get my bg checked as “precaution” for suspicious symptoms. Part of me thought I was being paranoid as there is no way I could be diabetic at my age (type 1 or 2). They didn’t have the instant blood sugar machine at the clinic so they ran a whole bunch of labs and sent me home. Hours later the nurse practitioner calls me to tell me my concerns were correct and my bg was 497 and I need to go to the ER. Fortunately I was not in full blown DKA, apparently I was on my way there and putting me on IV insulin was discussed. I was admitted for the night and got a ton of information and was, in general incredibly overwhelmed. I am a registered nurse so I already had a decent understanding of diabetes, insulin and meal time bolus dosing and I was still just completely overwhelmed. I can only imagine how hard this is for those with no medical background or training.
I am now home working on figuring out “my new normal” and have some questions:
-What are good apps you guys use for counting carbs, tracking your blood sugars and insulin doses? I am using My Fitness Pal right now and writing down sugars and insulin boluses in my notes app.
-What are good high protein/low-ish carb snacks that are easy to dose for? I am also considered underweight and in malnutrition right now.
-I am a meal prepper but most of the stuff I prep I am unsure how to count the carbs because they are complicated meals. Any advice on easier to prep meals/recipes for carb dosing?

Hello! I took 2.5 Zepbound, but I cannot find 5 anywhere. Pharmacies are not getting it in. I messaged my provider a week ago this message:

I feel so bad for all my messages... You are correct, insurance will not cover another 2.5 dose of Zepbound. The pharmacy said they have no idea when they will get anymore in. They are getting the 2.5 and 7.5, I believe he said. I'm not sure if the 7.5 is an option for me right now. What do you think would be my best and safest option? I think I really need something to continue with the appetite suppression. It has worked so well, combined with moving my body more and increasing my water in take. The pharmacist did say they are having better luck getting Wagovy in, if you think that might work. I'm open to anything to get and stay healthy. Thank you so much, and I am so sorry to be a bother.

For context i’m 16 Female and i was diagnosed with ADHD in May 2023 and prescribed Dexamfetamine (5mg) two tablets a day. in August that year i was prescribed 40mg of Vyvanse (one capsule a day). The first time i took the Vyvanse my mind was clear and i didn’t have any racing thoughts i felt like a ‘normal’ person….But the crash i experienced was terrible, i felt depressed and overall a in such a horrible mood and would cry over something so small. This happened for the first 3-4 days of taking it and then i just felt like my usual self.
How often i take it definitely fluctuates, i took it roughly everyday for 1-2 months and then i started to take it only a few days a week or a few times a month,sometime a whole week or more and then miss taking it forra bit. ( i forget to take it unless Im going to school, even though if i’m running late in the morning or stressed out, i will forget to take it which happened more in the past 5 months)
As of currently the last time i took it would be 2 months ago or more i’m not sure. I haven’t been able to specifically pin point if it has been working for me, I do think i’m more productive and focused when i take it; but it also suppresses my appetite so much to the point where i will only eat small nibbles of food on the days i take Vyvanse. This appetite issue started around a few months into being prescribed and went from rarely happening to very often. it fluctuates from making me STARVING hungry to the thought of food will make me vomit even tho i’m hungry and want to eat.
Yes. i am aware that seeking medical advice is the first approach! I got diagnosed and prescribed by a Paediatrician, he was an old man and actually retired near the end of last year. I voiced my concerns to him about the appetite loss and explained how some days i would either feel like a zombie, i would feel exactly the same as normal, or i would feel a slight “buzz” and an increase in my attention span although not super significantly for me to be like yep!! i definitely feel it’s working!!. He literally brushed all that off and would kinda ramble on in appointments about random shit (usually the appointments were like 20 mins max) He also gave me my dosage based on my weight,( i am 5’6 and 70kg)
Since he’s retired my GP has control of my prescription and in a year or so i have to get “re-evaluated for my need of the medication.” My prescription refill amount is 5 bottles of both Vyvanse and D-amph. Today i accidentally double dosed on Vyvanse so 40mg+ 40mg as well as two Dexies 5mg + 5mg ,which i’m prescribed to take in the morning along w the Vyvanse (it’s currently 2 am i cannot sleep) and i have experienced EXTREME suppression of my appetite, headaches, excessive sweat, my brain is acting faster than my body and my vision has gone weird,, i feel like im looking through Iphone Portrait mode, my eyes are drowsy and the only sensation i can focus on is my eyes. i do not feel like myself,,looking at my reflection is uncomfortable because i feel like im not me in my body but the only thing that is me are my eyes.,, its hard to explain hopefully someone understands what i am trying to say.
ANYWAYS sorry i rambled a bit... Apart from all that i was very productive on TWO assignments in school today, normally i cant even focus on one 😖 but i completed both these in one sitting.(i did use chat GPT for one but i rewrote it)
Im really satisfied with how productive this dose has made me but not with the side effects as i think i may have OD!,, correct me if i am wrong!! I have some questions if anyone’s willing to help a girl out x
Does anyone have any similar experiences with their ADHD meds?
Have you found increasing your dose more effective or was lowering it better?
What are some tips i could start doing to improve the appetite issue and feelings of dissociation. Maybe my dose it not high enough or too low??
How can you tell or track if your medication is working?
(P.S i do have a history of using 🍃 for about a year,,most recent time was in January and March of this year. Aswell as a nicotine addiction from october 2022 untill around New years 2024. For context i’m 16 Female and i was diagnosed with ADHD in May 2023 and prescribed Dexamfetamine (5mg) two tablets a day. in August that year i was prescribed 40mg of Vyvanse (one capsule a day). The first time i took the Vyvanse my mind was clear and i didn’t have any racing thoughts i felt like a ‘normal’ person….But the crash i experienced was terrible, i felt depressed and overall a in such a horrible mood and would cry over something so small. This happened for the first 3-4 days of taking it and then i just felt like my usual self.
How often i take it definitely fluctuates, i took it roughly everyday for 1-2 months and then i started to take it only a few days a week or a few times a month,sometime a whole week or more and then miss taking it forra bit. ( i forget to take it unless Im going to school, even though if i’m running late in the morning or stressed out, i will forget to take it which happened more in the past 5 months)
As of currently the last time i took it would be 2 months ago or more i’m not sure. I haven’t been able to specifically pin point if it has been working for me, I do think i’m more productive and focused when i take it; but it also suppresses my appetite so much to the point where i will only eat small nibbles of food on the days i take Vyvanse. This appetite issue started around a few months into being prescribed and went from rarely happening to very often. it fluctuates from making me STARVING hungry to the thought of food will make me vomit even tho i’m hungry and want to eat.
Yes. i am aware that seeking medical advice is the first approach! I got diagnosed and prescribed by a Paediatrician, he was an old man and actually retired near the end of last year. I voiced my concerns to him about the appetite loss and explained how some days i would either feel like a zombie, i would feel exactly the same as normal, or i would feel a slight “buzz” and an increase in my attention span although not super significantly for me to be like yep!! i definitely feel it’s working!!. He literally brushed all that off and would kinda ramble on in appointments about random shit (usually the appointments were like 20 mins max) He also gave me my dosage based on my weight,( i am 5’6 and 70kg)
Since he’s retired my GP has control of my prescription and in a year or so i have to get “re-evaluated for my need of the medication.” My prescription refill amount is 5 bottles of both Vyvanse and D-amph. I’ve recently been told to up my dose and that means I need to buy 2 prescriptions in one month which is not something I had budgeted for. I can’t get these meds through the nhs yet as I’m still trialling and trying to find the correct dose. I work part time and get disability, and these meds have changed my life. I am able to actually function and would very much like to be able to keep functioning so I can stay in work.

Now, there’s a couple talking points people tend to use to support this nonsense and I’ll talk about some here. If you can think of any more, talk about them in the comments!!
Some people (idiots) say you won’t be able to get a diagnosis if:
You’re in luck my friend!! There is hope. Some universities allow psych students, under the supervision of a professional, to do assessments, not something super in depth like an autism assessment, but easier to screen disorders. My therapist actually had this done on him and it was an in depth assessment too. And he was able to get his answers. It’s way cheaper than going to a full on professional. And it’s good, just not AS good. You can call ahead of time and ask the psych/therapist office how much the appointment will cost out of pocket. A lot of places are willing to give you a discount if you just ask to do a copay based off income. The reason why the costs tend to be so high (in the US) because insurance effs up the cost of everything. I’ve done this before. It’s generally cheaper than going through insurance. At my old therapy place, it was $50 an appointment with insurance. Without insurance it was $5 an appointment. Just don’t tell them you have insurance until you get the rate. Go to a non profit or state ran facility. Not nearly as glamorous as your private places, but if you’re desperate enough to self diagnose, take what you can get. One of my close friends goes to a non profit place and he gets therapy for free since they also base off income.
As a former woman myself, I can attest some psych professionals are sexist. But remember, a diagnosis CAN be changed as you talk to the person more and they get a proper history on you. I got a BPD misdiagnosis because I’m a trans man and I have bipolar disorder. I was dismissed. So I went to another professional who listened. After a couple appointments he was baffled on how I even got that diagnosis because I had maybe two symptoms? And those overlapped with my bipolar symptoms. It sucks. Anywho, enough talk about myself. But like in any serious situation, it’s up to you to advocate for yourself. Not in an ‘I HAVE THIS I KNOW I DO’ way. But in an ‘I don’t believe this diagnosis is right and here’s why’ way. This way works. Don’t be a know it all, they’ll just dismiss you.
Atypical presentations of disorders can exist, yes. But make sure you’re going to the RIGHT professional. See a neuropsychologist for an ADHD diagnosis. Not your damn GP. The neuropsychologist can spot the disorder from a mile away.
This just isn’t true.. you won’t lose your freedoms. A diagnosis can only help you, you don’t have to tell someone like your employer unless you’re a government employee. You can get accommodations and extra time off if you disclose your diagnosis.

This is my dog Jesco. We’re not sure what breed/mix he is yet. I got him 4 years ago from the humane society and they said he was a Beagle/hound mix. I don’t think that’s correct. His first seizure happened around 2 years ago in March after his second birthday. The vet we took him to said we shouldn’t worry since he only had one, so we took him home and he had another seizure the next month. The second time, I took him to the emergency vet and they gave us Keppra to start. He did really well with it until his third seizure that following October, which we added Zonisamide. He did really well until his one year, where he started having seizures once a month, then every 9 days. The vet, the same one who originally saw him after his first seizure, said this was our new normal and that we couldn’t increase any of his doses. I ended up taking him to another vet at the beginning of 2024, who immediately increased his doses. He went roughly 2 months without a seizure, but had one in March 2024. The new vet said she wasn’t worried about it because it had been awhile since he had any. He had another one about 2 months later, and the new vet added Pheno to his daily medications. I guess the point of this long post is, I used to have so much anxiety, I couldn’t sleep over this, and was kind of making myself miserable, so writing all of this is kind of therapeutic. This is my first post I’ve ever done like this, but reading everyone’s experiences has calmed me a bit. Does anyone have any advice/comments about Pheno? We have him his first dose this morning.

I’ve started the med on Friday and have had some pretty aggressive side effects, I’m also literally unable to eat more than an egg and a few crackers a day which is not sustainable. Because of this, I am in a service industry and self employed so I cannot bear to have worse side effects and cancel clients when I do another dose this Friday. I understand that my body can get used to all of this, but I am not willing to have back end issues in regards to work.
My understanding is that the 1/2 life is one week so on Friday when I take my second dose I will still have 0.125 in my system. If I inject 0.125, I will be back at that initial dose of 0.25, correct?
I understand the med is supposed to build up in your system, I understand I am just starting out and have time for side effects to dissipate, but at this exact moment I am unable and unwilling to go above .25 being in my system at one time as I am very med sensitive and cannot miss work. If I can even decrease my binge eating slightly I will consider this a win and can focus on lifestyle changes.
I am looking to hear from anyone on if this math is correct, and also if you had any success on a very low dose?
For context: 5’ SW: 156 CW: 152 (lost a pound a day this week likely inflammatory or water weight) GW: 125-130ish.

Let me try to sum up this story the best I can.
I was working for a company who had other companies for contract based work. When their relationship with my employer ended, I was offered a chance to work with them directly, for a 125% pay increase, so of course I went. I was hired to do the same job I had been doing, and was joining a team they had formed, who I was told had been trained. I was the only person from my previous employer hired for the position I had been doing for 4 years.
Two days after I was hired, I realized that their team hadn't been trained well, and this was 3 weeks before this department went live. I told my manager, "Beth" that the team wasn't trained and once my previous employer went dark, we were going to be in a lot of trouble. She wasn't responsible for the training, another "manager" was, and after sitting in her training sessions I saw where the problem was.
...okay this isn't going to be short, apologies.
The person training didn't like me after I told her that something she was teaching the team to do was incorrect and would cause problems, and she hasn't liked me since then. It's mutual so IDC. I tried to work directly with Beth to get my team trained. I was being disregarded and the day we went live, I had to not only do my work but correct the work of nearly EVERYONE else. No exaggeration. In a two week pay period, my paycheck was for double that. I eventually burnt myself out and had to take PTO. I also had to get on Adderall. At the same time, I started low dose ketamine. The job I had been doing with little to no problems was being done by an entire team, incorrectly, and stressing me out.
The manager that doesn't like me, we'll call her "Jess", kept creating teams that I should have been on but wasn't bc of personal reasons. It's 1 year and 3 months later, I'm still doing my same job, and was never promoted to any of the positions that I was capable of doing to the point that it was embarrassing.
Now I thought Beth and I were friends. Friendly? Something. But over the last 6 months, she speaks to me like I didn't literally save this department from the chopping block, and instead of putting her in the same place as Jess, I keep trying to get her to be nice to me, to talk to me, and it has me on edge every single day. It's the same scenario that I've played out over and over again with my mother, which is why I've realized it's a fawning response. Thing is, I can't stop doing it. Like right now, I jokingly said I needed a raise. She told me to email firm management, the same people who left me in my role, even though I could do others, and eventually hired people from my old employer to do one of the jobs I could have already done, and paying them more than me.
What I'd like to do is transfer to another department but I like my job! I just really need to get to a place where my day isn't dictated by Beth's mood or what feels like her dismissal of me at every turn.

Hi All,
I am on Minoxidil 5% twice a day and RU 30mg twice a day ( RU is mixed in waterless Min ).
I started taking B complex capsules a week back. My hair has gotten thinner ( hair shaft diameter reduced ) and having moe hair shedding as well.
That B-complex supplement has 100mg Vitamin B3 and 50mg B5 which are both way above the RDA. Other vitamins are not very high.
I have had similar issue when I took very high dosages of Vitamin B5 ( it was 250mg ) if I recall correctly in October 2023 and saw tremendous hair thinning and hair fall. I feel B3 is not the main culprit here but B5 is.
Has someone also faced hair thinning/loss after starting high dose vitamin B complex or multivitamin with high dosages of these vitamins ?

2024.05.13 13:05 rauhweltbegrifff Akathisia- can't sleep. Any advice from people who experienced this?

I keep jumping up when I fall asleep. It feels like a mix of dyskenia and akathisia. They both kick in once the seroquel is working.
I'm using mirtazapine and gabapentin. I had the mirtazapine from my old doctor.
Gabapentin works the best though. If I don't time my doses properly I'll feel the akathisia until the new dose kicks in.
I have to take the gabapentin correctly before I take the seroquel so I'm not having full on panic attacks. But it's still very difficult to fall and stay asleep. Respiratory dyskenisia is also affecting me and I'm out of breath all the time.

Looking for some stories from people who may have experienced the same!
Quick background, I'm 25 M 104kg healthy but can be fitter and leaner. Got H-pylori in March 2023 and since then I've been dealing with a lot ( the pylori was killed off quickly with double dose of antibiotics), had a scope and was diagnosed with "a perfect example of EoE".. google the image.. it looks just like that..
I take 40mg PPI each morning (Nexium) and a flixotide steroid inhaler morn and night (also, can people tell me the correct way to take this pls. Doctor told me to treat this like a regular inhaler but I don't think that's right)
I never used to get them before all of this, but I get palpitations whenever I'm gassy, or whenever I run around during sport or lie down (sometimes). I read in places that EoE spasms can cause palpitations. Anyone had this before? If I'm looking in the wrong place I'd love to hear your thoughts. I'm an open book and a dry sponge you could say.
Note: heart has been cleared for arythmias, clogging of arteries, and overworked (stress test passed)

Sup all. I’ve taken a huge interest in gnosticism in the past few months. Since the texts we currently have are likely a fusion of much older schools of thought (sethite, ophite, barbeloite) my current interest is trying to find what parts of different texts came from which gnostic camp.
(For instance, the Apocryphon of John is theorized to have combined the mythologies of 3 separate gnostic groups, and then had a healthy dose of Christianity and platonism plopped on top.)
I’ve been reading Irenaeus’ Against Heresies because it has some of the earliest known records of gnostic thought, and his section on Barbeloites essentially tells the first half of the Apocryphon of John, starting with the Monad and Barbelo, and ending with Sophia creating the demiurge.
There’s something weird about this chapter, though, and it really threw me off when I first read it. Sophia is referred to as “he” in most of the references. Only when she’s directly called the demiurge’s mother is she ever referred to as “she.” The fact that Irenaeus seems to jump between masculine and feminine really threw me off, and for several parts of Sophia’s story I thought the “He” pronouns were referring to a different character entirely. It took me several rereads to understand what he was saying, and I’m still confused as to whether I’ve understood it correctly.
I know this is kind of a shot in the dark, but is anyone familiar with Irenaeus and the Barbeloite myths? Does anyone know why Sophia is referred to as a male in this text, or if I’m simply reading it wrong? Did Irenaeus write the inconsistent pronouns himself, or is it a problem with the English translation?
