Hi, I’m a 1st year nursing student in an LPN program. During clinical, the pt I had was on the heavier side and when I went to take a BP I couldn’t hear anything at all. I tried both arms and even tested it on myself to make sure it was ok. My procter said there wasn’t any bigger cuffs that were allowed to use when I asked, so she said to just skip taking BP for that assessment. I had initially left the room with my partner in with the pt to get a bigger cuff and when I came back, I was stuttering a bit to find an appropriate explanation. I feel like I basically told the poor woman she was “fat” and I felt like I didn’t use the right words to explaining why the cuff didn’t fit/ I needed a bigger size. She didn’t take any offense she was super kind and understanding but I wonder if another pt was in her position, if she would be so kind. Idk I feel stupid like I have no idea what I’m talking abt, this is only like my 3rd day of clinical anyways so u know I have a lot to learn. But how exactly would you explain something like that without offending anyone?

Prior to becoming an ER Tech, I purely worked event EMS; and prior to that, I did CCTs. I honestly feel that CCTs were my "high life" as an EMT, as I had great CCT team. The nurses and RTs I ran with, did not get in our way and we each knew our role well.
Fast forward 3 years later, Im now an ER tech at a local ER. It's a rather small ER at 28 licensed beds. It's not a trauma center nor a STEMI center, and the hospital doesn't have an OB unit. We are however a stroke center and pediatric receiving center.
The longer I'm here, the more and more I despise coming to work. To start off, my salary is the lowest of the low. I was kind of tricked into the low salary by the hiring manager, as they posted a lower salary on the job ad than what is actually the lowest they are allowed to pay. Next is my scope of practice, which has been cut by what seems in half. EMTs are only allowed to transport patients, splint, restrain, and place patients on 3-lead monitors. The other tasks, are just restocking and cleaning beds. We are not allowed to take vitals, do any patient assessments, take blood glucose levels, or suction patients. We are essentially just cleaners and transporters for the ER.
I originally took the job to learn more, but that's not the case. I feel this job is more skill-killing than IFT. Not to mention, EMTs are belittled by nurses and doctors. Only a couple of nurses actually value us and understand our situation.
The cherry on top is, that the clinical director constantly walks around the ER and will write you up for anything he feels you shouldn't be doing. I have been written up for taking vitals on a syncope patient in triage.

So about half a year ago while I (27 AFAB at the time) was at work my arm began tingling. It happened while I was crocheting so I attributed it to poor posture but took note of the time (just after 3pm) and continued working until 5pm. By the time I got off work my pinky and ring finger on my right hand became difficult to use. I drove home (hindsight this was really stupid of me to do) and an hour later when it hadn't gotten better I asked my brother to take me to the ER.
While there I told them that given the symptoms I was having (numbness on one side, and partial paralysis) and that I had started a new birth control a month prior that I was worried about the possibility of a stroke. They checked for the more obvious signs of stroke (facial droop, delayed eye dilation, blood tests, etc.) and noted that I had weakness in my right arm as well as tachycardia. They asked my brother about my behavior and if I was acting off, which he said I was not.
When the blood tests came back they informed me that my d-dimer was elevated. I wasn't told how much it was elevated. They referred me to CT to check for a pulmonary embolism. CT showed a normal healthy chest and I was sent home with a diagnosis of pinched ulnar nerve.
Over the next two days I proceeded to be incapable of eating or drinking anything, if I slept it was not for more than an hour at a time and I woke up in a cold sweat. My arm went from tingly and partially paralyzed to dead meat attached to my body. I could not move it, and could only lift at the shoulder. I woke my brother up at 3am to take me back to the ER. I informed them of the new symptoms, as well as the worsening of the previous symptoms.
This time it felt very dismissive. I was not being heard. I got sent to MRI to check again for a pulmonary embolism which again showed nothing. I told them I felt like I had really bad brain fog and my arm felt like it was in ice water. I was told this was normal for a pinched ulnar nerve. They gave me nerve glides to do, a prescription for 600mg of ibuprofen, and a referral to a neurologist. I asked them before I left if it was a good idea to fly as I had a vacation starting the next day, I was told it was fine.
So I went on my vacation, flying from one coast of the US to the other. It was 7hrs after I landed and was picked up from the airport where it started to go downhill and rapidly.
I still couldn't eat, and I knew I wouldn't be able to sleep so I bummed a gummy off a friend and started getting ready to go to bed. After this is a complete haze for me but I will list the events as I was later told:
I dropped on Tuesday night. I remember bits and pieces of Tuesday night, and the following Wednesday, but I was not conscious and aware until Thursday morning. I woke up, realized I had a catheter, and I was PISSED.
In the few hours after the medical staff learned I was actually with it. I was informed not only that I had a stroke, but that I had a rare form of stroke. It was a Cerebral Venous Sinus Thrombosis. They were shocked that the previous hospital had not continued to look for something and dismissed me. I was then informed that my d-dimer was 1.03 (where a 0.50 was the maximum threshold for a normal d-dimer), my glucose upon being admitted was severely elevated, and they didn't know how bad it was because they had not yet been able to get a clear image of my brain.
Throughout my ICU stay I learned I was extremely lucky to be alive, you can't trust every doctor, and to always be accompanied by someone who will advocate for you. Also birth control sucks and my friends are amazing.
I spent my 28th birthday in the ER.
I now live in the state where I was finally diagnosed properly and I am communicating with a lawyer for a malpractice lawsuit. I have had up to 20 appointments a month just for things related to my stroke and am coming to terms with things that just might not get better. I'm taking 4 different medications for the after affects of the stroke, and I might just have to remain on 3 of them as it's possible I now have an epileptic disorder. We wont know for sure until my brain is given more time to heal but my recent EEG still shows abnormal brain activity.
I think overall I have a pretty solid case, especially given the shock of the paralegal as I explained what happened. But sometimes I second guess it and it worries me.

…okay, so after going through acute appendicitis and an emergency appendectomy while I had COVID in January of 2022, less than six months after giving birth to our youngest, I’m now nursing my husband in bed after he just got done vomiting pretty much everything he’s eaten today. He’s been suffering from increasingly worse abdominal pains for the last seven hours or so.
I’ve seen the man doubled over with gas pains (which is what he was originally thinking this was,) so I know his pain tolerance isn’t super high, but the locale of the pain he is suffering right now, plus the vomiting nearly 7 hours in, coupled with the fact that he does feel warm to the touch in the lower right quadrant of his abdomen and forehead (even though he is not complaining of pain on the right side yet, just a generalized central pain deep in his abdomen about 3 to 4 inches above his belly button,) is all, in truth, freaking me the fuck out. In five years I’ve never seen him like this.
Problem is, the ED nearest us would be lucky to have him triaged and tests ordered within a 6 to 8 hour window, I’m scheduled to work second shift tomorrow, I’ve already had to take a ton of time off due to literally every other emergency that could happen (I’m dealing with an ovarian cyst large enough to warrant surgery, I had to take an entire day to fix the tag on our only vehicle, the grapefruit on my ovary comes with a host of debilitating side effects that have me in and out of the office right now, and I’m literally six months into this new phase of my career. At the end of the day, I don’t care if it’s not a good look if I have to request PTO to take him in, but we don’t have anybody close enough to us to watch the baby and I don’t really feel that an emergency department is an appropriate setting for her right now, co-pay is through the roof…you get the idea.
So I guess I’m asking this: at the time of this post, it’s been about 30 minutes since he’s vomited, it was just the one instance so far, he says he doesn’t feel the ‘stabbing’ pain anymore, just pressure, but he’s snuggled up to me and is quite warm now, like bordering on feverish, and when I push on his abdomen, the warmth/pressure/deep pain behind and above his belly button (only when I push right now,) is just really worrying me. Enough to ask internet strangers if I should take him in. Might he have the beginning stages of appendicitis?
I tried calling the back of his insurance card several hours ago - they used to have a 24 hour nurse helpline where they would give limited medical advice, but it was better than nothing, however, this time I got a customer service rep, so I figured I would ask where they would give limited medical advice, but it was better than nothing, however, this time I got a customer service rep, so I figured I would ask the fine citizens here on Reddit. Worth the trip? I’m down for a paid day off, not so down for subjecting my family to a nasty ED waiting room for 8-12 hours if it’s NOT appendicitis.
He wants to get some rest right now; it’s just after 11 PM my time and truthfully, I’m working a combination of first, second, and third shift(s) with a maximum of seven hour “breaks” in between and we’re all exhausted. Should I let him sleep a little and assess when he wakes, or should I not even take the chance?
EDIT: he just told me he’s still having little sharp stabbing pains here and there, though not nearly as frequently as before (I woke him up to move him to a more comfortable position, it’s been an hour now since he vomited.)

Background: I'm a single Mom with an almost 9 year old son. He's currently finishing up second grade in our local public school and I'm torn between staying in public school for third grade, which is a different building with only 3rd and 4th grade in it, or going back to his private school which he attended for kindergarten and first grade. My son has an IEP due to his Autism, but is considered high functioning in comparison to others.
My son did an assessment through is previous private school which is a school designed for special needs children on the spectrum, and was officially accepted today. The Pros of this school is that he's already been there previously and knows the layout and what's expected. The classroom he would be in would have the same children as before, with the exception of maybe 2-ish kids that had left the school the same time we did. The classroom is a max of 16 children on the spectrum/ learning challenges, but does have two intervention specialists. The cons would be that the scholarship doesn't cover the full amount and I would be required to pay $2,200 for the school year, which as a single Mom in nursing school is a lot for me to be able to do each month, if they even accept payment plans. His Dad wouldn't be helping foot the cost (he already told me no). The school also requires uniforms which is both a pro and a con. I found that uniforms seem to be easier to deal with than everyday clothing, but the shirts alone are like $30 each since it has to come from a particular store and needs to be engraved with the school's name. There's no busing available so I would be required to drive him to school which is about 15 minutes away. The district would reimburse me $500 at the end of the school year since they don't offer transportation to the school since it's out of district.
As far as the public school goes, I live in a top 10 school district in my area. The Pros is that he would be going to a different building that's actually closer to home (less than 8 minutes away). The classroom size is larger than the private school with about 24 kids a class, but luckily he would have a second teacher in the class that's an intervention specialist. He would still get his speech therapy, be pulled into smaller groups during some activities, have the opportunity to go for a walk in the hallway if he needed a moment, etc. Pros here is that he would be in a mainstream classroom, which he's done well with, but I'll be honest, these public school kids have been mean this year. They've told my son that he isn't stylish (whatever that means to a bunch of 8 year olds), that he's weird, and has had some classroom disputes with two boys that he used to be friends with at the beginning of the school year which he isn't anymore. Along with that, one of the girls in his classroom has been an issue all year which i had brought up numerous times so I don't think they'll be in the same class next year (she literally hit him in the face with her lunch box).
The school recommended him stay in the district since his biggest issue is socialization and he would have a better opportunity to learn to better interact with people of all backgrounds instead of just children on the spectrum, which i agree, but I'm also aware that with the scholarship, the cost comes out of the district's budget so I could understand them trying to sway me one way so they don't lose the $40k.
The private school is also not in session 34 days out of the school year which is a bit more than the public school at 26. I'm only somewhat concerned by this because I'm his main caregiver and would struggle to find childcare on the days the private school isn't in session since they don't have any programs. They do have aftercare which is $10 an hour in the gym. The public school has a YMCA program, but we wouldn't be doing it again next year since we didn't have a great experience. The intermediate school has after school clubs, but another childcare center would be an option as long as i sign up early. The $10 hr charge is more expensive than the daycare center would be.

say you have a nurse, off duty, at a bar with friends. theyve had a few drinks and witness someone collapse. on immediate assessment its a classic BLS situation. nonreactive. no pulse. no breathing. there is no one else in the facility who can assist. if that intoxicated nurse begins to administer CPR until EMS arrives on scene, at which point they relinquish care, could they be held liable for damages to the individual/family (if they dont survive) or are they protected under the good Samaritan laws?
the reason i ask is that there is a big consensus within the medical community that no matter what, if you have been drinking, you should never provide any level of emergency care off duty. some even going as far as saying you shouldnt provide any level of emergency care period, regardless of intoxication status, off duty because as a medical professional good Samaritan law do not apply to you and you can be held liable for damages in the likely even the patient suffers injury or dies. take it from experience. it's very easy and common to break ribs doing CPR. and off hospital codes do not have the best survival statistics.
i think that sets a dangerous precedent where the people most capable of delivering care and saving a person's life, often the ONLY ones capable on scene, will outright refuse to do so because of the perceived fear of litigation following an emergency event.
is there any truth to that idea?
