Am I wrong for reporting this Dr to the Medical Board?
Basically what happened is I went to the ED for concerns of Serotonin Toxicity. Or Serotonin Syndrome (SS). I was taking an adhd stimulant and just started an ssri. And more recently just upped my ssri dose. Upon upping my dose, I began shaking, sweating, no appetite, didn’t sleep for 3 days, was having moments of disorientation and confusion. Spouse also said my pupils were dilated. As the days went on my shaking got worse. My muscles, specifically my legs hurt so badly. I wanted to sleep. But I couldn’t physically stop shaking to sleep. And by day 2, my eyes were shaking side to side as well. My prescribing doc had told me about SS. She said “you won’t have to guess if you’re experiencing SS cause it will truly be so awful, you won’t have to wonder. Before going to the ED I looked up SS on UpToDate (the medical version of Google that doctors actually use) to make sure I wasn’t being over dramatic, that it wasn’t something I could handle at home, and just make sure my symptoms even remotely matched up. And they did. They matched up perfectly. It was saying for a Serotonin Syndrome diagnosis you’d need to take at least one serotonin drug AND have one or more of the following symptoms, 1- muscle twitching or jerking without stimulation (had that), 2-Muscle Twitching when stimulated or stretched, and restlessness and excessive sweating (had all of those) 3- Rapid horizontal eye movements (yep) 4- Shaking movements a person cannot control plus exaggerated reflexes (yep) 5- a fever (did not have that)
And that SS is a clinical diagnosis. Meaning there is no bloodwork no testing to confirm you have it or that it’s ruled out. So if you get labwork and nothing is wrong like a heart attack for example, but you’re having allll the SS symptoms, and you’re taking 1 serotonin drug, then it’s SS.
So I was like well heck let me go rule it out at least cause I’m just getting worse as time goes on and the longer I take this new upped SSRI dose with my adhd stimulant. And it was symptoms from hades. It was not fun.
In the ED my bp was 160/105, heart rate was 160’s. I told them I had concerns for SS because of the meds I take and how I just upped my Prozac dose. My symptoms. Etc. keep in mind. I’m trembling and jerking uncontrollably for 3 days now, haven’t slept in 3 days either. It was horrible. Doctor comes in, I reiterate everything to him ( as quickly as I can bc my teeth are chattering from my shakes) They do labwork and a urine analysis.
While I’m waiting for my results. The staff is outside my door talking incredibly loud. They are talking about a pt a few rooms beside me. The pt name, birthday, what they received as care, the patients ailments, etc. literally everything about this pt. And how we happened to have the same doctor that night (there was multiple dr in the ED that night, we just happened to share one). I thought to myself “Gosh that’s sad. When the doctor comes in here, if he seems off or sad, I need to give him some grace because that must be really hard”
So the doctor comes in. And tell me looks like all my labwork is good, that I should just go home and get some rest and that I have 30 protein and 70 ketones in my urine so I should also drink water and eat something. I told him again, I would love to rest, but I have slept in 3 days and I can’t sleep bc of my symptoms. He then prescribes me a sleep aid and immediately walks out of the room. I was confused because he never once mentioned the SS concern? Like ever at all? Just said my labs looked good.
So the nurse comes in with my discharge paperwork. And I asked her if the doctor remembered which pt and concerns I was having. Bc I understood it was a busy night for them and mix ups can happen. But my concern for SS wasn’t mentioned. She said he definitely remembered me. That he just doesn’t know the reason for my shaking or my other symptoms and my labs look fine. I told her that’s good my labs are fine but there’s not SS labs so how are we sure we’ve ruled SS out? She rolls her eyes and huffs and leaves. She comes back with the dr and he rips open my ED room curtain and punches the light on. And he goes “Look Kid. You’re going home! You’re not in any emergency whatsoever!!” I said “that’s fine. I just want to make sure we’ve ruled SS out as..” and he cuts me off and says “Look. You’ve had a FULL work up. Your labs are fine. You being here right now isn’t going to get you the answers you want” I said “I understand my labs are fine, but there’s no such thing as Serotonin Syndrome labs, it’s an exclusion dx so how are we sure we’ve ruled it out?” And he goes “You know what. Let’s have psychiatry come take a look at you and do a psych eval for you. But. How about this? I’ll be nice about it. You can do it inpatient tonight, or outpatient since you’re leaving. Your choice” I was absolutely appalled by this tactic. But I could see it for what it was. He was probably emotional over the other pt. Or was on a power trip and hated me having questions. Or both. So I said “I’ll do it outpatient. Thanks” and I begin shakily taking my monitors off so I can waddle on out the ED best I can and go on home. As I’m taking my monitors off he goes “Nope. Stop. Something doesn’t seem right about you. Are you safe at home? You seem really emotional and off” just to give him something to get off my back I said “I’m good. Just about to get my period I guess” and he goes “Mm makes sense. Well sounds good kid!!” And leaves.
What I think is incredibly interesting is that according to the hospital they addressed my SS concerns during my ED visit. So this would now mean that SS can now be diagnosed via labwork and heart and lung sounds. And no longer being on one serotonin drug AND one Hunter Criteria. Never mind the fact I was on 2 serotonin drugs and had more than one hunter criteria. And the fact that UpToDate says “there are no labs to indicate serotonin syndrome” this is now a medical catalyst!!! A complete 180 has been found by this doctor and hospital. Truly a break through. But nonetheless, I felt the way he spoke to me was absolutely insane and aside from him not addressing my SS concern but quite literally threatening me with a psych eval because I had questions?

Since after World War II, linguists, due to lack of interest in Kurdish studies, have often neglected the pre-Iranic substratum in Kurdish and focused merely on the Irano-Kurdish layer. Of the Urartian language is not much known either (about 300 words), however, a strong majority of its known words have a identical or at least close equivalent in Kurdish. Urartian though closely related to Hurrian, sound more similar to Kurdish than does Hurrian; this may depend on the fact that Hurrian was spoken at an earlier time period. Mannaean was another Hurro-Urartian-related dialect spoken south of lake Urmia.
It is also worth noting that Armenian scholars have found a few Urartian words in Armenian, which appear to be borrowings via Kurdish.

2024.05.14 12:31 Impressive_Dingo_531 DM question: how to properly challenge an experienced level 3 group? Specifically when they can take on things they shouldn't be able to.

I have a group that is very good, not just experienced but work pretty well together. There are six of them. They show up to every session and know their class, they are a dream group and I love them. However sometimes when they level up I'll throw something at them that I know they can't defeat just to test them for so many rounds. For example they are level 3 and I tested them against the demon lord Fraz Urb'luu (look up stat block) they managed to incapacitate him for a single turn which allowed them all to use some of their best abilities and get about 160 health down before he got his first turn. Now to be fair the first lair action put them all to 0 instantly and he would have got them but he was just story, the idea was they need to feel like they are surviving, not winning or coming close.
So this brings me to actual boss fights I want them to win. I put them up against something that should be a challenge and they defeat it fairly easily. The answer many people have given in the past is "never use a boss against a party alone" "give him minions" however every time I try this it just adds so much time into combat, either I do too much damage in my turn if they all go at the same time or I take forever adding them to the initiative order and turns take so much longer.
Has anyone ran into this before where either it's a challenge and takes too long or it's good timing for the fight but not really difficult?
Should a fight take an entire session normally? I've always tried to have a bit of everything in each session for variety but maybe I need to accept that combat should happen not every session and when it does can take up the entire thing?
Advice greatly appreciated and specifically ideas of fun encounters for ways to make fights more fun are appreciated, other DMs PLEASE feel free to share encounters you were really proud of and your players really enjoyed thank you!!!

27 y/o female with Lupus diagnosis I had a laparoscopy done on 5/2 for endometriosis. All was going pretty smoothly. On 5/7 I noticed the incision in my belly button wasn’t looking very pretty. 5/8 I woke up to blood all over my shirt and once that happened it went downhill very quickly. Went to the dermatologist (my surgeon was unavailable) and she did a culture and prescribed me Doxycycline 100mg 2x a day. 5/9 I went to the ER due to the severity of this infection’s progressive symptoms and the recommendation for IV antibiotics due to having Lupus. My HR while I was at the ER ranged from 102-160, never went under 100. I was prescribed Bactrim in addition to the Doxycycline and sent home. I took the antibiotics as told and it slowly seemed to be getting somewhat better other than feeling flu like symptoms and tachycardia on 5/10. I emailed my psychiatrist as he had made some changes to my medication recently and asked if that could cause these symptoms, the answer was no. On the night of 5/11 the tachycardia was very bad. I was finally able to fall asleep after several hours of my heart racing and chest pain. Yesterday, 5/12 I noticed the incision was oozing and began to feel sick again… flu like symptoms and tachycardia. The glue from the incision came off in the shower and it smelled terrible. This morning 5/13 I got my culture results back and the result was “Heavy growth of Pseudomonas Aeruginosa.” The surgeon is of no help, I will leave it at that. I called my PCP and he was able to see the culture report . He immediately got me on a new antibiotic, Cipro 2x a day for 10 days.
My question is, I had been recommended by 2 doctors to get IV antibiotics when this initially started. I am wondering if this is something I still might need to do? I did a little research on the bacteria but I don’t know what “heavy growth” means…does that mean the infection is more serious?
Any and all responses are very much appreciated.

Why jump rope isn’t already touted as a leading running drill tool is completely beyond me. But then again…
I'm by no means an "experienced runner"—having started running in 2021 at the age of 34. So, at the time of this writing, about 3 years.
Despite this, I managed to silver-medal my age group in my first race ever.
And it was a 10k. And I was wearing barefoot-shoes.
And I had only been running before that race for about 3 months.
How the heck did I manage to pull this off?
The answer eluded me for a while. Then I remembered—ah, I’ve been jumping rope nearly every day for 2 years.
But how do those connect?
But first, why the heck would some guy start jumping rope at age 32?
About 2 years before I started running, I took up jump rope really just as a fun outdoor hobby.
Even though I was pretty inactive and a bit overweight, that’s not the reason I started skippin’.
One day, I came across some footage of boxer Lulu Hawton doing some jump rope training.
In addition to her seemingly effortless rope handling skills and rhythmic footwork, what caught my eye was a giant grin that spread across her face about 45 seconds into the video. While she was probably skipping to warm up for a match or a training session, something was abundantly clear.
She was having a blast.
And this was from a prize fighter! None of the usual boxer mean-mugging—she looked more like a kid on a carousel.
So, after buying a $10 jump rope on Amazon, I took to the driveway in my swim trunks (yes, I was so inactive, I didn’t own gym shorts).
And…whoo, did I suck.
After a few months of making puddles of sweat in my driveway as well as wheezing sounds so loud that I’m surprised the neighbors didn’t whistle EMS, I eventually got pretty decent at it.
And I lost about 45 pounds in 6 months—probably also from making some lifestyle changes merely to make jump rope less of a slog. Not the original plan, but hey, not too shabby.
After about a year, I found myself constructively critiquing other people’s beginner jump rope videos.
But how did that turn into running?
Though jumping rope is inherently enjoyable, 30-minute skipping sessions of staring at the wall without something in your headphones can be a bit drab.
One fateful day, about 2 years into being student of the jump rope, I began listening to the book Born to Run: A Hidden Tribe, Superathletes, and the Greatest Race the World Has Never Seen by Christopher McDougall.
Even before I got to the end of the book, running—just like jump rope— sounded fun**.**
Yeah, I know that sounds counterintuitive—unless you’ve read the book.
“I knew aerobic exercise was a powerful antidepressant, but I hadn’t realized it could be so profoundly mood stabilizing and — I hate to use the word — meditative. If you don’t have answers to your problems after a four-hour run, you ain’t getting them.”
Ok, ok—I’ll bite.
I proceeded to dive into all of the normal “Couch to 5k” running programs I could find and took my jump rope to a nearby park with a 1k walking path—sprinkling in running between jump rope sessions.
But something wasn’t adding up.
There was a lot of advice about walk-running to build endurance until one could run a block, two blocks, a mile.
Not to brag, but I wasn’t experiencing most beginner snags.
**“Ah, I know why—**I did most of my newbie wind-sucking two years ago!”
This isn’t to say I wasn’t still periodically sucking wind but after two years of consistent boxer skips and double-unders, getting gassed felt like part of the fun and not a medical emergency.
I also felt much springier than the average beginning runner—able to run for miles all over the city in the most minimal of footwear.
And so, I tried my hand at my first race—a donut-themed 10k. And silvered in my age group.
(Ok, there was only two of us…but my time was still respectable. 😂)
Running became an amazingly freeing activity, like getting my driver’s license for my legs.
But I still didn’t understand why running was coming easier to me than the average newcomer.
Digging still deeper, I unearthed another exciting revelation—this time from multi-decade sub-3-hour Boston Marathon runner and one of the foremost running experts on the planet, Dr. Mark Cucuzzella.
“Running with a jump rope is also an amazingly simple drill for posture, balance, and rhythm.”
In other words—form. Overall technique.
Digging a little keeper and experimenting on myself, I discovered just how similar proper running technique and proper jump rope technique were.
Both require:
And so many other commonalities. The list unraveled before me on every run.
And like running, without proper technique, jumping rope just doesn’t work—though the consequences are different.
For a jump roper, due to the lower impact, the risk of injury is quite minimal.
Most newbie rope slingers will report sore calves, slightly tender Achilles tendons, and the odd shin splint if they go full Rocky at it. No need to worry, though—most of these injuries see themselves out as the skipper becomes more experienced.
However, for runners, the injury story is more severe.
The next time you’re at a park with a good path, take a seat on a bench and watch the runners. See if you can spot folks reaching far out in front of them with straightened legs—smashing heels into the pavement.
This style of running results in everything from screaming knees, plantar fasciitis, lower back pain, to hips issues.
But why do all of these occur to new runners, but rarely to new jump ropers?
Most new runners commit a major physiological no-no when they begin their running journey: they treat running like fast, aggressive, airborne walking.
“Well, what is it supposed to be?”
Synchronized jumping.
Simply put, proper running is nothing more than a series of coordinated single leg jumps through space with each landing compressing the springs for the next stride.
To compare this synchronized jumping to the aggressive airborne walking of heel-led running, you can test these in just a few seconds.
Step 1: Stand up.
Step 2: Kick off your shoes.
Step 3: Jump up and down three times.
How did you land?
Probably on your mid-foot, knee bent slightly, with your weight stacked above your pelvis.
And did you use your compressed “leg springs” to launch you into the following two jumps?
Oddly enough, if you were to add a jump rope to this, you would on your way to spinning side swings like Lulu Hawton.
If you were to take this same technique one foot at a time moving forward, you would be running in a way that increases speed, preserves stamina (springs!), and drastically decreases your likelihood of injury.
Let’s try the same test with a few tweaks.
This time, jump, but land on your heels.
Your knees probably remained fairly straight and you felt the impact in your ankles, knees, hips, and possibly even your lower back.
Now, imagine attempting to jump rope this way.
It simply doesn’t work.
Not only would there be no second jump due to the lack of spring but the pain would stop you in your tracks—even in cushioned shoes.
But if jump rope technique and proper running technique are nearly identical, what are aggressive heel landings doing in running?
While a jump roper landing on their heels would resemble Frankenstein’s monster in an express lane to an orthopedist, this is how many people perform the aggressive airborne walk—aka, a heel-striking, over-striding run.
But why do we run this way? Well, our shoes let us get away with it.
Thick heel cushioning and a bit of forward momentum do a great job of masking the pain of repeated blows against every joint up the chain—for a while, anyway. Eventually, the chickens come home to roost in the form of stress fractures, meniscus tears, plantar fasciitis, “runner’s knee,” IT-band syndrome, and more.
Not to brag (and maybe to knock on some wood), I have never experienced any of these injuries in my three years of running.
Is this because I’m some kind of running genius with all of the cheat codes? Haha, I wish! It’s simply sheer luck that I started out with jumping rope before running—an activity that shares the same injury-preventing techniques.
So, are the shoes totally to blame? No.
It is possible to run with proper form in shoes with raised, cushioned heels. But it’s not as easy.
When your heel is totally cushioned, you will be able to run with a heel strike in the same way you can hit your head against a brick wall while wearing a football helmet. And in both instances, it will eventually become less about the forces outside of the foam and more about the forces inside the cushion against each other that do the most damage.
“So, how can getting a jump rope help me become a better runner?”
Jump rope is a tremendous training tool for runners for the same reason why running barefoot can also be helpful—the feedback is immediate.
Though running with inefficient and injurious form is possible, the feedback from doing so isn’t so immediate. When it comes to jumping rope, however, you won’t get through too many skips if you don’t learn to utilize the springs in your legs. The rope doesn’t pull punches.
So, get a rope and get started.
If you’re new to jump rope, I would recommend acquiring two pieces of equipment.
Firstly, find a jump rope with a little bit, but not too much, weight to it. The weight will help you feel the position of the rope during it’s entire rotation and remain in better sync with your wrist spins
My favorite rope for this purpose is a 7mm PVC model called the Hererope, which costs a whopping $15. If you find this to be too thick or heavy, a cheap 5mm PVC model will work as well.
Secondly, to protect your rope and provide a nice jumping surface, I would recommend a large foam-rubber exercise mat. My favorite is a massive 78” mat for $32—which is probably the cheapest jump rope mat you will find.
When it comes to footwear, barefoot is ideal. This will help strengthen and mobilize your feet—including your likely overly-supported neglected arches.
And just how does one begin to jump rope?
Start with short seasons hopping with both feet—maybe 30 seconds on, 30 seconds rest. Aim for minimal muscular activation, instead, using the recoil of your tendons and ligaments for suspension and launch as much as possible.
From jumping with both feet, move onto learning an alternating leg bounce—essentially a jog skip. Right, left, right, left—all while keeping an imaginary belt level with the horizon.
By now, you’re essentially running in place with an extremely efficient technique.
Now, apply your jump rope skills to your running!
This is going to seem quite bizarre, but it is possible (and even beneficial) to take your jump rope for a run.
And there you have it!
You may find it quite helpful to return to this drill once or twice a week. Also if you find your form slipping a bit or becoming slugging mid-run, feel free to skip imaginary rope to try to correct your technique mid-stride. It will restore lightness and springiness to your running.
I still find myself bringing my wrists to my pockets and spinning imaginary jump rope handles if I feel my technique is collapsing a bit or if my running is becoming less springy.

Hi Doctors,
I hope someone can help me with this.
My mom 62 year old, Female, Hispanic, 5’ 3” she weights about 160 pounds, meds: Carbodopa / Levodopa, was diagnosed with Parkinson’s, no, she does not have tremors and many neurologists say that she doesn’t appear to have Parkinson’s.
The reason she’s on dopamine now is because she took a scan and the result came back as “suggestive as Parkinson’s”, the dopamine makes her happier and it gives her energy.
Original symptoms, She walks sort of twisted, like a limp her arm is weak all on the same side. Her muscles freeze, She does have scoliosis the orthopedic doctor said she’s had it all her life but I always looked at her back growing up and it never looks like it looks now. Sometimes when she eats apples her hand swells up… random but she doesn’t eat much but could she have diabetes?
Her leg muscle is basically locked inward, she woke up with it super locked and hurting today. Is this part of her Parkinson’s or should we also check into another condition too?
We have taken her to so many doctors and not many have the same answers for us, one said she had a stroke (came out clear) and another thought gout, and I am not sure, any ideas so that I can ask doctors to check? Thank you!

Hi Disneylanders! Back stateside, so as promised, here's a trip report from my three-day visit. This covers transport, staying at the Disneyland Hotel, several upscale meals, Premier Access, and my experiences with French vs English. (For Disneyland, CA fans, I'll make a separate posts with some comparisons and a different AMA.)
My stats, for reference: solo traveler (30sF) from California; fairly frequent visitor and amateur scholar of Disneyland, CA. Not a first timer but it had been 12 years. Competent in French.

Transportation:
I took the TGV direct from CDG, so this was my first stop in the country, since I figured (correctly) that Disneyland energy would shake me out of jet lag. It's an easy trip and quick exit from Marne-la-Vallée, much smoother and faster than the RER. Baggage claimed to feet in the hotel lobby took less than an hour.
Disneyland Hotel and private entry:
The hotel was still guest-only while I was staying there. Beautiful premises and worth every penny. The whole place is full of charming details, and the views of the whole resort are unbeatable. It also offers the best food and drinks I had during my stay (will cover that next).
My room was an entry-level Superior near the far side of the West Wing. Very generously-sized, even by American standards, and well-appointed. Vaguely Little Mermaid-themed, and I had a small terrace (most rooms didn't seem to, but mine was rounding a corner).
Since you can only get in on the east (lobby) side, if you're staying in the West Wing, plan an extra couple of minutes of "commute" time to get from one end of the hotel to the other. The good news is that your walk takes you through the corridor with picture windows offering twin views of Disneyland Park and Disney Village, as well as past the bar and restaurants and shop. That said, it only took about 5 minutes total for me to get from the room to the park through the DLH guest-only entrance.
Speaking of said entrance: this might be one of the best perks. The guest-only entrance never had a wait, even three minutes before Extra Magic Hours opened. I was second through the turnstiles. There's an exit on that side as well, and save for my afternoon at Studios, it's the only entry and exit point I used.
Sit-down food and drink:
I'm from a very foodie/cocktail pioneering city in the US and have pretty high standards. I found the quick service food, on the few occasions I grabbed it, to be mediocre, but the sit-down meals were surprisingly good. Drink quality varied widely. I have rated them based on a combo of food quality and atmosphere.
My assessments:
Disneyland Hotel Breakfast: I was one of the last seated on both days, and they seated me right next to a window overlooking the park in the Lumière section of the restaurant. Viennoiserie and other things were good, but the highlights were the fresh smoothies and the eggs. I've never had such a silky scramble in my life. Food 5/5 for the eggs alone. Experience 5/5.
Captain Jack's (lunch): The Caribbean version of Blue Bayou, and as charmingly themed. Nothing like being greeted with "bonsoir" in the early afternoon. Food was not great, though, and my cocktail was sugary and weak. Food 2.5/5, experience 4/5.
L'Auberge de Cendrillon (dinner): Immersive Fantasyland space and pretty good 3 course dinner. I requested the characters not interact with me, since I'm there for atmosphere, and they complied. Food 4/5, experience 4/5.
Walt's (lunch): Great view of Main Street, fun story, decent but not great food/wine. Loved touring the different themed rooms, and they were happy to let me. Food 3/5, experience 4/5.
La Table de Lumière (dinner): Delightful experience, stunning interior, excellent food by Disney standards, enjoyed the champagne tasting and a beautiful cocktail to start (they froze a rose inside an ice spear). The royal couples did in fact all stop to talk to me, even when I made it clear I didn't need them to; it was easier to just comply than explain. I don't fault them for it. Food 5/5, experience 4.5/5.
Fleur de Lys (nightcap and one midday cocktail break): Charming and comfortable interior with royal crests, and pretty darn good cocktails (though they put soda water in their old fashioned, which is heresy). I sat at the bar and chatted a lot with the staff, and they kept putting nice little treats in front of me to snack on. The drinks are expensive, but it's a great adults-only refuge. Cocktails 4.5/5, experience 5/5.
Agrabah Café Restaurant (lunch): Moroccan/Mediterranean buffet, pretty good quality. The space is incredibly cool and meticulously detailed: best story design of the restaurants in my opinion. The carafe wine was decent and an amazing value. Food 4/5, experience 4.5/5.
Premier Access:
I'm allergic to lines longer than 15 minutes and, in California, am really good at circumventing them through planning. Paris has much lighter lines in general, at least during my visit, but when I saw 30m and up with no single rider line, I'd pay a few euros to skip. As a solo visitor, I was a walk-on for everything but the Avengers coaster (that queueing system is a nightmare) with the Premier pass.
I tried out both individual and daily passes. Would recommend the individual, but not the daily. Not sure how they price things dynamically, but the day pass cost like 160 euros for me. I was rushing to get my full value out of it all day, based on individual pass cost, and failed to break even. It was way more stress than fun.
French vs English (edited for clarity):
I often get clocked as not French before I even start speaking, so cast members addressed me in English unsolicited about a third of the time, but I did feel like I got treated better (and certainly got more interesting conversation, secrets, tips etc) when I answered their English with French. (For reference, I’m probably a borderline B2/C1 at the moment, with an accent most people don’t pin down as American but can tell is non-native.) Happy to share more in AMA.

I've spent over a decade slowly taking on more and more responsibility and having all my free time and hobbies evaporate. Letting go of things I like because he doesn't. Not doing something easier (like spaghetti for dinner) because he doesn't like it. But I can survive it day to day. The thing that I don't think I will survive?
This man would be the death of me if I had a severe or longterm illness or disability.
I am pregnant with our 3rd child and have been diagnosed with mild preeclampsia and put on bed rest. It started a couple weeks ago on Sunday evening with a headache. I took my blood pressure and it was 187/106. I tell my husband and he minimizes it. "It's probably just stress". I knew better but I was exhausted and felt like shit. So I went to bed.
Next day my pressure upon waking was 140/90s. I checked it again mid day and it was 160/100. So I called the nurses line and they never called me back.
I called my husband on his way home from work and told him I would need to go to the hospital. "How much is that going to cost us?" Queue the bad attitude. when my husband got home that evening, after he went to his friends house, after I cooked everyone dinner, after I had helped one of the kids shower, put the kids to bed, I told him I had to go. He snapped at me. "Why didn't I just go earlier?" As in take 2 kids to labor and delivery triage so that I would be stuck in a small room trying to entertain and control them, getting my blood pressure checked every 10 minutes trying to bring it down.
I asked if he wanted me to take his car or my truck. If he wanted me to leave the car seats. "Why would you do that?" I left the car seats. Truck wouldn't start. So he had to come out and fix it(the cable to the positive side of one of the batteries was loose). He told me I was dumb for not figuring it out myself in the dark feeling rundown and stressed and then went inside. I drove myself to the hospital 30 mins away.
My BP was 197/100 and stabilized to 165/90s. They gave me medicine and did labs, and NST(baby is good). They admitted me for 24hr urine. I called him at 3am to update him. "You need to call your mother later I don't have any PTO". A lie. He's already committed his PTO to hunting trips so he's technically out in the sense he would have to give up one of them.
I text my mom and sisters who are all concerned. My mom agrees to come to my home to help care for our girls. I get a call later that day from the school because the eldest was having a hard time so I let them know I was in the hospital and that dad had to get her up for the first time. Husband miraculously manages to follow my directions and picks up kiddos medicine at the pharmacy and drop it at the school and add my mom to checkout list.
He brings my girls and mom to the hospital to see me with the short list of things I needed (underwear, hair brush, Tooth brush). He didn't bring me dinner or snacks. He takes the kids out to dinner when he left.
Over the 2 days my BP gets down on bedrest and my protein in urine comes back in 400s. Mild preeclampsia. I'm discharged and drive myself home. Relieve my mom of duty and thank her immensely(eldest behavior report was much better that day). She even cleaned my house some.
Husband gets home, I cook dinner, he asks for sex(I decline). I get kids to bed and go to bed.
Husband doesn't understand bedrest, asks for sex every day(that'll be a no), and is very snappy and rude, hangs with his friend in the evenings, until the Sunday when he is slightly nicer. He denies being mean to me during this time, of course
Monday my doctor puts me on permanent bed rest, follow up with the MFM, and blood pressure medication because my numbers at my appointment were 160/100 and 150/110. I tell my husband. He still doesn't understand bedrest. But he's nicer as he expects nothing to change. It's when things impact him that he starts snapping at me.
And that's what I realize. This is the kind of man who leaves his wife when she has cancer. This is the kind of man who would stress me into a worsening medical condition and death to ensure I'm punished for stressing his life. And that's what his attitude is. He's stressed about the situation so he's making it harder on me as a way to punish me. He minimizes the situation because addressing it, helping and getting care might impact his life or day. He will not make a sacrifice on my behalf. He has to have something in it for him.
Now maybe your thinking he doesn't do well when people are sick or injured. The opposite. He's amazing for his friends or family when they or a loved one is sick or injured. Calling daily to check on them arranging to visit them in the hospital or their home. Seeing if they need anything. Just the other week a friend fractured his neck and he was there for them.
It's just me. It's me who isn't a person or someone to be cared for. I'm a thing that when it's broken he gets frustrated at and blames and takes it out on. The thing that's supposed to make his life easier by bringing in additional income(except he makes 3x as much as i do so i shouldnt forget that), doing all the child care including during my work hours(except he took the kids with him for 2 hrs the other weekend and they all came back grumpy), all the cooking(except he cooked himself breakfast the other weekend and gave the kids some of the bacon) all the chores(except he put away the dishes 3 weeks ago one time), every bath, every bedtime, all the grocery shopping and pharmacy runs(except when he had to go while I was hospitalized), and can I pick up truck parts from Advanced and drop off this drive shaft at one shop and pick up a transfer case at this other shop, and scan in this paperwork. And "no" isnt an acceptable answer. And bedrest isn't really a thing.
I've gotten no support post c section from him the last 2 times. I've been driving myself around basically the day after discharge both times. He went to work while I was still on the mag drip less than 24hrs after birth of my first. His boss and coworkers asked him WTF he was doing. After the 2nd(zero complications planned csection) I was vomiting in recovery and he "needed a cigarette" and was gone for 4 hrs, got himself food brought me nothing. He denies he was gone for that long but by the time he got back I'd already peed in the toilet twice.
But thats not what happened according to him. Just like he denies asking me for sex every day post hospitalization this time around(not 5 mins after he asked for sex, no less.) Denies that I do all the chores because he works on the vehicles. We have 5 running trucks. We do not need 5. But he's trying to sell a couple, for what he thinks is a fair price. He says at least.
He drives me nuts with finances. How can we both not have enough money for pizza 1 night a week but he can buy 2 parts trucks this month. I started making the pizzas myself for the kids. The cost is actuallythe same as takeout. I do personal pan pizzas. He says I should only make mine a half as I don't eat as much and only mine was left over the other week. The week I made pizza hours before being hospitalized and had no appetite post discharge. Apparently it was a waste.

I really need help figuring out why I'm still gaining weight. I'm gonna add a bit of backstory so that all the details are there. I apologize in advance it's been a whole journey.
I (25F, 300lb) have a heart issue which has caused me to get inactive over the years. Heartrate > 110bpm usually around 115bpm. Around 160-180 when walking or standing. It's likely POTS according to my doctors, but they aren't completely sure and I've been fighting for years to try to get an answer and I just can't afford to try to chase it down. They've told me that even if that is what it is there is nothing they can do that isn't already being addressed.
In high school when I was 15-17 I used to be very anorexic. I tried to solve the issue on my own- eventually led to binge eating disorder but I'm currently trying to work on that (I'm on medication for it). I'm not perfect but it's a process.
I also used to eat nothing but junk food. This is likely do to my ARFID, I've had this issue my entire life, it has been a huge process.
So I have slowly over the past two years been changing my diet and getting medical help for issues with my body. (When I can afford it anyways, I'm American, health care is expensive).

When I started this, I was 23-24 and weighed 253lbs. I am 25- turning 26 in three months. I weigh 301lbs.
I can not figure out why I continue to gain weight despite eating less and exercising more. at the very least, I'd expect it to be stabilized but that isn't the case. It makes me demotivated and exhausted. I really don't want to get weight loss surgery. It can cause long term health issues and I honestly just can't handle anymore on top of what I already have. Also I want to be able to do this for myself.
My goal is currently to get back to 250 but my extreme long term goal is to get to 150. I'd be satisfied with 200.
I'd appreciate any advice. Tell me what I'm doing wrong please. I know I'm not moving very fast, and that I could be doing more extreme changes but I'm trying to build habits that aren't going to go away, It's very hard for me to keep a routine so this has taken me a long time and with zero results I'm just ready to give up.

I’m new to the PC gaming community and have recently built a PC with the latest RTX 4090. My monitor supports 4K resolution at 144Hz, but most of my games are running at 160-180+ FPS.
I’m wondering what adjustments I can make in the NVIDIA Control Panel to achieve the highest image quality considering that I use 4k for games (mostly first-person shooting so FPS is important, but 4090 is outperforming for most of my games).
I’d like to limit the performance to 144Hz so that the graphics card can utilize any additional resources to enhance image quality rather than pushing FPS beyond 144 which has zero benefits due to monitor limits. Are there any articles that summarize the right settings and balance to have?
Update 1: Specific questions I have are:
  1. should I use Image Scaling in NVidia Control Panel (NVCP) or not? I understand it's beneficial if you are playing below native monitor resolution. But what if you are using the native one? What setting to use then
  2. I saw many recommendations for G-Sync On/Off with Frame Limits On/Off. but cannot find any definitive answers what's the best option. Some are saying "G-Sync + V-Sync ON in NVCP" was recommended before but not the case for windows 11
  3. What is the difference limiting FPS in game vs NVCP(controller by the driver) ? some information I found was better use NCP limit
  4. What is the correlation between "Max Frames" option and VSync On/Off?

Hi, I started noticing pots-like symptoms in my senior year of high school, and just finished my sophomore year of college. Every day im fatigued, I get frequent headaches and have digestive issues, I flip flop all day between being freezing and sweating, my heart rate spikes (I can feel the palpitations physically and can see them on my Apple Watch, my HR rises 30+ bpm and can go up to 160 on bad days) when I stand and I get pretty lightheaded and usually can’t see well for a minute or so. Since I was a kid, I’ve had these pains that I can’t find much of a cause for, I’ve searched online for hours and have asked other people with dysautonomia and haven’t had any luck so I thought I’d try my luck here. It’s not constant, and it only gets bad every month or so. The pains feel like they’re in my joints, they’re like dull aches that just feel warm, like my bones are warm? When I was a kid they would mostly be in one ankle or the other. Then in my teens I started feeling the symptom additionally in my wrists, and now sometimes when the pain comes on it will be from my ankle to my calf. It feels like a warm dull ache. I’m looking into POTS specialists in my state to go to in the meantime, the first time I tried to see a cardiologist they heard I was about to start college and told me it was just anxiety. The second time they did my orthostatic blood pressure (poor man’s tilt table) but the whole test was about 3 minutes and they told me my results were normal, they waited one minute in between testing my lying down, sitting, and standing hr. It was a renowned hospital too, so I was shocked that I left feeling so heartbroken that a true answer to my symptoms wasn’t looked into enough. I have that map with dysautonomia specialists in different US regions but life has been crazy the past couple years. Sorry for the long post, just wanted to see if anyone else has experienced this type of pain since I know people with POTS are more likely than other people to have other syndromes and chronic pain.

But firstly, people can't remain tense 24/7 in a fight in every muscle, it takes too much energy/stamina (which is why Kiryu just waited for the moment that Ohma could no longer remain tense in their fight, and Mark Myers eventually got tired too), on top of slowing your movements down, so it's something experienced fighters usually only deploy discretely and not continously during a fight.

It's noted for example Waka had to Clench his muscles at the moment of impact to defend against Muteba's spear, and if he didn't, he would have lost. So when you see Ohma, ON-GUARD for Wakatsuki's liver blow and using indestructible to guard against it ( thus excercising his maximum durability potential), you can't compare that to Julius getting hit from outside of his awareness ( who is OFF-GUARD due to directing too much of his attention at the fake blast core and thus at his lowest possible durability), it's comparing apples to oranges. You need to compare apples with apples and oranges with oranges. Technique is important, hitting a person at the right time and at the right angle and striking them in a vital point when they're off-guard is more important than simple raw power.

Hi so shortly after ordering my first 2.5mg dose, among the emails i received from Medexpress was whether I’d be interested in taking part of research on GP1-medications with Dscout. I had to do a 5 min video answering a few questions. Yesterday I received an email from Dscout to say I’d been accepted and would need to spend roughly 30 minutes per week for 8 weeks on tasks and I’d receive £160 after 4 weeks and then another £160 at the end of the 8 weeks.
I’m just wondering if anyone else was offered this by Medexpress and then accepted? I guess my main question too is I’ve never heard of Dscout before so is it definitely legit and will payout?

My family origin comes from Awadh State (Kingdom of Oudh). We migrated to Nepal like 160 years ago (approx) to madhesh and we are living here currently. By race we are Awadhi People and my grandfather and their father's were mostly speaking to Awadhi Language but now my father, me or my next generation or my whole family is Nepali Speaking Family. But due to interest in history I learned Sanskrit and Awadhi from my grandfather. During Learning Awadhi I went down, read and understood to Tulsi Das's Ramcharitmanas and during learning Sanskrit I went and learned Sanskrit from my grandfather, priest and YouTube, during Learning period I read a of most of the books like all the Vedas (4 of them), Puranas, Upanishad and Mahakavya (like Mahabharat and Valmiki Ramayana).
After reading most of the book from Hindu mythology and system I felt a lot missing, there were a lot of question which raises in my mind which this book's or any Hindu Mythology book can't give answer to me even Brahmans and Pandit (they gave me answer same the book have). Fast Forwarding to my Engineering I was completing my Graduation from Delhi where I met a Muslim Girl (I liked here a lot), She was kind of crush to me. We went in relationship and I told her my dilemma and questions about God (which was related to Hinduism, Islam is not entered in my life yet). She told me why don't you read Quran it might give you an answer. She told me if you want to know about Islam then read Quran don't look at to Musalman (Muslim). Let me conclude here that she was not trying to convert me at this position, she tried to help out in which she thought right. Her every question is solved by Quran (what she said) and thought my problem could also solve by Quran) It took me 5 months to read Quran and most of the main Hadith and then I was thrown into black hole after reading this Muslim's religious book. Same problem a lot of more question to me and told to my girlfriend and she also started having issue with her religion book and then we both were thrown in the big black hole. I started reading most of the Old Testament, new Tastament, Psalms, Book of Genesis, Kojiki, Avesta, Guru Granth Sahib, A Guide to Tipitaka and many religious book.
After reading this ton's of knowledge and losing around 2 years I started getting my answers in very different way. I found that while reading this book's make sure you also read about their civilization (past civilization and present civilization) and how and when their god formed and shown. Also read their religion critics book. You also need to know about who was ruling that time what people thought or what was their thought process. Who was the king and how did king ruled. you need to also take geography and climatic condition of that time.
After concluding this all. I got a very fundamental solution or answer to this that "We don't know whether their is existence of God, I don't know but Yes all the God's till we follow (whether you are Hindu, Buddhist, Muslim or Christian) are people made God, they all are made on purpose (some by society, some by kings, some by family) and they had or have a purpose when they were designed or made, right now many of the God or Goddess don't have a purpose but still we follow but some have a purpose (which we don't know). All the traditional dresses, Food are societal personal holding are made for survival and if the society can't explain the scientific reasoning behind anyone of them then It is thrown to the God or the Divine power.
It's up-to you how to handle your God or Goddess, if there is something Great like concentration or self healing or realization or you are getting a better life is getting through prayer's and pooja path then you can follow it or if it is harming you or making you more fundamentalist then you should stop praying.

Hello, i would like to ask you guys'n'girls before i even try again, ill try to explain the case as easy as possible but also as understandable as possible.
EDIT: Its D-160 Visa i got denied. Just for vacation.
So, i was trying to get a VISA to US in 2019 wich led me to a red paper and denial of visa as the interview was ended, i felt like they didnt even tried to ask me anything, just wanted to see the criminal record wich has a prisonsentence from 2013, did time in 2014 for 9 months (6 in prison) for some stupid shit that happend in 2011, it qualified as i understood for Moral Turpidity or something. I tried to google this and its not easy to find any information if im denied forever or if i can apply again..?
Can add there is a few traffic fellonies to, but thats just a few for speeding, drivers licenes got suspended. But this was 2012. Have not even been stopped in a traffic control since 2014 so im a normal "dude in the street"..
It like this, im now 30, turning 31 this year and this stuff is way back in time, im a father with a good job and my own house etc, i know this dosnt have anything to say as i had this when i applied first time too, they didnt even ask of anything..
So, my question, now its 5 years since ive applied last time, is there even worth the stress and time to try again? Or do i have to wait for "forever" to visit the land i want to most.
Wish and hope you guys can answer?
I can also say that back in 2019 i got accepted a ESTA, but i didnt wanted to take the chance of stay at the border so i tought the safest was to get a VISA. That was mabye very dumb.

Hey yall, I’m slowly working on my pets stats, but as I do I’m looking to upgrade her equipment. Right now she’s got a Ylana’s Blaster, Greater Healing Scroll, a Scuzzys Comb, and a Mystical Tablet.
Obviously I’m not close to maxing out my stats; but do yall have any recommendations for ways I could beef out her battle set? Or at this stage should I just be focused on getting her to the next tier of stat boosts?
