I want to preface this by saying I know varicose veins during pregnancy can be considered “normal” and because of this I hadn’t previously mentioned them to my OB.
I am 32, this is my third pregnancy, I am healthy BMI (5’4” and 130 lbs) with zero chronic health issues. I am a very “veiny” person in general and pre-pregnancy was 108lbs so they were pretty prominent but no varicose or pain or issues related to them.
I am now 23 weeks pregnant and an RN in a step-down / critical care unit. I work 12 hour shifts and have total-care patients (need to be turned, some vented, and requires physical ability to provide care). I also want to add I do wear compression stockings. The strongest I could find OTC. When I’m not at work, my veins are in severe pain and they extend from the ankle to behind my thigh and into my groin. This is in my right extremity only. The skin also itches and it’s become to the point I have small ulcerations from itching. After working one shift, I have +2 edema and my shoe barely fits.
I have just emailed my OB, but am wondering if it’s just pregnancy related and not a reason to be reaching out to her or this is in fact reason to be somewhat concerned for the sake of my venous structures within the extremity long-term. My mom and aunt both had surgical intervention to remove theirs and I’m hoping to eventually do the same if it’s appropriate.
Any input would be great on the opinion regarding working 12.5 hours on my feet with this kind of varicose/ what seems to be venous insufficiency :/

Today I will be briefly explaining the practice of Gematria. Gematria comes from ancient Jewish mysticism. It is the practice of combining the letter with the number, with the word. It is said this is how God created the world.

Step 1: What is Fat Necrosis?

As a professional plastic surgeon, I understand the concerns patients may have about complications following a Brazilian Butt Lift (BBL) procedure. One of the most common complications is something called "fat necrosis." In this blog, I'll explain what fat necrosis is, why it happens, and how it's typically managed.
Fat necrosis is the death of fat cells that can occur after a BBL. It's important to understand that this is a natural consequence of the procedure and happens in virtually all patients, even if they don't notice or feel it. When we perform liposuction to harvest fat for the BBL, we're essentially taking fat cells from one part of the body and transplanting them to the buttocks. During this process, the fat cells get disconnected from their original blood supply, and some of them will not be able to establish a new one in their new location.

Step 2: The Survival Rate of Transferred Fat

Studies show that approximately 85% of the transferred fat survives the BBL procedure. The remaining 15% will not be able to integrate and will instead undergo necrosis, or death. Superficial fat necrosis, while alarming in appearance, also usually resolves on its own without the need for medical treatment. The body will naturally expel the dead fat, and the area will heal over time. Attempting to cut or drain the area can actually lead to worse outcomes, so it's important to resist the urge to intervene and instead let the body do its job.

Step 5: Minimizing Fat Necrosis

As a plastic surgeon, I take several steps to try to minimize the risk of fat necrosis during a BBL procedure. This includes carefully harvesting the fat, avoiding large, dense deposits, and evenly distributing the transplanted fat to ensure adequate blood supply. However, even with these precautions, some degree of fat necrosis is virtually inevitable.
The good news is that fat necrosis, while common, rarely poses a long-term problem for the overall appearance and shape of the buttocks. As long as patients are aware of the condition and understand that it's a normal part of the healing process, they can rest assured that the final result of their BBL will still be excellent.

Not so long ago, Kailua (the city of “two seas” as the name translates from Hawaiian) was known for its golden sandy beaches with turquoise water - a surfer's paradise! Also, Kailua was one of the residences of the kings - the rulers of Hawaii. Barack Obama, when he was president, moved his office here for the winter. Now the city, located just 20 minutes drive from the center of Honolulu, has become associated with the Russian Orthodox Church and its shrines, first of all - with the world revered by Orthodox people myrrh-flowing Iverskaya Montreal (Hawaiian) icon of the Mother of God.
The Mother of God in the altar was painted by Matushka Anna Kalinina from Moscow. Nectarius Youngson, the keeper of the myrrh-flowing icon, serves as a deacon in the temple. He shared with us the difficult history of his parish, told us about the miraculous discovery of the Hawaiian icon and about the miracles and healings that occur through prayers before this icon.

  • My family is native Hawaiians, we have lived on the island of Oahu for many years. My great-great-grandfather came from China and married a Filipina. There was a wonderful priest there, Father Peter Salmas. He was going to baptize me, but then the Greek Archdiocese sent him to California. Then I began to go to the temple of the Russian Church Abroad. At that time it was a small mission that rented space in the Episcopal Church. When I first went to their service, it seemed to me that angels were singing, although there were only two people singing on the choir. I was charmed by the service itself, the atmosphere in the parish, and the kindness of the parishioners.
Father Seraphim Rolman was serving there at that time. He agreed to baptize me the following Friday and asked me what name I would like to be baptized with. I said that I had read about a wonderful man, St. Nectarius of Aegina, and I would like him to be my heavenly patron saint. “Oh!” exclaimed the priest, “this Friday is just his commemoration day".
So I was baptized on the day of the memory of St. Nectarius of Aegina according to the old style. For me it was a confirmation that I made the right choice and came to the church where I should be.
On Sunday I was blessed to read the Gospel, and I received Holy Communion for the first time. And after the service, Father Seraphim gave us everything we needed for services and told us that Archbishop Anthony (Medvedev) of San Francisco was sending him to serve in Montana, and for the time being there would be no priest in the parish.
By the way, in the same parish I met our future priest, Fr. Anatoly Levin, who was then a longtime subdeacon.

Averky, knowing that we needed a permanent priest and that our subdeacon would make a worthy shepherd, began to persuade his Japanese wife Emiko to agree to ordain her husband. Fr. Anatoly was at that time a professor at the university in Honolulu, and after retirement he could devote all his time to the temple.

In 1984, Archbishop Anthony (Sinkevich) of Los Angeles came to us and told us that the Iveron Icon of the Mother of God had begun to myrrh-streaming in Montreal and brought us a copy of it. Vladyka suggested that we dedicate our parish to the Iveron Icon of Our Lady of Montreal. So our community became the first in the world to be dedicated to this icon.
Since 1986 we had Hieromonk Thomas (Delano; a former Hollywood actor who became a monk in 1978) serving us for five years, and then again for almost ten years there was no permanent priest.
But we were gradually building our parish. Sometimes I would get sad and call Fr. Averky in Jordanville and almost shout: “Why can't we just go to the Greek church?” But Fr. Averky told us to keep praying and that there should be more temples, not fewer; that the Mother of God would help us.
And so God sent us Fr. Anatoly. That's when we began to have a real regular liturgical life.
At that time we had only a few families who supported the parish: Russians, Ukrainians, Serbs, Romanians, local Hawaiians.
At that time we served in a tiny office converted for the church (only 18 square meters!)
Fr. Anatoly tried to tell as much as possible about the temple, and the parish gradually grew. We served then in a tiny office (only 18 square meters!) converted for the temple. But what a joyful atmosphere there was! I remembered the words of Vladyka John (Maximovich). When his congregation in Shanghai was deprived of its cathedral, they were forced to serve in a garage, and many people cried. And then the saint consoled them that this place is greater than any cathedral, because here is the Body and Blood of Christ. We felt the same way when we served in our little room. Ten years later we rented a bigger place in the center of Honolulu.
And the church we have now is a miracle!

  • Five years ago our long-time pastor gathered for retirement (now Fr. Anatoly is our rector emeritus and continues to serve to the best of his ability). We needed a younger priest. In 2016, I just went with an icon and several priests to Alaska. One priest I liked there was Fr. Athanasius Cohn. He had lived with his family on Kadyak Island in Alaska for many years, and he had an islander mentality.
Archbishop Kyrill of San Francisco and Western America considered him a good candidate for our temple and sent him to Hawaii. Our parish is mixed: now there are Russians, Serbs, Bulgarians, Ukrainians, Palestinians, believers from Alaska, and we needed a missionary priest who would be able to spread Orthodoxy on the island in the future, to bring people to faith, so that they would seek Christ and follow Christ to the temple.
Two years after Fr. Athanasius came to us, in 2018, we started looking for a new place for the parish. But we didn't have much money, especially since the parish decided to pay the rector a salary for the first time in more than 20 years so that he wouldn't have to work a secular job as well. After all, it is no secret that abroad almost all clergy, in addition to serving, also work. There are priests-computer scientists and priests-doctors, construction workers, truckers and even cleaners. And in Hawaii, life is expensive.
It took us a long time to find a suitable place to rent. We thought maybe we could buy it. We looked for two years, and one day we got a call that there was a former mission in Keilua, not far from Honolulu, that was moving to Texas. It was a Baptist congregation with a small church, a school that needed to be remodeled.
We liked the place. The pastor told us that Jehovah's Witnesses and other denominations wanted to buy the premises, but he wanted to sell it to a real Christian parish.

Then we started looking for a bank that would help us with the money, but we couldn't find one. And then something interesting happened. We found an Orthodox bank that gave Orthodox churches money on credit for repairs, expansion, and remodeling, but had never before given money to buy a church. Father Athanasius and I applied to them, but before that we served a prayer service to the Mother of God. And they accepted our application! And the entire management of the bank, starting with its president, was very willing to help our mission. We launched a fundraising campaign, and people from all over the world sent money, which we were able to make as a down payment.
All of this was happening during the coronavirus pandemic. We bought the spaces of our future church in March 2020, Easter services were held at the old location, and my father, the contractor, began working on remodeling and remodeling the buildings.
Last year was a year of much work. And all along we had some obstacles that came up. But as soon as the obstacles came up, the solution came!
When we started to renovate the masonry on the outside of the church, the mason took the money for the work and disappeared. But on the same day another mason came and offered to help. We would have gladly taken it, because at that time we were just making an ambo in the church. But he didn't even know the exact dimensions. My father said to me: “Let's build the ambo, then we'll decide. Mother of God will help.”
We assembled the iconostasis given to us, and it turned out that it exactly fits our ambo!
The iconostasis was delivered by container to Hawaii. We assembled it, and it turned out to fit our pulpit exactly! As Fr. Anatoly said, the iconostasis was made 100 years ago just for us.
About a year ago we completed the reconstruction of the church building and the neighboring school building, kitchen, library, and began to serve in our new church.
Even though the new church is a 15-minute drive from the former church, the very thought of driving through the mountains proved psychologically difficult for some of our members who live close to downtown Honolulu. We have lost some families, but gained more. We have Sunday school, the children serve, and sing in the choir where my matushka practices with them.

  • Fr. Nektarius, people usually see you in churches, where you visit with your myrrh-flowing icon. But few people know about your secular life, about your mother in particular.....
  • Blythe grew up in a not very church-going family, but she loved music and sang in the choir at the same Epiphany Episcopal Church in Honolulu, where our Orthodox liturgy began at the end of their service. But this we discovered later. In the mid-1990s, she converted to Orthodoxy and was given the name Iya in baptism. Interestingly, the same weekend that Blythe was baptized, I was tonsured a reader.
We married, and when I was ordained a deacon in 2018, Iya became a matushka. She is of Japanese descent, a math teacher by profession, and teaches at a Catholic high school. In the temple, she leads the choir and teaches the younger generation to read and sing.

  • I was a policeman, and after I was badly injured in the line of duty, I switched to investigative work. Almost at the same time, the icon became myrrh-flowing, and I had more opportunity to pilgrimage with it. On trips I take my computer with me and work. When there is a lot of work, one of the attendants is in the temple with the icon, and I work with documents - in the church office or in the car.
I love my job, but I still look forward to the time when I retire and can devote more time to the icon of the Mother of God and the temple.

Very many I remember are either simple parishioners or students at Holy Trinity Seminary in Jordanville, St. Tikhon's Seminary and St. Vladimir's Seminary.
Some priests come up to me and ask, “Do you remember me? I was a seminarian and I was friends with a girl. Now I am a priest, she is my matushka, and we have four children!”

I raised my hands and found that the shelf was wet and the smell of roses was coming from there. I picked up an icon and an oily liquid - myrrh - trickled out of it. The myrrh flowed right from under the dust on the icon. Then drops began to appear on the icon itself.

We knew about the myrrh-flowing Iveron Icon of the Mother of God of Montreal because our parish is named after her, but I had never seen it myself, it had never come to Hawaii. I was told that Brother Joseph Munoz was going to come with the icon, but while he was living on earth, he was not able to do so. Perhaps after his martyrdom he was here with the icon.

Hello,
I have a 7 year old pitbull who keeps growing a large non-cancerous cyst/lump on her back. About a year ago I noticed a lump growing and brought her to a vet to get it tested for cancer. The vet stated it was non cancerous and surgically removed it. Since then, this lump keeps growing back to the size of a baseball/softball every 4-6 months and I get it surgically removed because it looks like it will pop.
Approximately 1 month ago, I brought her in to get it removed and the lump grew back even faster to the same size and that’s where I am today.
I brought her to a different vet for a second opinion, they tested it for cancer, and stated it was non-cancerous again. I forgot the term but they described it as many blood pocket cells with pus. Before another surgery they want to try steroids.
Putting her down isn’t an option for me since she does not appear to be in any pain but is there any other options other than surgery that would be effective in reducing this? Or if I opt for surgery is there anything the vet can do to prevent it from growing back so rapidly? I can imagine surgery is hard on her so frequently…

We are lost and defeated and scared to go back to court in fear of BM gaining more custody.
This is going to be long because last time I asked for advice people kept asking for more details (rightfully so) so I am going to try and be as clear and concise as possible. My husband and I have joint custody of his 4 year old daughter. Her mom only gets 30 days in summer and spring break. Our situation is strained, she will not talk to my husband so I am the mediator. I do try to be as unbiased as I can, because I believe in coparenting (I am from a broken but very happy and healthy family, so I know it’s possible and really made our lives better as kids, full of love) however she says mostly the right things, posts on TikTok about how much she loves her daughter and what not, but her actions don’t show that at all. She also bashes my husband for being an abusive cheater, where in fact she was the abuser and we have multiple witness statements from his coworkers, his bosses, and his friends that had witnessed her behavior in their home and at his. She was also extremely unstable and suicidal to the point my husband would have to remove the knives and forks, and hide medications to keep her from trying to commit suicide, which was also witnessed by coworkers (she’d go to his job mad looking for pills and go ballistic on him because he wouldn’t give them to her. He was prescribed meds for his broken back) this is all before I was in the picture, and I did ask her side of it and she said they were best friends and super happy until he left for a job for a week and then it fell apart. I did witness the way she talked and yelled at him and got mad if he was with other women AFTER they divorced and would call them names.
When he went to that job for a week, she absconded with their daughter and then cut off contact. He had fought for her for about a year but didn’t have a very good lawyer and basically they said it wasn’t kidnapping because that’s her mom (yes legally true). He didn’t see her or talk to her during that time except when he would give her his entire paycheck to support them (about 7k a month). She also still had full cover healthcare and dental. His BM made the arrangement that she gets his full check after bills are paid (while he had to live with a friend and still had to pay her bills and his), he was not allowed to be with anyone else and she spun it as if it was for their daughter, no strangers around her kind of bs, even though he didn’t get to see her and they lived in different states. During the time we were first together, she had called and was all you wanna talk to your daughter, this was out of no where not scheduled so him and I were together, and she heard me singing in the background and freaked out and called me some whore and that she didn’t want whores around her baby and then hung up. When him and I got together I told him he did not need to pay her 7k a month legally, and if she wants actual child support, then they need to go to court and get it figured out. I figured she wanted the benefits of his money, and having another woman threatens that, and the fact she spun it in a way that made it seem like it was for her daughter… smh.
We lived in Hawaii, they were in Texas. When he cut off the money, she went ballistic. Chain calling like 50-60 times, calling him names, having family call and threaten him. We hired a good lawyer and got together the witness statements and receipts of texts/money transfers and went to court. We waited three hours for her AFTER our scheduled time. The judge (a woman) kept asking her bailiff person to call her and call her until she answers. We were there an hour before, ready to go. She was no where to be found and no one could get a hold of her. After waiting three hours, all the other parties there were done, the judge had them call her one more time and she answered. She was asleep (it was 3pm) and didn’t even know court was set for that day and time (even though she got three notices from our lawyer about the date and time). The judge was giving her legal advice on lawyers and that she needs to get her act together because she could only do so much to help her (isn’t that illegal?). She granted us temporary custody until trial. Daughter is 2 years old at this time.
When we went to pick her up with police, she had a 102 fever (BM said she ran hot when she slept), a yeast infection and rash, was drinking out of a newborn baby bottle with Coca-Cola in it, and her teeth were black. She gave us one stuffy, no shoes, one diaper, no wipes, no clothes in a little backpack. Once we had her and were trying to ascertain her condition and talk with her, she couldn’t speak. She could say no and that was the only word she knew. She couldn’t eat, she would cry and point at her teeth and when we looked they were bleeding. She needed emergency dental surgery and the gums were infected, causing the fever. She needed occupational therapy, speech therapy and they thought she was autistic because she was so behind on development.
Fast forward, next available surgery date was during her mom’s first spring break visit. We thought that was great, her mom would be there and we knew that would make her more comfortable. Her mom accused us of scheduling it then out of spite because “how is she going to spend time with her when she has to be in a hospital sedated and then recovering and on meds“ she canceled her flight out and didn’t come. She didn’t call or ask about anything until three days after and just asked how it was.
My stepdaughter would refuse and throw a six hour tantrum if she was given water, she wanted candy and soda (she’d point and cry for it at the grocery store). She also didn’t sleep, like at all. Maybe three hours in a 24 hour time period and then would crash once or twice a week. Fast forward to first summer visit, she lives with her parents and has no license. She drove up with a friend to pick her up and drove back. She said she didn’t drive at all, but worked at a daycare (the irony) and would take my stepdaughter with her so she. Was driving her around illegally, we just couldn’t prove it.
One of the terms was she could not be left with her maternal grandfather alone because two other female cousins had accused him of sexually abusing them, which she would go out and leave her with them. She also left her with them alone for two months while she had her during the year my husband had no contact with her so she could go live it up in California.
I have been attempting to be the glue because she talks sweet and calls me pet names and has said how sorry she is to my stepdaughter and that she knows better now. I told her the dentist said no sugar for two months and absolutely no candy because of the amount of fillings she had and crowns and root canals. i also provided her entire medical record from the time we got her and she had the ability to contact any of those providers. She sends me a picture of her eating a marshmallow jello mix at daycare. Fast forward to the present, it’s been two years now.
My stepdaughter is doing great and healthy, and talking, and recovered. her mom does annoying little things like teaches her how to flip off her dad and that it means I love you, she also tries to get her to call me her new mommy instead of mom, but as long as she is happy and healthy so am I. We correct those things and ask her mom about them but she just puts the blame on someone else. She has never stayed in the same place with her mom, every visit is somewhere new.
Each visit, she comes home sick or needing medical attention, nothing so severe but I mean come on. She has zero coughs and colds with us, and always comes home sick from her moms? She was only at daycare that first visit so it isn’t that. This last visit she came home with a stye, probably from the glitter and makeup her mom puts on her. When we asked her about getting care for the stye, she said she took her to urgent care and gave her a cream for it. Well, the cream is an OTC one from Walgreens, and so I asked her for the dr name and number so we had the info for follow up with her pediatrician the following day, because her stye was huge and she could barely see. She lied, she didn’t actually take her and tried to tell me she went in there and the dr said it’s no biggie and so she didn’t have to pay she just told her the cream and sent her on her way. My stepdaughter has full coverage insurance, and we gave her a copy of her insurance id for the purpose to cover anything that may come up, so she wouldn’t have had to pay. She also mentioned she took her to a friends friend who is a psychiatrist off the books to get her evaluated for herself because she didn’t believe the evaluations from the multiple therapists and doctors she saw while with us and that dr told her she only didn’t talk because she just needed her mom and to know she was safe. Couldn’t have been the 8 months of speech therapy that helped her talk right? Nope, just needed to see her mom and boom fixed. Cue the eye roll. Anyways, the stye left untreated turned into a hardened chalazion. Per the dr recommendation, we waited a few months to see if it would go away, and it didn’t. They had to surgically remove it. The day of surgery the only thing her mom asked was if she looked cute with an eyepatch. She said she felt bad about it because she felt it was her fault.
I try to always stay neutral so I just said okay thank you and gave her the details of the surgery even though she didn’t ask. Again, she has always had the same rights to contact any of the providers and we never withheld information. She apparently has which again, is illegal right? I keep trying to repair the relationship so we can all try to be coparents and in better terms, but then she does something like this and now I don’t know what to do.
She isn’t outright abusive, just neglectful. She showers her with candy and sweets and treats and toys and so my stepdaughter loves it there, but doesn’t realize that’s why she has four teeth at the age of four. Her mom has also never paid any kind child support or anything, ever. We really want to trust her and repair this, but everytime we have a deep convo she lies to me. My stepdaughter obviously isn’t getting the kind of care she needs there (basic hygiene and medical) and we’re just lost and defeated. We don’t fight with her, we don’t judge her, I even told her I understand her situation and frustrations because I was a single mom for a long time. I do understand that position, but it makes it harder to excuse her behavior because my daughter is healthy and taken care of and I did that on my own, yet she says it’s because she was on her own and my husband is to blame for it all. She left him and accused him of cheating when she got caught cheating. That’s why she left originally.
Other things to note: I have legal guardianship, was granted during first order. multiple late drop offs from visits, last minute scheduling pick up arrangements (like day before). We’ve just been documenting it all, and we were told we cannot bring back the original cases of abuse and neglect because it has been visited in court already, unless it’s to prove a pattern. She also doesn’t exercise all of her visiting rights, only the 30 days because a week isn’t worth it. We invited her on a family vacation with us, fully paid, and she said it was only three days and it wasn’t worth it. She also is coaching my stepdaughter to say horrendous things, she told me i threw her through a bathroom door, then got confused and said no it was daddy, then kept naming different people it was. Her psychiatrist says she shows a lot of coaching, because when asked about our home life no issues, but when asked about her moms, she says I don’t know and that’s all she will say about her mom is idk.
Any advice? Maybe hire a PI? We are so scared to go back to court because depending on the judge, she may have favor and we get less custody if it goes wrong, even with all the evidence. We aren’t looking to terminate her visitation, we are looking to enforce the decree as she has violated it multiple times and take away the visitation she doesn’t exercise and never has. We also are considering pursuing child support and just putting all that into a bank account for my stepdaughter so she has it when she gets older. We also want to put a clause that we need more than 24 hour notice for visitation travel plans and whether or not she is taking her, sometimes we only get a text a few hours before. I read a lot about meds you are all taking but I have not heard anyone mention methadone. I was not given a choice when I was at Kaiser, they said for long term chronic pain my only choice was methadone. I was on a high dose back in the day, I currently take 25mg a day and can live my life. I have no issues, take no extra, never asked for an increase. At 68 with a 3 acre farm and no family to help, my wonderful dr left to care for family, leaving me in the care of an office full of non pain med dr’s . Her ‘replacement’ decided to taper me (and way too fast) off of methadone. Not reading info in my chart as to why I take pain meds- because of severe back problems that I was told are not surgically safe. My only option has been my current meds and I have managed to reduce them from the 35mg to 25mg although it’s not perfect I can at least get up and have a life. Has anyone else been prescribed methadone? Funny thing I could go to a methadone clinic for addiction and be given twice what I take now. But that’s not the issue- so I just wondered if anyone else had been prescribed methadone-
