I participated in the UCSF LIINC study today. For those unfamiliar with it, it’s a program at UCSF that studies patients with Long Covid. Specifically one thing they’re looking for is evidence of viral persistence.
They performed an FNA (fine needle aspiration) on my lymph node today which they plan to send to UPenn to analyze. The FNA procedure itself involves getting numbed in an area where a lymph node is and then they do a biopsy on it. It’s not as painful as it sounds actually. I didn’t really feel much except a couple of pricks from the Lidocaine they gave me. No other side effects really except for a sore neck.
One interesting thing the doctor who performed it said was that all the long covid patients he’s seen have had very small lymph nodes which he said is unusual if your body was fighting a viral infection. It kind of makes me wonder if viral persistence isn’t the cause of LC. They did find evidence of viral persistence in the gut though which is interesting. Maybe the immune system is being suppressed?

Hi! I could really use some advice and experience from nurses out there. I’ve been working various CNA positions over the past six years and I do feel majorly burnt out. The main factor (I think) is the number of patients CNA’s are assigned. I work in a position right now with pretty acute patients (ICU stepdown) and usually have 13 patients, although the other night I had 19 because we were short. It feels heartbreaking because I don’t get to spend the time with patients I know they deserve. I’ve kind of put up this emotional wall between myself and patients which doesn’t feel healthy.
I’m not sure if I want to work in bedside when I graduate, but I do know I want to get further education (either NP or MD, not sure yet - that’s a whole nother can of worms). I worked in hospice for quite some time as a CNA and I loved it. I’ve been anxious about the idea that if I’m this burned out in my position, I will start nursing when I graduate in August already burned out. I think that I would love working in hospice again but I don’t know if experience in that field would be looked upon favorably with grad schools. I’ve been telling myself that only having four patients will feel so much better than a typical CNA ratio, but I’m not sure. I didn’t feel burned out in hospice probably because I only had 6 patients at most, and the work culture was fantastic. This makes me consider staying at the bedside, especially if I like the internship I’m about to start on an oncology floor.

My hospital insurance is horrible but I'm covered by parents still, and I've been having problems with the low census policy (see my previous posts) but other than that, everything is pretty good about the floor.
My problem is that I am a creature of the early morning; I work night shift now and it's eating away at my mental and physical health. I've tried everything I can to adjust and I just can't. I talked to my manager about it, she said there were no positions available for days and that one likely wouldn't br available for another year or two. Fair! But I cannot wait that long. So I have applied to day shift nursing positions, and today I got an offer on a medsurg unit in town. Day shift. Like I should be happy about this, but I just have a gnawing feeling medsurg isn't for me. I was a tech in nursing school on a medsurg unit during covid and that made me dislike it, honestly. Aside from the fact that I did not get the best vibe from the director of the department (who interrupted me during the interview and gave unsolicited advice) when I interviewed, they were very transparent that almost all their patients are "very high acuity" and its a common thing on that floor, and that I would be expected to know a lot within my 6 week orientation. And I'm just being honest, that's not my cup of tea. So I'm thinking I'll stay where I am on nights for now. But once i hit my 1 year mark, I plan to look for more options in specialties I'd like. I've applied for wound care, urogyn clinic, school nursing, pre admission testing, and pre and post surgical and have gotten turned down by all I've applied for.
I guess one of my questions or concerns is that, am I being turned down from these jobs because I only have 10 months under my belt? Two rejection letters said they were looking for more experience. Could it be that being in such a specialized area of nursing (mother-baby) is limiting me on my progression? Considering that's the only place I have RN experience??? Have any other postpartum nurses or nurses in a specialty been able to get other positions outside of your specialty? I'm interested in wound care, psychiatry, public health, and rehab nursing. Do I need to to medsurg to get into these specialties if I only have mother baby experience???

My results from an echo I had a couple of weeks ago were just uploaded to my patient portal. I haven't heard from my doctor yet, so I plan to ask him. Everything was normal (which was better than my last echo, which showed mild LVH), but this raised an eyebrow for me. I have no idea what this means, so wondering if anyone here might?
"The inferior vena cava demonstrates a diameter of <=2.1 cm and collapses >50%; therefore, the right atrial pressure is estimated at 3 mmHg."

2024.05.14 23:41 chadbulled Trane cleaneffects- upgrading to new AC unit

The parents have about a 10 year old Trane Cleaneffects unit. Every time I am at their house I get itchy eyes and allergies. Sometimes in the past I smelled what seemed like chlorine type smell to me when the unit kicked in which I wonder if it might be ozone. Both of them have health issues so I am wondering if this is actually a decent AC unit or problematic, I don't really want them breathing in ozone, but I don't know much about these things and can't seem to conclude if they actually produce ozone in significant quantities.
I've also seen some black gunk coming out of the ceiling vents. I plan on having the ducting replaced when they re-do the home.. I'm fairly certain it's 20 years old and disgusting.
Anyway, they are re-doing their home next year and want a new AC unit. Any particular suggestions? Prioritizing air quality for asthma/heart patients.
House is a 5 bedroom 2500 square feet home in FL.

I am an immigrant and i am planning to apply for 2025 fall or winter semester because I need to complete the requirements, it is my first time going to school in Canada and I don't really know much about the education system in here. I am a 39 year old former college dropout. I worked for 9 years in accounting as a staff accountant. I went back to school in 2020, and finished a Management BS with a concentration on management information systems and business analytics. After I graduated in 2023, I continued on to graduate school where I am currently in a dual MBA/MSBA program (not a top 50 program I don’t think) in the Boston area, that I will finish in May of 2025. I transitioned to an entry level position with the government that is half project support and half technical work. What should I do next? I am unsure what to do. I plan to stay in government so I can take advantage of PSLF, but I want to know what I can/should do with my degrees and background to maximize my potential earnings. Should I pursue a PhD? What kind of positions should I aim for? I pursued this degree path because I do believe that data is the newest commodity and understanding it would be key to ensuring my educational background and skills would be suited for future demand curves, am I on the right track?

2024.05.14 23:35 sadphrogs I’m quitting my current internship tomorrow for a different one, any advice on how to do it nicely?

I’m pretty nervous about quitting ngl. I’m 17F and have worked at my current company for over a year now. This company sorta planned on me staying, even though I never confirmed that, so I’m worried they aren’t gonna be happy. I went through the job list the past year and I have done over 50% of them, so I’m worried they aren’t going to be able to handle me leaving? But at the same time, I feel like a company shouldn’t really be relying on the high school intern for engineering?
Anyways, my school got an email last week from this massive multi billion refinery company near me, in which they offered me an engineering internship because they saw I placed at a drafting competition at states. This job pays less than my current, especially considering my current job said they would give me another raise if I stay for the summer, but I decided it was worth doing so I could diversify a bit. It’s also just a massive company that will look good and I will learn a lot, no matter how it goes.
My plan to quit is to talk to my boss and hand him a written letter. I want to make sure to give them two weeks, but I’m still nervous they might be upset I didn’t give more of a notice? But I also didn’t know I would get this job until a few days ago. Does anybody know how to break the news lightly? My boss is pretty nice, I honestly just feel a bit bad because I currently have the most open jobs and everybody in engineering is buried, and they will be especially buried when they have to split my jobs up.
Also does anybody know how to make a good first impression for my new job? My new internship usually only offers to college graduates who are related to a person already employed, so it’s a big deal for me. My main concern is just that I don’t have as much education as the other interns. I’m pretty good at AutoCAD, but my highschool and work doesn’t use anything else so I don’t know solidworks or inventor well. Also just general knowledge I might be lacking, like higher math skills and engineering principles. I had these same stresses with my first job, and that went okay, but this job and company is 1000x bigger than that.
