2024.05.04 05:10 deka27 Radiation therapy for FIC

I have a 1yo MN DSH who had a PU sx at 4mo due to penile trauma incurred at 7wks old from littermates suckling. Since his PU, he's had 2 sever bouts of FIC flares, this current one required a u-cath for a week.
He's on Purina UR wet, with water added to increase water intake. He gags at the smell of hydra care so that won't help. We did a Solensia injection a week ago, he's finishing a course of abx, gets .5ml 50mg/mL Gabapentin q8h, .5ml Prazosin BID, Dasuquin 1 chew SID, 1000mg Fish Oil SID. We're likely going to start Adequan when he fully recovers from this bout, and I have Feliway diffusers going. Even pushing oral water intake, I can't get his spec gravity below 1.025 (rDVM wanted me to test at home daily so I bought a refractometer and have been using it while he's been catheterization this week). Recent rads showed no radiopaque stones, AFAST showed SIGNIFICANT bladder inflammation. Chemistry was WNL, except slightly elevated ALT and slightly decreased Creatinine.
He's been on UR, Dasuquin and fish oil for 6 months. Gaba, Prazosin and Onsior during flares.
I talked with his rDVM about adding Dantrolene as it seems he is constricting while urinating almost everytime, causing very spastic / spurting urination rather than smooth long streams. He looked into it, said it was a very expensive drug and wanted to try Fluoxetine first. We also looked into Bethanechol, but he was concerned about side effects.
Has anyone here had experience with Radiation therapy for FIC? I schedule a consult with a local specialty clinic for the end of the month, but would really love some more info than the few webpages I've seen about it.
Is there anything else we should be considering or trying that we haven't yet?

TL:DR: Classic FIC disaster having reoccurring flares that cause blockages so I'm considering Radiation therapy.

This is my first time posting one of these types of questions. I know how detailed people need to be in order to get informed responses so please bear with me in the comments as I get back to you the computer specs that are requested. For now, here is the issue: Only recently have my h.264 exports had this weird black and white glitch that FLASHES at random. It doesn't seem to show up in Premiere Pro (though I have seen it once) so it's not an issue on the timeline. This is the before...

Something seems to change between the "GLITCHY 1280x720 preset" and the "NON-GLITCHY 4K preset" but my untrained eye can't seem to figure out the root issue and I'd love to be able to export the smaller-sized videos again without the glitches so let me know what I can look into to figure this out.

SMALL UPDATE: I'm noticing the issue might be specifically linked with the aspect ratio as I exported using the 4K settings but changing ONLY the aspect ratio and the glitches returned with a vengeance. Still experimenting.
SECOND UPDATE: While every version of 1280x720 h.264 export I tried through Media Encoder yielded the glitch, I switched the Renderer to Mercury Playback Engine Software Only and the glitching seems to have disappeared. Is there an obvious root issue here? And can I eventually switch back to GPU Acceleration or will I have to endure the x2 exporting time from now on to avoid glitches? I wanna try and figure out EXACTLY what's happening.

Hello all. I went in this past Tuesday because of some pain in an upper molar. The tooth was bad and it was not unexpected when the dentist said we'd need to pull it. But first he wanted to put me on amoxicillin q8h x7d and reschedule the extraction for next Tuesday. Great. That will probably relieve some of the pain, which up to then had been completely manageable with a rotation of ibuprofen q8h and acetaminophen q8h.
Fast forward four days and I am now in excruciating pain that is not managed AT ALL with OTC NSAIDs. The pain becomes face-numbingly unbearable after each dose of amoxicillin.
Have you heard of anything like this happening? Should I be worried? Is the amoxicillin having an unintended effect on the tooth or infection? I am going to an emergency dentist in the morning but I'm PAINFULLY curious as to why my pain spikes after each antibiotic dose.

Species: Dog Age: 4.5 Sex: Female/Spayed Breed: Border Collie-Australian Shepherd Mix Body weight: 22-24kg General Location: Southwestern Ontario, Canada History: -Diagnosed ideopathic epilepsy approx. 1 year ago, progressively symptomatic, presents as full grand mal -Currently, approx. every 2 months has seizures for about 2-3 days, 4-6 hours apart and last anywhere between 1-3 minutes -Currently taking Keppra 750mg q8h
Clinical signs: She often presents as clingy, anxious, afraid during the pre-ictal phase and will try to jump into my lap. She had three seizures recently over the course of two days (timing for a seizure is normal; she was due and we are managing) however since then, she will randomly come to me as clingy, anxious, and afraid - like she is about to have a seizure, but does not. This has happened 2-3 times a day for the past 3 days. The presentation lasts about 10-15 seconds (which is consistent with her usual routine of pre-seizure behaviour) and then she enjoys her pets and cuddles and curls up to fall asleep
Question: Is it common for a dog with ideopathic epilepsy to show signs of a pre-seizure without having seizure activity? Is this foreboding of a seizure to come - should I still expect a seizure to come? (Typically, understanding that there are specific circumstances and individual cases. This is just all new to heus.) Can I give her an extra dose of medication to potentially decrease that threshold of a seizure?

However, I've noticed that when I compare by Path of Building character to the ones in the guides I follow, my damage stats are wayyyy behind despite having what I think is pretty great gear. I noticed yesterday evening that I had made a major talent mistake (I kept precise technique after swapping to crit stuff) that helped a bit, but things seem to still be quite off. I feel like I'm making another one or two small but impactful oversights that I just can't seem to spot.

I was running in good profits and it was just because of my winning streak.
I have no sentimental control whatsoever and I have paid dearly for it ....

As I had already mentioned in the previous Post I don't use stoploss, I didn't for a trade last week.
I scalp using high volumes and once the trade backfired, I kept waiting for miracle which I knew will not happen.
My mentality is somewhat like this:
Bought something at say 100
Profits: Goes to 120, comes back to 115, I sell it at 120 using a limit order and then it goes upto 200
Loss: Goes to 90, I wait for cost, comes to 100 I wait for some profit(atleast 80 rupees to cover brokerage), goes back to 75, then I keep hoping for a reversal and then finally exit at around 25, thus blowing up my entire account.
I was always aware of Stoploss and GTT orders but I thought "Stoploss is for pussies who backout and are fooled later". But wrong ! I was the actual pussy who didn't have to cut losses early.
Today I used the GTT stoploss and Target order while buying and I earned some petty cash from it but I felt so good about one thing, how light I was feeling, how stressfree, rather than staring at the screen like a maniac.
My sincere advice to fellow traders :
Start using stoploss! I used it today for the first time and it is actually a good feeling to be stressfree (Naked options mein bhi risk limit kar dene se you are killing one of the biggest downside of the instrument)
Put SL and Target while buying the option ..... And then just keep sliding (only upwards!!!)the window to maximize profits ....
Trailing SL is a great way to chase a rally without risk .....
Guys don't trade like me, a trade gone wrong has to be accepted and damage is to be minimised. Capital rahega to loss wapas aajayega, capital gaya to tum gaye market ke baahar.

2024.02.21 06:55 Ok_Marsupial728 Sudden delirium as primary pneumonia symptom? Could anything else be happening? I am so desperate to get my mom back. Any ideas are welcome. Hospital MDs seem perplexed.

Writing for my mom (66F). Height 5' 8", ~220 pounds, Ashkenazi Jewish, primary complaint: delirium maybe secondary to pneumonia?, or maybe medication-induced?, no drinking, no smoking, no recreational drugs, we are in northern California
I will share any labs requested and have tried to summarize them. I am an only child and have no family other than my mom and the past month has been the worst of my life. I cry all the time and am so scared.
Current meds in hospital: IV doxycycline q12h, IV cefepime q8h, weaning off depakote, starting risperidone .5mg, levothyroxine (unsure but very small dose), omeprazole 20mg once a day, Adderall 10mg in the morning
At home she also takes uqora supplements (these have actually really reduced her UTI frequency). I haven't brought them into the hospital yet but since she'll be there a while I probably will.
My mom has had multiple sclerosis for about 20 years (probably longer, but it was dismissed as anxiety). Her primary symptom is leg weakness; she uses a powerchair and can stand and pivot with minimal assistance from others. I live with her and have not noticed any cognitive changes due to MS, though she does have ADHD and has her whole life, I suspect. She is a full-time professor and was teaching until the end of last semester; she took this semester off to edit her textbook.
Here is a timeline of what's happened:
Jan 11 - 15? - upper respiratory infection, negative for COVID, everything starts after this
Jan 18 - she forgets my birthday, I begin getting concerned, sometime between this and the 22nd I suspect she has a UTI and she takes Keflex (culture shows either contamination or colonization), this appears to help the confusion briefly
Jan 22 - she begins to get more and more confused
Jan 24 - starts hallucinating that there are people
Jan 25 - brought into ED, thinks she’s in South Africa, significant hallucinations
Jan 25 - Feb 8 - hospitalized for encephalopathy and delirium, thought is microseizures from Ampyra usage (for MS, she had been on this drug since it was FDA approved, ~15 years) and URI triggered delirium, gets nosocomial UTI and rx’d 7 days of Bactrim, significant improvement by discharge (discharged with Prednisone 40 mg, Zyprexa 5 mg, Depakote 1000 mg), CT shows possible infectious nodules in lungs but these are never followed up on. Sometime around February 2nd or 3rd she begins showing improvement, but before that, she has been extremely combative, had to be in restraints, and was hallucinating and delirious more or less constantly. She did have some very lucid moments even in that time period, however.
Feb 8 - 12th/13th - appears to be doing well, still some hallucinations and confusion but felt to be on upward trend by both of us - standing and transferring well after a few days, tells me about how for 30 seconds “I thought I was in Miami but then thought, that’s crazy!”, reading the New Yorker, score of 28 when I administered MMSE (I work in healthcare research and felt competent to administer) though has moments of confusion, remembers passwords, short-term memory improving, has home health PT and is very motivated (BIMS administered by home health PT, perfect score), fairly consistently AOx4, talking about editing her textbook manuscript. She is eating very well and keeps telling me how happy she is to be home. She is very traumatized by her hospital stay and seems to remember much more than you would think for someone who had delirium.
Feb 13 - I notice a cough, but it seems to improve throughout the day. I dismiss this as that we have just turned the heat on, and that always dries our sinuses out.
Feb 14 - Bactrim has stopped a few days prior, Zyprexa stopped by her PCP due to concern of side effects (makes her feel very groggy and sleep a lot), feels she is on upward trend, has a PCP appointment where her long-term PCP notes that while she is confused at points, also has quite a few higher-than-college-level responses, she complains of a sore throat that night
Feb 15 - ?? - wakes up feeling extremely cold and confused and thinks she is in London or Thailand, I call 911, readmitted for encephalopathy/delirium likely due to pneumonia that Bactrim was potentially holding at bayOn readmission on the 15th, her temperature got down to 91.8F. It has since risen to around 98F. She was initially hypoxic in the mid-80s, but now is satting 94-98 on room air. Her encephalopathy and delirium continue--she is consistently only ao1-2. She has only a very mild cough and only pain from her IV, no complaints of chest pain. Initial ER x-ray showed either fluid overload or pneumonia, follow-up CT of chest showed pneumonia (" LUNGS: Interval development of patchy opacities in the right upper and middle lobes. Mild interval progression of airspace consolidation in the right lower lobe. Interval development of patchy groundglass opacities in the left lower lobe. Technical factors or superimposed disease prevent exclusion of pulmonary nodules (#PULX)." She was started on vancomycin and cefepime, but when MRSA screen came back negative vanco was removed. Now she is on doxycycline and cefepime after a chest x-ray showed maybe progression of pneumonia, but they were unclear on this. Again, her oxygen is fine, and she doesn't seem to be in any pain except from her IV. You wouldn't know she had pneumonia if it weren't for the delirium, which I'm told is very uncommon.
Urine screen on admission was clear, no UTI.
She does have lucid moments, but now she's just terrified out of her mind in all of them. Like she was saying "I can't make decisions, this is so scary" and talking about how "this is the most awful thing to ever happen to me". She seems to understand she has pneumonia, and starts crying when she thinks about it. She is not consistently aware that she's in the hospital.
The working theory from the MDs is that this is all one long delirium episode starting from the initial URI, which was unintentioanlly held at bay from causing pneumonia by the Keflex, then unintentionally held at bay again by the Bactrim. Once she was off that, it could progress into a full pneumonia. I'm just getting scared because there's been no improvement in her delirium yet (they say that there has been a little, but I don't see it and I'm there 12 hours a day). Will she make a full recovery? I've read that delirium is more distressing than it is serious, but I'm so scared. I'm also wondering if this could be something else entirely, given that delirium presenting as the primary symptom as pneumonia seems very rare to me. What is the prognosis like? Her labs have all been pretty okay (WBC WNL, slightly anemic but like 11), slightly low platelets (~100) but she always gets slightly low platelets when she has infections.
CT of head has been normal, MRI of brain was stable MS-wise from 2019, nothing else remarkable on it.
They also have said they haven't really seen a case quite like this before, and when I've pushed on the % chance they're right that it's the pneumonia and once that's treated her delirium should clear, one doctor said 66% and another said 70%.

Hi all,
I'm building a new system with a local company in upstate NY. Really excited to have solar for the first time but I'm a total noob! Looking for a true "off grid" solution with battery backup. The company has sold me a 17.22 kW REC 410 system with Enphase inverters (iQ8H). This will meet 100% of our need. I also want battery for storage in the event of prolonged power outage and they recommend partial home enphase iQ batteries. They initially recommend 4 for 20kWh storage but I asked for more so we agreed on 8 (for multiple winter days without the sun).
My question is now that the Tesla PW3 is out, should I consider that instead of the enphase? My rep really likes the enphase and recommended against the PW3, but most of the specs weren't available until now.
What are you thoughts? Appreciate your advice!

Husband (48M) nonsmoker, uncontrolled moderate HTN, has H/O epilepsy but under control. Takes depakote, testosterone shots, occasional drinker (A few beers a month, occasional mixed drinks a few times a year).
Had temporary crown 6 wks ago to replace a really old filling. He waited almost a month to go back instead of going back after 2 wks. We was in a moderate amount of pain before the filling was removed, and it never got better. 2 wks ago permanent crown placed. When numbess wore off his jaw was hurting really bad.
A week ago he was seen again bc pain never got better. Dentist said it was a reaction to the numbing injection. Was prescribed cyclobenzaprine 10mg q12h, and ibuprofen 800mg q8h not helping even a little.
Is this something that needs to be seen immediately, and should we try to get in with a new dentist (more details in comment)?

I (34F, Hispanic) am a relatively new T1D. I was taking metformin 500 mg, but urgent care put me on steroids for pneumonia and my sugar went up to 685, requiring two ER trips. They increased my metformin to 1000 mg twice a day and put me on glipizide 10 mg in the morning. My PCP also put me on Ozempic 0.25 weekly. According to my CGM, my glucose has been running 90-200 during the day. But at night it's been dropping two or three times per night to the high 60s. My CGM alerts me and I go have a snack or drink some orange juice. I do experience Dawn Phenomenon where my sugar spikes to 140 right as I'm waking up. What can I do to prevent these lows? I do often have a pretty big meal right before bed. I'm concerned that if I adjust my metformin nightly dose, my fasting sugar will get worse. Sorry about formatting. I'm on my cell.

I've (34 F) recently started biting/grinding my teeth throughout my day. It's not when I'm nervous or anything. It's all the time. If I think about it, I can make it stop, so my psychiatrist doesn't think it's TD. I have other tics like tapping my fingers and rocking back and forth. My doctor prescribed Cogentin to help with my symptoms and I started taking it today, but I just read online that Cogentin should not be prescribed for TD as it can make it worse. I don't know if that applies to motor tics as well. Please advise. Thanks.

I am a 34 y/o female Hispanic. I have several medical and mental health conditions and take several medications, listed below. I quit smoking cigarettes Jan 1st (using NRT), rarely drink, and do not do drugs.
I am getting a tingling and sometimes crawling sensation, along with pressure, in front of and inside my left ear. There's been times where I've been sure there's a bug crawling in and around my ear, but I've looked in the mirror and also had other people check with no results. I know there's a sinus there, and I did just get over a cold. I did not have this sensation during my cold. The only other things I think it might be is a tactile hallucination (is that a thing?) or nerve irritation. I have not experienced any pain with this sensation.

Part 2 – The pen was in another dimension
When we talk about joke writing, a lot of models and types of constructions stand out. One of the most simple to apprehend is the three part structure, the typical joke construction. A lot of movies, TV shows and comedians use it, and while telling a simple joke to a friend, you might use it without knowing it. The three parts are different and their definitions slightly change with the person describing them, but they roughly remain the same, and there are : the framing (or set up), the narrative (or build), and the punchline. The framing is the overall context necessary to understand the joke. It can be said right at the beginning, to make sure the audience understands. In a movie or TV show, the framing is already set in previous scenes, so it is not necessary to place it within the joke. Here’s a framing : « A guest is ordering at a restaurant ». The narrative is the beginning of the joke itself, it is a series of actions and/or dialogues that builds towards the third part, the punchline. Here’s a narrative : « The guest in the restaurant asks the waiter if he can order what the man at the nearest table is eating ». Finally, the punchline is part that’s supposed to make people laugh. It’s the very last part of the joke, the one that makes sense of everything that was set up before. It can be an unexpected twist, a physical impact, a weird offsetting, a pun, anything. To finish the joke I started, here’s the punchline : « The waiter responds : No sire, this man intends to eat it himself. » Unexpected. The waiter didn’t understand the order. Funny. I guess…
To me, there’s no better illustration to how funny this construction can be than misdirection jokes. If you’re a fan of The Simpsons, you already know them. It’s a type of joke where the framing and the narrative induce you into thinking a situation is happening or will go a certain way, only for the punchline to surprise you by doing a 180, turning the entire thing in a new direction, in most cases absurd or surreal. To come back to the ZAZ, in their movie Top Secret, we have this scene. I give you the framing : we’re in a military base, intruders have infiltrated the place and have been spotted, an officer is calling his superior to inform him, and this happens : [Top Secret-1]. So in this scene, the narrative is that the ringing phone seems huge in comparison to everything else, but as viewers we simply think it’s because of the perspective, the phone is not really THIS huge. Then the officer comes to take it, and as he does, punchline, the phone was completely out of proportion since the very beginning.
Another example, in the Simpson episode And with Maggie it makes three. The whole episode is a flashback telling us how Marge became pregnant with Maggie. At one point, she asks her sisters Patty and Selma to not say it to anybody. It is then followed by this scene : [Simpson-2]. Patty calls one person to announce the news to them, then the transition, her position and the fact she ends up at the very end of the phonebook make us think she has called every person on it. And then, surprise, she has just called the two people we saw her talking to. They just added the transition to make us think otherwise.
You have guessed correctly, I love this type of joke. And it’s pretty fair, because Elliott from Earth has very good ones. Take this scene from Episode 8 [Scène Late]. When the Hive director talks about her late husband, the phrasing and her body language make us think he passed away. Then the punchline reveals he was actually alive, it’s just that we and Frankie misinterpreted the word « late » and that she decided to act like she was sad, probably to fool us into thinking she can feel some kind of emotion because actually, she’ll turn out to be the real villain of the show. Or maybe not. But you’ve understood how the joke works. And if you have a good memory, you may have realized the scene with Lord Kallous seemingly choking another person is also a misdirection joke.
Of course, every joke doesn’t have to strictly follow the three parts structure to the letter. Sometimes, a comical element can just be implemented in the middle of a scene without any narrative. In this scene from Episode 1 [Scène Vélo], the broken bicycle is introduced at the beginning of the scene. We just see it while the characters talk, and at the end, it just falls and that’s it. There is no real narrative. No need to specifically focus on it. No need to make it look suspicious. Just placing it and using it when you need to is enough. Not the gag of the century, but it’s fine. Other jokes surprise the audience by not ending with the punchline but using it as a way to create a new comical element. Take this scene from Episode 13 [Scène Ash]. The three parts are crystal clear. Framing : Elliott and Mo are looking for someone to eat with. Narrative : They decide to eat with Ash, who’s about to bite a sandwich. Punchline : Ash actually has fire powers, which causes problems. The joke could’ve stopped with Ash igniting his sandwich, it would’ve been enough. But as we think it’s going to end, the stakes suddenly escalate, with the character putting his plate, then his table, on fire, and responding to all of this by saying he wasn’t hungry in an apathetic tone. In just a few seconds, we had three new comical elements. A nice way to expand a simple joke without giving the feeling it’s getting dragged on.
To end this part on joke writing, I’d like to talk about an important thing to have in mind. It may have already happened to you, but when watching a comedy, maybe you have laughed at a seemingly insignificant thing, something that was not necessarily the primary focus of the joke, but that was still funny. That’s because, when writing comedy, everything that happens on screen, every little detail, can be a source of laughter. And for a change, one of the artwork that understood this better comes from Japan. And that is : Assassination Classroom.
For those who don’t know it, Assassination Classroom is an anime that came out in 2015. It is about a normal class of young japanese students who are tasked by the government to kill their teacher, Koro-sensei, a squid-like creature who has the ability to move extremely fast, has destroyed the moon and is about to do the same thing to Earth. Your eyes don’t deceive you, that’s the actual story. The anime follows the daily life of these students and their teachetarget and is punctuated with their regular attempts at killing him, attempts which are doomed to fail of course, because there wouldn’t be a show in the first place if they succeeded in the first episode. Behind this delirious synopsis, the anime is actually a lesson in comedy. It is still today one the funniest anime, and overall one the funniest piece of media, I ever saw. And one of its greatest strengths is what I just discussed, that being its capacity to create dozens of excellent sources of humor out of overall pretty simple jokes.
But let’s take an example. In this scene very early in the anime, several students attempt an assassination on Koro-sensei by walking towards him with a friendly face to surprise him. Of course, they’re going to fail. Imagine for a few seconds how you would write such a scene, and now look at how the author wrote it : [AC-1].
Let me ask you a question : how many comical elements do you see in this scene ? Personally, I count 9. 1 : the student running towards Koro-sensei while being way too overly friendly. 2 : Koro-sensei being genuinely touched by their mark of affection. 3 : the student keeping their ridiculous, uncanny friendly face while trying to stab him. 4 : Koro-sensei doing all of a sudden a 180 in his behavior while spotting the threat. 5 : Him being cool enough to switch their blades with flowers. 6 : The fact the flowers he used were actually planted by the students. 7 : The students acting like their teacher’s mistake was the worst act of malice they’ve ever seen (I remind you they’re talking to a monster who’s going to destroy Earth). 8 : Koro-sensei being so devastated he decides to put new flowers on the spot. 9 : The student starting to act like a drill sergeant and commanding him.
In one minute, we had 9 occasions to laugh, and each joke managed to be cohesive by directly being linked to the previous one and the next one. Not only that, but it is also coherent with the characters. I haven’t said anything about it, but all of Koro-sensei’s reactions in this scene, from his love to his students to his desire to be cool with them, are genuine and an essential part of his character. Same for the students acting like he’s equal to them. It also moves the plot forward, because not that long after that, there is a new scene dedicated to the consequences of this one. And having that many sources of comedy in less than one minute, it is not a whim or doing nonsense hoping something will work on a misunderstanding. It’s effectiveness. It’s understanding where humor can come from. And in this case, it also implies to know who the characters are.
Here’s another example. During one of their training sessions, the students play cops and robbers, with their teachers being cops. One of them chases them while Koro-sensei plays the part of the prison officer. In this scene, a group of students have been captured and one of their friends is trying to find a way to free them [AC-2].
I’m gonna do this again : how many comical elements do you see in this scene ? For me, there is : 1 : the student doing his weird dance thing to communicate with his teammate. 2 : Koro-sensei being full method actor and acting like an american cop caricature while being interrupted. 3 : The student bribing him with a photo of a girl in a bikini (which also fits the characters as both the student and Koro-sensei are described during the show as being quite pervy). 4 : The students deciding not to free one of the prisoners (because it’s someone they don’t get along with in the show). 5 : The other teacher being reasonably angry. 6 : Koro-sensei inventing a ridiculous excuse for the escape. 7 : Him suddenly paying attention to the girl on the photo in the middle of the conversation. 8 : The fact the other teacher was actually talking the whole time next to a bunch of captured students, which is also quite good narratively as it links this scene to the next one, where we see what happens with these prisoners.
Do we find that kind of humor in the G-team’s work ? Of course. Take this scene in Elliott from Earth’s episode 4 : [Scène feuille]. One last time. How many comical elements ? 1 : Mo promptly eating an alien’s leaves/hair because he’s a vegetarian. 2 : Him ripping off the head. 3 : A new head growing to replace the old one. 4 : The new head politely asking for the old one like it’s nothing. 5 : Him dismissing the whole incident by talking about the old one needing an haircut. 6 : The two heads starting a small argument about the haircut like an old married couple or two best friends. Honestly, for a scene that’s just here to introduce the “Touch with your eyes'' rule that Mo will follow during the episode, I find it pretty impressive.
I’ll stop there, but I think you’ve understood. Writing comedy, it’s not just pausing the action to do stupid things. It needs preparation, coherence. And it asks to take advantage of every bit you’ve been given. Out of that, it also needs good timing, and to think of a good way to show it. And speaking about it…
Part 3 – A tool disguised as car keys
What if we talked about Mise en scène. Or, if you prefer : Directing. The ways scenes are shot and composed. A part of composing humor that is often overlooked, despite being of its most important parts. Let’s get back to this small scene this Ash, shall we ? [Scène Ash]. Let’s abandon the writing and look at how the scene is shot. The scene where Ash burns his surroundings is framed with a single, 10 seconds long, perfectly still shot. It shows the entirety of the actions and only stops after Ash’s speaks to cut back to Mo and Elliott. And so, do you think the scene would’ve been funnier if this shot was intercut with inserts of the objects catching fire, of Elliott and Mo’s horrified expression or of other people screaming in fear at the scene ? To me, no. Absolutely not. Because the shot doesn’t need it. It is clear and definitive. Adding such things would cause two problems : they would be pointless, as they would overexplain something we already see by ourselves ; but they would also ruin the tempo of the joke by making it too long and dragged on for its own good.
Directing is a tool to tell a joke as much as writing. The way a scene is shot, the timing, all of this is a way to direct the viewer’s attention towards what he is supposed to look or not, which information he’ll receive, what he’s supposed to feel. And in the same way having a character explaining the joke is a great way to make sure no one will laugh, underlining constantly the part that’s supposed to be funny will just annoy the viewer. No one likes to be taken for a fool.
A good comedy directing is a directing that lets you appreciate the funny part without making it overly obvious. And by doing that, it also lets you the freedom to explore the joke by yourself. Look at this scene from the Rick and Morty episode Gazorpazorp Junior. In this scene, Morty has given birth to an alien, and his family is trying to figure out what to do with it, and this happens [RM-1]. Just like in the previous scene we analyzed, past the shot of Rick holding the robot, the entire discussion is framed in one single shot. At the start, the entirety of the action consists of Rick giving his instructions and making his sarcastic joke about the 90's to Jerry. After that, it divides itself into two sub-actions. On the left side of the screen, Morty, Beth and Summer are debating about the alien baby, and on the right side, we can see Jerry thinking about how to respond to Rick before going to the door to shout his answer. Framing these two actions in one shot is a brilliant decision as it gives to Jerry’s action a sense of discrete ridiculousness and communicates with the images how his anger and reflexions are vain and pointless, because he’s literally thinking about a response to a silly comment right next to a big discussion. It also lets us the freedom to switch our attention between the two arguing and decide which one we want to follow, making us active in the process and letting us decide when we laugh.
If you split the scene and alternate between close-ups of the family and close-ups of Jerry alone, the whole effect falls apart. Everything becomes flat, the joke becomes too “on your nose”. It doesn’t work anymore. For a deeper approach on the importance of directing in comedy, I advise you to watch this YouTube video from the videast Le Fossoyeur de films (yes, the same guy as before), who perfectly resumes the subject, and who has partially inspired this post [Ref 2]. It’s in french, but there are english subtitles if you need.
Hum…
So fine, letting freedom to the viewer is important. Not overcutting a scene is important. Are there other things to say about directing ? Well, let’s look at this scene from Episode 6 of Elliott from Earth [Scène tête]. Okay, at first glance, the joke is pretty straightforward. Frankie is facing The Head and they make a joke about their shape. The gag could just stop there. Ha ha. Pun. Funny. Then it is followed by a member of The Head laughing on his own because he understood the pun a few seconds late. Again, the joke could stop here. Ha ha, he is dumb. Really funny. Except, in the middle of the cringey silence, there is this two seconds shot of Frankie… who does nothing, who just stares at the scene with a semi-smile on her face. And… Why ? What does that shot mean ? Is Frankie tempted to laugh at the joke but decides to stay serious ? Is she annoyed by The Head’s antics and is trying her best not to show it ? Is she starting to have a mental breakdown ? Is she struck by a sudden revelation and is secretly reevaluating the value of life, showing her newfound achievement with a weird smile ? I don’t know. And that’s the point.
This shot is funny because it doesn’t gives us any explanation. The character’s expression and what she’s feeling is left in the field of ambiguity. We have the choice of filling the gap and to interpret it the way we want. This makes the joke malleable depending on the viewer and allows us to make it more funny than just having Frankie saying out loud how she feels. Plus, by implementing this in the middle of what is otherwise a pretty standard joke, the show creates an offsetting that works extremely well. The first time I saw this scene, I bursted out laughing.
This reminds of a scene from an old Simpson episode, Simpson Tide. In this episode, Homer becomes pilot in command of a military submarine. His actions end up causing a resurgence of the URSS. Then, in a scene later in the episode, this happens [Simpson-1]. Just like in the scene with The Head, the joke seems pretty straightforward. We have Grandpa starting to say nonsense on the air, which is very funny, and as his speech reaches its climax, as we hear the punchline, the character stops talking and… we have 3 entire seconds of silence. And it’s not an awkward silence like what we have in many comedies, where everyone in the scene acts embarrassed to say this is the moment we’re supposed to laugh. It’s three seconds where the only character on screen is completely placid and no one reacts to what he said. We think it’s going to cut to the next scene, then the scene starts dragging, we’re expecting something to happen, anything, but no, it ends with… basically nothing. For three seconds. I don’t know if you realize, but in comedy, three seconds is a lot of time. It gives the viewer the time to analyze the joke, think about every possible outcome, turn off the TV, eat, go to a party, see long-lost friends again, decide to go on a trip to Brazil with them, go back and forth, be traumatized by their trip, start a therapy, getting over their trauma, start a new goal in life, set it in motion and start learning to play guitar, and maybe the basics of Chinese if they’re fast. You do not put such a long gap unless you know what you’re doing. And there, the ambiguity of the silence, its purpose and the fact that it’s here without any explainable reason, it creates a blatant but small unexpectedness, a play on our expectation of comedic timing, which is rather funny.
This is another lesson we can learn from comedy. Just like in every other type of writing, letting gaps in your jokes, unexplained parts, is very important. Not only does it make the viewer more active, and therefore more responsive to the gag, it can also lead to a better creativity, to unexpected parts that could never exist otherwise.
In both Gumball and Elliott from Earth, despite the fundamental differences between the shows, the directing has the decency of leaving us to explore what’s happening. The shots are usually quite wide, with a lot of field depth, and several details are lifted up here and there. We have the time and opportunity to explore each frame without having to pause. This might even work better in Elliott from Earth, as the rhythm is slower and we have less wacky weird things to focus on. Its shots allow us to breathe, and they have the decency of not giving us everything on a plate. Take this scene from the show’s Episode 15 [Scène lettre]. It’s by watching for the tenth time I just noticed the blackmail letter is made of cut-out pieces of newspapers, like in a crime movie… even though it’s a hologram. It’s a small detail, yes, but it’s a brilliant one.
And be cautious. I’m not saying good humor is just doing very abstract and ambiguous things, or never cut. It is important to not make the joke too obvious, but it is also important not to fall into the opposite trap, and making it so abstract nobody will understand it was supposed to be comedy. The important thing is to find a good balance. And for that, there’s no universal rule. Each case is different, and each joke has to be taken one by one. And having an expressive direction, making things hyperbolic, can work. For example, in the famous “social media debate” scene from the Gumball episode The Best [Gumball-4], the directing is uber-expressive, with close-ups, deutsch angles, shaky cam, big sound effects and dramatic music. But that’s the joke. The two characters are having a childish argument, and it is filmed like an action scene. If you use a more calm direction in this scene, to quote a great man, it’s not that it’s not funny, it’s that it’s not a joke – and I’ll stop with French references nobody will get. Again, the most important is to understand what the gag needs. There’s nothing revolutionary…

9/25 Day 0: I’m not gonna lie, it was pretty bad. All I could do was eat popsicles at first. Main tip: buy an ear, chin ice pack!!! That thing saved my life yesterday. I took the liquid norco 2 times and the 10p dose made me extremely nauseous even with zofran and eating jello and applesauce. Ended up taking liquid Tylenol in the middle of the night. I ate Mac and cheese, pudding and popsicles today. Water and pedialyte. Ear ice pack to the rescue again last night! Gross mucus, scabs? Coming out of throat. Idk exactly what it is. Yellow gunk that tastes and smells pretty bad 🥴
Day 3: no pain with swallowing water. I ate scrambled eggs this morning and soft tacos + refried pinto beans from Taco Bell for dinner today! The beans were a little sticky but the tacos went down surprisingly easy. Ears a little sore after dinner but the ice pack took that away. Scabs are white and look good. I’m nervous for when they start coming off 🥴 Tylenol has been enough for me again today. Holding in yawns is the worst. Also, tmi but I have horrible diarrhea.. unsure if it’s the antibiotics, my period, the meds, diet, or all of the above. Started taking a probiotic and eating activia yogurt.
Day 4: talking is much easier today. Mostly just pain in my ears after eating. Managing with Tylenol. I had a smoothie, oatmeal, yogurt, chicken flavored rice and potato bacon soup today. Scabs look good, red outline so I’m wondering if they’re starting to slough off? Hoping mine don’t break off harshly. Diarrhea stopped, thank god lol. I also wanted to add I have been gently gargling in the morning at sometimes before bed with 1:1 solution of water and hydrogen peroxide (per doc order) which has definitely helped break up mucus and other gunk in throat. And if you don’t have a tung brush, get one!! Be gentle.
Day 5: pain is different but not intolerable. Throat feels kinda tight, “dry” even though it’s not. Burning in throat and ears. I think my scabs are slowly sloughing off. I ate a chipotle burrito bowl today with chicken (no spicy stuff) 😋 switched to pill Tylenol 1000 mg every 6 hr. I shot my compound bow today against my better judgement but was very careful. I plan to go deer hunting for opening day of bow season in the morning. I’m going to take it easy and sit in a blind. Hopefully it isn’t too much!
Day 6: I managed to hunt both morning (blind) and evening (ladder stand) today. I don’t think it was too much! Definitely got tired and out of breath walking through the woods a lot faster than usual but not bad. Scabs are definitely falling away. Some more uncomfortable than others but not too bad. Tylenol spaced to q8h 1000mg. I ate McDonald’s breakfast burritos and a hash brown, iced coffee, slushee and cheese curds today. Don’t judge my bad eating habits 💀 just chewed everything up real good and everything went down smoothly! Day 6 night update: noticed a small black scab on the right side that started bothering me earlier in the day. Idk if it was already there (covered by white scab) or new.
Day 7: more burning today, definitely scabs coming off. A little worried about this small black scab, but hoping it’s nothing and comes off when it’s supposed to. Any suggestions on dealing with a black scab? Is this normal? All my other scabs are white. Right side burns pretty bad after eating. I ate yogurt, Kodiak cake (pancake one), applesauce and shepherds pie today.
Day 8: thankfully, the burning/stinging has improved. My throat is more itchy today. More and more pink skin is showing through the scabs. Eating the same kind of foods! Nothing crispy yet.
Day 9: I’m sick of the yellow mucus/scab sloughing. It tastes horrible (musty, poison-like). But everything seems to be healing well! Still some white scabs and the black scab is down to a little dot now. Pain is mostly discomfort, throat tightness, muscle fatigue. All I want is chips and salsa and a margarita!! 😂
Day 10: I see why people call this the turning point day. I feel almost normal. The remaining scabs and mucus STILL taste and smell gross. Still spitting green, yellow stuff. But my throat looks pretty healed, still some white in the deeper spots. Swallowing is fairly normal. Still sticking to softer foods, just nothing crunchy for now. The right side hurts worse when swallowing for some reason (smaller, but possibly deeper? Side). Q8h 1000mg Tylenol still but will probably try to go without for as long as I can tomorrow.
Day 11: similar to day 10. My throat really only hurts after eating and when I first wake up and it’s dry. I ate potatoes and eggs with hot sauce today! Didn’t burn 😊 intermittent green, yellow gunk continues. Q12h 1000mg Tylenol.
Day 12: same ol same ol. When does the mucus stop!? 😭
Day 13: throat is itchy! Sore after talking and eating all day. Mucus seems to be improving. Mouth is kinda dry. Pretty much stopped Tylenol except once at night.
Day 14: throat is less itchy today. There’s like a very thin white film on some spots but mostly pink tissue. The tissue is a little lumpy but I’ve read that’ll smooth out. Spitting gross mucus and scab residue has stopped 🙏🏻 behind my nose is kinda stuffy and mucusy, but hoping that goes away with time. I’ve also been avoiding sneezing so that probably doesn’t help but I’m scared to sneeze lol. Eating mostly everything but will be avoiding crunchy stuff for 1 more week to be safe! No more Tylenol. I’m stopping day to day updates but I plan to put a little month update when that time comes 😊
Tissue results came back as “follicular lymphoid hyperplasia.” After looking it up, it’s basically inflammation of the tonsil cells which could be cause by viruses, bacteria, etc. Surprisingly, no hidden tonsil stones noted but some yellow liquid and debris was. I would usually find this yellow/green liquid with small stones when cleaning my tonsils.
I’ll continue to update! Post questions, advice, etc. 😊

But my throat looks pretty healed, still some white in the deeper spots. Swallowing is fairly normal. Still sticking to softer foods, just nothing crunchy for now. The right side hurts worse when swallowing for some reason (smaller, but possibly deeper? Side). Q8h 1000mg Tylenol still but will probably try to go without for as long as I can tomorrow.
Day 11: similar to day 10. My throat really only hurts after eating and when I first wake up and it’s dry. I ate potatoes and eggs with hot sauce today! Didn’t burn 😊 intermittent green, yellow gunk continues. Q12h 1000mg Tylenol.
Day 12: same ol same ol. When does the mucus stop!? 😭
Day 13: throat is itchy! Sore after talking and eating all day. Mucus seems to be improving. Mouth is kinda dry. Pretty much stopped Tylenol except once at night.
Day 14: throat is less itchy today. There’s like a very thin white film on some spots but mostly pink tissue. The tissue is a little lumpy but I’ve read that’ll smooth out. Spitting gross mucus and scab residue has stopped 🙏🏻 behind my nose is kinda stuffy and mucusy, but hoping that goes away with time. I’ve also been avoiding sneezing so that probably doesn’t help but I’m scared to sneeze lol. Eating mostly everything but will be avoiding crunchy stuff for 1 more week to be safe! No more Tylenol. I’m stopping day to day updates but I plan to put a little month update when that time comes 😊
Tissue results came back as “follicular lymphoid hyperplasia.” After looking it up, it’s basically inflammation of the tonsil cells which could be cause by viruses, bacteria, etc. Surprisingly, no hidden tonsil stones noted but some yellow liquid and debris was. I would usually find this yellow/green liquid with small stones when cleaning my tonsils.
I’ll continue to update! Post questions, advice, etc. 😊

25F, emergency laparoscopic cholecystectomy done at 1pm this afternoon. No complications other than small amount of bile spillage. Discharged home 4pm.
Hx: PCOS Endometriosis Autism vs adhd Gallstones IBS
Meds: Prozac 40mg Concerta 36 mg Ritalin 10mg Trazadone 50mg
Prescribed following surgery: Dilaudid 1-2 mg PO q8h last dose 2mg at 2200 Tylenol 1000mg q6h last dose 1900
Current problem: I have one incision site that is severely painful now to the point where I can’t walk standing straight up, and have extreme trouble getting up from a lying position. Earlier when I tried to get out of bed I felt what was almost like a tearing sensation. Now there is constant sharp pain no matter what position I am in and incredibly sore to touch the dressing over it. The incision is located slightly to the right of the centre of my abdomen just below my ribs. None of the other incisions are painful, just sore to touch. Dressing over umbilicus has a small amount of dried blood that has come through the dressing, all other dressings are dry and intact. My temperature was 39.2 in right ear and 38.1 in the left ear at 2230. Every time I have checked it there is a significant difference between both sides with the right ear having higher temp, not sure if that’s relevant. I have been having intermittent episodes of profuse sweating since about 7pm. Negative for COVID and all common viruses via NPS swab done today.
I don’t have any supplies with me at home so I can’t take the dressing off to assess it and then re dress it. I’m wondering if I can wait until my doctor opens in the morning to see her sometime tomorrow or if this is a more urgent issue?
