2024.05.15 01:36 Kitchen-Lemon1862 i got three ppl we cannot stand to transfer out yesterday!!!

so this past week i’ve just quit putting up with the bs ppl have given us and started to tell ppl how it is, respectfully tho.
  1. the first person called bc they wanted their allegra d 12 our prescription to be filled bc they’ve tried for two weeks and we haven’t been able to. i checked their profile and it said “pending inventory” so i went over to check and we didn’t have any. so i asked the pharmacist if we have had it recently since this person has been waiting for two weeks.
the pharmacist said the warehouse has been refusing to send it, and we checked and no pharmacy that’s part of our chain has it in a 20+ mile radius.
i got back and told the patient and they went into hysterics and i told them, “i’m very sorry, but the warehouse has not sent in any, and we’ve requested it multiple times. no pharmacy in 3 letter has it in a 20+ mile radius. i have no control over the warehouse deciding to send us medication or not.”
the patient then said, “so you can’t just give me a partial fill?” and i said, “no, we don’t have any i’m sorry” and then patient said, “oh so what am i supposed to do now? die?” i said, “you’re not going to die without allegra d, but you can call another pharmacy and ask if they have it if you really need it. or you could go up to a different pharmacy and buy it.” 10 minutes later we got a call from wags saying they need us to fax over the persons prescriptions for a transfer.
  1. someone calls asking for a prescription to be filled and i checked and they needed a PA, i said, “i’m sorry, your insurance needs more information from your doctor we are waiting for your doctor to send in a PA.”
the patient said, “no, my doctor sent in the prescription so no one needs any information from them now fill it.” i then said, “i don’t need anything, your insurance needs information from your doctor, so you will have to call your doctor or insurance and talk to them. it takes 3 days typically for a PA to go through as well.”
the patient then said, “again, neither you or the insurance needs anything from my doctor, MY DOCTOR SENT IN THE PRESCRIPTION!!” i said, “im sorry you need to tell your insurance that, not me. you can call your doctor and ask them about it. a lot of people need prior authorizations.” they then said, “that’s it i’m going to wags they will never have this problem!!” and i said, “okay that’s fine.” and hung up.
5 minutes later wags calls for their prescriptions to be faxed over. and yes, ik wags will need a pa but i was just glad to see this person to go.
  1. patient calls saying it’s ridiculous they have to pay $1 for their prescription and ask if it’s ran through their insurance. i said, “yes, and it shows you’ve always paid $1 for this medication.” the patient says, “no, i never pay, i have insurance and that’s what insurance is for, are you dumb?” i said, “no, i’m not dumb, a lot of people’s insurance doesn’t make their medication completely free.” they then said, “nooo no no, that’s not how insurance works. i’ve had insurance my whole life, and once you have it you don’t pay for anything! i’m going to grocery store pharmacy, they’ll never make me pay!!” and a few minutes later they were transferred

2024.05.14 23:45 Accurate-Address2559 Staffing Rubrics/Budget?

Looking for staffing rubrics for both pharmacist and techs in the retail setting. Context around your hours and services are very helpful to support the rubric used.
How many prescriptions to how many hours of each you are budgeted, etc.
I am in a leadership role for a growing small chain pharmacy. We are open 40 hours/week, no weekends, no immunizations, no MTM in our dispensing pharmacies. Appreciate anything you have to share so we can better evaluate our existing staffing rubric and develop it for anticipated growth.

2024.05.14 21:09 bbdoublechin SO ready to get the hell away from shopper's drug mart

Ever since my local shoppers raised their dispensing fee higher than what my (really good) benefits covers, I've been itching to switch to a local pharmacy.
Shopper's is giving me all the motivation I need. The last THREE times my spouse or I have gotten texts saying our meds are ready for pick up, we arrive to staff saying "oh weird, they aren't. anyway it'll be at least a half hour wait."
We aren't on meds because they are fun. We are on them because we need them to function, and because we have conditions which make it more difficult than the average person to do things like get all the way to a pharmacy regularly.
Another recent irritation is when I have no more refills left on a prescription, I will have the EXACT same interaction:
Me: I need a refill of (medication). Pharmacy staff: You're out of refills. Blank stare Me: Okay... Can you fax my doctor to request more? Pharmacy staff: You want us to fax them? Me: Yes. Pharmacy staff: I mean... We can... It will probably take them a couple of days to get back to us though... Me: Okay. Thank you.
Mind you, I've been getting my meds refilled by them for 13 years, and this has only started in the past year, if that.
My doctor doesn't prescribe 10000 refills at a time, so maybe I am the crazy one, but it literally feels like getting them to do anything is like pulling teeth.
I don't even mind that the local pharmacy I am switching to is slower and charges $1 for faxing my doctor, because they're a small operation and they've gotten a massive wave of new folks fleeing larger chains.
Shopper's has no excuse. They're insanely profitable.
WOW. The music just cut out (I'm waiting 30 mins for meds) and they did two announcements:
Did you know sunscreen can expire? Make sure you stay safe and purchase new sunscreen if you haven't checked the expiry date!
Security zone D, security zone D (fake automated message to scare people out of stealing).
Buy more, watch the fuck out, and don't you dare ask us for a thing. Got it.

2024.05.14 20:09 MediocreGrocery8 Costco pharmacy and diabetes supplies?

We love our Costco pharmacy, the people are great, the service is great. It is a vast improvement over the large retail chain formerly used by my elderly parents, both on Medicare. Costco has come through every time when someone has been sick and needs a prescription today. (Not so much at the large retail chain; good staff, poor corporate management is what I've heard?)
The only weird thing: Costco pharmacy reps have told me that they can't fill a Medicare patient's prescriptions for diabetes test strips, which at retail can cost $1-2 per strip, yikes. (Fortunately, we can get this Medicare prescription filled at their former pharmacy, and it's easy to plan ahead for this prescription.)
Does anyone have any insight on this? Thanks!

EXPLORATION
  • Juanicipio:
    • Infill drilling at Juanicipio continued in Q1 2024 from underground aimed at upgrading mineralization in areas expected to be mined in the near to mid-term. During Q1 2024, 11,271 metres were drilled from underground.
    • Surface drilling focused on expanding and upgrading the deeper zones and broader regional exploration started in April 2024.
    • During 2024, Juanicipio plans to drill a total of 50,000 metres, with 33,000 metres from underground and 17,000 metres from surface.
  • Deer Trail Project, Utah:
    • On May 29, 2023 MAG started a Phase 3 drilling program focused on up to three porphyry “hub” target areas thought to be the source of the manto, skarn, epithermal mineralization and extensive alteration throughout the project area including that at the Deer Trail and Carissa zones. In late 2023 an early onset of winter snowfall impacted the commencement of the third porphyry “hub” target, which is now expected to be drilled in 2024. The two completed “hub” holes to date total 2,738 metres. Both holes intercepted alteration and mineralization in line with what is expected on the edges of porphyry systems. Follow-up drill targets are planned for summer 2024.
    • With the early onset of snowfall, Phase 4 drilling focussed on lower elevations commenced in the last quarter of 2023 and continued through Q1 2024, aimed at offsetting the Carissa discovery and testing other high-potential targets in the Deer Trail mine area. During Q1 2024, 1,208 metres were drilled at Carissa with results pending.
  • Larder Project, Ontario:
    • Drilling targeting Cheminis and Bear totalled 5,391 metres in Q1 2024. Targets tested include down plunge extension of the high-grade double knuckle at the Bear East zone and extending the Cheminis south mine sequence down plunge.
    • Cheminis Update: Follow-up drilling of the Cheminis South Cadillac-Larder Break (“CLD”) mine sequence down plunge is planned to test below the most recent intercepts. Hole GAT-24-026 intersected a new zone on the north side of the CLB within a fuchsite-silica-albite altered komatiite grading 3.9 g/t gold over 16 metres with 2 higher grade shoots associated with albite dykes (see Table 1 below).
    • Bear Update: Utilizing the updated model and incorporating the updated data from recent drilling, the Bear East zone was successfully extended down plunge by up to 1,100 metres depth. Hole GAT-24-024NB intersected gold mineralization on both sides of the CLB which confirms the presence of either another structural trap at depth or the continuation of the “double knuckle” zone at surface. Gold mineralization intersected on the north zone included 9.4 g/t gold over 2.2 metres within a strongly altered komatiite with syenite intrusions and 1.6 g/t gold over 4.2 metres on the south zone within the south iron-rich volcanics (see Table 1 below). Bear East remains open in all directions.

JUANICIPIO RESULTS
All results of Juanicipio in this section are on a 100% basis, unless otherwise noted.
Operating Performance
The following table and subsequent discussion provide a summary of the operating performance of Juanicipio for the three months ended March 31, 2024 and 2023, unless otherwise noted.
https://preview.redd.it/9txq8snzze0d1.png?width=720&format=png&auto=webp&s=0089cf57573e8834bdb641cf9e055619e6f6c5b2
(1) Equivalent silver head grades have been calculated using the following price assumptions to translate gold, lead and zinc to “equivalent” silver head grade: $23/oz silver, $1,950/oz gold, $0.95/lb lead and $1.15/lb zinc (Q1 2023: $21.85/oz silver, $1,775/oz gold, $0.915/lb lead and $1.30/lb zinc).
(2) Equivalent silver payable ounces have been calculated using realized price assumptions to translate gold, lead and zinc to “equivalent” silver payable ounces (metal quantity, multiplied by metal price, divided by silver price). Q1 2024 realized prices of $23.73/oz silver, $2,112.27/oz gold, $0.92/lb lead and $1.08/lb zinc (Q1 2023 realized prices of $22.93/oz silver, $1,959.50/oz gold, $0.94/lb lead and $1.43/lb zinc).
During the three months ended March 31, 2024 a total of 325,081 tonnes of ore were mined. This represents an increase of 45% over Q1 2023. Increases in mined tonnages at Juanicipio have been driven by the operational ramp up of the mine towards steady state targets.
During the three months ended March 31, 2024 a total of 325,683 tonnes of ore were processed through the Juanicipio plant; no ore was processed at the nearby Fresnillo and Saucito processing plants (100% owned by Fresnillo). This represents an increase of 47% over Q1 2023. The increase in milled tonnage has been driven by the Juanicipio mill commissioning and operational ramp up to nameplate capacity over the course of 2023.
The silver head grade and equivalent silver head grade for the ore processed in the three months ended March 31, 2024 was 476 g/t and 713 g/t, respectively (three months ended March 31, 2023: 363 g/t and 530 g/t, respectively). Head grades in Q1 2023 were lower as low-grade commissioning stockpiles were processed through the Juanicipio plant. Silver metallurgical recovery during Q1 2024 was 89.1% (Q1 2023: 87.0%) reflecting ongoing optimizations in the processing plant.
The following table provides a summary of the total cash costs 5 and all-in sustaining costs 5 (“AISC”) of Juanicipio for the three months ended March 31, 2024, and 2023.

Equivalent silver ounces sold have been calculated using realized price assumptions to translate gold, lead and zinc to “equivalent” silver ounces sold (metal quantity, multiplied by metal price, divided by silver price). Q1 2024 realized prices of $23.73/oz silver, $2,112.27/oz gold, $0.92/lb lead and $1.08/lb zinc (Q1 2023: $22.93/oz silver, $1,959.50/oz gold, $0.94/lb lead and $1.43/lb zinc).
Financial Results
The following table presents excerpts of the financial results of Juanicipio for the three months ended March 31, 2024 and 2023.
https://preview.redd.it/4zym5dqzze0d1.png?width=720&format=png&auto=webp&s=d25c91cabc7f6c6b35ff61acacc0160f3dfac2e7
Sales increased by $72,207 during the three months ended March 31, 2024, mainly due to 179% higher metal volumes and 2% higher realized metal prices.
Offsetting higher sales was higher production cost ($9,409) which was driven by higher sales and operational ramp-up in mining and processing, including $3,545 in inventory movements, and higher depreciation ($14,083) as the Juanicipio mill achieved commercial production and commenced depreciating the processing facility and associated equipment in June 2023. Operating margin increased by 21% to 52%, mainly due to operational leverage and the lower reliance on the nearby Fresnillo and Saucito processing facilities.
Other expenses increased by $2,159 mainly as a result of higher extraordinary mining and other duties ($872) in relation to higher precious metal revenues from the sale of concentrates and higher consulting and administrative expenses ($2,690) as an operator services agreement became effective upon initiation of commercial production (the “Operator Services Agreement”), offset by lower exchange losses and other costs ($1,566).
Taxes increased by $20,980 impacted by higher taxable profits generated during Q1 2024, and non-cash deferred tax credits related to the commencement of use of plant and equipment in Q1 2023.

Navigating geopolitical trade dynamics: Identifying opportunities and business implications amid uncertainty
https://preview.redd.it/gjtyiubtsd0d1.jpg?width=1200&format=pjpg&auto=webp&s=cd01eb2583f6c0757ed047ed278dca16f5cc4abd
The landscape of global trade is undergoing a profound transformation, shaped by ongoing geopolitical tensions and shifting alliances. Recent events such as the Russia-Ukraine conflict and escalating US-China disputes have not only disrupted established trade flows but have also redefined the geopolitical contours of commerce. According to projections by UNCTAD, global trade witnessed a significant downturn in 2023, reflecting a decline of approximately 5% compared to the previous year’s record levels, amounting to a staggering $1.5 trillion reduction.
European Central Bank President Christine Lagarde aptly captured the essence of this transformation, noting a fragmentation of the global economy into competing blocs. This fragmentation underscores a fundamental shift in global trade patterns, with nations increasingly gravitating towards politically aligned trade partners. This trend, often referred to as “friendshoring” and “nearshoring,” signifies a departure from traditional trade dynamics.
The United States, for instance, has diversified its trade away from China, redirecting its focus towards countries such as Mexico and Vietnam. Mexico, in particular, has emerged as a crucial trading partner, surpassing China in terms of trade volume with the US in 2023. Similarly, the EU’s trade with Russia witnessed a significant decline following the Ukraine crisis, leading to a surge in trade with alternative partners, including China. These shifts underscore the growing influence of geopolitics on global trade patterns.
Clear identification of opportunities is crucial
In this evolving landscape, clear identification of opportunities is paramount for businesses seeking to navigate geopolitical trade dynamics successfully. The emergence of nearshoring as a viable strategy underscores the importance of agility and adaptability in supply chain management. Countries such as Mexico and Vietnam have positioned themselves as attractive alternatives to China, offering new avenues for trade and investment.
Germany’s experience provides a compelling case study of nearshoring in action. As trade with Russia dwindled, Germany sought alternative suppliers within Europe, thereby reducing its dependence on distant partners. Similarly, Poland witnessed a surge in imports from Germany, signaling a shift towards regional trade networks. These developments highlight the strategic imperatives of diversification and resilience in an increasingly uncertain global landscape.
Preparedness for uncertainty: Informed decision-making in global trade
Against this backdrop of geopolitical upheaval, business leaders must equip themselves with the tools and insights needed to make informed decisions. The rise of geopolitics as a prominent factor in corporate discourse underscores the need for proactive monitoring and geopolitical risk analysis. From supply chain disruptions to regulatory uncertainties, businesses must anticipate and mitigate potential challenges to maintain operational continuity.
Leaders such as Larry Fink of BlackRock and Elon Musk of Tesla have emphasized the importance of strategic diversification in mitigating geopolitical risks. By establishing a presence in multiple regions, companies can enhance their resilience and adaptability in the face of uncertainty. Moreover, constant tracking of geopolitical events and regulatory shifts enables businesses to seize emerging opportunities and mitigate potential threats proactively.
In conclusion, navigating geopolitical trade dynamics requires a nuanced understanding of evolving geopolitical trends and their implications for global commerce. By conducting geopolitical risk analysis, and embracing agility, diversification, and strategic foresight, businesses can position themselves to thrive in an increasingly uncertain world.

2024.05.14 12:56 Stuart104 Can anyone recommend a good non-chain pharmacy in Inwood?

My experiences filling prescriptions with the CVS on Dyckman have been less than stellar (to put it mildly). Thanks!

2024.05.14 11:54 enthusiast19 Diagnosis code question

Every time I transfer my prescription from one pharmacy to another, I get told Rx is on hold and that they have to contact my doctor for a diagnosis code even though previous pharmacy already had obtained that info from my provider on the same script (same Rx number).
Now, in times of shortage, when I often have to transfer Rx from one pharmacy to another for them to even tell me if Mounjaro is in stock (although some will tell without transferring), I’m once again being told the same Rx with the same script number needs a diagnosis code even though Rx was simply transferred from one location to another within the same chain.
Anyone else experience something similar with diagnosis codes?

2024.05.14 02:12 smallcapsteve Biden Signs Ban on Imports of Russian Nuclear Reactor Fuel

President Joe Biden on Monday signed legislation banning the import of Russian enriched uranium, starting a 90-day countdown until limits on shipments of the reactor fuel take effect.
Biden’s signature also unlocks some $2.7 billion in spending, previously approved by Congress, to build up domestic uranium supplies for US nuclear plants. That money was contingent on the federal government imposing limits on Russian uranium imports.
The move, which comes as Russia’s invasion of Ukraine grinds into its third year, carries risks. Russia is America’s top foreign source of the fuel, supplying about a quarter of the uranium used in US reactors, according to Energy Department data. Russia makes about $1 billion a year from those sales. Cutting off that supply could raise uranium prices 20%, according to Jonathan Hinze, president of nuclear fuel market research firm UxC.
But the legislation lets the Energy Department issue waivers allowing the import of Russian enriched uranium until 2028 if no alternative source is found or if the imports are determined to be in the national interest.
There’s also the possibility Russia could retaliate. In December Tenex, a Russian state-owned uranium supplier, warned American customers that the Kremlin might preemptively bar exports of its nuclear fuel to the US if lawmakers imposed a ban.
Fletcher T. Newton, president of Tenex-USA, Inc. said in an interview the company “fully intends to honor all of their contractual commitments in the United States.” But Tenex, he said, has no control over whatever action the Kremlin might take.
US nuclear plant owners have been planning for a ban. A spokesman for Constellation, the largest U.S. nuclear plant operator, said the company has worked with suppliers to secure enough fuel to power their operations into 2029, with contracts extending beyond then.
But analysts say there may still be smaller plant operators that haven’t fully prepared for the disruption. And fuel supplier Centrus Energy Corp., which is based in the US but relies on Tenex for the majority of the uranium it delivers, warned in its most recent annual report that a ban would pose “a significant risk” to the business.
“While we have other sources, they are not sufficient to replace the Tenex supply,” the report said. The company plans to apply for waivers.
The US was once a leading supplier of enriched uranium but lost its edge in the industry decades ago. The country now has just one commercial enrichment facility in New Mexico, owned by Urenco Ltd., a British, Dutch and German consortium. Centrus, based in Bethesda, Maryland, began production in October at a pilot project in Piketon, Ohio, and eventually expects the site to supply as much as 900 kilograms a year of specialized, highly enriched reactor fuel to be used in a new breed of advanced reactors.
Centrus also has secured approximately $900 million in conditional sales commitments to support a plan to produce conventional, low-enriched uranium reactor fuel, Chief Executive Officer Amir Vexler said Wednesday on an earning call. The company intends to compete for billions of dollars in funding the Energy Department will make available to support domestic enrichment, he said.
The Energy Department plans to use the $2.7 billion to help rebuild the domestic supply chain by creating a guaranteed buyer of American-made reactor fuel. Companies that could benefit from the spending include ConverDyn, a joint venture between Honeywell International Inc. and General Atomics that provides uranium conversion services, and Global Laser Enrichment, owned jointly by Silex Systems LTD and and Cameco Corp.
“To reduce – and ultimately eliminate – our current dependence on Russian uranium for civilian nuclear power reactors, the most important step the US government can take is to invest in US commercial enrichment,” the White House National Security Council said in a statement.

UNDER THE MOUNTAIN
A potential nod to the Schiehallion, a cone-shaped mountain often referred to as the center of Scotland. It's name comes from the Gaelic Sìth Chailleann, meaning "fairy hill of the Caledonians." On the southwestern side of the mountain is said to lie the well-concealed Uamh Tom a' Mhòr-fhir, a cave serving as an entrance to the underworld.
Note: As the Schiehallion lies in the middle of Scotland (the Night Court) and not the middle of mainland UK (Prythian), the Sciehallion could instead be representative of Ramiel.
THE WESTERN ISLES
May refer to the Outer Hebrides, also known as the Western Isles, which is an island chain off the west coast of mainland Scotland. They are also sometimes known as Innse Gall, a Gaelic term meaning "islands of the foreigners/strangers" which was originally used by mainland Highlanders when the islands were ruled by the Norse.
ACOTAR role: The Prison is located in the Western Isles.
THE PRISON
Though located in the Inner Hebrides of Scotland rather than the Outer, the Prison may be a nod to Beinn Shiantaidh, which is Scottish Gaelic for "holy mountain." It is located on the western side of the Island of Jura and is the second highest peak of the Paps of Jura.
Fun fact: Pap is of Old Norse origin and means breast.
VALLAHAN
Valhalla is the hall of slain warriors in Norse mythology, located in Asgard, where those who die in combat live in peace under the leadership of the god, Odin. They are said to dwell in Valhalla until the events of Ragnarök, where they will then march out to fight in aid of Odin against the jötnar, a type of being in Germanic mythology.
ACOTAR role: A faerie territory located in the northwest portion of the Continent.

ACOTAR role: One of the territories of the human lands on the Continent, ruled by Queen Vassa. They are said to be a horse loving people.
ROSEHALL
Rosehall is the name of a remote hamlet in the Scottish Highlands.
Fun fact: The 2nd Duke of Westminster acquired the Rosehall estate and his mistress at the time, Coco Chanel, redecorated the interior to her own liking. Winston Churchill, a close friend of the Duke, also visited the estate.
ACOTAR role: Mentioned in ACOFAS and is theorized to be the place where Azriel's mother resides.

ADDITIONAL LOCATIONS

VELARIS
Possibly derived from the Latin vēlo, meaning "to veil, cover, wrap" along with "to hide, conceal" and "to clothe in." Could also derive from the Latin vēlum, meaning "to veil, curtain" and also indicates a tarpaulin acting as cover for outdoor spaces.
THE WALL
No singular reference, but the presence of a wall separating one race from another, often humans from mythical beings, is rife throughout history, mythology, and fantasy.
One from each category that has loose ties to ACOTAR: (1) Hadrian's Wall, ie the Roman Wall, a defensive fortification of the Roman province of Britannia. It spanned from coast to coast, cutting across the entire width of Northern England. (2) In Norse mythology, the Master Builder asking for the hand of the goddess Freyja as payment for constructing a wall around Asgard to defend the gods. (3) The English village of Wall in Neil Gaiman's Stardust, named after the wall separating the land of Faerie from the mortal realm, highly guarded and cautioned against for mortals seeking to pass through.
THE MOONSTONE PALACE
Romans linked moonstone to the moon goddess Diana (goddess of wild animals and the hunt) and Ancient Greeks linked it to their lunar deities Artemis, Hecate, and Selene.
SIDRA RIVER
Sidra is a name of Latin origin and means "Goddess of the stars" or "like a star."
RAMIEL
Ramiel is a fallen Watcher, a type of Biblical angel, one of the 20 Watchers that rebelled against God by mating with mortal women, therefore creating the Nephilim (Shadowhunters, anyone?). Ramiel means "God has thundered." Ramiel is sometimes conflated with Remiel, also known as Jeremiel, meaning "God shall have mercy," as is listed as one of the seven Holy Angels. He is the archangel of hope and is responsible for divine visions and guiding the souls of the faithful into Heaven.
POOL OF STARLIGHT
No plausible connection found, though there is a famous pool in Greek mythology—a pool Mnemosyne presided over in Hades, counterpart to the river Lethe. Dead souls would drink from Lethe to wipe their past lives from remembrance when reincarnated. Those who drank from Mnemosyne's pool remembered and halted their transmigration of the soul.
ATHELWOOD
Ethel (also aethel) is an Old English word meaning "noble." Athelwood can mean "noblewood."
ACOTAR role: Morrigan's estate located in the Night Court.
GOLLIAN MOUNTAINS

CESERA
No direct reference found, but may allude to Caesarea, the name of numerous cities and locations throughout the Roman Empire.
ACOTAR role: One of three temples where the feet of the Cauldron were hidden.
SANGRAVAH
No direct reference found (and I lost my mind searching for one). Closest I came to was the similarly sounding Angharad, a feminine Welsh name, long associated with Welsh royalty, history, and myth, including Arthurian legend.
ACOTAR role: One of three temples where the feet of the Cauldron were hidden.
GREENFIELD VILLAGE
No direct connection found, though Greenfield is the name of several villages of England.
ACOTAR role: A neighboring village to where Feyre grew up and where Isaac Hale's wife is from.
WINDHAVEN CAMP
No connection found, though a separate Windhaven also appears in Tower of Dawn in the TOG series. Altun—Windhaven being the rough translation—is located in a three-peaked mountain range and is the home of Sartaq's hearth-mother and family.
Fun fact: Windhaven is the name of a science-fiction novel by George R. R. Martin and Lisa Tuttle following a group of humans who crash-landed on a foreign planet and learned to craft their own wings out of the spaceship wreckage.
THE WOLF'S DEN
No connection found, BUT fun fact: there is a named Wolf's Den castle in George R. R. Martin's A Song of Ice and Fire series (aka Game of Thrones) that now serves as a prison. "All the days were much the same inside the Wolf's Den, and any change was usually for the worse." A Dance with Dragons, Ch 29
ACOTAR role: The name of the worst tavern in Velaris that Nesta is known to frequent.
SILVERSPRING CREEK
No plausible connection found, BUT fun fact: "Silver Springs" is a song written by Stevie Nicks, performed by Fleetwood Mac, and is so strongly ACOTAR-coded even my non-loving playlist-to-book self can't deny it. If you want more bread crumbles that SJM is a Stevie Nicks fan, "Rhiannon" and it's lyric origins have potential ties to the Rhiannon of TOG.
You could be my silver spring
Blue-green colors flashin'
I would be your only dream
Your shinin' autumn ocean crashin'
Time cas t a spell on you,
but you won't forget me
I know I could have loved you
But you would not let me
ACOTAR role: Silverspring Creek is a stream running through Feyre's human village, near where she hunted rabbits.
STRYGA'S CASTLE
No direct reference found, but due to Stryga/the Weaver resembling a female demon akin to a vampire in Slavic mythology (see the Monster section above), it is possible Stryga's former home, before she was confined to her cottage, is reminiscent of Bram Stoker's 1897 Dracula and the Transylvanian Castle complete with three vampiric sister brides who entice men with their beauty and charm before proceeding to feed upon them.

The Night Sky

ARKTOS
Arktos, also written Arctus, means "bear" in Ancient Greek. Arktos was a centaur who fought against the Lapith spearmen. The Arktos Megale (aka Ursa Major, the Great Bear) is one of the original 48 constellations listed by Ptolemy who drew on earlier works by Greek, Egyptian, Babylonian, and Assyrian astronomers.
ACOTAR role: One of the three regarded holy stars (the furthest left star) of the Illyrians and serves as part of the symbol for the Night Court. The echelon of Arktosian is reserved for Illyrians who survive the Blood Right but fail to reach Ramiel.
CARYNTH
May refer to Corinth, an ancient city that was one of the largest and most important of Greece, or Carinthia, the southernmost Austrian state in the Eastern Alps, noted for its mountains and lakes.
ACOTAR role: One of the three regarded holy stars (the middle star) of the Illyrians and serves as part of the symbol for the Night Court. The echelon of Carynthian is reserved for Illyrians who scale the summit of Ramiel during the Blood Rite and are considered elite warriors.
ORISTES
Orestes (also spelled Orestes) means "stands on a mountain" and he is the subject of several Ancient Greek plays and myths concerning his madness and purification.
ACOTAR role: One of the three regarded holy stars (the furthest right star) of the Illyrians and serves as part of the symbol for the Night Court. The echelon of Oristian is reserved for Illyrians who make it to the Ramiel during the Blood Rite but don't reach the top.
Locations not listed: A slew of various more minor locations have not been listed, including the Four Market Squares of Velaris, certain cottages and houses, and even the Ironcrest Illyrian war-camp.

THE CAULDRON
There are many magical cauldrons present in various mythologies, one being the Pair Dadeni in Welsh mythology, a magical cauldron able to resurrect the dead, and the coire ansic of Irish mythology, the cauldron of the chief god of the Tuatha Dé Danann, which never ran empty.
TRUTH-TELLER
Possibly inspired by Fragarach, an Irish mythological sword known as "The Whisperer" or "The Answerer." It was the sword of Nuada, the first high king; it was said that no one could tell a lie with Fragarach at their throat and that the sword inflicting wounds from which no one could recover.
THE HARP
May refer to the uaithne, the magical harp of the Dagda, the chief god of the Tuatha Dé Danann in Irish mythology. It could control men's emotions and change the seasons.
THE MASK
In Egyptian culture, death masks were designed to guide the spirits of those mummified into the next world.
THE UNKNOWN 4th TROVE ITEM
Seen in a vision by Nesta via Lathys, which we now know to be the Horn, and may refer to the Gjallarhorn of Norse mythology, the sound of which is said to herald the beginning of Ragnarok.
ATARAXIA
Ataraxia, in Ancient Greek philosophy, was a state of calmness untroubled by mental or emotional disquiet, ie tranquility of the mind. It was considered the ideal mental state for soldiers entering battle.
ACOTAR Role: Nesta's Made sword.
GWYDION
Gwydion is a name meaning "born of the trees" and is the given name of Gwydion fab Dôn, a magician, hero, and trickster of Welsh mythology. Caer Wydion, the castle of Gwydion, was the traditional Welsh name for the Milky Way.
ACOTAR role: The Starsword and twin to Truth-Teller.
NARBEN
No direct reference found, though narben in German means "scar."
ACOTAR role: A lost sword from the Dread Trove.
OUROBOROS MIRROR
The ouroboros is an ancient symbol of a serpent or a dragon eating its own tale, first present in ancient Egyptian iconography and the Greek magical tradition, the term itself deriving from Ancient Greek. It has been used frequently and extensively throughout fantasy literature. It often symbolizes the cycle of life, death, and rebirth.
BOOK OF BREATHINGS
The Books of Breathing are a set of ancient Egyptian funerary texts that are intended to enable those deceased to continue their existence in the afterlife. It is a simplified form of the Book of the Dead.
VERITAS ORB
Veritas is of Latin origin meaning "truth." It is the name for the Roman virtue of truthfulness and in Roman mythology, Veritas is the Goddess of Truth. Universities and colleges such as Harvard use the term veritas as their motto.
SYMPHONIA
Symphonia is Greek in origin and was used to denote a variety of musical instruments.
AMARANTHA'S NECKLACE AND RING (JURIAN'S FINGER AND EYE)
Jurian's finger bone and eye, kept by Amarantha, are reminiscent of relics and reliquaries, often religious in nature. Relics typically consist of the physical remains or personal affects of a saint and the reliquary is the container that holds them.
FAEBANE
Possibly inspired by wolfsbane (aka aconite or monkshood), a poisonous flower ancient Greeks would poison arrows and bait with to hunt wolves. It is lethal and often fatal and is an oft-used plant and poison within the fantasy genre.
Fun fact: In Shakespeare's Hamlet, Hamlet himself was killed with an aconite-laced blade.

DAEMATI
Potentially a combination of the Greek word dae (to give) and the concept of mati, labeled as "the evil eye" in Greek classical antiquity. Daemati may effectively mean "to give the evil eye."
THE WEAVER'S SONG
The Weaver's song, sung when Feyre was stealing Rhysand's ring from her cottage, is similar to "The Twa Sisters," a traditional murder ballad dating back to mid 17th century England. It tells of two sisters who travel down to a body of water where the older one pushes the younger one in. A harp or a fiddle is then made out of the murdered sister's body, which plays itself and sings about her murder.

2024.05.13 22:06 gojane9378 Good News - PHL region women's health provider Axia supportive!

I am pleased to report that my annual visit with my long time women's health provider went very well. The female physician will manage my HRT! I was very comfortable w her knowledge and attitude. Axia is a women's health chain in the Philly region of the US. My HRT has been w a cash pay gyno. She was good; however, I wasn't comfortable w the disconnected care. Like- I have a gyno for a pap and mammo & -another- gyno that's HRT only? What if I need care or diagnostic testing connected to the HRT? I am very relieved. I've been on HRT for 7mo. She was surprised I'm still bleeding and spotting. During the pelvic, she thought my uterus seemed thick and ordered a vaginal U/S. I was like can I just get a uterine ablasion? She responded one step at a time (which makes sense). She even agreed to continue my testosterone cream w the same compounding pharmacy! I just feel like I'm in a less fractured care situation. I hope the bleeding, uterine lining, fibroid possibility will be figured out though! Again, just wanted to share this good news that there is positive momentum for menopause care and providers who will manage HRT!! Hurray!

2024.05.13 20:19 ikieneng My fanfiction - Episode 2

My fanfiction - Episode 2
The next part is here! This episode is actually so long that I'm going to split it, so today, you're only getting part 1 of 3.
DISCLAIMERS (the same ones as before)
The point of this fanfiction is not to be a straight-up continuation of events with the same themes, intensity, and tone. If you go into it with those expectations, you are probably not going to like it. Rather, it’s supposed to be how I wish things went if these events were real life. The resolution you want for a real-life situation isn’t often the right choice for a show, but it can be incredibly beautiful. Think of what you’re about to read to be a separate show then.
Episode 1 of this fanfiction begins after the episode “2:00” (season 2 episode 4), so it replaces the episode “Cake” and the ones that follow it. This fanfiction expects you to have seen the entirety of seasons 1 and 2, so you should watch those first.
I myself bursting into the story here. The narrator and me are the same. While my character is like 95% real me, don’t take events about my life described here as facts. Some aspects of my life have been changed for the story. In my head, I started writing like an “alternate me” character in 2016, fulfilling a lot of the things that I wish I had in life, adding that to my story. I’m not really from Ukraine. I speak fluent Ukrainian as a foreign language, I started learning it in 2014, and I’ve talked to tons of people from there, but I’m not from Ukraine. I also don’t have as much money as I do in the story. I wish lmao.
If you want to post your own fanfiction, feel free to do so! To get your own post flair for your fanfic, and to appear in the side bar, please message me.
SEASON 2 EPISODE 6 - “ATTIC”
Part 1 (day 1)
So I'd be scheduled to work with Sean in the kitchen again, maybe a few days later, and Leanne would be "allowed" out of the attic again and have some time in the kitchen with me, like with Tobe in the real show (which Dorothy only started to allow because her strategy of pure torture and isolation wasn't working...). Julian would be there to watch us while Dorothy is at work (it would be around 10-11 AM) and Sean is shopping for new parts for the dishwasher.
https://preview.redd.it/0c1nl07xi80d1.png?width=780&format=png&auto=webp&s=e80511c2313074880382dd5b1773a6a046c0c90b
This time, we’d be told to cook something for Sean, following his recipe, maybe filling squid with a mix of mashed vegetables and spices.
This time, we’d be told to cook something for Sean, following his recipe, maybe filling squid with a mix of mashed vegetables and spices.
With Julian there, we’d be pretty limited in the things we can talk about, but the air would be noticeably lighter because of my gift for Leanne that neither of us can talk about yet, and you could tell on her face and especially in her eyes how much that has touched her. I’d imagine we’d talk about the stuff either of us likes to cook. She’d tell me that she doesn’t actually cook that much on her own. I’d be surprised and ask what she likes to eat, and she’d open one of the drawers and show me the cans upon cans of tomato soup and tell me she likes to just warm up a can every day and add some side dish on the plate, like toast, and something to drink, usually just water. She’d be a bit embarrassed about it because her experience is that people think that’s weird, but I’d tell her that I think that’s cute, and she could tell on my face that I mean it. I’d tell her how I eat obscene amounts of Ben & Jerry’s, even more so in the summer, and how her eating so much tomato soup reminds me of that, and I’d tell her how I drink sooo much Cola Zero that I’ve built up a lot of resistance to caffeine. “I can drink a whole two liter bottle, take my meds, and then go to bed just like that” (Side note: Coca Cola’s US website actually lists a two liter bottle among their sizes. Is that correct?) She’d be amazed and almost not believe me, but I’d show her my almost empty two-liter bottle in my backpack, with a little bit of condensed water from my fridge still on the outside, and she’d look at me with big eyes, bewildered and amazed, and we’d both chuckle before Julian tells us to stop. Quite confused, I’d ask why, like, what’s wrong with us laughing, and he’d tell us something like me not being there to have fun. I’d ask “Mister… What’s your surname?” - “Pearce” - “Do you have employees, Mister Pearce?” - “My father does, and I wouldn’t hire you anyway.” - “Oh, good, I wasn’t going to apply for a job with you in the first place. I can’t say I’m surprised that nobody wants to work for you.”
Leanne would be proud that I’m sticking it like that to Julian, and before he even has a chance to reply, she’d ask him “Could you please get us some wheat flour from the basement?” - “You want me to get you a fucking bottle, too?” - “Two would be nice.” - (Julian rolls his eyes) “I think I’d get two for myself, so I don’t lose it with you both!”, and he goes into the basement. As soon as Leanne can hear the door shut, she would suddenly tell me to fill up a bag she takes out from under the sink with ALL of the water bottles in the kitchen and a lot of the food in there that can be eaten as is and doesn't require cooking, and she’d tell me to do the same with my backpack, quietly go up to the attic, and hide there, so I can’t be seen if Julian comes upstairs, anticipating that she will be left alone in there again for days without food. I'd be confused at first, but she'd frantically beg me to do it immediately, and I'd trust her, I’d nod and say “okay” and do it. Julian would come back, and she'd pretend that I left. Julian would command her around again to finish up in the kitchen, and soon after, he'd lock her in the attic again, not knowing that I'm there.
I'd be shocked and really confused and concerned after realizing he just locked us in, and in that moment, she'd come to me, begging me on her knees to get her out of there, crying. At first, I’d just look around in shock with my jaw dropped, but then, I'd just hug her and just comfort her and let her know I'll do it. I'd feel so sorry for her... I wouldn’t know yet how long she’s been locked up for and why, but that wouldn’t matter for me to decide to help her. Really confused, I’d ask her to tell me what’s going on and why she’s locked up there. I’d be so shocked.
Once she’s calmed down enough after begging me to help her in full desperation, we’d sit down on the mattress. She’d tell me the full story - from the moment she first arrived at the Turners' in season 1 to now, including Dorothy’s brutal acts of violence and the pranks Julian and Sean played on her in season 1 to drive her out of the house, but she'd only mention the Church of Lesser Saints in passing as that's another really painful and complex topic she doesn't want to get into, and she wouldn't tell me about reanimating the doll yet because she knows how unbelievable the truth sounds (she tries not to let anyone know about her powers anyway). She'd stop several times while telling me all that because it's so hurtful, and I'd just comfort her and hug her . She’d cry out that it’s her fault and that she never should have come back. I’d just tell her that none of this is her fault and that she didn’t “come back” because she was taken against her will. “You can’t blame yourself for any of that. It’s not like they gave you a choice. It’s not your fault.” I’d tell her that she deserves none of the things they’ve done to her, I’d be absolutely horrified by them. She’d tell me about Sean’s visits to her, how he stopped coming upstairs after she was buried alive,
https://preview.redd.it/nfcpodj7j80d1.png?width=780&format=png&auto=webp&s=51f4fc8c9cf9bbc64ed04133019220890cb18b05
and how he told her stories about Dorothy to try and make her (Leanne) feel sorry for her (Dorothy), so she can tell him where Jericho is. “I told him that Jericho died, and he just said nothing and stood up and got out after a while. He knows that there’s nothing I can do…” While she cries on my shoulder, I’d just comfort her, rubbing her back, and just express my absolute shock at the things she’s telling me and just try to make her feel at least a little bit better. I’d be like “Those are the worst things I’ve ever heard… He actually defends her? Like, he got you out of a hole in the ground and still told you that she’s ‘ not a bad person ’?” Leanne would nod. “Oh my God, what a piece of shit… What an absolute piece of shit… I’m so sorry you’re going through that… She’s trying to kill you, she’s starving you, she forces you to use the bucket, and… Oh my God, I’m so sorry…”
I’d just let her cry for a little while as I’m comforting her. I’d cry myself, just so shocked and horrified and scared. Unsure if that’s what she wants to do, she’d ask me if I’m going to call the police. I’d ask her how long she thinks we’ll be in here, and she’d say she doesn’t know. “Do you think someone will come up here before tonight?”, and she’d shake her head. “Mr. and Mrs. Turner don’t come up during the day anymore”. I’d suggest we wait until everyone’s asleep tonight and then try to find a way out. She’d say that she’s already tried everything, and I’d be like “Of course, but now, we got a lot more options because you’re not alone up here anyway. Like, okay, that’s gonna sound really hard, and it probably is, but I can try to step up on your shoulders and reach the skylight, stuff like that. If we can sneak out without them knowing, that’s probably a lot safer for you than calling the police while we’re unarmed and the Turners are not. And the last thing you need right now is another traumatic situation”. She’d look up at me, surprised that I’m even considering her well-being like this. “And if we don’t manage to get out tonight, we can still come up with a plan. What do you say?” And she’d smile and nod. I’d smile back and rub her back and say “Heeey, it’s gonna be alright. I’m getting you out of here! Until tonight, let’s just make the best of it!”, and she’d smile really hard at that, which would really touch me, seeing her smile like that because she has hope now, and she’d almost cry.
“So how did you like the cake?”, I’d randomly ask her. She’d look me directly in the eyes and tell me how delicious it was, full of joy, and show me the porcelain baby and say that she wants to keep it. I’d be like “I told you you’re special!” with a big smile and embrace her over the shoulders as she’s smiling back. She’d go “Thank you so much, Daria!”, and I’d be like “Of course!”
I’d then go “Hey, let’s eat some of this stuff! You must be starving!”, and we’d divide the food and water we got upstairs, dividing it into rations for three days (just to be sure…), making her ration for today a bit bigger because she hasn’t eaten in days. Because calling the police would create a dangerous situation for us (and it’s not like there are any lengths the Turners wouldn’t go to), we’d keep that as a last resort if we run out of food, “but let’s see what we can do tonight”. Among the food in my backpack would be every single can of tomato soup from the kitchen and a can opener 😊 Even though it’s cold, the soup would be like heaven to her! And I’d be like “Mmm! Hey, honestly, this is way better than I expected!” - “You like it?” - “Yeah! I thought it might be a bit dull, but there’s, like, what’s in here? I think there’s some celery, definitely some salt, and there are some chunkier bits, like, yeah, this is pretty good!” This is her comfort and favorite food, and because it’s such a rare choice, I don’t think anyone has ever told her that they like it (even the way Dorothy said “You do love that soup, don’t you?” in season 1 kinda communicated that she found it odd or weird),
https://preview.redd.it/m0qd3ffbj80d1.png?width=780&format=png&auto=webp&s=3ca1365daff79f8b126460f21423e516ee5e9dd7
and she’d love hearing that! I’d ask her if she’s ever had Ben & Jerry’s, and she’d say she hasn’t. “I’m gonna give you some when we’re out of here, you’re gonna love it! My favorite flavor is Cookie Dough S’Wich Up, it’s like vanilla ice cream mixed with cookie dough, Oreo pieces, and brownie pieces, and also some chocolatey stuff mixed in with the vanilla in some spots!”, and that would sound great to her, she’d look forward to it. And I’d give her the rest of my Cola Zero. She’s probably had some before, but right now, she’d enjoy every bit of it.
I’d take out my two phones at some point (I actually do have two - an iPhone 15 Pro Max and a Samsung Galaxy S5 Mini lmao, but there was no 15 Pro Max at this time, so I’d have an iPhone 13 Pro Max here) and give her the Samsung because, like, we don’t really know for sure if anyone will come upstairs before tonight. I’d add my own number as a contact as well and add her as a contact on my iPhone under the number of the Samsung phone, so this way, she can call the police herself if the Turners find me upstairs with her, and the police can find out where I am if anything happens to me, and vice-versa.
After we’re done eating, knowing that we’re left with like ten to eleven hours until we can try to get out, we’d just sit there on the mattress saying nothing for a few seconds. Breaking the silence, I’d look around a bit and ask “Did the attic look the way it did when you… you know, or did you decorate it like this?”
https://preview.redd.it/x38rzmzdj80d1.png?width=780&format=png&auto=webp&s=18f45af349fce7f3783887e7e2eb3a27c7c27ced
She’d say she did. “It’s beautiful, especially with the lights and stuff! You’ve got a good eye!”, and she’d smile a bit in embarrassment. I’d be like “Hey, I mean it! No need to be embarrassed!” and then say “Have you heard about, like, I don’t know what to call it, but some department stores have LED chains that you can stick to surfaces and control the color of with a remote. Most of them can even fade back and forth between colors. When we’re out of here, if you want, I can show you some. If you already like these lights, you’re gonna love them!”, and she’d smile and say that that sounds great! I’d show her some on my phone, and we’d imagine putting them up around us and talk about our ideas that we couldn’t realize because we obviously just wanna get out of there, and we’d talk about where we’re gonna go once we are. I’d suggest my place, a small rental house like twelve minutes by bike from there that’s technically in East Lansdowne, where we can stay at least for a little while, and she’d say she’d love to, but to please take her somewhere safe where the Turners can’t find her, somewhere far away, certainly not this close to them. I’d suggest going to a hotel, and that would sound great to her. We’ll probably have to go to my place first just to get my stuff, but yeah, we can go to a hotel from there. I’d again just reassure her that it’s gonna be alright.
She’d bring up that I told her I’ve spent an exchange year in California, and I’d say that yes, I’ve spent a year in San Diego and add “Still the best year of my life.” She’d ask why, and in her own words, she’d say that we talked so much about HER life, and she wants to know some more about me. I’d tell her I wasn’t the first one in my family to do a student exchange year. My sister went to Denmark from 2008 to 2009. “You got a sister?”, Leanne would ask, and I’d tell her I got two and ask about her family, and she’d tell me she’s an only child. I’d tell her my sisters moved out, or rather, they were forced out, in 2007 (I think) and 2012, so I was alone with my parents for several years, which felt a lot longer. “Time already flies by at twenty-three now”. And anyway, at the time, I thought my mother, who did most of the “parenting”, wanted to make me happy by letting me go abroad for a year, but in retrospect, it’s pretty obvious that she just wanted to be rid of me for a year, the same as when my sister went to Denmark. In retrospect, I remember how many arguments she and my sister had after she came back, which was partly because she definitely enjoyed being rid of her for a year, and then, she had to “deal” with her again. Leanne and me would just lock eyes, and I’d say “We both got terrible moms” and chuckle because of it while still having sad expressions on our faces.
I’d say “Anyway, my first choice were the US, and I got placed with a family in San Diego, California, or [sãn ˈd̪je.ɣ̞o] in Spanish”. Surprised, she’d ask “You speak Spanish, too??”, and I’d be like “Yeah, but not back then”, and in awe, she’d ask how many languages I speak. I’d answer her in each language before saying what language I was just speaking in - fluent Ukrainian, English, and R*ssian, rather good Spanish, some French and Dutch, and I’ve forgotten most of the Finnish and Azeri that I used to know, and I’d show her on Google Maps where that’s even spoken. She’d be really impressed ahaha, and a bit embarrassed about herself. I’d be like “Hey, it’s okay, you don’t need to compare yourself to anyone. Life’s not a competition”, and that would be a really important lesson in life for her, certainly very different from how she’s been taught to think before, especially by her mother and by the Turners. She seems pretty smart anyway, and when I tell her that, she’d be really surprised and flat-out say no, and I’d be like “You’re super resourceful in the kitchen, like, you can come up with solutions to problems pretty quickly, you’re great at remembering details, and you already know that you got lots of special skills! I’d say you’re pretty smart!”, and she’d smile again before making a sadder face and telling me that she never went to school because the Church never let her. I’d be like “That’s… That’s horrible. If you want, there are YouTube channels out there that maybe you can use to learn about all the stuff you missed out on”, and she’d nod a little with a bit of a smile. “Doesn’t say anything about your intelligence anyway. Credentials are only credentials, and school is pretty terrible at telling you how smart you are.”
Back on talking about California, I’d tell her about my year there from August 2014 to August 2015. While there, I didn’t even realize how free I was all of a sudden because I was away from my parents for the first long time in my life. I changed a lot in that year because I didn’t have them look over my shoulder and judge my every move anymore. My mother even took me by the hand outside up until I left for America, and suddenly, I could socialize with whomever I wanted, I could stay outside of the home after school, like, AT ALL, the family I was with, they actually cared, like, I could talk to them, I could just ask when I needed stuff, and they didn’t force their restrictive values and stuff on me, I could just be me, y’know? I’d tell her that my time in California was also the first time I had a girlfriend (a little hint there ahaha), which my parents would have gone ballistic over if they found out. Leanne would ask if her and me still speak, and I’d be like “No, not in a very long time”, and she’d ask about the family I stayed with in San Diego, if we’re still speaking, and I’d be like “Oh yeah, we do. They know about where I live now, about a lot of the things that’s happened in my life since 2015, and yeah. We actually talked just a few days ago.”
“If it’s not too personal”, I’d ask, “do you still talk to your parents?”, and she’d shake her head and say she doesn’t, and that she doesn’t wanna talk about that, maybe another day, and I’d be like “Okay” and respect that.
Back on talking about California, I’d say “ignorance is bliss”, so to come back from America a year later (we already moved to Kyiv City before I left for America), where nothing had changed, with how much I had changed in that year without realizing it, my home life became horrible as a result. I suddenly realized that my parents having loud arguments several times a week is NOT normal, and I began to realize that my mother probably never cared so much about me and my autism diagnosis (which I got in 2006) because she loved me, but because she used it to cash in benefits for it all these years. I have no idea how much she received, but one time, I saw the bank statements of my parents’ shared account, and there were the equivalent of like $8000-$9000 in there, while I only received the equivalent of like $30 per month as an allowance. For years, a health inspector would come by once a year to check up on me (mostly by just talking to my mother) for continuing the granting of the money she exploited me for, and for years, she'd taught me to act like - literally - the most mentally disabled person ever during those check-ups, either ignoring the inspector completely and acting like they're not there at all, or cowering up in a corner and pretending I'm terrified. This way, she cashed in the money that's granted for the care of people whose level of disability is comparable to that of late-stage dementia patients… While the government was already struggling financially! Living in a normal environment for a year really changed me, and I didn't notice it until I came back, when I finally stopped playing along, which would make everything worse for how I was treated, and just one month later was when my parents broke up and decided they wanted a divorce, which made my world crumble even more than it already had.
If I didn’t have feelings for her, I’d probably just call the police, but because I do like her in that way already, I’d just go the extra mile and comfort her and ask her if she can tell me what’s going on and stuff, assuring her that I’ll get her out of there.
If Leanne was a completely different person and I didn't have feelings for her, I'd probably call the police, but when you're slowly starting to fall in love with somebody, you just wanna make sure they're safe and be really careful about this. I haven't gotten to a lot of the stuff in my life yet because it's a long story, but with how Leanne and me both went through parental abuse, parents who worked really hard to make us feel horrible, strict religious abusive upbringings, horrible punishments when we left religion, feeling so left behind in our development because of our upbringings and struggling to succeed in the wider world as a result (it's so hard to actually find someone who understands what that's like. I feel so much comfort and understanding knowing that Leanne can really relate to this!), and falling into the traps of other people who used our lack of experience and agency, we both went through so many similar things in different ways, and I'd think we'd bond soooo much over that, knowing and feeling how much we both understand each other through the similar things we went through, that would bring both of us so much comfort! I think we'd not just be great, but great for each other, not only through our similar experiences, but also through our similar personalities and values, like how we're not fitting in with people. I love her peculiarities so much that people just call weird, like how she eats sooooo much canned tomato soup, how she arranges everything so tidily, like her plates or her food in the kitchen, or how she keeps bugs she tries to reanimate.

Something I'd notice so easily at this point in the story already are the ADORABLE ways she reacts to things with her face and verbally! Some examples of what I mean by that are her short pauses before she speaks if she doesn't know how to answer right away,
https://preview.redd.it/u5yj2u9nj80d1.png?width=780&format=png&auto=webp&s=b3fdaaa7db350b8664b4f0391e5ba57621cae47a
how she answers non-verbally sometimes like smiling and nodding instead of saying yes,
https://preview.redd.it/4nkz9lvnj80d1.png?width=780&format=png&auto=webp&s=509294d626b4e85f75a630214fecb5a836f6955f

the way she moves her eyes when something's awkward,
https://preview.redd.it/7pits5koj80d1.png?width=780&format=png&auto=webp&s=45bd06c9198a480c47e61c8f8b3256fc958a7202

that is SO INCREDIBLY CUTE, I adore it so much, it adds so much to her personality! I'd notice that so hard already and absolutely adore it!
And even though we wouldn't know much about each other yet, we'd already notice and really like these things about each other. And up there, when I tell her a little bit about my life like I just have, she'd really feel for me, too and comfort me back, and that is honestly so wonderful to imagine for me ❤️ She'd sit there next to me and listen a lot and look at me, and embrace me a little bit. She wouldn't really know how to do that yet because it's not something that people ever did for me or taught her to do for others until I just came along, but she'd now know how good that feels and do it for me, too, as best as she knows how, and that's the best thing about it!
Late that night, when we think that everyone else is almost certainly asleep, we'd try to find anything we can in the room to get through the door of the attic without being loud enough to wake anyone up (because then, we'd both be screwed), but there would be nothing we could do to get out right then and there without the Turners waking up. If there was, Leanne would have been long gone already. We’re both twigs lol, so we’d step up on each other’s shoulders to try and reach the skylight, but it would be too high. We’d look for long solid objects to try and reach the skylight, but anything we find wouldn’t be enough. I’d double-check the door to the other part of the attic, and it would be locked. Smashing any doors would wake everyone up at night and make them come upstairs during the day. The window is locked, and it’s way too high for a safe fall anyway. So we'd make plans. If, in three days, we're still locked up, we will call the police because we'd have no other choice, but if Sean or someone else comes in by then, Leanne would be ""given"" a few hours out of the attic again, and they would leave the attic unlocked like they did before
https://preview.redd.it/xlgdf3mrj80d1.png?width=780&format=png&auto=webp&s=1b517528b4d494774c463ee320cd0ded84367809
because they'd think no one else is in there, and I would leave all the rations in the attic, sneak out of the attic, and leave through the basement. I'd leave the Samsung phone with her, so that she can reach me after I get out of there and prepare to get HER out of there. The plan would be for me to go home, taker a shower, type up everything I've seen into a PDF file addressed to the police, and send the PDF file to one of my internet friends (I actually have such a hard time making friends, another way in which Leanne and I are so similar and would really get each other). (I’m changing his name for this story for privacy reasons) I'd probably choose my friend Liam for this. I'd tell him that if I'm not back online telling him I'm safe in 24 hours, that he should then open the PDF file and call the Philadelphia police and read it all out to them. If I lived in America, I'd definitely get a gun lol. In Pennsylvania, I'd actually be able to just go buy one, I'd pass the requirements of the instant background checks. I'd get my gun, pack up everything we'd need for the next couple of weeks, and get a taxi to like one block away from the Turners', and with my gun for intimidation (considering that they're holding her hostage, this would probably fall under acting in defense of a third party), lock the Turners in the bathroom, have Leanne come downstairs, and leave with her, get to the taxi, and drive off, out of Philly, and be safe from the Turners for now.
But we'd leave the details for the next day. By this point, I'd need to tell her about some of my medical conditions, like night terrors, which usually fade during one's youth, but for me, they never did, so that she's prepared if they happen and doesn't get too freaked out, and so she knows that they're completely triggerless and can happen to me even after the best of days. I’d tell her that people don’t usually remember their night terrors at all, so she knows that I will act like nothing happened in the morning because I literally won't remember, it's only sometimes that I know that SOMETHING scary happened, but I rarely ever remember the night terrors themselves at all), and also so she knows to make sure I won't hurt myself or her if I have an episode, and also because it's a safety issue in this situation, to make sure that the Turners don't hear me, because if they did, we’d be screwed. And I'd have to tell her about my PTSD (because of the war in Ukraine, I’d tell her I’d lived through the first three days of it), which gives me nightmares, and to please wake me up if she notices I'm having those. We'd share the tiny mattress and covers that are up there, say good night, and fall asleep next to each other! Because I wouldn't have my meds, she'd fall asleep first, and I'd just look at her for a while 😊

2024.05.13 14:08 Successful-Bison9429 29 years old, 5 '11'', 80 kg, I walk for over two hours three times every week, I never smoke, I drank just a glass of beer in almost a month, but my blood pressure doesn't seem to lower at all.

Here's some context to make it not look like the typical high blood pressure rant:
There may be a genetical tendency in the males from my mother's side to high blood pressure. My grandad, who is 82, developed it at the age of 40, my uncle developed it along with other metabolic diseases (though he did nothing to keep his health and weight in check, the result being that he developed diabetes at the age of 45). My father, who is 70, also found out to have high blood sugar levels, but he's still far from developing diabetes.
That being said, I actually suffered from LOW blood pressure until I turned 27. The first high blood pressure reading was in late 2022, which coincided with me stopping hiking and a long series of highly stressful events, among which the defense of my Master's thesis, my scholarship being halved (with the following financial hurdle) and the (figurative) loss of several friends because of the war in Ukraine.
Understandably, I have developed a crippling anxiety that, as far as I can tell, seems to be perceived by others even when I am feeling calm.
Once my studies were over, it took me several months to relax myself and, eventually, in August 2023, I passed a medical assessment test to become Cabin Crew. I still remember that the urine samples results were OK, which means the possibility of diabetes being behind my hypertension seems unlikely (not to mention that I pee 5-6 times every day and I never had any diabetic attack, unlike my uncle). However, the blood pressure reading was still high, 140/90, but the doc assumed it was just white coat syndrome because of the high BPM. In fact, I never felt sick or had any excruciating pain in the chest (such as angina).
Even more strangely, my pressure has remained high despite that fact that I drastically limited my intake of salt, caffeine (I stopped drinking espresso last year), tea (I usually drink camomile/herbs) and processed food (I usually eat by McDonald's and similar chains less than twice every month, and I limit the consumption of sweets and other treats to parties or travels).
To sum it up, I pay attention to my diet and I am also thinking of practicing yoga and hitting the gym again after a two year hiatus.
In spite of all of this, my BP readings at the pharmacy stubbornly remain high. 148/80 last week 30 minutes after walking up and 146/95 today after breakfast. In both cases, my BPM were high (>100), which may still be explained by white coat anxiety, but after one and half years, I'm starting to believe there may be indeed something behind these constant readings.
I am currently waiting for my urine and blood results, but my gut instinct is telling me there won't be any odd value. My urine sample looked like everyone else's (no cloudiness, bubbles, etc).
If a balanced diet doesn't seem to lower my blood pressure, is there any other way to lower it without taking medicaments?
Thank you all for your time.
Note: I'll keep this thread updated until I find out the actual cause with my doc.

2024.05.13 13:02 artemON2121 (Handels, Commercial, Business) engineer vs Ing. Engineer (Engineering Technology) - finding the right fit for my goals.

Hey everybody. So, I am currently faced with quite a decision to make as I am approaching my university education. What I really want to get out of this post is some advice, comments, and corrections that would allow for a better-informed decision between the 2 programmes and an understanding of their alignment with the background I seek for accomplishing my future goals. This post will be quite long, so thank you in advance for your time!
A little background:
I am a Ukrainian national who has fled the war and is currently living in Belgium. I am currently completing my last year in the British educational system, where I do A-Levels in Chemistry, Biology, Maths, and Further Maths (good grades in all). Overall, I have a very broad range of experiences and interests. A few years ago, I got acquainted with programming: Python (Django), C++, JS—all the basic stuff, at quite a basic level. Mainly used the knowledge in WebDev. Participated in Olympiads, robotics workshops, and visits to KUL`s cytogenetics labs. Later, I got experience in creating and managing volunteer events, where I collaborated with big institutions. Also, I am currently in the process of co-founding a new organisation on a bigger scale: more people, personal office space, etc. I got acquainted with creating and managing organisations, and I enjoy it. When consuming content, it is often technology-related: the science behind it and the implementation of it; its impact; its management; and the creation of supply chains. I am communicative, with team-leading ability, and am not afraid of taking on responsibilities. English, French, Ukrainian, and Russian languages. I am planning on learning Dutch. X-shaped profile.
My goals and desirable background:
I am not an R&D person; I don`t see myself as an inventor or an on-site production manager (for more than a few years). I am leaning more towards business management (resources, innovation, and technology management). I want to have a deep enough knowledge of tech and science, which would allow for a deduction of the science behind existing and emerging tech, its utility, and its applicability to my position or possible entrepreneurial pursuits. Even though you certainly cannot predict your future, I want to finish my studies here, get great working experience in great companies located in Belgium to build up my competence, and eventually return to Ukraine. Where I not only feel a great connection and responsibility for post-war reconstruction but also see a lot of opportunities in the post-war vacuum. I want to participate in the reconstruction and build-up of new industries in Ukraine: the energy sector, pharma, military, data, and compute centres.
What is my vision and understanding of each programme (correct me if I am wrong in anything there)?
(Handels, Commercial, Business) engineer:
A trademark degree in Belgium for management/economics. Good mathematical and statistical component. But it does not seem to have an extensive scientific component: a bit of software development, a bit of physics and electronics. Very versatile, quite reputable, and recognisable both in Belgium and abroad. After the bachelor, I want to continue with the same master in KUL, or if I stumble upon something with a bit more technology-related content elsewhere. From my understanding, the bachelor will allow for the latter option as well. This degree is reputable amongst employers and thus will be a good door opener to internships and positions in Belgian companies and multinationals.
Ing. Engineer (Engineering Technology):
Engineering Technology is a quite recently “academized” degree that focuses on the application of known engineering principles in projects and industry. It allows for specialisation in the 2nd year but gives a common background in the 1st year. It is less theoretical than, burgerlijk, civil engineering (polytechnical engineering), thus resembling the more hands-on structure of US degrees. After finishing the bachelor’s in engineering., I thought of the master's in management engineering available at KUL and other unis, which could allow for interesting experiences in companies due to the profile. A Bachelor in Ing, will provide a more solid scientific foundation.
Questions regarding each programme:
· Was my assessment of the programmes wrong in some ways?

(Handels, Commercial, Business) engineer:
1) Is this multi-disciplinarity an advantage? Will it not create the impression of "jack of all trades, master of none.”?
2) Is the scientific component of the course sufficient for my goals?
3) What position within companies am I looking at as a graduate?

Ing. Engineer (Engineering Technology):
4) There seems to be a ceiling of opportunities or difficulty in progression in Ing. rather than ir. engineers both for the first jobs and later in a career. Is the assessment generally true?
5) The degree is recently academized, so foreign universities and employers might not be very familiar with it. Are my options limited after graduation, purely based on this fact?
6) What are the positions within the company that I am looking at as an Ing. engineer + M.Sc. in management engineering?
7) Will the profile of Bachelor in ing.engineering+ Management Engineering (or other management degree) be valuable or considered a weird disjunction?

Other questions:
8) Comment on what you think will be a better education path for my background and desired goals, preferably with some explanation.
9) How robust is the Belgian engineering scene in terms of technology adoption and creation, particularly for gaining relevant work experience?
Any other comments and feedback are greatly appreciated.
Thanks a lot.

2024.05.13 03:22 guymon Advice for someone investing in Absolute NNN in CA?

Hello! My parents are planning on selling their current commercial property (industrial warehouse+office space) and start looking at Absolute NNN properties instead as they start to want to retire and ideally spend little to no day-to-day time managing a property.
They are in California and their broker has been showing them places like chain restaurants and pharmacies. Is that pretty much the core of the market? Are there any good resources for knowing what to look for in an Absolute NNN agreement, or any advice you might have?

2024.05.13 02:44 Rainyfriedtofu Vertical consolidation of healthcare: the goods, the bads, and the might not work out. bahaha!

Hello Fellow Apes,
This is the only time that I have to write so I'm going to make anymore post regarding vertical consolidation since UNH is being sue like a catholic priest over its current monopoly of healthcare.
https://news.bloomberglaw.com/health-law-and-business/unitedhealth-faces-justice-department-antitrust-probe-wsj-says
With that said, let's get this show on the road. Vertical consolidation, often referred to as vertical integration, is a strategy used by companies to gain control over their entire supply chain. This approach involves a company expanding its business operations into different stages of production or distribution that are normally operated by separate businesses. We're going to use UNH as an example here, but on the surface, UNH is pitching their vertical integration in a positive light.
UnitedHealth Group, through its Optum division, is actively engaging in vertical consolidation within the healthcare sector. This strategy involves integrating various healthcare services under one corporate umbrella, which has been a significant trend in the industry. Optum encompasses a wide range of health services, including pharmacy benefits management, healthcare providers, clinics, and data analytics platforms. By consolidating these services, UnitedHealth aims to create a more integrated healthcare delivery system that can provide more coordinated and efficient care. We'll go more into this later.
This type of vertical integration allows UnitedHealth to manage both the provision of healthcare services and the insurance coverage aspects, leading to "potentially" lower healthcare costs and improved health outcomes. However, this consolidation also raises concerns about market competition, as it could limit choices for consumers and control more of the healthcare process within a single company's ecosystem.
This consolidation trend is not unique to UnitedHealth. It reflects a broader movement within the healthcare industry where large entities are increasingly integrating providers such as hospitals and physician groups to create comprehensive healthcare networks. This trend is expected to continue and possibly accelerate, influencing how healthcare services are delivered and accessed in the United States.
Health insurance companies have increasingly engaged in vertical consolidation as a strategy to streamline operations, reduce costs, and expand their market influence. There are several ways in which these companies are using vertical consolidation to funnel consumers into their health services business segments:
  1. Many insurers are buying or partnering with hospitals, specialist clinics, and primary care practices. By owning these providers, insurers can direct their policyholders to use these in-network services, which are often cheaper for the insurance company due to controlled pricing and streamlined services.
  2. Insurers are acquiring or creating their own Pharmacy Benefits Managers (PBMs). These entities manage prescription drug benefits on behalf of health plans. By controlling the PBMs, insurers can influence prescriptions and direct consumers to specific pharmacies or mail-order drug services, often owned by the insurer itself.
  3. Some insurers are integrating vertically by acquiring companies that provide specialty medical services, such as dialysis centers, mental health services, and rehabilitation facilities. This allows them to keep these often expensive and frequently used services within their network, potentially lowering costs and increasing the use of these services among their insured population.
  4. Insurance companies are also developing or acquiring digital health platforms that offer telemedicine services, health apps, and other digital health tools. These platforms often direct users to preferred providers or services that are part of the insurer’s network, promoting an integrated approach to healthcare that keeps all aspects of patient care within one system.
  5. Through vertical consolidation, insurers are pushing more into value-based care agreements where providers are paid based on patient outcomes rather than services rendered. By owning healthcare providers, insurers can better implement these programs, which aim to reduce unnecessary services and focus on preventive care.
Vertical consolidation can also give health insurance companies a form of crowding power in the marketplace. Crowding power typically refers to the ability of a company to dominate certain areas of the market, which can edge out competition and influence both pricing and availability of services. By owning multiple layers of the healthcare delivery system—from insurance to providers to pharmacies—insurers can gain substantial control over the entire healthcare experience of their consumers. This integration allows them to dictate terms and conditions to both consumers and other healthcare providers who are outside of their network. Controlling more steps in the healthcare value chain allows insurers to potentially set or influence pricing at multiple points—from the cost of medical procedures to the price of pharmaceuticals. This can make it difficult for smaller, independent providers or new entrants to compete effectively. Furthermore, With their own networks of providers and services, integrated health insurers can direct patients to use these in-network services, thereby increasing the volume for their owned services while potentially limiting the volume going to competing providers. This can crowd out independent providers and smaller competitors who cannot offer the same range of services or prices.
Vertical consolidation can also provide insurers with extensive data on consumer behavior, health outcomes, and cost efficiency. This information can be used to optimize their own services and pricing models further, strengthening their market position and making it harder for others to compete on the same level of efficiency or personalization. Large, vertically integrated companies often have greater resources to influence healthcare policy and regulations. This can lead to a regulatory environment that favors large, integrated entities over smaller competitors or new entrants. While crowding power can lead to increased efficiency and potentially lower costs for consumers, it also raises significant concerns about competition, consumer choice, and the overall health of the market. Regulators often scrutinize such consolidations closely to ensure they don't harm consumer interests, and this is why UnitedHealth is being sued by DOJ.
For Medicare Advantage (MA) plans, which are private plans offering Medicare-covered benefits, vertical consolidation can play a significant role in managing financial risks and costs, particularly in the face of changing policies from the Centers for Medicare and Medicaid Services (CMS). One of the primary ways vertical consolidation helps is through more direct control over costs. For Medicare Advantage plans, controlling healthcare costs is crucial since they receive a fixed payment per enrollee from CMS. By owning providers, MA plans can directly influence the cost of care, reducing overall expenses and potentially offsetting losses due to policy changes that might reduce reimbursement rates or impose new cost-sharing requirements. Vertical integration allows MA plans to streamline care delivery. For example, they can implement more effective care coordination and management practices across their owned networks, which can lead to better health outcomes and reduced hospitalizations—a significant cost factor. This efficiency can help balance out negative impacts from policy changes by keeping patients healthier and reducing expensive medical interventions. Owning a broader swath of the healthcare delivery system gives MA plans access to comprehensive data across the care continuum. This data can be used to identify cost-saving opportunities, manage chronic conditions more effectively, and tailor preventive measures. Such data-driven strategies can help MA plans stay financially viable even when CMS policies become less favorable. However, with the recent earnings, we are seeing many companies failing on their cost modeling.
The twist behind all of this is the whole process might not be all rainbow and butterflies. In a somewhat new findings in the JAMA health forum, vertical consolidation might actually drive higher utilization and spending. In the words of Moocao, "Integration of health systems allows PCP to refer to higher-level specialists if they can't solve something. Before integration, they have to think really hard before referral"
https://www.fiercehealthcare.com/providers/newly-integrated-pcps-steer-patients-toward-systems-higher-utilization-spending-study
Despite previous studies, vertical integration isn't beneficial for healthcare access or coordination. This is why the Biden administration and other lawmakers are scrutinizing mergers that lead to greater vertical integration and provider consolidation. This scrutiny includes proposed updates by the Federal Trade Commission and the Department of Justice to antitrust guidelines, specifically considering the impacts of vertical integration. There is ongoing debate, with some stakeholders arguing that consolidation can help providers survive economically challenging times, while others believe these mergers cause significant market harm without benefiting patients.
Anyway, I hope this helps explain to you why you are seeing so much consolidation, but the cost saving isn't showing up, and why these giant healthcare companies are being sued.
