Hi everyone, I’m a third-year Caribbean IMG facing a decision about applying for PM&R residency THIS cycle or the NEXT and could use some advice from those who have been through this.
  1. Background: It's taken me six years to complete medical school due to several factors, but I passed Step 1 on my first try. My timeline is tight with cores finishing by the end of June, leaving July for taking Step 2, and August for my only away rotation to secure my only PM&R letter of recommendation right before submitting ERAS in September.
  2. Research and Education Initiatives: My current research experience includes two case presentations in internal medicine, but no publications. I have the opportunity to publish, though not directly related to PM&R. I've also participated in a PM&R-specific journal club and Grand Rounds at the local University hospital, and attended an AAP conference.
  3. Volunteering and Leadership: In college, I participated in 3 years of back-to-back medical mission trips to Honduras. During medical school, I’ve engaged in some volunteering, but not PM&R-specific opportunities like adaptive sports which I am going to do if I decide to postpone my match. I also served as a clinical peer mentor and was the AMSA volunteer chair at my school.
  4. Letters of Recommendation and Away Rotations: I’ve been advised I need two PMR letters, but will likely only have one from my away rotation before applying. I can only manage one away rotation due to time constraints, though I've heard two or three are ideal.
  5. Concerns: The main red flags in my application are the extended time taken to finish medical school and the limited number of PM&R-specific experiences before applying. I feel like the only way for me to push for this cycle is to get a stellar (240+) step 2 score. But even then, I don't feel like I'd be a well rounded PMR applicant.
Any insights or advice on whether to push for this application cycle or wait another year would be greatly appreciated!

Staying put in Europe after the Germany update, the Emerald Isle is put under the spotlight, as we see the island nation attempt to confront the reality of being a small country in a large world. Will the Irish people maintain their independence, or are they to become the subjects of a foreign power once more? And of course, this release also includes a few of the usual tweaks and changes for elsewhere all around the world.
This rework is filled with content all across the political spectrum, and has been a long time in the making, so we hope you enjoy it!

Baltimore’s Francis Scott Key Bridge collapse

On Tuesday, the most iconic Baltimore’s Francis Scott Key Bridge collapsed.
The reason for this unfortunate incident was that the giant cargo ship collided with Baltimore’s Bridge.
Six people were presumed dead as they were missing, and their search was suspended until Wednesday morning. Those six missing people were construction crew filling the Bridge’s potholes. Honduras’ Deputy Foreign Affairs Minister Antonio García told the Press that a Honduran citizen, Maynor Yassir Suazo Sandoval, was missing. He said he had been in contact with Suazo’s family.Guatemala’s Consulate in Maryland said in a statement that “Two of the missing were citizens of the Central American nation.”The Consulate of Mexico in Washington said through the X platform that Mexicon nations were also missing in the incident.
Cargo ship crew members issued a mayday call a moment before collision so the traffic could be stopped to cross the Bridge. Authorities had just enough time to prevent cars from crossing the Bridge.
The cargo ship was seen to be moving fast, with black smoke coming out of it, and lights were also flickering on and off just a few moments before it struck the central support system of the Bridge, causing the whole Bridge to collapse within seconds.
According to the data of Marine Traffic. The Dali, headed from Baltimore to Colombo, Sri Lanka, and flying under a Singapore flag, is about 985 feet (300 meters) long and about 157 feet (48 meters) wide.
All crew members and the two pilots on board were accounted for, and no injuries were reported.

I saw this today and almost dismissed it but then I remembered hearing that they are evacuating the Caribbean Islands. Now it's rumored that this plane is being escorted right to there... The post I saw also kept saying EBS is coming soon. Emergency Broadcast System 10 days of some kind of warning followed by 3 days of EBS. Elon also just did a cryptic post saying if we deny being in a burning building no one can help us.

2024.03.01 16:00 galaxxybrain Are my parents being gullible tourists?

My parents are on vacation in Roatan, Honduras right now with another couple their age who live on the same street and are friends. They were approached by a seemingly kind young man on a beach (I think) who started chatting with them. They were invited back to this man’s house and went. I believe it was a short walk to get there. They sent me pictures of this man posing with his entire family: siblings, a few young kids, grandma, mom etc. they clearly don’t have a lot, shack house with dirt floor, mom& grandma sell fruit to bus drivers to make a living etc. They were showing them the herbs/plants they grow and use for medicine etc. My parents and their friends gave them $200 USD and told us “we feel like we’re best friends with them” We love them so much, they’re so kind etc.
Then they tell us that this young man is going to pick them up today and drive them 40 minutes to a nice golf course where he can get them 1/2 price entry so my dad and his friend can golf and my mom and her friend can relax at a nearby resort.
Red flags are going off everywhere in my head because I know Central America does not have a reputation for being all that safe but everyone tells me Roatan is a great place. If these people are trying to swoon my parents and their friends for money, or worse etc then they were shown just how easy of targets they are (willing to come to the home, willingly giving money etc)
Of course I can’t help but think of the worst case scenario. Them getting kidnapped, held for ransom, robbed, etc. why do they want to split up the men and the women? Don’t really know what to do… I told them not to be so trusting but they’re adamant this young man and his family “are legit” Half of me is saying “this is totally fine, everything will be okay!”
Thoughts?!
UPDATE #1 My mom told me they got to the golf course / resort and everything is fine, they like the guy in question. They paid the guy $70 per couple to get them there, 45 min drive. I guess the guy wasn’t clear with them on whether this $ included the drive back, that makes me kinda wonder if he’ll want a lot more for the trip back. He also got my dad and his friend into the course for $100 (likely 9 holes, my dad hardly does a full 18 anymore at 69 years old) but from what I’m finding online this would have been much much cheaper had they booked this little excursion independently. Sounds like they’re getting ripped off when it comes down to it but doesn’t sound like they’re in danger. I will post a second update when I hear back from them later tonight when (hopefully) they make it back to their main hotel. Thank you so much everyone for all the advice and lookin’ out! I’m glad to know I wasn’t overreacting..
UPDATE #2 They made it back to the hotel they’re staying at! YAY! Such a sigh of relief for me. Sounds like this young man wanted more money, but my dad negotiated out a very reasonable price. He was always kind with them and they got back with no problems. Overall, it was my rich American parents helping out a local with some extra cash and my parents actually did enjoy their experience. I feel a little bad for kinda freaking out, but would rather be safe than sorry. I can’t thank everyone enough for all the advice and comments. Had no idea this post would garner this kind of attention but safe to say I actually learned SO MUCH from everyone’s comments and gave me some things to think about and some things I wanna talk about with them. Y’all are the absolute BEST!!!!! <3

For example, you could redesign Sweden’s flag to include yellow, blue, and red. You could NOT redesign Sweden’s flag to include red, blue, and green - it needs to include both yellow and blue because they are in the original design. You also could NOT redesign Sweden’s flag to be red and yellow - it needs to have three colours.
The shade of colours you use in your redesign must be very close to the same as included in the original flag. Variation is permitted so long as it would not be noticeable at the distance a flag is normally flown at.
There are no limits on how you use the three colours in question - you can redesign the flags in any way you choose. But they MUST have three colours and no more or no less.

The Guatemalan Revolution

The Guatemalan Revolution began in June 1944. After decades of foreign economic domination that propped up military strongmen, the people of Guatemala had had enough. Their discontent with the administration of President Jorge Ubico, a military dictator who styled himself “Central America’s Napoleon”, came to a head on June 25th 1944, when school teacher María Chinchilla Recinos was murdered at a peaceful demonstration. Her death sparked a long summer of general strikes, suspended constitutions, Presidential resignations, and sham elections.
No longer able to keep control of the Guatemalan masses, who resolutely rejected Ubico’s hand-picked successor Juan Federico Ponce Vaides, Ubico and his dogs were forced to flee into exile in the United States. Under the leadership of the junta that succeeded him, lef by Major Francisco Javier Arana, Captain Jacobo Árbenz Guzmán, and the civilian Jorge Toriello Garrido, Guatemala had its first ever free and fair elections, in which the people of Guatemala selected Juan José Arévalo as their President.
Arévalo’s election marked a sea-change in Guatemalan politics. While he did not meaningfully challenge the economic domination of American-owned corporations like United Fruit Company, the more liberal political environment of his Presidency allowed labor and communist organizers, many of whom were released in a general amnesty early in 1945, to organize publicly for the first time. Things were looking up for ordinary Guatemalans for the first time in a long time.

The Flood

Then came the flood. Guatemala was blessedly spared the worst effects of the flood itself--the mountainous interior that hosts most of the country’s major cities was never seriously threatened by the rising sea levels--but no country could truly be spared from something so world-changing. By mid-1946, Guatemala’s principal port, Puerto Barrios, had been subsumed by the waves, leaving the export-oriented Guatemalan economy withering on the vine, and putting government finances into a tailspin.
Matters quickly went from bad to worse. Mountainous Guatemala quickly became a bastion of hope for people living in lower-lying Central American countries. Through 1946 to 1948, hundreds of thousands of refugees fled to Guatemala, El Salvador, and Honduras, stretching already meager government resources to the point of collapse.
In this period of global catastrophe, two major political trends emerged in regional politics. The first was a substantial demographic shift within Guatemala. The Maya-dominated lowlands of the Yucatán peninsula was among the first places drowned by the flood. Cut off from the rest of Mexico by poor infrastructure, made worse by the flooding of the state of Tabasco early in 1946, many, if not most, residents of Yucatán fled to Guatemala. By the end of 1947, the population of Petén Department, once one of Guatemala’s least indigenous departments, had a Maya majority.
The second was a rapid strengthening of relations between several countries in the region, most notably Guatemala and El Salvador. Arévalo and the President of El Salvador, Salvador Castaneda Castro, had been in talks to reform some sort of Central American Union since early 1945, and the need for coordinating a broader response to the crises brought on by the flood only accelerated those talks. The two signed the Pact of Santa Ana in 1946, committing both parties to deepening cooperation between their countries, and later signed the Pact of the United Republics of Central America, the treaty that led to the creation of Centroamérica, in early 1947.

The Centroamerican Counterrevolution

Despite the concerted and coordinated efforts of Guatemala and El Salvador, stability and living conditions in Centroamérica rapidly deteriorated through 1947 as the rate of sea level rise increased dramatically. With global maritime trade more or less non-existent as the rising sea swallowed all ports capable of handling traffic, the export-oriented economies of Centroamérica foundered.
The rural poor, deprived of the already pitiful incomes the export economy had afforded them, organized to demand the government immediately nationalize and redistribute the vast estates of foreign companies and Guatemalan elites, who together controlled some 70 percent of the country’s land. In the countryside, already a place that was often far from the government’s reach, peaceful organizing sometimes turned violent, with peasant unions responding to perceived government inaction by redistributing the land themselves--a process that often involved a painful and violent end for those who owned the land, who began organizing armed groups of their own to protect their families and their property. In most cases, this process had a clear ethnic cleavage involved, too, as the rural poor were overwhelmingly indigenous or Mestizo, while the landowning elite were overwhelmingly Ladino.
When word reached the Centroamérican elites that Arévalo was seriously considering a national policy of land redistribution to try to restore stability in the countryside, it was received extremely poorly, making the Arévalo administration a good number of new enemies in the Guatemalan elite. They found a sympathetic ear in the Chief of the Armed Forces, former Arevévalo ally Francisco Javier Arana, and in the Salvadoran Chief of Police Osmín Aguirre y Salinas. Meanwhile, old enemies were stirring. Ubico, never one to let a good crisis go to waste, had taken advantage of the chaos in the region to return to Central America, setting up shop in the neighboring Mexican state of Chiapas.

For the March 2024 contest, we are going to be doing a national redesign challenge.
The challenge will be to redesign the flag of a country which only uses two colours, and make a redesign using ONLY three colours - the two colours the flag currently features and ONE other new colour.
For example, you could redesign the flag of Sweden using blue, yellow, and purple, or the flag of Nigeria using green, white and red. You could not redesign the flag of the UK since it has three colours (blue, red, and white), and if you redesigned Poland but used red, white, green, and gold that would be too many colours.
We have two options of lists of possible countries for you to chose from, and we'd like the community to select which list to chose

The First Option

Redesign any of the countries whose flags currently have only two colours - 37 countries - (Albania, Austria, Bahrain, Bangladesh, Canada, China, Denmark, Finland, Georgia, Greece, Honduras, Indonesia, Israel, Japan, Kazakhstan, Kyrgyzstan, Latvia, Micronesia, Monaco, Morocco, Nigeria, Pakistan, Palau, Peru, Poland, Qatar, Saudi Arabia, Singapore, Somalia, Sweden, Switzerland, North Macedonia, Tonga, Tunisia, Turkey, Ukraine, and Vietnam.)

Option Number Two

Redesign any of the countries whose flags currently have only two coloured stripes - 8 countries (Austria, Indonesia, Latvia, Monaco, Nigeria, Peru, Poland, and Ukraine)
