A US lawmaker is accusing Amgen of “putting profits before patients” over its decision to continue marketing a high dose of a pricey cancer treatment instead of a lower dose that is less expensive and not as toxic to patients.ABOUT LUMAKRAS (sotorasib)
At issue is a medication called Lumakras, which is used to treat non-small cell lung cancer and which won conditional regulatory approval three years ago. At the time, the Food and Drug Administration required Amgen to run a trial confirming earlier test results, as well as a so-called post-marketing study to examine safety and effectiveness at different dosages, in order to gain full approval.
Last December, the agency determined the confirmatory trial was not acceptable and asked the company to run yet another trial, which must be completed by February 2028. Meanwhile, the company was allowed to continue offering a 960-mg dose of the drug, which the FDA originally did not believe was optimal, since a study released last October found its efficacy was similar to that of a 240-mg dosage.
Lumakras was approved by the FDA on 28 May 2021 as the first treatment for adult patients with non-small cell lung cancer whose tumors have genetic mutation KRAS G12C and who have received at least one prior systemic therapy. This was the first approved targeted therapy for tumors with any KRAS mutation, which accounts for approximately 25% of mutations in non-small cell lung cancers. KRAS G12C mutations represent about 13% of mutations in non-small cell lung cancers.Lumakras was approved under Accelerated Approval pathway and reviewed under Project Orbis, an initiative of FDA Oncology Center of Excellence, that allowed concurrent submission and review of marketing application by multiple regulatory agencies; besides FDA, the application was reviewed by UK MHRA, Brazilian ANVISA, Australia’s TGA, and Health Canada.
The approval was based on the study of 124 patients with locally advanced or metastatic KRAS G12C-mutated non-small cell lung cancer with disease progression after receiving an immune checkpoint inhibitor and/or platinum-based chemotherapy. The major outcomes measured were objective response rate (proportion of patients whose tumor is destroyed or reduced) and duration of response. The objective response rate was 36% and 58% of those patients had a duration of response of six months or longer. As an American, I find that attitude shocking. We have an Innocence Project here in the US precisely because juries are sometimes wrong. Justice is sometimes not served. Sometimes the police lie, or plant evidence. Sometimes eyewitness are mistaken. Sometimes medical examiners are not board certified and are less than competent. Sometimes evidence goes missing or is corrupted. There are so many reasons why innocent people are convicted every day.
So, I’m curious how many people here would be interested in a truly open forum where we think critically? In posting this question, I’m breaking no rules of the sub. She is suspected of being behind the deaths of at least 14 others, aged 43 to 104, as well as attempting to kill five more. All were given insulin whether they needed it or not, quietly injected while others were not around to witness it.
If Pressdee has killed that many people, it would make her one of the most prolific serial killers in the US and certainly the biggest in the state of Pennsylvania.
Her case has parallels with that of Lucy Letby, the British paediatric nurse who in August was found guilty of seven counts of murder and seven of attempted murder, and Harold Shipman, the doctor based in Yorkshire who killed an estimated 250 of his patients, making him Britain’s most prolific serial killer.
The defendant has admitted to harming, with the intent to kill, all patients named in this Affidavit. Her conduct spanned approximately five years over the course of eleven facilities in Armstrong, Allegheny, Westmoreland and Butler Counties. Her criminal conduct exhibited a pattern of behavior. PRESSDEE would often work the medication cart, administer insulin during the night shift when staffing was lowest and the facilities were quiet. The victims would often remain undiscovered until morning. PRESSDEE often took steps to ensure her victims would expire prior to shift change so that they wouldn't be sent to the hospital where her scheme could be discovered through medical testing such as a C-peptide tests [sic]. If PRESSDEE sensed the victim would "pull through" there is a pattern of her taking additional measures to try to kill the victims before they could be sent to the hospital by either administering a second dose of insulin or the use of an air embolism to ensure death. Despite PRESSDEE's efforts to conceal her actions, the investigative team uncovered the evidence detailed above in this Affidavit. The evidence along with PRESSDEE's confessions demonstrate sufficient probably cause to support the issuance of attached criminal complaint.While Lucy Letby was arrested prior to the beginning of Ms. Pressdee's crimes, the allegations of Letby having administered air injection and poisoning by insulin were not made public until the beginning of Letby's trial in October 2022, ruling out the possibility of her having operated as a copycat killer. Ms. Pressdee's confessions also support the validity of the charges against her, even though they weren't brought until Lucy Letby's case was already public knowledge in the UK.
