November 11th 2022: Emailed northwell health for the first time, they emailed back saying to call. I was too anxious so I avoided it for a few months.
Called northwell a few months later but got too anxious talking to the person who picked up. They were being normal and talking normally, it was just personal anxiety on my part.
October 2022 - Early March 2023: Spent time talking to trans friends and family members about their timelines and processes for top surgery.
Looked into Penn medicine for a bit but wasn’t happy with the surgeons there, specifically as a nonbinary person. The patient navigational team however is lovely.
March 2: emailed Penn health patient navigation
March 3-10: correspondence and phone calls w patient navigation (absolutely wonderful people, some of the easiest phone calls I’ve ever had) Got lots of into on surgeons, things I’d need, processes etc.
Date unknown: phone call to Penn medicine asking about surgeons and possibly setting up as a patient (v long wait time on phone) Surgeon I had heard good things about only works w CHOP program and I’m was too old for that program. Other surgeons I was v iffy on.
March 23rd: Back to square 1. Called northwell again to set up an appointment. Everyone I spoke to was really nice. Could have set up an appointment within the week but decided to wait till the end of the semester. Scheduled a trans care and primary care appointment for May
Couple of calls In between for confirmations. Trans care appointment got moved around a bit and ended up being moved to a phone call.
May 8th: Trans care call: Basic preliminary questions like: Emergency contact, what you’re looking for, are you thinking of looking into hormones, experience w dysphoria or dysmorphia, mental health, and eating/nutritional concerns, things you might want doc to know, piercings or tattoos, do you do any drugs or drink often, etc. total call time was about 20 minutes. Doctor was incredibly kind, I still experienced a good deal of anxiety but the call was super easy, welcoming, and friendly. Got sent contact referrals for the surgeons, as well as trans-friendly therapists under my insurance.
May 9th: started looking at list of therapists and making respective emails and calls. Checking per session costs and double checking insurance. Most charge 100-150 per session. Got in contact w one.
May 10th: Called w first therapist talking about what I’m looking for, where I am in this process, if parents are supportive, and talking about costs. She was very friendly and affirming, wants to have a few sessions to get to know me and my situation before writing a letter. Understandable and expected, but frustrating.
May 15th: Primary care appointment: Went to northwell health primary care, parkinglot was a little scary (just a large lot with a lot of cars) but everyone working there’s is super kind. Office is incredibly affirming, pride flags and lgbtq+ art everywhere. Gave my insurance card, filled out some paper work, got called in pretty quickly. I have a needle phobia and medical trauma so I was panicking a bit in the office, nurse was good w me about it and doctor was very kind, I just requested to not have any blood work done that day and that was totally fine, so I could schedule that at a later date and go w a friend. Recommended to get blood work done before scheduling a consult w a surgeon. Also prescribed me a single dose anxiety med for the bloodwork which I was very happy about. I found over time that the anxiety meds unfortunately do little to nothing for my panic attacks personally when it comes to needles but regardless having a doctor acknowledge and respect that fear and listen to me was incredibly helpful and reassuring.
May 30th: Got blood work done in a different lab, went w a friend. Scheduling for that is super easy, I think I did it online actually I don’t entirely recall. they do take walk ins but I made an appointment to minimize complications and make sure I could prepare properly. Front desk/lobby area was a little spooky, but I think that is mainly just bc of my social anxiety. They take a urine sample, you give them your prescription, eventually they call you over for blood work. Quick and easy, tech was v nice and having a friend with me was incredibly helpful. Probably the best I’ve ever done with a needle despite the fact that I did still panic and get very lightheaded lol.
Got blood work results back within the next couple days, all looks a-okay! Neat :)
June 15th This day was incredibly difficult. I had my first session with a therapist to establish some ground knowledge around my dysphoria and the way that I view myself. Top surgery is something that I know from research and related experience Can be difficult and expensive to get and can take time, so much of my prep work has been on the understanding of taking things a step at a time and just knowing that the current way things are doesn’t have to be forever. It allowed me to be able to live with myself while prioritizing my health better. This read to the therapist as “not having the level of dysphoria [she’s] come to expect and look for in someone who is trans” and was largely based off the fact that I don’t want to go on hrt. Past that point I started to break down because now my method of learning to live with myself felt like it was actively going to work against me and prevent me from getting top surgery. I’m not good at talking about my dysphoria, I can’t imagine it’s easy for anyone, especially to a stranger I just met. It was rough, and I felt incredibly mentally drained after ending the session.
June 19th Called it quits with the first therapist, I felt incredibly disrespected and the one session we had put me in a mental spiral for days. It can feel some times in this process like the people you have to get permission from need you to be severely depressed and unable to wait another second for this procedure just in order to take you seriously.
After I left that therapist, I immediately got back to the list to find someone new. Spoke to a new therapist via email, but my insurance is kinda weird (Blue Cross Blue Shield out of state) so its off putting to some people. This therapist recommended I go through the office she started out at (Heart and Soul Counseling)
————- Time Skip ——————
IM BACK its time for some record keeping. Got super overwhelmed and lost the energy to document my process for some time so here goes.
HEART AND SOUL COUNSELING: My experience w/ this therapy office was mostly good. The person in charge, Jesse, was absolutely lovely and responsive. Never spoke in person, but any text/email interaction was prompt, respectful, and kind. The office is stellar with email/text communication, so I only ever had to call them once when I was initially inquiring about the office. This is something I wish all therapy/counseling centers did better, eliminated a ton of my anxiety and hesitation to speak to therapists.
I got set up w someone as quickly as possible and established what my goal was (to acquire letter document for my surgery team). I attended multiple session w the therapist, she was a kind lady but the sessions were unfortunately p miserable for me. We didn’t fit well, but I was willing to stick it out rather than backtrack on my process. She also did not invalidate me or accuse me of not being trans which was a major step up from my first therapy experience. Once I acquired my letter I did stop therapy there, I kindly explained to the therapist that it wasn’t a good match, but I may honestly explore my options at the office in the future. Receptionist there was also lovely and they had a cool fish tank.
———- CONSULT STARTS HERE —————
July 19th: CONSULT!!! My mama and I went to Dr. David Whiteheads office for a consult. Parking was a nightmare so I’m super glad I didn’t have to drive for this one (ty mama). Consult went really well, and the staff were all super friendly. Dr. Whitehead is cool, very chill energy and a bit intimidating, but I’m scared of everyone so that’s nothing new. First question he asked me is what I wanted/what he could do for me which caught me more off guard than it should have? I didn’t realize going into this process how many times people ask you what you’re having done even if it’s already written down, because there’s so much variety in what you can look for in the results.
We talked about the procedure, went through a slideshow n stuff, and discussed how I wanted a flat chest w/ no nipple preservation. They made sure to specify that my mental health professional letter had to include that I did not want nipple preservation because thats technically a “non-standard” appearance. Also had the first breast exam I’ve ever had in my life. Can’t say i’m a fan (not that I need to worry about that anymore!) Took pictures n measurements n such, and also discussed recovery supplies and care w me and my mom.
August 9th: After a plethora of painfully awkward therapy sessions, a decent amount of crying, and a couple breakdowns in friends cars/backyards, I got my therapist letter and sent it to the surgeons office. It ended up needing minor revisions to which I contacted Jesse from Heart and Soul and he got me the revised letter immediately. Unfortunately the surgical coordinator was out of office for the rest of the month the next day ;w;. Is how it be.
September 12th: Got a call from Surgical coordinator mid-painting class that I stepped out to take. Started discussing surgical dates!! She was kind enough to email the dates to me which was lovely because I was absolutely shaking/mind blank haha. There was an option for January 8th which felt like an absolute miracle the way it would work with my school schedule. It would give me a solid two weeks recovery time before spring semester began. Because it would be a couple months out, I was asked to contact her in the second week of October to submit documents to insurance.
(Timeline note: earliest date offered was in early December)
October 10th: Documents sent to insurance, predetermination started
October 30th: Received mail from my insurance approving my procedure as medically necessary (YAY) But! This is also where things get,,, fun! Dr Whitehead’s surgical coordinator, Alyssa, is a blessing and was very helpful and prompt with me despite the fact that I had to email her pretty constantly during this general time which I still feel bad about.
Around this time, my mom got diagnosed with breast cancer, which I reported to the surgical coordinator because it influences my family history (grandmother also had breast cancer). It was asked that I get genetic testing done because this could impact my surgical procedure. Now I’m handling the setup on this between helping my mom in her process setting up consults and considering her options because there of course is a lot of crossover to the steps I’ve already completed and am familiar with.
November 1st: Very kind person at cancer genetics calls me, sends me a family history questionnaire to fill out before I can be scheduled to see a genetic counselor. Filled out the questionnaire the same day.
November 8th: Called cancer genetics to check about scheduling, office was not open so left a message. Got a call back later in the day. I have a virtual appointment with a Genetic counselor Tuesday the 14th. Current plan is a mailed saliva genetic test but I’m going to ask if theres anything I can do to get results/materials quicker. If I can’t get results/feedback by December 8th my surgery date may get deferred.
Trying not to stress too much because there is little to nothing I can do about this, and I just don’t want to be sad. I’ve kept telling myself throughout this process to not get excited and not let myself believe anything is solid because something could happen at any time that might mess up my schedule or plan, and If I convince myself I’m in the clear, those changes will hurt a lot more. So far I think thats been a good move, because this really sucks.
My surgery date is still officially scheduled as of now as well as my first post-op. I will also ideally have pre-surgical testing done December 18th should I be cleared by genetics in time (Fingers crossed!)
ALSO! Def lean on friends if/when you can during this process. It can absolutely be challenging, and having a support system is incredibly important and helpful. I’m super lucky to have really lovely and supportive friends that are around to listen to me and send me pictures of stupid little animals.
November 9th: My mama is scheduled for her double mastectomy on December 4th
November 10th: Did some shopping with my mama for recovery supplies for double mastectomy/top surgery. Having watched a million and a half transition/top surgery videos and tiktoks and having read all the blogs and posts and tweets makes you a great support for someone suddenly faced with an upcoming double mastectomy! We might go shopping this weekend for some button ups and zip ups for her, clothes shopping is better done when you can try stuff on
November 14th: Meeting w genetic counselor: Victoria Webb, one of the loveliest medical care workers I’ve ever met. Had a virtual appointment with her to discuss and set up genetic testing. I explained to her about my situation w the proximity of my surgery and tight deadline as well as my willingness to do a blood test instead of a saliva kit to get results quicker. She was so incredibly kind and good with me, ended up being able to do a saliva kit and get results in time she deserves every good thing in life.
December 18th: pre-surgical testing: This was at the main hospital, everyone was really nice but I had a really bad panic attack despite being on Xanax.
The process is sort of like getting a physical. Measurements like weight and blood pressure get taken, lots of preliminary health questions. The people working with me were really kind and I was very open with them about my anxiety, it was visually apparent though anyway because I started crying the second we even started talking about the blood draw.
Once the equipment was actually brought into the room I started to panic. Both of the women working with me were really kind and helpful and tried to distract me and keep me talking the entire time, but I did still have a really horrible panic attack. Every muscle in my body locked up and I lost all my color, took a bit to get back to a spot where I could move and talk properly because my speech was affected too. It was a bit scary but funny to think about in post. Thanked the medical staff for being patient w me as always, a good portion of the anxiety is also guilt about making things harder for them. Got through it tho. Def eat before presurgical if allowed, I didn’t and that probably didn’t help!!
———- SURGERY DAY ————-
January 8th:
Ok so surgery day:
This day was very scary. Got my phone call the Friday prior for my surgery time which ended up being 1pm and I was asked to arrive around 11. Got there at 10 and went in at 10:30.
Called up to check in then in waiting room till someone brought me back to change. I told her right away about my anxiety with the iv bc that’s legit all I could think about. Got changed right after. I was generally shaky and a little disoriented the entire time because I was panicking but everyone was very patient with me. Clothes and belongings go in a bag in a locker and you get two gowns one that faces back and one that faces front. I was given underwear and a pad as well because lucky me I got my period a couple days before my surgery.
The pre-op area is a lot of little cubicles with curtain divider things, blue soft chairs, and medical equipment. Everyone I met and spoke to was very kind, but any time someone even suggested starting my iv I would panic. I was informed it would have to be placed in my hand and that terrified me, I’m especially anxious and sensitive about my hands and fingers. I think doctors and nurses tend to misunderstand exactly where my fear is with needles and ivs. It isn’t the pain that scares me, but the concept of veins and and anything being in them. Even writing this right now is horrible so I’m going to stop w any further detail. I spent the entire two-ish hours of pre-op absolutely terrified about this iv.
I wasn’t really keeping track of time but dr whitehead came in to do markings for surgery. They had cool rainbow socks on,big fan. Having your chest drawn on and just like, moved around n shit is such an experience. Felt bad because I kept losing my balance but doctor Whitehead is cool and I am 98% less scared about them now.
Probably my most favorite person I met during my entire hospital experience was the anesthesiologist. I know he told me what his name was but I couldn’t focus on or retain information at the time. He told me we could essentially put me to sleep with gas before putting the iv in and for the first time in probably a solid week I felt like I could calm down a little. He took a look at my hand and arm to check my veins which always does freak me out a bit but I’m more used to that kind of thing at this point and I know nothing bad is going to happen. One of the nurses came in with the iv equipment and he let her know that were going to wait till in the or which was also incredibly helpful because I absolutely panicked when I saw that little supply kit again.
V nice lady brought me into the or, I’d never been in one before it was cool. They had a little music speaker which was really cool. Took off blue jacket gown and they helped me onto the table. They put a warm blanket over my legs and my chest to help me calm down. Before long they gave me a mask w fun happy sleepy time gas, they let me keep my arms on my chest for a while which was really nice because I was still scared. I started getting loopy pretty fast but I still heard when someone mentioned where the iv equipment was and panicked a little because of that. I remember feeling them take my hand for that but never actually felt anything happen. Just some fear but the gas was v helpful obvi. Someone said they would see me in a little bit, and then I was groggily waking up in recovery.
Recovery was a little rough bc the iv was still there (fully wrapped up so I couldn’t see it though which was rad) but I was still really anxious about it until it was taken out and when it was taken out. For anyone that struggles w this i did not feel them remove it, just the tape. Everything was mentally much easier after that. After a while, going over instructions w parents, a cracker , some ginger ale and some juice, my dad helped me Get dressed and I was helped out to the car in a wheel chair. Ride was smooth bc of remaining numbness and meds except a few Bumps in the road
TOP SURGERY GOTTEN
My post op date was scheduled for Jan 17th and that’s the day I got my drains out followed by several post op check-ins. First week of recovery was miserable but things exponentially approved each day past that, and I went back to school in person two weeks post-op with driving and item-carrying assistance from friends!

Hello. Stupid question but I’m confused. I had my baby in February this year at the hospital through c section so ofc they did some blood tests there after surgery. My question: can I still get my wellness yearly exam that includes blood test free of cost this year at my pcp? Or will they charge me since I had some blood tests done after c section? I would like to get it but want to make sure I don’t get charged as I already have enough medical bills from delivery. My insurance does offer free preventive care usually.

So almost a year ago I (24m) was involved in an accident as a pedestrian. I looked at solicitors to make a claim and chose one which at the time had good reviews. They were very cooperative in the early stages replying a day or two on every enquiry I had, as this was my first time making a claim.
At the very back end of last year, I received an offer from the person’s insurance which was a little lower then expected. I was told not to accept this as I had a medical coming up to assess the damage.
At the start of this year my medical report came back and I was told my claim was worth 3 times the amount of the initial offer. They began negotiating and I was told I would here back in 21 working days, but 2 months went by with nothing.
I tried contacted them to no avail, so I mentioned I may get another firm to take over as it seems I’m being avoided. I got a phone call 2 days later apologising and they said I should hear an offer in 2 weeks as they have all the evidence they need.
Again now another 2 months have past and nothing. I’ve tried contacting them multiple ways but I don’t get a response. I recently looked at the reviews again and have seen they’ve been getting alot more negative reviews. The positive reviews I first saw were when people first made there claim and all the negative are people saying they’ve waited a year or 2 and nothing, with only one person I saw actually get there compensation.
What should I do? This is my first ever claim and I live in the uk. I was told it was an open and shut case with all the evidence cctv etc. Do I get another solicitor to take over? Do I contact the ombudsman?

I need the otolaryngology medical community’s help, as many physicians have been stumped by my situation. Admins, please read before deciding to delete. I am not a medical professional, but I possess a unique situation that cannot seem to be resolved by the medical professionals available to me. I am not sure how to phrase this without breaking the subreddit’s only rule, so I will try to express it in a manner oriented to my obsessive curiosity to understand my (undiagnosed) condition.
 
For starters, I have no credentials to confidently say anything I am about to; some of it may even be jargon or irrelevant, but I will try my best.
 
Eight years ago, I weighed around 285 lbs. I began an exercise routine that caused me to lose about 20 lbs within a short amount of time. During this period, I developed persistent discomfort in my left ear. I noticed the discomfort after visiting my aunt, where there was a 500’ elevation difference. When I came back home, my ear would not pop. Over the course of a week or so, the ear pressure eventually equalized, but everything sounded muffled.
 
This led me to my first ENT visit. The ENT didn’t really give me much time, and paired me with an audiologist who determined my hearing was perfect. After hearing the unfortunate news of no diagnosis, I asked why I have to pop my ears to hear consistently. I was brushed off and told to see a TMJ specialist. The TMJ specialist told me I’m fine.
 
This is where my independent research began. I determined that my “popping” was actually me forcibly equalizing the pressure in my Eustachian tube by repeatedly moving my jaw. I went to an in-network ENT. This ENT took offense to my self-diagnosis of Eustachian Tube Dysfunction (ETD) and told me my issues were allergy-related. I was referred to their allergy department and put on immunotherapy shots. After completing my treatment and reaching maintenance, my issue persisted.
 
Frustrated, I looked online for the highest-rated ENT in my state. This doctor did not accept my insurance, so I decided to pay out of pocket to get the issue resolved. This doctor’s niche was rhinoplasty, and he was the first (and so far last) to give me the time of day. He performed a nasal endoscopy and a CT scan of my sinuses. Both tests revealed nothing of use. He noted I have a slightly deviated septum, but not nearly enough to cause any of my symptoms. However, he did note the sinuses on the side where I’m experiencing discomfort were inflamed. His diagnosis was chronic sinusitis. I was prescribed Azelastine 0.1% (137 mcg) nasal spray (an antihistamine). I used this medication for about a year in conjunction with Flonase, but nothing changed.
 
Once again, I sought out a different ENT. This ENT also took offense to my description of symptoms because I used medical phrasing (ETD). After a brief consultation, I was told once again that my symptoms were allergy-related. I went back on allergy shots, reached maintenance, and once again the issue persisted.
 
At this point, I gave up and didn’t bother refilling my prescriptions or finishing my shot treatment. Over the course of a year or two, I developed a severe sinus infection that went several months untreated. It came to the point where I was unable to breathe out of my nose at all, affecting me to the point my family members became concerned, as now there were comorbidities (such as sleep apnea). I eventually saw a pulmonologist who made it clear I needed to see an ENT and get my sinus issues fixed.
 
Thinking that maybe if I stuck with the same ENT and followed the trial and error process, I went back to the last ENT I saw. I was diagnosed with a sinus infection and prescribed medication. For the first time in months, I was able to breathe again after excreting from my nose what looked like petrified mucus. With this small victory, I returned to the same ENT and expressed my frustrations. I passionately described the location of my discomfort and explained that I believe the Eustachian tube problems I’m having are not the issue but a symptom of something else. I explained that I feel a mass or some sort of inflammation in my throat and mentioned the inflammation found by the previous ENT who performed a nasal endoscopy.
 
That was a mistake. After expressing my frustrations, I was diagnosed with "globus hystericus." Initially excited to finally have an explanation and a treatment plan, I was disheartened when I was told he could no longer assist me and suggested involving different medical professionals. When I got home and googled my new diagnosis, I realized he was referring me to a psychologist. Needless to say, I didn’t return.
 
Unable to accept my new reality, I resorted to more independent research. Before I even say this, I want to strongly express how much this improved my symptoms. Mewing, changing my tongue posture, and stopping mouth breathing drastically improved my Eustachian tube pressure over the course of a year. Using a jaw exercisegum daily also significantly reduced my ear pressure issues. Although the discomfort was reduced, it was not fully resolved. Sometimes I’d use a massage gun on my jaw, chin, bridge of my nose, head, and neck, which helped with mucus and provided temporary relief, especially lower in my neck near my chests, this seems to what felt like drain mucus/fluid. I am 120% able to the breath better after, but the “mass” still felt present.
 
After a while, my circumstances relocated me to a different state, giving me a new opportunity to finally resolve this. I saw a new ENT, and I gave the full rundown of my symptoms and history. I mentioned every medical ailment, bad habit, and quirk I have ever experienced. I mentioned my frequent regurgitation from overeating, how sometimes ground beef or rice seems to get stuck somewhere in the back of my throat, how I sleep on the side where I’m having discomfort, and how I pick my nose in the morning. I also mentioned that I can feel when my ears are producing wax and can remove hot wax with a Q-tip in real-time. I mentioned having tubes as a kid (as I mentioned to every ENT). This ENT diagnosed my issue as being caused by the acid in my regurgitation irritating my sinuses. I was prescribed Omeprazole 40 mg. I took this for a while but realized it was a bandaid on a bigger issue. I needed to prioritize my weight and eating habits.
 
I eventually started measuring my portions and eating slowechewing my food better, which fully resolved the regurgitation issue. However, I still occasionally get food stuck somewhere in my throat and definitely stuck in my soft palate. I have had my tonsils and adenoids removed when I was a toddler, but I can’t help but wonder if something grew back and is trapping food (I assume this would show in a CT scan).
 
As of recently, my latest symptom is hairs coming from the back of my mouth. Initially, I thought they were beard hairs, but after the fifth or sixth time, I became fully certain that single strands of hair are coming from the side of my mouth where I’m experiencing discomfort. I can replicate this over a long period of repeatedly opening my jaw (like I have Tourette’s) in an effort to equalize the pressure in my ear. I did some research, and it sounds like it could be a million different things. So once again, I have an ENT appointment coming up.
 
Now we’re caught up. I started to about a year ago still use the nasal spray and Flonase daily, and I even have a tablespoon of raw local honey. I have ZERO allergies. My ear pressure has progressively gotten worse over the past month, with the frequency of hairs increasing due to my constant need to open and close my mouth for pressure relief. I am not sure what to even tell this new ENT. I need experts to help me navigate the landscape of this condition.

2024.05.14 10:03 Devil-Wears-Nada- Pain in arms when coughing

Hello,
This sounds stupid but I’m going to ask it anyway. 25F for reference. Sometimes when I cough especially hard, it is like a radiating pain from my chest to my forearms. It’s difficult to describe but it’s like I can feel it in my bones, like I’ve literally rattled my skeleton in my arms and hands. It’s quite painful, and it lasts for about 5 minutes at a time. Every once in a while I will actually lose sensation in my hands and wrists, and lose grip strength for those 5 minutes. My hands shake and it is just a deep, sharp pain that slowly ebbs.
For context I do have high blood pressure, but that’s the only condition that I think would be applicable in this case.
I don’t have health insurance, so can’t go to the doc and I’m honestly thinking (and hoping) this is just a weird body quirk and not a medical problem but figured I’d ask. Hopefully someone says my skeleton is just telling me to stop coughing and that’s that. Lol.

Hi! I need (ok, I'm desperate for) a job (even a temporary job is fine) where I can make at least 3000 USD in the next few weeks. I recently quit teaching, but if there's a teaching job that pays now, I'll do it for sure. I was a math and chemistry teacheprofessor (mostly math), and also taught a STEM success class that largely focused on research skills and writing. I also have solid skills in data analysis, item analysis, data annotation and labeling, editing, curriculum design, assessment design, LMS admin ...really anything related to teaching, learning, and data (but I'm not a great coder). My dream job would combine data analysis and education.
I know it's a HUGE ask and likely won't go anywhere. I have a large medical expense (no insurance, but it's necessary) coming up and they want full payment first. Even though I have decent credit (TransUnion, anyway), my debt to income ratio is super high because of all my student loans. So I can't get a personal loan.

I’ve been in therapy for almost my whole life and have gone through probably 7 of them (usually because they move and my insurance no longer covers it). On the first session it’s mostly the legal details they have to write down, I don’t know the specifics. It’s questions like, “what are some traumas?”, “what are your family dynamics?”, “are you on medication?”, “have you been to a psychiatric hospital”, etc. I HATE the first day. When I come to a new therapist it’s usually because I’m in deep shit and really just need to talk to someone, not to rehash old trauma. Is there a list of things therapists need to have written down/documented? Because I’d really just prefer to create a list, skip the legal part and deal with my current situation, and leave early so I’m not making anyone work overtime. I have no idea if that’s even possible but it would be SO nice.
Tl;dr: What things do therapists have to have documented after the first visit? Is it possible to make a list with the requirements and move on to other topics?

Long story short I can't afford to go to the Dr nor a podiatrist. I have a bad ingrown toenail to the point I can't walk properly, have to do a weird side foot angle walk.
So I'm needing to get some medical supplies, numbing injection? Scalpel, some stitches & anything else I might need to numb toe and cut it out myself. Where does one get these things? I've been Epsom salt bathing it for 2 weeks & lifting it up cutting it out where I can which has made it slightly better. But it's so painful even brushing the blanket across it at night has me seeing red in pain. Its gotta go, where do you get this kinda stuff?
No I don't qualify for a CSC No cant take out a loan No don't have private health insurance

I was put on some medication for nerve pain however I had a pre existing condition of absence seizures ( I made it clear to all doctors) that I had under control. But recently I found out I am having my seizures more often due to the medication and that the damage is permanent. Now I have to go on seizures medications now too. So I am trying to figure out if I have to pay out of pocket or does workers comp insurance have to pay for it?

I (23 FTM) have multiple chronic illnesses, both physical and mental, that prevent me from really doing anything. When I’m not having panic attacks, I’m having horrible muscle spasms or my back is in so much pain that my legs give out. I either forget to take care of myself, or when I remember, I don’t have the physical and/or mental capacity to complete the most basic tasks. I rely on my fiancé to assist me with everything as my family, whom we are living with, doesn’t think I need the support. On top of helping me bathe, getting me something to eat, or reminding me to use the bathroom, my fiancé works full time. The entire time he is at work I am stuck in the same spot most of the day until he gets home. As a result he is constantly exhausted with no time to himself, and has a very minimal social life.
I’m too disabled to work, but not disabled enough for assistance. I’ve been denied three times. I have no insurance currently as we can’t afford it. I’m supposed to be going to free clinics in my town for both medical and mental health, but the medical clinic closed down, which stopped me from getting my meds. No more meds, I can’t leave the house, as I was already having a hard time going out while taking them. My therapist kept canceling and missing our zoom appointments and I had to find out two days before my last scheduled appointment that I had to have an earnings statement from the DoL to continue seeing her. I can only get this done in person.
It’s been two months, I’m unsure how I’m supposed to get anything done when my fiancé is only off on one weekday, I have massive panic attacks just stepping on the porch, and I can’t walk for more than like two minutes. I need more support than he can give and neither of us know what to do. Does anyone else know what we should do?

2024.05.14 08:17 Chance-Question-6630 4 year old Dog Sudden Anemia-Multiple Transfusions

My 4 year old sheepadoodle is currently in ICU and just received her 2nd transfusion. 3 weeks ago she cut her paw pad, and it got infected. The vet put her on antibiotics and oral anti itch (wasn’t familiar with/didn’t understand the need).
After about 2 weeks she finished that cycle and seemed back to normal. This weekend however she woke up on Friday and was not herself. Saturday morning she fell on our morning walk and we rushed her to the ER. Her red blood cell count was 10. She has received two transfusions and her RBC is only up to 20.
The emergency vets are more concerned with stabilizing her. I am concerned with understanding the underlying condition. They do not think she was exposed to any toxins or got into anything largely due to a lack of vomiting (we recently moved into a new home, she doesn’t get into things but maybe something was left out from prior owner). Also do not believe it is cancer because of her young age. They assure me their immediate care would be the same (steroids and blood transfusions). Seems most likely she has an immune issue which was exasperated by a new antibiotic and anti itch?
I am getting less hopeful, she has been refusing food which likely isn’t uncommon given the medications she’s on. Any guidance on how I can a) better work with her team or b) am I unreasonable trying to find root cause in tandem with immediate stabilization. It’s been a tough week. Her pet insurance is capped and finances are not an issue..but want to ensure we are managing this effectively and efficiently.

I have tried working other jobs in finance and insurance. But I am utterly miserable and kinda suck at it to be really frank. The only way I kept my job was by kissing the right butt. I had a job in finance and got fired. I now work in insurance and while my team lead thinks I'm a dullard who moves too slow, my supervisor and the head Honcho at this office loves me.
Anyway, I hated working in human services, especially case management, when I got paid peanuts just to do the stressful job itself. I loved my clients and gave my heart and soul to my work. I watched children with disabilities learn to walk, talk, communicate, and stay in school. One parent even reached out to me to say her kid got into college; he was in middle school when I first worked with him and I feel old now. I also worked with people who made a bad decision with addiction. And the thing is, they weren't bad people. I learned that they were depressed, lonely, and scared people who didn't know what options they had when it came to managing pain, mental illness, or suicidality. I watched them beat addiction and become good friends, family members, and spouses. I once saw this young woman being welcomed into her parents home once again, keys to the house placed in her ha ds, when she completed one year of sobriety from heroin. And that meant a lot considering how she was so consumed by addiction that she stole from her family to get drugs. To see someone be taken back like that pulled my heart strings.
People told me I was empathetic, strong-willed, and dedicated. But I got tired of it at some point. I was sick of being blamed for something that was simply out of my control. I'm there to help people not force them to make decisions. I got tired of having no work-life balance and sick of bringing my stress from work, home. I also got so sick of being obsessed with my work (that was my fault though), and taking it to heart when someone still ended up dropping out of school, went back to jail, relapsed on drugs, and so on. I also got tired of not being a le to meet my needs. I have medical bills and health issues of my own to tend to, and will need to pay more if I had a family. It's pretty upsetting knowing your friends who have the same level of education as you make significantly more than you, all because a younger version of you felt money didn't matter.
I tried helping people by simply being a good friend, family, or neighbor. Many of my clients needed that and wouldn't have been in their horrible situations if they had that. Yet it wasn't enough because I'm not good at anything else.
When I say I'm good at helping people, I mean that I fit right in and actually felt like I was a part of something. My clients were the ones who were good at solving their problems, I was just there to help navigate.

Ewan ba, pero feels like being in 30s is seriously depressing. Lalo pa if hindi ka pa financially free, and in general ang gulo ng buhay mo.
Sa age na ito, hindi na rin ganoon ka effective ang gasgas na mga advice na, "bata ka pa naman," "you have your life ahead of you", kung sa age na ito e wala ka pang ipon, wala pang asawa, wala pang naipundar at kapag di ka nagtrabaho, kakalam ang sikmura mo.
Never thought na ganito pala ka-cyclic, ka meaningless at ka-nakakapagod ang maging adult. Minsan gusto ko na lang umiyak tapos hindi ko alam ang dahilan kasi ang daming problema and di ko alam kung alin sa kanila ang iniiyakan ko.
I know na di ako naging wise sa pera. Until now, after 10 years, ako pa rin ang breadwinner. And I support my family by doing a job that I don't like kasi hindi rin naman ako naka graduate. Mag build ng meaningful connections sa mga tao at di yung papauto sa kung sino yung may itsura at maboka, yun ang pipiliin. Aalagaan mabuti ang health ng parents para hindi na i dialysis ang tatay ko, at hindi na maging diabetic ang nanay ko na marami na ring maintenance.
At syempre, gusto ko i-enjoy ulit ang maging bata. Yung malaya pa mag isip. Di pa nilalamon ng sistema. Alam pa kung sino siya at hindi parang nakadikit na sa trabaho at responsibilities ang pagkatao niya. Yung hindi magu guilty pag naglaro ka ng online game o nanood ng anime maghapon, kasi bata ka pa naman. At, kung katulad lang ng panahon ngayon dati, pwede ka kumita sa paglalaro.

In a RAG application with any vector databases connected. How can I deal with ambiguity in the user query? What kind of tool/ prompt can I define so that my agent asks the user for further questions when a query is not very clear or not enough information is given to give a solid correct answer. I have a 4 tools with a ReAct agent ( create_ReAct_agent ), one for using the vector databases as a retriever, one for handling irrelevant queries, one for handling generic user greetings and one for handling ambiguity. The other tools work well but the tool for ambiguity looks like it's never used as the agent always retrieves docs even if the context is relevant yet ambiguous.
One good product that can handle this is Perplexity which prompts the user for further clarification when an ambiguous question is asked.
I want to handle ambiguous nature related to my documents in the vector store without the LLM assuming anything on its own. As an example, if I am creating a medical chatbot that can help people know about different health insurances, doctors in their areas and which insurances those hospitals/doctors accept and user asks "Which doctor should I visit?". The agent should ideally be asking the user what problems they're facing or any other relevant information to give a proper answer, rather than just saying here are 10 most important kinds of doctors you have to visit.....
It should ask about patient's age, medical issues, medical history, the more clearer it can be on what the user really wants the better answer or can generate rather than giving some generic response based on the documents in the vector store.

While polling has consistently showed the election as a close race, almost all polls had the incumbent President, Margaret Smith, winning by 1 or 2 points up until the election. Revenue Act of 1965
The Revenue Act of 1965 would take a more progressive approach to taxation, increasing income taxes up to 7% in the highest tax bracket; all while lowering income taxes down by 4% for lower income households. The Act would also increase the Social Security Tax to 8%.

Death of former President, Theodore F. Green: May 19, 1966

This morning, former President Theodore Francis Green passed away in his Rhode Island home at the age of 98, marking the oldest President at the time of his death. Green was a member of the Democratic Party and briefly the Anti-Fascist Alliance, taking charge from his previous position as Secretary of State after the sudden assassinations of sitting President Earl Browder and Vice President Upton Sinclair. President Green helped uncover the “Business Plot” orchestrated in part by J.P. Morgan Jr. and Prescott Sheldon Bush Sr, the latter being the father of sitting Texas Congressman George Bush.
President Theodore F. Green led us through the horrors of the second world war after the sudden attack on Pearl Harbor, resigning his post and organizing a special election the year following the conclusion of the war itself. He was instrumental in the foundation of the United Nations and eventual foundation of both NATO and EATO two Presidents later. He was, and still remains a national hero in our hearts. President Henry Jackson, among former Presidents and dignitaries are expected to show up for his public funeral in Providence, Rhode Island. The public has been allowed to pay their respects at his grave site before his proper burial et to take place from May 19 at 9:00 AM to May 20 at 9:00 AM.

Supported already by the majority of the country, Scoop Jackson directed Congress to pass a full embargo of all raw and manufactured Cuban goods on entering the United States through any port or checkpoint.

With the law being signed by the President in August that year, he would make a speech in Miami celebrating the passage of the act, glorifying its protections of American, anti-communist goods. Scoop would face some backlash over his anti-communist posturing, as the Labor Party has a small (but noticeable) sect of Communists in their ranks.
The Saigon Summit
In July of 1965, after riots against the French government in Saigon, and the breakout of a guerrilla war in French Cambodia, a summit was called in Saigon to determine the future of the city. President Jackson, President Ho Chi Minh, and President Charles de Gaulle met within the French administrative building to discuss the recent riots in the city and future between Saigon and Vietnam. Although much of Vietnam was granted total independence from French rule in 1950, French Saigon remained a thorn in Vietnam's side. France wished to keep as much of its dying empire as possible, and no one would fight harder at that than Charles de Gaulle himself. President Jackson wished to keep the peace and eventually coerce Vietnam into rejoining EATO.
Talks were messy at times, as yelling could be heard from the chambers the talks were being held in, but the three would come to an agreement. Saigon would be administered by a joint Vietnamese-French government, and policing and law would gradually transition to local and Vietnamese systems. In return, Vietnam would promise to not get itself involved in the Cambodian guerilla war.

Invasion of Saigon

In December 1966, a clash between Vietnamese and French police during a riot led the Vietnamese side of the Saigon Transitional Government to call on Vietnamese military aid. Within hours, the Republic of Vietnam marched into the jointly occupied city. Rumors immediately began amassing that the Saigon police force worked with the Vietnamese government in order to cease Saigon before the transitional period was up. Although these rumors were just that, President Jackson was surely worried when the news hit him the next morning; alongside the French Ambassador asking for an audience with the President.
French Ambassador Hervé Alphand would share with Scoop three things:
  1. France intends to treat the invasion of Saigon as an act of war.
  2. France is already mobilizing troops to southern Cambodia for a naval invasion of Vietnam.
  3. France intends to call on the force of NATO and EATO to defend “France in her hour of need.”
No matter how Jackson tried to argue, Alphand was keen on these points. Jackson would argue that the incident be investigated by the United Nations to determine whether it was an act of war; while Alphand threatened that American delay on the issue could lead to French withdrawal from both NATO and EATO. Jackson, reportedly furious, refused to be threatened by a “dying empire”. He denied meeting with any French foreign dignitary for the time being until they promised to allow the UN for an investigation.
The French response was quick, with France officially leaving both NATO and EATO on December 18, 1966. The French declaration of war and further campaign into Vietnam began on the 20th. With naval and air landings concentrated around Rach Gia, Can Tho, My Tho, Saigon, and Vung Tau, the Second Indochina War began. Although Australia would provide weapon assistance, the other nations within both NATO and EATO held their breath on what to do. France had left the two most powerful military and economic alliances in the world, and President Jackson could not be more angry.

Glasgow Conference of 1967

With the war having gone on for nearly three months, and French military forces having begun to get bogged down by the Vietnamese harsh tactics; Can Tho remained the only major French-held territory in the young Republic. And although Vietnamese war tactics were questionable at best; much of the world was united in believing the French declaration of war was not entirely justifiable; with President Scoop Jackson and General Secretary Leonid Brezhnev at the forefront of organizing peace efforts within and without the UN. Although the United Nations have begun investigations into both the Vietnamese invasion of Saigon and the French declaration of war, they both had gotten bogged down by the surrounding war effort.
It was agreed upon by several major powers to meet in Glasgow with French and Vietnamese delegates to discuss an armistice. The United States, United Kingdom, Soviet Union, and the People’s Republic of China agreed to enforce the following terms:
  1. Saigon and surrounding territories that formerly made up the French Vietnam Territory following the 1950 Treaty of Manila shall be ceded to the Republic of Vietnam. Saigon and the surrounding territories shall become a United Nations sponsored demilitarized zone until an official peace treaty between the 5th Republic of France and the Socialist Republic of Vietnam.
  2. French military and bureaucratic personnel shall be allowed free and safe passage out of the cities of Can Tho and Saigon; sanctioned by the United Nations Peacekeeping Force. The French and Vietnamese governments must release all prisoners of war; sanctioned by the United Nations Peacekeeping Force.
  3. Saigon officials implicated in the initial invasion of the city on December 16, 1966 must release all official, personal, and private documents to the United Nations Office of Legal Affairs for investigation.
  4. Vietnam must retain its promise from the 1964 Saigon Summit to not aid or abet Cambodian guerilla forces or rebels.
  5. All combat between the 5th Republic of France and the Socialist Republic of Vietnam shall cease and abide by the above rules, the United Nations, and Geneva Conventions.
Although both nations had much to say and change in their favor, the above is the final version of the armistice agreed upon by all parties. The armistice paper was signed by:
  • President Henry Martin Jackson of the United States
  • General Secretary Leonid Ilyich Brezhnev of the Soviet Union
  • Prime Minister James Harold Wilson, Baron Wilson of Rievaulx of the United Kingdom
  • Chairman Mao Zedong of the People's Republic of China

  • Foreign Minister Nguyên Duy Trinh of the Socialist Republic of Vietnam

Although the Glasgow Conference was seen as a great triumph of diplomacy between the major powers, Taiwan (the Republic of China) was greatly hindered in its geopolitical influence for the time being. President Jackson had recognized the People’s Republic of China the week prior to the Conference; Communist China would replace Taiwan's spot as a permanent member of the UN Security Council within the month.

The Better Society Plan

Plans drawn up between Pro Tempore Lyndon Johnson, Representative Claude Pepper, and Speaker Adam Powell Jr. would be taken to the President's desk following the first relatively calm year in the administration's history. Although much of the work on marketing the plan would be placed on Scoop himself; Johnson, Pepper, and Powell would act as the main sponsors of each piece in Congress. What would become the beginnings of the “Better Society Plan” would officially pass both houses of Congress throughout mid 1968.
Cheap Food and Housing Act of 1968
A large bill authored primarily by Speaker Adam Powell Jr. and Secretary Hubert Humphrey; the Cheap Food and Housing Act would cover extensive social programs. Although, with weak support in Congress, many Republicans were able to push to soften these programs and add their own agendas on top of them. The final contents of this massive bill were as follows:
  1. A federal Food Stamps program would begin and be administered and funded by the Department of Health and Welfare. Certain imported foodstuffs would receive a 15% higher tariff. All American citizens that either fall below or are less than 6% above the poverty line would be eligible for the Food Stamps program.
  2. Store-bought meat products will receive price controls to fit the monthly income of the average family. The Federal Government will cut 60% funds toward GMO Agriculture, Meat, Fish, and Poultry research.
  3. Houses that take up less than a certain area size will be price capped based county-by-county income. This job is in the hands of State Governments. (Apartments are not covered in this)
  4. Housing discrimination shall be made illegal based on identity.

Medical Bill Reduction Act of 1968
This bill was authored by Representative Claude Pepper and Secretary John Gardner in order to fundamentally reduce medical expenses for the youth, elderly, and medically unable. The bill however was weakened significantly by the Republicans in Congress, only allowing for those receiving Social Security benefits to have reduced medical expenses paid for partially by the Department of Health and Welfare; no matter if the recipient is signed on with private insurance or the Public Option.

Apollo 8: Americans on the Moon in November 1968!

Thanks to streamlined efforts by Presidents Dwight Eisenhower and Margaret Smith the past 11 years, NASA and furthermore America were able to place the first men on the moon on November 12, 1967. In a speech made on national television that night in the hour following the conclusion of the live coverage of the moon landing, Scoop Jackson would put much of his thanks on the “Greatest mind our nation has ever had,” referring to Dr. J. Robert Oppenheimer. Oppenheimer, since 1961, has been placed in a secondary charge of the Apollo missions and a potential moon landing until his resignation in January 1967 and death the following month. Dr. Oppenheimer's expertise in theory and former President Smith's dedication for space exploration are likely candidates as to the victory America achieved that night.

The United Nations has concluded their investigation into the potential legality and coercion in the events leading up to the invasion of Saigon.
“While France has made compelling arguments for the contrary, regarding available documents and other pieces of evidence, the Vietnamese military occupation of Saigon was not a result of coercion, manipulation, embezzlement, bribery, or corruption within the Republic of Vietnam. The invitation of Vietnamese armed forces into the territory limits was done by the legal Vietnamese co-government of said territory, and therefore, is deemed a semi-legal occupation of the city. The United Nations upholds the results of the Glasgow Conference.”

Richard Nixon is back at it with his 4th attempt at a Presidential run, and if he wins the nomination or is selected as a running mate, 3rd attempt on a Presidential ticket. He is generally a moderate, but is definitely the wildcard. Despite his past of losing elections, he is somehow the safest, and perhaps most dangerous, to the Jackson administration.

The Governor of California has perhaps one of the most charismatic voices in the nation, and is definitely a threat should he receive the Republican nomination. While he is charismatic, he is also the most Conservative of the major players for the Republican nomination. Reagan has instituted a mix of conservative and liberal policy as Governor of California, but has spouted rhetoric like all the former dixiecrats; just without blatant racism. Scoop believes Reagan is not only a credible threat to his Presidency, but also a threat to minority groups nationwide.

Despite neither Kennedy having decided to throw their hat in the ring this year, 1960 and 1964 Presidential candidate John Kennedy has received some support among anti-nixon moderates for the head of the ticket later this year. He has an air of charisma around him, much like his fellow Republican Ronald Reagan, but Kennedy has only commented on the matter stating he is “far too tired” for 3 Presidential runs in a row. The Senator's health is seemingly beginning to fail, as well. Despite the unlikeliness of the matter, Jackson is prepared to deal with Jack Kennedy again if he wins a draft.
Opposed to his older brother, Governor Robert Kennedy has remained Non-Partisan since the fall of the Liberal Party 3 years prior. Bobby has had moderate support from both parties since the beginning of his governorship in 1963. Despite this, and probably with wishes to go against one of his brothers, Bobby Kennedy has denied to run or entertain a draft movement in his name. Scoop has declared Bobby to be of little threat.

The long-serving Senator and son of former President Huey Long has walked the line of conservative, liberal, and progressive support throughout his career. In recent years, he has become more supportive of progressive social policies, and definitely leans economically toward Labor; but his reach across rural southerners matches a more populist approach. Long has already declared his independent candidacy for President. If Nixon isn't one, Long is certainly the most dangerous wildcard if he plays his hand right. Scoop will closely watch him.
