2024.05.14 18:39 TheRabbiit Lucky mate for me

I haven’t been playing much lately but counting my lucky stars to have gotten mate in this one (I was black). I have rarely gotten such a see saw game. Pgn pasted below. My thoughts in the game:

Opening : I thought he was being a bit silly moving c and d pawns and then the a pawn like he did.
Move 11: He goes Qb3 and I can’t find a counter. Can’t move the knight which is now attacked twice or he gets my f pawn and rook and looks totally bad. If I move Be6 to defend the knight, the bishop blocks my rook from defending the e pawn. (This was the correct move and in game I didn’t see the line that puts me up material if he takes the seemingly free e pawn). I went Bxd3. Frankly for no good reason why other than to get a pawn for what I thought was a lost knight, and maybe clear the f file for my queen in future. In hindsight would have been better just to lose the e pawn even if I didn’t see the engine follow up.
Move 14: Checked with the knight and the subsequent takes were as expected. I was thinking to checkmate eventually on Rg6. Didn’t see the counter of Ra4, which he could then bring to the g file to block the check, protected by a pawn. Neither did my opponent though.
Move 18: didn’t take the bishop with the queen because I thought that would let his king escape to e2 then d1, then his knight or bishop could block and there would be no chance of mate. Engine showed taking with queen was correct though as taking with rook meant he could bring his queen to e3 and force a queen trade. I didn’t see that. I only saw Re3 which I dismissed because that would be M1 for me with Qxf2, his rook blocking his queens’s protection of the f2 pawn. My opponent didn’t see the queen move though and after Rxh2 mate was unstoppable.

Well that’s it I guess. Feel free to let me know what I could have done better

Hey, I'm looking for some answers to chronic tireness, itching, face rash, brain fog...etc...and I know these symptoms are common in lots of illnesses. I had some recent bloods done and I can see that I'm borderline low on several things, which has been consistent for a few years but GP's have never picked it up.
Heamatocrit is always just below reference values at 0.35, red bloods cells just above reference 3.8, all white blood cells always low but just above reference range. Platelets are mid range, iron is mid range.
Is anyone able to interpret these for me and give me some insight? I'd like to go to my GP to ask for some further investigations as I've been dealing with these symptoms for years but never get any answers.
When I was looking at doctor Google aplastic anemia and other non-iron deficient anemia came up as a possibility. As I have an intermittent spotty rash below my eyes aplastic is something I've been looking into.

2024.05.14 18:22 devicecontrol001 blood test results show low wbc, low neutrophils, and others?

I'm 28F, only taking antidepressants and iron supplements for anemia.
So I received my blood test results and have my pre-scheduled follow up appointment tomorrow but I wanted to see if I could get some insight here?
white blood cell count is low (3.3), as is neutrophils (1228), hemoglobin, hematocrit, mch, and mchc. There's also trace leukocyte esterase in urine.
I am a bit freaked out because my cousin was recently diagnosed with colon cancer and is going through chemotherapy now, so of course my mind is going 0-100 in what the results could be indicating. Iron levels were normal, which was a surprise cuz I've been easily fatigued that I thought would be related to the anemia.
Any insight would be greatly appreciated!

Hey friends,
I had a good first week, dropped 9 pounds, altho I put 3 of em back on after cheat day and haven't been able to significantly move it down since, just yet.
I've been absolutely blown away by just how much I'm eating, and not out of necessity, but out of a sheer hunger.
First, the stats:

37YO Male, able bodied, active, healthy or, at worst, on the high end of normal of all blood metrics
6'3, 213lbs at start, 204 before cheat day on Saturday, Tuesday of week two today, and at 206.8.
Body fat of 15% at start, 14.3% pre-cheat, and 14.6% today.
And maybe the most important variable: Vegetarian (lacto ovo)
Taking Potassium, Calcium as defined in the book, and Magnesium at night for bed. Skipping the PAGG based on research on this sub and other sources.

*Standard breakfast * I'm having two eggs with about a half cup of whites with spinach and an assortment of frozen veggies into a scramble, probably a cup of spinach (compressed) and a cup of other frozen veggies , a full can of lentils, 560g)
Invariably hungry three hrs later, and I'll either do a couple spoons of cottage cheese, or suffer thru until a bigger lunch.
Lunch is roasted veggies, marinated in Olive Oil and some harissa spices, sometimes with salsa, and sometimes with tahini (but only if I haven't had the cottage cheese)
Evenings will be a burrito bowl of sorts, lettuce, onions, a shit ton of veggies, totalling about four to five cups worth of veggies, and a combo of black beans and refries beans, with a bunch of salsa.
And even then, I'd get into night time feeling hungry, so I'd finish off with a tbsp of peanut butter and my magnesium tablet before bed.
It's sort of crazy to me how much I am eating.... I have never eaten this much volume so regularly before. I've seen other vegetarians on the sub say they've been fine with some tofu here and there, so I do some extra firm tofu to amp up protein, but usually not more than twice a week.
And I noticed if I drink a pea protein shake with some veggie greens and spinach at breakfast time, I dramatically reduce my hunger thru the day. I know that's breaking a bit of a cardinal rule, but I remembered the book saying that snacking means I wasn't having enough protein, and I figured the best time to do that was breakfast to really make sure the Ratios of breakfast tipped heavily in the favour of protein vs carb/fat.
Roughly speaking, I'm throwing about 150g of protein into my body everyday (including the shake).
Activity wise: I'm usually moving at least 60 mins a day between commuting to meetings by foot and bike, as well as morning walks. I'm also doing the three day a week fifteen minute workouts Tim mentions he did in the book. (kettlebells M and F, dumbbells and sit-ups on W)
All told, by the end of the week, my Garmin watch calculates me as having done about 600 to 700 intensity mins a week, usually in the neighbourhood of 600-900 active calories burned.
I'm asking for the judgement/review because despite feeling like I'm following it as I'm supposed to, I'm sometimes struggling to imagine how this much food is okay to consume Hahaha... And also because the slight uptick on the scale is annoying, post-cheat day.
Also: I'm super tired by 830 pm. Like I'm usually very energetic, but by 830/9 I'm just counting down the hours til bedtime, usually in bed by 1115,and asleep by 1130pm. (I will say that as someone who used to take a long time to fall asleep, I am very nicely fatigued since starting SCD and fall asleep quickly.)
Would love your feedback to know if this is "business as usual", and if anyone has comments on what feels like a setback in the increased weight and fat%, the fatigue, and any other thoughts of advice.

2.3.5 Controversial views and perceptions of the clinical picture of MCAS
2.3.5.1 Alternative diagnostic criteria ("Consensus-2")
In 2011, the group of authors led by Gerald Molderings from the Institute for Human Genetics at the University Hospital Bonn and Lawrence Afrin published their own approach to the diagnosis of MCAS (51). In contrast to the diagnostic criteria of Valent et al. (1), the focus here is more on clinical symptoms. In 2016, Afrin et al. published a list of the most common symptoms that could indicate MCAS (52). Furthermore a questionnaire was developed and published on a website (53), after the answers to which the suspected diagnosis of MCAS could be supported or ruled out depending on the scores obtained. In a recently published publication, the working group referred to their diagnostic criteria as "Consensus-2" and compared and discussed them with the criteria of Valent et al. from 2016 (so-called "Consensus-1") (51). An important difference to the criteria Valent et al. 2016, the authors consider the symptoms not only as the main main criterion, but also a much wider range of previously unexplained symptoms (111 unexplained symptoms (111 possible symptoms (44)) as the most important indication of a a mast cell-mediated cause (14 symptoms in Valent et al. (1)). As The diagnosis of MCAS is considered confirmed if the main criterion is present together with a secondary criterion and possible alternative diagnoses have been excluded. The secondary criteria, in turn, are based on observations made at the time of the 500 people with suspected MCAS at the time of the first publication (44). Further differences between the two consensuses can be found in the laboratory parameters to be determined. For example, the researchers from different disciplines, which according to their own statements can draw on a wealth of experience of of over 10,000 MCAS patients (diagnosed according to their own criteria, nota bene), consider CgA to be specific for mast cells in addition to tryptase, among other things (44). The counterargument of the lower specificity compared to serum tryptase is granted a certain validity in the addendum to the "Consensus-2" published in 2020 (44), however the differential diagnoses with elevated CgA values should be easy to rule out and other markers are also never 100% specific. However, another group was already able to show in 2017 that CgA should not be used as a marker for mast cell disease(49). Furthermore the group of authors of the "Consensus-2" counts heparin as an important marker for MCAS, which should be determined after venous congestion using a blood pressure cuff (54). This maneuver was reported to cause irritation of excessively activatable mast cells with release of heparin in the congested area. Interestingly, the following section mentions markers such as IL-6 or tumor necrosis factor (TNF) which, due to their lack of specificity, are not used in diagnostics, but only in the evaluation of a successful therapy. The authors of "Consensus-2" criticize "Consensus-1" for, among other things the lack of definitions for a treatment response, whereby the "Consensus-2 does not provide any concrete proposals for evaluating or monitoring the response to therapy. Another point of criticism is the lack of exclusion of other comorbidities or differential diagnoses, such as CFS, EDS and irritable bowel syndrome, as clinical indications of MCAS. [...] In return, the AAAAI expressly points out that there is no evidence to date of a connection between CFS or EDS and MCAS. Overall, the clinical picture of MCAS is so complex and heterogeneous that a precise definition of a diagnostic algorithm is not possible at the present time. Molderings et al. therefore propose the acceptance of both the "Consensus1" according to Valent et al. and their "Consensus-2" until more precise findings are available through research. The resulting disadvantages, such as the the poorer comparability of patient populations in scientific studies would weigh less heavily than those resulting from the rejection of "Consensus-2" (an underdiagnosis due to criteria that are too restrictive according to the authors). On the other hand, the large number of non-specific complaints that are supposedly associated with MCAS harbors the risk of inflationary diagnosis.
2.3.5.2 Presentation in the lay press
An expansion of the MCAS definition with the use of non-validated clinical and laboratory chemical parameters for diagnosis is frequently found in the lay media, above all on websites, but also in the specialist literature. Increasingly, patients with (suspected) MCAS are organizing themselves with commitment and are increasingly organizing themselves into interest groups such as MCAS Hope e.V., which campaigns for the recognition of MCAS "as an independent disease". In addition They also network those affected and their relatives and carry out public relations work, which aims to make the clinical picture known to a broader public. This expansion of the diagnostic criteria described above increases the risk of a misdiagnosis of MCAS and overlooking the underlying disease, which may be easily treatable. On the other hand, such an erroneous diagnosis can also lead to the use of unnecessary or potentially harmful therapies for MCAS and supposed comorbidities (20). Shortly after publication of the review paper "Doctor, I Think I Am Suffering from MCAS: Differential Diagnosis and Separating Facts from Fiction" by Valent et al. a self claimed affected person started an online petition in which she demands the authors and the publishing Journal of Allergy and Clinical Immunology to remove the article (55). Among other things, they criticize the criterion of the tryptase increase, which is too harsh and would therefore prevent many patients from being diagnosed. The clinic also does not typically manifest as anaphylaxis, contrary to what is described in the paper, since mediator release in anaphylactic degranulation differs from that in piecemeal degranulation. Finally, the author of the petition, who sees herself as a "patient spokesperson", reports on personal experiences of frustration and feelings of frustration and rejection that were conveyed to her by doctors in the course of her medical history. The petition has so far reached just under 3,000 of the targeted 5,000 digital signatures (as of December 2020) and shows in particular how emotional the issue of the topic of MCAS is being observed and discussed not only in professional circles, but also among patients. Apparently, some patients find the diagnosis of MCAS to be the last explanation for their multiple non-specific symptoms and hope for more acceptance in scientific circles.
2.3.5.3 Difficulties in making a diagnosis
In recent years, despite the existence of consensus criteria, a (suspected) diagnosis is often made in practice, even though these criteria are insufficiently fulfilled. In some cases, the MCAS diagnosis is also increasingly used for otherwise inexplicable conditions that cannot otherwise be explained. The evaluation of symptoms without a known direct connection with the release of mast cell mediators, for example from the neurological or psychiatric spectrum, as a manifestation of the disease leads to a further dilution of the MCAS diagnosis (43). In the "Bonn" questionnaire, the vast majority of the items asked are not based on the consensus criteria formulated by Valent et al. for example they see the sonographic evidence of an enlarged liver as an indication of the disease (53). The measurement of a tryptase elevation in acute relapse, as required by the diagnostic criteria is difficult to implement in practice, whether for reasons of time, capacity or billing. Targeted therapy trials with maximum specificity with regard to all possible decisive mediators are not possible without prior measurement of urinary metabolites and, in the absence of criteria or measuring instruments often do not produce satisfactory results (43). Last but not least, the wide range of possible differential diagnoses, such as for example from the endocrinological, neurological, psychiatric or cardiovascular area, further complicates the diagnosis (43).

Hello, My recent bloodwork came back with these flags. I have been exhausted lately. The levels are only slightly below the minimum…are they low enough to cause symptoms? Will I need to treat with supplements or just through diet? Waiting for my doctor to call me back…but they aren’t super attentive there…maybe I need a new provider who looks deeper into things?

2024.05.14 17:47 11HE Worried about lipid profile test results

I got my lipid profiles results from a blood test and am really concerned - I've looked at the wiki page which is very helpful but I still don't understand my results.
Background info: Age 30, female, cycle daily for 30 minutes and work as a nursery teacher so fairly active, eat a relatively good diet (whole foods - oats, nuts, don't drink caffeine, some fruits/vegetables). Suffer from severe anxiety so wondering if there's a correlation with cortisol/adrenaline levels but unsure. Not on any medication. History of family having high cholesterol (both mum and dad - dad from 50s, mum from 60s). Started taking vitamin B complex tablets daily around 4 years ago.
I didn't get the blood results to check cholesterol, was actually to figure out what might be causing epilepsy-like symptoms/loss of consciousness so was a shock to see these results.
Lipid profile results:
Cholesterol: 7.2mmol/l (ref range 2.5-5.0) Cholesterol:HDL ratio: 2.7 HDL Cholesterol: 2.7mmol/l (ref range 1.2-1.7) LDL Cholesterol: 4.0mmol/l (ref range 0-3.5) Non-HDL Cholesterol: 4.5mmol/l Triglyceride: 1.2mmol/l (ref range 0.4-2.3)
Other results from blood tests that were flagged was my MCHC (g/L) and that is 318 g/l (ref range 320-360) so is low but red cell count is within normal range from other results received.
If anyone could help me that would be great. Just wondering if anyone can see a pattern here or looking for general advice please. Thanks so much! :)

The third and last was in JLA #58. The League had to do an extremely complicated maneuver with the Moon, dragging it as quickly as possible into the Earth's atmosphere to bring oxygen to the Moon and fill it with fire (all at high speeds), removing it at the last second. Not just move it. So it stands to reason that they would want as many hands on the task as possible. They were also quite injured and tired and subsequently suffered even more blows from the White Martians.

This is heavily taken out of context. What Brainiac-13 claimed was that Superman did not have the power to move HIS planet away from him, referring to the War World, which Brainiac had taken control of. This distinction is not mere semantics, since Brainiac-13 has just absorbed the universal energies of Imperiex with which he wanted to cause a new Big Bang that would destroy the current universe and replace it with another. These energies were going to allow Brainiac-13 to remodel the entire universe to his whim.

5) "SUPERMAN WAS KNOWN BY A PLANETARY ATTACK AND A MOON EXPLOSION":

The figure Batman gives for the moon's mass is incorrect, but assuming he's right, that would be 1.8x1026 Joules. Multiplying both energies, the result is an explosion of 1.06x1046 Joules or solar system. Just what the title says. I took a blood test one month after discontinuing the BCP while I was having a light brown period. (It's not uncommon for me to have a period right after stopping the pill so I don't think this "counts".)
The results were:

Prolactin: 8.3
Estradiol: 180
Testosterone: 0.9 (??)
LH: 4.2
FSH: 4.4
Anti Mullerian Factor: He got weirdly tight-lipped about this so I'm going to ask the gp on my phone appt Thursday.
According to the ob/gyn, these are "normal" results. But he eventually conceded that I should take a few months off the pill to do the tests again because between never properly sexually maturing and feeling old and tired at 34, I have a feeling it's a very unusual and severe case of deficiency I've had over the course of the past two decades.
BUT what bothers me is that he insists birth control is all I'll need if my ovaries are indeed non-functional and I just... *facepalm* I'm so worried about what an uphill battle I'm going to have. This year is going to be unbearable, I just know it.

2024.05.14 16:13 MyFerrariMakesMeCry 69-year old father recently diagnosed with AML from having MDS. Please tell me what to expect going forward and how to best approach this.

Hello all, first post on this sub. Firstly, many thanks to everyone for the amount of useful information you give us and for how openly you talk about your own experiences. I know it cannot be easy but it's invaluable and I appreciate you for it.
My (26m) father was diagnosed with a very aggressive lymphoma back in 2019, he fought like a lion and underwent devastatingly strong chemos/treatments which saved his life but caused myelodysplastic syndrome. This obviously caused him all sorts of issues with blood platelets, white/red blood cell counts, haemoglobin et all. He has undergone 9 rounds of chemo in the past 1.5-2.5 years in order to "control" the disease. There were two occasions last year where we had to rush him to the ER because he woke up barely conscious, incoherent, with an incredibly high fever and in a frightening state; he always recovered shortly thereafter and spend a month plus in the hospital on each occasion. He was never considered for a bone marrow transplant due to age and his general health (relative to the disease) but he did get stem cell treatment which have apparently not done their job (I say this as somebody who is ignorant in the field, I'm not even sure what they were supposed to do in the first place).
A recent routine bone marrow biopsy returned a blast cell count high enough to warrant a diagnosis of AML. My father is entering hospital today to receive additional treatment to complement the one he already receiving - he is doing well and is always the same but they want him as an inpatient so they can be on "standby" in the event that whatever happened last year happens again and he's due to remain there for 30-40 days. My father is 69 years old and a plastic surgeon; he's a strong, hard-working man and has been his entire life. Took the treatments and side effects like a champ - I've seen him weaker but not frailer, I've seen him despondent but unyielding, I've seen him depressed but never tired of living. He's always been well (relatively speaking for somebody with his condition) but he's obviously had side effects of the physical (strength, endurance etc) and mental kind.
My father is my best friend and the center of my life and today I am hoping that somebody here can tell me what to expect moving forward. How should I approach this? What should I prepare for and what important information should I be aware of? I know that prognosis numbers/statistics for AML are very unfavourable but I also know that there are a number of factors that skew these excessively so I am approaching this knowledge erring on the side of caution. I have seen so much differing info on this sub alone - some say it's treatable, some say it's treatable for younger people but devastating for older, some were just saying nothing but offering well wishes; what is for certain is that this is a horrible thing to go through and a merciless ailment. I would everyone to give me their experiences, their knowledge, their everything so that I may best approach this situation.

I've been running a code that generates a login page for a desktop app. By logging in succesfully, the user would be sent to the main UI of the app ("Mainwin.py"). However, everytime I tried to log in, it gives me an error about 'pymysql' not being found, even though I already have it installed. The error is caused by a file named "actividad.py", which is used to show what user has been using the app lately:

What's strange is that when I run the "Mainwin.py" file, it all runs smoothly. It's only when I run from the login page code that the error is happening. Here's the code of the login page for reference:

My mother(53)was diagnosed with low grade bcell lymphoma treated with BR regimen(5 cycles) and was complete remission since September(last cycle) last year. Her blood counts were some what normal except wbc. She use to respond to gcsf but after her last cycle she has been refractory of gcsf.she was checked for mds the. By fish which came negative.her wbc was 0.4 for 40 days after that dr tried her to give Prednisolone and the wbc bounced back and dr started to tapper off till march after one month of stopping Prednisolone her wbc tanked again and she caught fungal infection in lungs.up until now her hb, platelets and rbc were all normal except wbc which tanked to 0.4 again.she was getting treatment for febrile neutropenia along with gcsf although she was not responding,she was not responding to voriconazole so she was started on liposomal amphocetrine b and caspofunngin after that her hb and platelets drop.she is currently on cyclosporine.seeing drop in platelets and hb dr ordered for bone marrow biopsy aspiration came normal but in biopsy little.percentage of cells abnormal according to pathologist (note she was stopped on gcsf just 2 days before biopsy) .they have again sent for fish mds.what are chances of mds as she always had her other blood markers normal except wbc (until her anto fungals were started) , previous report done on nov showed fish mds negative.how should we move forwards.apologies for my bad english.

(The vet said her glucose results can be dismissed as it was due to too little blood being in the vial during the test.)

I've got other questions I'd like to ask about her treatment plan, such as if there are things to try before going fully in to the science diets because I know how much that is going to change her life upside down and I'm a bit worried about some of the things I've read about using them when in the early stages due to the possible reduction in protein doing damage to the kidneys. If it seems safe I'd would like more time if possible to try her on other things first to see if her levels can at all be brought down. (Without wasting any time if there seems no time to spare. Whatever is best for her to help her as much as possible).
I'm also a bit confused about the food plan as from what I'm reading it's to reduce her protein and phosphorous which don't seem to be at elevated levels right now?

Hello,
This is a rather difficult post as I am looking for guidance. My 12-13 year old cat (I cannot remember what year I got him) was diagnosed with intestinal lymphoma. I have never had a pet have cancer so this is all new to me. He started losing weight about a year ago and chalked it up to us moving to a new home and was stressed. A few months ago he had a very aggressive yeast infection build up in his ears somehow that I treated. Many times we have tested him for cat leukemia and FIV that were all negative. In the last few weeks he started getting skinny and lethargic. I thought he had an infection so I brought him to the vet and they gave him different meds. Lab work showed some elevated levels for his white blood cell count and areas for kidney and liver. We leave and the next week after being gone for a trip he was worse. Brought him back to the vet to have them feel a mass that’s about the size of your whole thumb. They said this could be intestinal lymphoma. They couldn’t do an ultrasound that day but based on their new labs compared to two weeks ago they were certain it was cancer. He also was lightly jaundice in the ears. I have taken him to get fluids, pain meds, and nausea medication for him. I just don’t know what to expect as he’s really declined. What has your pets portrayed when they were having cancer? He’s so skinny and weak that requires a diaper for his incontinence. I feel like he does not have much more time and I feel so guilty. Like I missed something or is there anything else I can do to help him. Anything would be helpful.
