With respect to MRD, have never seen such a fast developing market - real clinical unmet need - enthusiasm among patients and physicians - excited to be apart of this market. Exact Sciences will catch up in MRD due to the following:
1) World class commercial capabilities - precision oncology reps are experienced, know physicians/territories, how to sell and how to get access to leverage these relationships to launch CRC MRD next year.
2) Exact Sciences' Nexus Platform - IT infrastructure to help providers save time obtaining prior use authorizations. This billion dollar company investment will allow providers to obtain prior use authorizations and order MRD tests more quickly and easily.
3) Better Performing Product -
Partnering with West German Study Group and
NSABP (National Surgical Adjuvant Breast and Bower Project) To build out evidence around MRD tests - Goal is for Exact Sciences' MRD tests to have best in Class Evidence. Exact Sciences' MRD tests measure more mutations in blood than some of the first mover companies in space, and will have best in class evidence. Investors will hear more about performance of MRD assays and evidence in the back half of this year.
Blood-based screening assets - crowded marketplace - Use case for Cologuard Blood will be limited.
Great idea in concept - Bert Vogelstein is a pre-eminent researcher in the field and wrote a 2005 paper on the subject of CRC screening blood test. He concluded that detecting circulating tumor dna from precancerous lesions/polyps is impossible - you can’t find what’s not there. According to Kevin Conroy, if you’re not finding pre-cancer, you don’t have a true screening test.
The real power of colon cancer screening is finding and removing pre-cancerous polyps which may result in Stage 1 disease - that is the goal. As a result, it is highly unlikely that blood tests to screen for CRC will end up in the USPSTF guidelines and quality measures b/c they are unable to detect precancer. CRC screening blood test will end up being more of a niche market. Commercial payers will not be too excited to pay for something which is not in the quality measures. Probably won’t find out if blood CRC screening will be in the quality measures until 2028-29 (After likely USPSTF in 2027). Howver, fee for service medicare advantage has agreed to pay for blood based crc. USPSTF meets every 5-8 years on CRC screening (August, 2014 - May, 2021). Predict next Meeting in 2027. This cycle think 6 years until USPSTF and then quality measures 2-3 years later
Management expects two or three blood tests to be approved by the FDA, including Cologuard Blood. Don't believe blood tests will ultimately be a big overhand for investors as Cologuard will still grow market share even with other FDA approved CRC blood tests as it is the most effective CRC screening test. GI societies recently weighed in - not recommending blood tests for frontline CRC screening. The growth of Cologuard over the coming years will be what excites investors, not CRC blood test.
Question to CFO Aaron Bloomer: What is most underappreciated or misunderstood about the company?
CFO is excited about pipeline. The company has spent many years developing these pipeline products, a number or which are slated to launch over the next few years. Don’t think investors appreciate the impact these pipeline products will have on patients, on revenue and the Company's growth profile, as well as on margins.
Excited about mid-teens growth in Cologuard and along with MRD and other new products coming online in the next few years which will provide both the Company with both leverage and diversification.

First lets take a quick look at some of the trades the Blue Jays have done mid-season in 2024: - IF Tanner Morris (26) to MIN for $$$
- Some sad news off the get go...after hitting just .149/.281/.213 with a .494 OPS in AAA, Morris has retired from baseball mid-season. Placed on the voluntary retirement list until his current contract expires at the end of the season.
- LHP Jimmy Robbins (26) to TEX for $$$
- Has taken the role of emergency depth for the Rangers AAA team and has pitched just 7.1 innings and is rocking a 8.59 ERA, .321 OBA & 2.18 WHIP.
- RHP Wes Parsons (31) to CLE for $$$
- Filling in his usual role as he uses up his last year of options. Has pitched 4.0 innings in MLB allowing 0 ER and 14.2 innings in AAA having a solid 1.84 ERA. Solid depth pitcher for them but not a mainstay.
- RHP Mitch White (29) to SFG for $$$
- After allowing 6 ER in 10.0 IP with TOR he was DFA/Traded to SFG where he allowed 7 ER in 5.1 IP before again being DFA/Traded to MIL where has has allowed 2 ER in 3.2 IP. Seems like teams think they can fix him considering they are trading for him. MIL has also been hampered with injuries and internal demotions to their bullpen so his stay might not be long when arms return.
Now lets take a look at some of the players that left the organization in the off-season through trade/waivers that were at one point on the 40-Man roster: - IF Santiago Espinal (29) to CIN for RHP Chris McElvain (23)
- Not a great season for Espinal as he has declined even further from his past seasons in all aspects of the game. He is hitting just .200/.252/.284 with an .536 OPS. His defensive metrics suggests he is now a negative player on defence as well. Due to injury he has been given a solid amount of playing time too. McElvain has been solid in A+ but still looks likely to be a future reliever.
- UTL Otto Lopez (25) to SFG for $$$
- DFA due to the middle-infleid depth the Jays had was then DFA by the SFG even when they were awarded another option year with him. MIA got him for free on waivers as the only claiming team and he has been very solid for them. He earned a quick call-up after having a 1.526 OPS in a week and is carving himself a role in the Majors with some solid hitting and strong defence. He is batting .282/.333/.564 with an .897 OPS in 39 PA. His early power display is very unsustainable though. He is already half way to his career minor league high.
- RHP Yosver Zulueta (26) to CIN via DFA Waivers
- There was a lot of anger amongst the fanbase with his loss on waivers and while his stuff is still good the production is still no where near where it should be. In 14.1 IP in AAA he has a 5.65 ERA, .267 OBA & 2.02 while striking out 18 and walking 13. Still not sure how he remained on prospect lists as long as he has. He just hasn't been good for awhile now. He does give the vibes off the guy that will randomly break out at age 30/31.
- C Tyler Heineman (32) to NYM to BOS via DFA Waivers
- Remains the trusty #3 catcher we knew him as. He is just 0-for-1 with 1 HBP at the Major League level but is staying warm in the Red Sox minor league system until he is needed once again. Batting .257./395/.286 with a .681 OPS in AAA. Severn is an upgrade in all departments though except the lack of magic tricks.
Finally, lets take a look into the players that left the organization in free-agency: - 3B Matt Chapman (31) to SFG (3 Y x $18 M w/ Two Player Opt-Outs)
- Easily the most talked about departure this season and it feels like beating a dead horse at this point but Chapman has been super terrible. Hitting just .206/.258/.341/.599 with a 73 wRC+ (1.0 bWAR / 0.1 fWAR). His strikeout rate remains as high as ever but his BB% has been nearly cut in half. He has already made 6 errors and his defence has dipped further for the 4th straight season. Chapman has just 2 extra-base hits over his last 82 PA despite having some of the best swing-rates and exit velocities in baseball. Looks like Chapman will be a SFG for the full 3 seasons.
- RHP Jordan Hicks (27) to SF (4 Y x $11.0 M)
- Arguably the only "big" contract the Giants have signed that have given them their value for their money. Converting back to a starter has worked wonders for him as he has posted a 2.44 ERA / 3.21 FIP, .220 OBA & 1.15 WHIP. He has taken a good 3 mph off his fastball which has led to a decrease in strikeouts, whiffs and more harder contact against but it has led to an increase and sustained improvement in control. 5.0 IP does seem to be his limit most nights and there are some long-term questions about stamina and durability but so far he has been worth ever penny. If things do now wrong e can always be converted back to an RP and his contract still won't look bad.
- 2B/OF Whit Merrifield (35) to PHI (1 Y x $7 M w/ Club Option for $8.0 M)
- Age seems to have properly caught up to Whit who is just hitting .188/.261/.300 with a .561 OPS in 80 AB with is just a 62 wRC+ (0.1 bWAR / -0.1 fWAR). His savant page portrays him as one of the worst hitters in baseball and his defence is now of negative value. Phillies had him hitting lead-off for 3 straight games recently where he went 1-for-15 before being demoted again.
- RHP Adam Cimber (33) to LAA (1 Y x $1.65 M)
- After struggling with injuries and inconsistency at the end of his Jays tenor he has rebounded back with LAA to being a solid mid-usage arm. Posting a respectable 4.00 ERA, .214 OBA & 1.00 WHIP. His numbers are misleading though as outside of 2 meltdowns he has been shutdown. He is one of the heaviest used bullpen arms in baseball and has only allowed an ER in 3-of-19 appearances. Should fetch the Angels a decent prospect at the trade deadline.
- RHP Jay Jackson (36) to MIN (1 Y x $1.3 M w/ Club Option for $3.0 M)
- Jackson has not had a great season with MIN as he is rocking a 6.20 ERA, .244 OBA & 1.28 WHIP.He has allowed at least 1 ER in more than half his appearances. It seems like MIN will likely not pick-up their club option on his contract at this rate. It is unfortunate to see after how important he was for us last season. Glad he got his money though.
- SP Hyun Jin Ryu (37) to Hanwha Eagles (KBO) (8 Y x $1.6 M)
- Ryu signed the largest contract in KBO League history this offseason which keeps him in the organization until his mid-40's. In 49.0 IP he has 5.33 ERA, 1.45 WHIP & 47 SO vs. 14 BB. Haven't watched him nor seen his game-to-game production but the face value numbers don't look great. To be fair he was likely never playing most of the contract anyways.

2024.05.16 12:45 SpecialistRegret7258 Biochemical pregnancy versus early miscarriages
Need help trying to figure this out- We've been TTC second baby for the past 3 months or so. Regular cycles 25-27 days bleeding X4 days. No prior GYN/OB Hx. PCOS panel NL this yr. LMP 4/5/24. May cycle never came, and I Got several positive home pregnancy tests on 5/9 which was cycle day 35. Very excited and shared with our parents and siblings. Had no issues conceiving last baby and no prior losses...calculated to be about 5 weeks based on LMP-Was also having some mild nausea, breast tenderness, bloating- same symptoms as last pregnancy... I work in an OBGYN office so I was eager to check my serum levels- But my Serum beta HCG in the office Monday 9/13 was very low... like 16- I called my OBGYN and he said to repeat the beta level on Tuesday- if it's even lower then likely will be a biochemical pregnancy or early miscarriage.
My serum progesterone also has dropped by about 50% with each check-and now serum beta HCG is down to 1- so doubt I had a late ovulation or a viable pregnancy with these levels. No bleeding whatsoever- but don't patients with a biochemical pregnancy usually have a heavier period that essentially comes as planned? In patients who miscarry don't they usually start bleeding pretty quickly with a nonviable pregnancy? It's Thursday now and I still have no bleeding whatsoever. Had some mild cramping yesterday but essentially resolved after some OTC ibuprofen.
The NP in my office and sonographer have offered to do a transvaginal ultrasound on me now to try and see if we can figure out what's going on? I'm 2 weeks late now, so not really sure when to expect the next "period" if this is a pregnancy that still hasn't resolved... IF we scan now Should we still see a corpus luteum in theory if it was an early nonviable IUP? What if it's a blighted ovum or an early ectopic? I'm not scheduled for "first OB appt" until the 28th but I just want to make sure it's nothing to worry about. You can't have an ectopic with a beta that has gone all the way down to 1, right? Any reason I should continue checking HCG levels or hormones at this point? Is it safe to just go ahead and assume I'm not pregnant? Anybody that's had a biochemical pregnancy/blighted ovum/early miscarriage etc. have any experience to guide me? How long after the beta went down to undetectable did you have bleeding? Or get a period? How long is TOO long to go without either seeing bleeding to indicate pregnancy resolution or another cycle?
I don't want to mess things up for future periods or opportunities to get pregnant again. If I go another two weeks and skip a second period, would I need a D&C? Or do a provera withdrawal bleed? Just trying to figure out what this all means.

Hi there! I'm kind of new to this and looking for support. I've only been off of testosterone and publicly detransitioning for a month now. I'll write some of the backstory out and then my questions/concerns about my former transition and what to do next. I might ramble, my apologies. I have a lot to say, and I am definitely feeling emotional about it all. It's a big deal.
I'm (27F) woman and I was transitioning as an FTM for over 5 years. I have a bit of a complicated transition history that stretches back into my earlier teen years, but I medically transitioned in my 20s for that 5 years. I only went on testosterone and never got any procedures done. However, I passed fairly well when binding and with my facial/body hair growth. I was diagnosed with gender dysphoria but I'm not sure if that was correct or not. Honestly, I think I struggled more with body dysmorphia than anything gender-specific. I've had a very traumatic life (just saying that for context, I don't try to live my life in a victim mentality, my trauma has just influenced my decisions) and I went through puberty early so I was more aware of sticking out than someone in general would have. I genuinely did feel like I was transgender at parts of my life, but I don't think I would have identified that way if I had received better mental healthcare and guidance for how to accept and love and care for myself as the capable woman I am.
Overall, I think my transition was successful in a way because I accomplished a lot of life goals during the process and connected with myself. I finished undergrad, got into graduate school, worked hard in academia and in jobs, became a professional artist and writer and much more- all things that I never thought I was smart enough to do before transitioning. (I know, that sounds terrible :c ) Ultimately though, transitioning was leading me to an unhealthy place. I started to uncover a lot of traumas in therapy linked to bad male figures in my life and I started to piece together that perhaps I was trying to become the positive male figure that I never had before. Honestly, becoming that "man" did help, but the whole time it was just me. I'll never have a father, and I'll never get the peace and innocence I had before former abusive relationships, but I should have understood that my inner strength was inside of me as a woman the whole time. I didn't have to be a trans man to access that. I never experienced life as a single adult woman since I was always under negative male control, so this new phase of life is an entirely new world to me in a way. It's exciting, fresh, scary, and I am feeling so many things. I don't feel bitter about transitioning but I'm silently highly critical of my past situation and the way I regard other transitions I see IRL and online.
Towards the last two years of medically transitioning I started to experience really weird symptoms. There is a possibility that some of this is tied with long covid because I got seriously ill a couple of years ago, but I noticed symptoms become worse when dosage went higher and better when dosage was lower. Eventually, my doctor put me on what he called a "nonbinary level" which got my levels to somewhere around 99 when he said they should have been at 300. This was the point that I took medicine into MY own hands and told HIM that I wanted to be on that low dose because I felt better on it even though he wanted to increase it for some reason.
The symptoms I was having were heart palpitations, chest pains, muscle aches, loud joint cracking and pains, weakness and prone to injuries, vaginal dryness and pain, dizzy spells, and more probably that I can't think of. I have brought up these things to various doctors especially the last one and none of my "transgender healthcare professionals" seemed worried/interested enough to investigate what was going wrong other than give me blood tests or prescribe me a medicine that I wouldn't take. I felt like I had to handle my own treatment plan by becoming uncomfortably self-aware of my body situation and adjust my weight, supplements, and testosterone based on what would make me feel the least crappy. I felt abandoned by the medical field and I'm having to search for competent clinicians now who can see what's actually happening with my health. Thankfully most of the negative side effects have either subsided or lessened considerably but I don't know how much damage was actually done. I'm very grateful I still have my breasts and that my reproductive system is working a bit better, though I haven't gotten my period back yet.
Another frustration that helped me choose to detransition was understanding that the trans space is actually more dangerous than people want others to see it as. I hate saying that it's dangerous because I don't want to fearmonger ever, it's more of a logical observation. The amount of people that were predatory towards me when I was a trans man is shocking. I have trust issues already and I thought that presenting as male would protect me, but there were several who knew how to manipulate my insecurities and benefit from my vulnerability. A couple of months before I made the choice to detransition, I was a victim of a very violent SA from a "friend." I was hospitalized and injured for weeks if not months and it made me reconsider everything during the intensive therapy. My eyes opened to the reality of "chasers" who just wanted to feminize me, enact their kinks on me, and objectify/brutalize/humiliate me for my inherent womanhood. The people that claimed to love me as a trans man in theory, hated me as a woman in practice. It was horrific.
I'm so so so thankful that other detransitioners are being vocal about their experiences and reaching out even though it's a hard thing to do. I feel like I've been saved from a lifetime of extra pain and challenges that I was tricked into. My experience could have been much worse, and it was already not good.
While now I'm learning how to love myself more and navigate the world and processes of detransitioning, I'm also facing my mindset changing towards transitioning. At this point in time, I do personally believe that some transgender people do exist with gender dysphoria, but I think it is much rarer than the dominating narrative seems to suggest. I've started to hear more about AGPs and one of my friend groups almost all transitioned to become "trans women" but they don't seem to put in any effort to socialize or appear like women. They only tend to have sexual discussions and have started to seem very predatory and creepy towards me and others. One of them is convinced she has a period too. I have distanced myself from them because they have crossed my boundaries and I feel very threatened. But I feel like I can't confront them about the offensive gender issue because they'll think I'm transphobic when in reality, I feel like they're being misogynistic and hurtful.
Has anyone else experienced difficulties in healthcare/bad hormone side effects? Or what are your thoughts on dangers in the trans community? Do you have any tips on how to navigate the early stages of detransitioning? I just filed for my name change and am hoping to get laser removal soon! :)

I'm not sure if I'm being emotional or on the way towards PPD. I'm very easy to upset, have upset others who are only trying to help. My partner is being super supportive, particularly at night taking on most of the nightly duties. Issue comes in how I feel. I don't feel adequate, almost like a bad mom. Though I'm receiving help, it feels like in redundant and unneeded. Like my bf can do everything on his own. I had a c section and due to being bed bound for a while, he has to take up more responsibility. It's great for him as it helped foster a relative with our newborn. This is where my issue lies because now he has a strong[er] connection with baby and it just make me sad. All of this comes to a head today when I try to breastfeed only for her to deny my nipples. We had been bottle feeding breast milk in the night which is great because it allowed me to sleep but has created a preference in baby towards the bottles. This makes me feel like a bad mom and like I don't matter. Baby isn't latching properly which exasperated the issue..
I'm not sure if I should spend a psychosis or a postpartum doula...

2024.05.16 02:34 mamz_leJournal Help! Is my shape difficult or have I just not figured it out yet?
I feel like I am having trouble figuring out my shape. All unpadded bras I have tried to far seem to be a shape missmatch.
I have fairly small breasts but I feel that my size is bigger because I have quite a lot of immediate projection. Unsupported I have pretty pointu breasts and about 2/3 of the tissue is on the bottom. I have wide roots and wide set breasts (there is about an inch gap at my sternum and get almost half the way under my armpits). They are high on my chest.
Now the part I am confused about is my roots’ height and my fullness. I would say my breats are wider that they are tall. I would tend to think I have short roots. In fact I am pretty sure I have short roots, as either I get gaping on top of a cup starting at my armpit or I get quadding. However when I press on my breasts the tissue tends to get quite higher, up to maybe an inch under my clavicule, while being maybe 2 inches over my armpit.
As for the fullness, the fact that my unsupported breasts are fuller on the bottom and that I have gapping in most bras would have me think I am FOB. However, when I do the leaning test the entirety of my tissue gets on top and almost to my chin and there is absolutely no volume under the nipples. They also get full in the middle even though a lot of it is usually under my armpit. So I guess that would mean that I am FOT? My experience with bras also make me feel like this is the case as as soon as my breasts are compressed slightly I get a very full and top heavy look and get quadding. Let’s say I loosen the straps the slightest now I just get gaping. This is the part that is confusing to me.
I feel like I have very soft tissue while it being quite dense at the same time? Maybe it’s just because of my size but they are self supporting, but the shape looks like it’s not supported that well if that makes sense? The fact that all my tissue tends to move easily but in a rounded shape as soon as these is pressure makes me think there is some density to it?
I would be really tankfull is someone can help me narrow it down. Even more if you get suggestions of styles that may work for me.
Here are the bras I have tried so far (the calculator recommended 30DD/30E
- my regular everyday bra is a la senza plunge style in 32D and it way too big in the band. The cup has enough projection and volume but I get gaping at the top and I can’t get the straps tight enough. With the same model in 32 C I get quadding and tissue escaping in the armpit / wires cutting in.
- cleo marcie 30DD. Not a bad fit but not a good one either. The band is perfect. The cup feels like it needs more depth. I can’t get the straps right as I either get a bit of quadding or the lace in the center is wrinkly. The height of the cup feels good but the straps are uncomfortabe in my armpits, while the wires are just wide enough. Also the top part of the gore moves away from my body when I move. I am thinking maybe a 30E would fit better
- panache jasmine 30DD. The band feels ok though it could be slightly tighter. There is enough immediate projection and the bottom part of the cup fits well, but the gore is almost as high as my roots and the top part is isn’t filled up at all. It is uncomfortable in the armpit. I got this bra because it is supposedly good for FOT folks but it has me thinking I must be FOB, at least fonctionally? The weird part is I tried a fantasie bra in 30F in the past and it fitted almost the same. Maybe a 30D would fit me better (if at all).
- the best fit of my supposedly correct size is panache Ana in 30DD. The band could be tighter as I am very comfortable on the tightest hook and it is new. The cup size feels right and there is no gaping. However I get a weird look with the side support (which I though would benefit me)as it pushes everything flat to a point where that part of the cup wrinkles even though it clearely isn’t too wide and it gives me kind of a pointy look from the top.
I am thinking about maybe trying polish brands for their projection and shorter cups but I worry that the wire would be too narrow.
It feels like half cups could be a good next step? I remember a few years back during a trip to europe I came across a bra I liked but that was in a size I felt could never fit me (something like 28F, but I can’t remember exactly the size). I decided to try it anyways and it was the best fitting bra I had ever put on. Just like cinderella’s shoe. I ended up not buying it and I still regret it to this day. I think it was a half cup. Would that make any sense? Does a combinaison of a larger and very short cup work for me?

2024.05.16 02:06 LynnB369 PTSD/ Jealousy/ sadness am I just overreacting?
Hi guys I have barely posted on here but I do try to read all posts when I can. My baby has been home for 4 months now. She left the NICU when she was 4 1/2 months old. She’s now 7 months old. We are so happy she’s been home and growing. Anyways. I still think back to my delivery and how traumatic it was and they journey of a rollercoaster in the NICU was a surprise. I look back and think that I wish to have carried my baby once she was born and do skin to skin and connect with her right away, and be able to breast feed her too. After I would discharge I would go to visit her everyday while still healing from my C-section. I would see new moms have their child in there arms or the dads carry them in the car seats. It gave me a sense of anger and sadness. I would break down crying or just hide the fact that I was so sad that they had a healthy baby and mine was here living in the hospital her whole newborn life. It was killing me inside honestly and I was just getting emotional a couple days back because of it. Am I the only one??

2024.05.16 01:14 zaahiraa a few different questions for my 4 week old- chest sleeping/eating to sleep/sleeping on back in bed
hi! my adorable baby is only 4 weeks old!! she a cute tiny little angel who pretty much exclusively cries while awake. every once in a while i’ll feed her before diapers and she’ll just look at me but usually she’s just crying until i feed her to sleep.
i used to use the Eat, Activity, Sleep method, but she conks out after eating, and because she usually is crying while she’s awake, it’s hard to want to wake her for “activity” and she absolutely won’t just fall back asleep on her own after that wailing.
because of that i started doing MAMS method - Milk, Activity, Milk, Sleep and will feed her immediately upon waking, then that her sleepily to diaper (where she wakes up fully) clothes change, bouncy ball, whatever, then milk again before she conks out the second time.
So my question is when it comes to the second fed and sleeping part, we usually do chest feeding/sleeping for all naps, but i feel like she eats SO MUCH, will nod off, jolt awake, then start all over again. i’m worried i just will run out of milk cause she’s still awake and i have to end up switching breast and she’s still eating and then comfort nursing. she’ll fall asleep for maybe 5 or 10, wake up, start eating again. so my second feeding her to nap feel like it takes dang near an hour! then she finally falls asleep (like now 🥰) and will be out for a couple hours.
i think im wondering how much milk is too much?? i just feed her constantly all day because she cries for it. when she comfort nurses i definitely swap out the pacifier, sometimes she takes it, sometimes not.
she doesn’t seem to be too gassy, because she always burps when i burp her and she farts pretty regularly but it doesn’t look like she’s in pain. could that be why she cries so much? i figured it was just that she’s a vocal opinionated baby and some baby’s just cry. also she’s only 4 weeks.
i’m also sleep deprived so im sorry if this was too long!!!!! also can i cosleep in bed with her but be laying on my back? i’ve read all the sleep rules but am still unsure of this one. i have a degenerative disk and c curl all night is miserable. so far i side lay feed her and when she falls asleep i like sleeping on my back with pillows propping me up. is that okay?? she is still low and not by the pillows and i sleep with no blanket but warm pjs.

2024.05.15 23:25 Mental-Pomegranate55 FNG only on one side?
My surgeon said I will likely need a FNG on one side due to it being much larger than the other side. Has anyone just had one breast that needed a nipple graft. I'm scared of the nipple graft anyway, but now I'm scared my nipples will look totally different from each other afterward. I believe my surgeon said my nipple on the larger breast is like 40cm from the breast fold. I currently wear a K sized bra, and she said after surgery I can expect to be a C or D cup.
