I have a question. I got denied for Gerd/ acid reflux 9 years ago. I have documented in medical records that I went to the ER and thought I was having a heart attack. The abdominal exam did show some mild epigastric discomfort to deep palpation. I was given Zantac 150 mg twice a day. Fast forward today. My acid reflux has gotten worst over time. I have changed my diet. I don’t eat foods that contains acidic foods, nothing spicy. Do I file a new claim since I was denied 9 years ago?

Can someone explain why D would NOT be a viable answer? No history of serious illness but guy hasn't seen a doctor in 20 years and A1c 8.9% with BP 150/90 - so I assumed diabetes/HTN and jumped to hyaline arteriolosclerosis (protein accumulation in the renal arteriolar walls). I see the logic of A (BPH -> hydronephrosis -> elevated Cr) but why are D and possibly hyaline arteriolosclerosis incorrect? Thanks!

12 months of misery, 12 months of pain. Malnourished, depressed, suicidal. 185cm and dropped to 150 pounds. Every night I was ready to die in my sleep from malnutrition.Even though smoking is bad I was doing 20-30 cigarettes a day, taking xanax, PPIs, zantac.
Finally my stomach decided to improve and although I am not healed and will never heal I feel alive again. I can digest foods and have energy. I am in my 30s and this chronic gastritis has made me stupid and immature.
But now that I feel human I want to run, lift weights and take vengeance from life. But I have fear. Will it come back? Will my heart and lungs collapse? I mean I was 1 year in bed eating nothing.

ANALYSIS:

• Dollar risk reversal continues to point lower, traders expect dollar to fall in medium term.
• Can get short term volatility from PCe today. It seems from the fact that skew on TLT points higher, that traders are expecting an in line or soft PCE.
• I showed also that Deutsche and Wells Fargo seem to point to an in line or soft PCE too. I however, have doubts, as CPI and PPI came hot and PCE can follow. This can push dollar higher to 104.6, but let’s see. If it comes soft, dollar can fall back to 103.3.
• JPY is rising today on BOJ’s Takata pointing to inflation target coming in view. This is increasing chances of monetary tightening. Risk reversal on USDJPY continues to point sharply lower. Definitely expecting this to sell off soon.
• A lot of todays price action will depend on PCE. We saw Market take a breather last couple of days, which is healthy. Skew on mag7 starting to point higher again. If we see good PCe can be a push back to nasdaq 18k and SPX 5100. NVDA taking a breather but the bullish positioning remains, so this can lead QQQ higher soon.
• Whilst hedge funds are rotating out of tech, SPX money flows continue to surge, which is bullish overall market. Not seeing big signs of a major pullback still. Just short term volatility ahead of uncertain data.
• Probably, if the market dips, it will eb short lived.
• Traders continue to be short VIX another sign that it won’t be a long term sell off.
• Main struggling Mag7 continue to be GOOGL and AAPL. Others show positive skew.
• SNOW getting killed right now on the CEO change. The guidance was a miss but they pointed to the fact it was conservative, as is common practice when new CEO comes in.
• I think can present an opportunity. For intraday plays, may be an idea to play another Cloud stock which is getting dragged, such as DDOG. MDB or ESTC
• On SNOW, 166 is my buy price, but it may not hit. Will be watching the candlesticks for intraday reversal.

DATA LEDE

• JAPAN retail sales
• Rose 2.3% YOY in January, in line with expectations, in line with last month. Still one of the lowest readings in months. The previous 11 months have been in the range of 4-7% increase.
• So retail sales growth still quite weak in Japan.
• Japan Industrial production very weak MOM, down 7.5%, worse than forecast, most negative MOM reading in over a year.
• Despite weak numbers here, JPY moved higher as BOJ’s Takata says inflation target is finally coming into sight, and central bank needs to consider flexible steps for an exit.
• AUSTRALIA retail sales:
• Came 1.1% mom, slight miss on 1.5% expectation.
• German Retail Sales (Jan)
• Fell by 1.4% YOY, slightly less than the 1.5% drop expected, and better than last month
• France CPI (Feb) - first read, will set precedent for eurozone CPI tomorrow too.
• Came slightly hotter than expected
• YOY was 2.9% vs 2.7% expected
• MOM was 0.8%, vs 0.7% expected.
• Spain: Inflation rate YOY was 2.8% hotter than the 2.7% expected.
• SO FRANCE & SPAIN INFLATION BOTH HOTTER THAN EXPECTED.
• Note: Despite hot CPI in France, PPI which was also released came at lowest level since 2009. So mixed inflation readings there. Maybe temporary tick up and PPI points to lower inflation after that.
• France Finance minister says that France is winning in the battle against inflation.
• Swiss GDP growth rate YOY was 0.6% vs 0.7% expected
• QOQ was 0.3% vs 0.1% forecast.
• So mixed GDP numbers, CHF moved lwoer on the data initially, then recovered higher.
• GER Unemployment Rate (Feb) - expecting continued weakness. MOM unemployment change to be positive again after surprising negative last month. Effect will be overshadowed by German CPI.
• Unemployment rate rose to 5.9% from 5.8%
• Unemployment change 11k vs 7k expected.
• Weaker employment numbers than expected, points to increased recession risk.
• German CPI (Feb)Expected to have ticked lower again.
• US Jobless Claims
• US PCE Price Index (Jan) - note core PCE has fallen on last 2 months YOY, and markets expect it to tick lower again, but we may see tick up in line with PPI and CPI. Personal income reding will be key for pointing to job market tightness too.
• Chicago PMI (Feb)
• Risk reversal on DXY and TLT shows no sign of panic from traders, but let’s see.

MARKETS:

• SPX: Yesterday, SPX traded mostly between 5060 and 5075, ahead of PCE. No big volatility, as traders wait for PCE and don’t seem to expect any major surprises.
• IN premarket, traded the same range, moving higher from 5060 to 5075 during Asian session, before getting rejected back to 5060. Low of premarket was 5051.
• Nasdaq: Remember key level is 18k, thats 0.89% above spot and will be in focus if PCE comes soft. Yesterday moved lower between 17,920 and 17800. In premarket, rose to 17900 before falling back to 17800.
• DJI: Yesterday, opened the day lower at 38,750, before moving higher to 38,950.
• During premarket, it dragged down with CRM earnings sell off, back to 38,750. Recovered slightly during asian session back to 38,950 and then sold off the gains again back to 38,800. Continues to trade a range basically.
• GER40: Continues to move higher, got a surge in open, no signs of panic really here.
• HKG50: Trading near a trend line from early Feb. Surged in first hours to 16,680. Then pared the gains. Trading back at 16,450, more or less where it was yesterday
• CHINA50: Same trend, pushed higher initially, then pared gains. Trading at 11,850. Key level still 12k
• OIL: Pared gains yesterday after EIA inventories showed excess supply , was also trading close to the key 80 level. Gains initially were as dollar sold off. Flat today around 78.
• GOLD: flat, awaiting dollar movement from PCE.
• BOND YIELDS: moved higher after hot France and Spain CPI and Germany Unemployment numbers coming weak. Ahead of PCE today

FOREX:

• JPY -JPY moved higher as BOJ’s Takata says inflation target is finally coming into sight, and central bank needs to consider flexible steps for an exit.
• AUDUSD - initially moved higher as Hong Kong MARKET moved higher. Then pared those gains back to below 0.65.
• EURUSD moved up to 1.85 then rejected and starting to come down. Muted movement before PCE.
• USDJPY moved lower to 149.6
• GBPUSD trading flat around 1.264.

EARNINGS:
SNOW: - primarily down on earnings because of the fact that the CEo stepped down. Said they have lowballed guidance to set a lwoer bar for new CEOs. Said it was a conservative guidance.

• Current quarter EPS beats by 2%
• EPS of 0.35 beat by 94%
• Dollar net retention rate was 131% from 151% last year. But this was misleading as they were ramping customers up quickly
• Product Revenue Guidance:
• Next quarter: 747.5m, miss by 3.3%
• Full year: 3.25B, miss by 7.2%
• 2025 operating margin of just 6% vs 9.5% expected
• AS mentioned, these were conservative guidance
• CEO steps down, the previous Senior VP of AI was named the new CEO. This appointment helps to push them towards AI.
• New CEO said he wants to a accelerate innovation for customers.
• Still said they’ve had a strong bookings quarter. Partly at expense of pricing.
• Said they want to hire 1000 employees and invest heavily in AI.
• Seeing strength in financial services and retail.
• Sees multi year pent up demand for products they will release.

CRM:

• Said they are seeing an improving buying environment and increasing bookings trend.
• Current quarter:
• Revenue of 1.97B in line with expectations
• EPS of 2.29 beat expectation by 1%, more or less in line.
• FUll year guidance:
• Revenue of 37.85B missed by 2%
• EPS of 9.72 beat by 1%
• Guidance wasn’t bad, but more or less in line with previous trends, so didn’t exactly accelerate despite recent price increases and product bundling.
• Starts dividend, increases share repurchases to 10B.
• Not a bad quarter here.
• Story is getting better on improved execution and AI.
• Talked a lot about AI on the earnings call. And how it will help provide their customers with better productivity.
• Said they stand in an advantageous position because they have the data to support the Ai models.

• Very strong earnings report . Note Goldman Sachs have OKTA as one of their high conviction plays.

• Revenue beat estimates and their previous guidance by 3.2%
• EPS of 0.52 beat by 0.12.
• EBIT beat estimate by 25%
• Beat FCF estimate by 63%
• So very strong numbers:

NARI earnings:

• Downgraded by Piper Sandler following the print to 55 from 85.

ANALYSIS:

• Dollar risk reversal continues to point lower, traders expect dollar to fall in medium term.
• Can get short term volatility from PCe today. It seems from the fact that skew on TLT points higher, that traders are expecting an in line or soft PCE.
• I showed also that Deutsche and Wells Fargo seem to point to an in line or soft PCE too. I however, have doubts, as CPI and PPI came hot and PCE can follow. This can push dollar higher to 104.6, but let’s see. If it comes soft, dollar can fall back to 103.3.
• JPY is rising today on BOJ’s Takata pointing to inflation target coming in view. This is increasing chances of monetary tightening. Risk reversal on USDJPY continues to point sharply lower. Definitely expecting this to sell off soon.
• A lot of todays price action will depend on PCE. We saw Market take a breather last couple of days, which is healthy. Skew on mag7 starting to point higher again. If we see good PCe can be a push back to nasdaq 18k and SPX 5100. NVDA taking a breather but the bullish positioning remains, so this can lead QQQ higher soon.
• Whilst hedge funds are rotating out of tech, SPX money flows continue to surge, which is bullish overall market. Not seeing big signs of a major pullback still. Just short term volatility ahead of uncertain data.
• Probably, if the market dips, it will eb short lived.
• Traders continue to be short VIX another sign that it won’t be a long term sell off.
• Main struggling Mag7 continue to be GOOGL and AAPL. Others show positive skew.
• SNOW getting killed right now on the CEO change. The guidance was a miss but they pointed to the fact it was conservative, as is common practice when new CEO comes in.
• I think can present an opportunity. For intraday plays, may be an idea to play another Cloud stock which is getting dragged, such as DDOG. MDB or ESTC
• On SNOW, 166 is my buy price, but it may not hit. Will be watching the candlesticks for intraday reversal.

• JAPAN retail sales
• Rose 2.3% YOY in January, in line with expectations, in line with last month. Still one of the lowest readings in months. The previous 11 months have been in the range of 4-7% increase.
• So retail sales growth still quite weak in Japan.
• Japan Industrial production very weak MOM, down 7.5%, worse than forecast, most negative MOM reading in over a year.
• Despite weak numbers here, JPY moved higher as BOJ’s Takata says inflation target is finally coming into sight, and central bank needs to consider flexible steps for an exit.
• AUSTRALIA retail sales:
• Came 1.1% mom, slight miss on 1.5% expectation.
• German Retail Sales (Jan)
• Fell by 1.4% YOY, slightly less than the 1.5% drop expected, and better than last month
• France CPI (Feb) - first read, will set precedent for eurozone CPI tomorrow too.
• Came slightly hotter than expected
• YOY was 2.9% vs 2.7% expected
• MOM was 0.8%, vs 0.7% expected.
• Spain: Inflation rate YOY was 2.8% hotter than the 2.7% expected.
• SO FRANCE & SPAIN INFLATION BOTH HOTTER THAN EXPECTED.
• Note: Despite hot CPI in France, PPI which was also released came at lowest level since 2009. So mixed inflation readings there. Maybe temporary tick up and PPI points to lower inflation after that.
• France Finance minister says that France is winning in the battle against inflation.
• Swiss GDP growth rate YOY was 0.6% vs 0.7% expected
• QOQ was 0.3% vs 0.1% forecast.
• So mixed GDP numbers, CHF moved lwoer on the data initially, then recovered higher.
• GER Unemployment Rate (Feb) - expecting continued weakness. MOM unemployment change to be positive again after surprising negative last month. Effect will be overshadowed by German CPI.
• Unemployment rate rose to 5.9% from 5.8%
• Unemployment change 11k vs 7k expected.
• Weaker employment numbers than expected, points to increased recession risk.
• German CPI (Feb)Expected to have ticked lower again.
• US Jobless Claims
• US PCE Price Index (Jan) - note core PCE has fallen on last 2 months YOY, and markets expect it to tick lower again, but we may see tick up in line with PPI and CPI. Personal income reding will be key for pointing to job market tightness too.
• Chicago PMI (Feb)
• Risk reversal on DXY and TLT shows no sign of panic from traders, but let’s see.

• SPX: Yesterday, SPX traded mostly between 5060 and 5075, ahead of PCE. No big volatility, as traders wait for PCE and don’t seem to expect any major surprises.
• IN premarket, traded the same range, moving higher from 5060 to 5075 during Asian session, before getting rejected back to 5060. Low of premarket was 5051.
• Nasdaq: Remember key level is 18k, thats 0.89% above spot and will be in focus if PCE comes soft. Yesterday moved lower between 17,920 and 17800. In premarket, rose to 17900 before falling back to 17800.
• DJI: Yesterday, opened the day lower at 38,750, before moving higher to 38,950.
• During premarket, it dragged down with CRM earnings sell off, back to 38,750. Recovered slightly during asian session back to 38,950 and then sold off the gains again back to 38,800. Continues to trade a range basically.
• GER40: Continues to move higher, got a surge in open, no signs of panic really here.
• HKG50: Trading near a trend line from early Feb. Surged in first hours to 16,680. Then pared the gains. Trading back at 16,450, more or less where it was yesterday
• CHINA50: Same trend, pushed higher initially, then pared gains. Trading at 11,850. Key level still 12k
• OIL: Pared gains yesterday after EIA inventories showed excess supply , was also trading close to the key 80 level. Gains initially were as dollar sold off. Flat today around 78.
• GOLD: flat, awaiting dollar movement from PCE.
• BOND YIELDS: moved higher after hot France and Spain CPI and Germany Unemployment numbers coming weak. Ahead of PCE today

FOREX:

• JPY -JPY moved higher as BOJ’s Takata says inflation target is finally coming into sight, and central bank needs to consider flexible steps for an exit.
• AUDUSD - initially moved higher as Hong Kong MARKET moved higher. Then pared those gains back to below 0.65.
• EURUSD moved up to 1.85 then rejected and starting to come down. Muted movement before PCE.
• USDJPY moved lower to 149.6
• GBPUSD trading flat around 1.264.

EARNINGS:
SNOW: - primarily down on earnings because of the fact that the CEo stepped down. Said they have lowballed guidance to set a lwoer bar for new CEOs. Said it was a conservative guidance.

• Current quarter EPS beats by 2%
• EPS of 0.35 beat by 94%
• Dollar net retention rate was 131% from 151% last year. But this was misleading as they were ramping customers up quickly
• Product Revenue Guidance:
• Next quarter: 747.5m, miss by 3.3%
• Full year: 3.25B, miss by 7.2%
• 2025 operating margin of just 6% vs 9.5% expected
• AS mentioned, these were conservative guidance
• CEO steps down, the previous Senior VP of AI was named the new CEO. This appointment helps to push them towards AI.
• New CEO said he wants to a accelerate innovation for customers.

• FUll year guidance:

• EPS of 9.72 beat by 1%
• Guidance wasn’t bad, but more or less in line with previous trends, so didn’t exactly accelerate despite recent price increases and product bundling.
• Starts dividend, increases share repurchases to 10B.
• Not a bad quarter here.
• Story is getting better on improved execution and AI.
• Talked a lot about AI on the earnings call. And how it will help provide their customers with better productivity.

ANALYSIS:

• Dollar risk reversal continues to point lower, traders expect dollar to fall in medium term.
• Can get short term volatility from PCe today. It seems from the fact that skew on TLT points higher, that traders are expecting an in line or soft PCE.
• I showed also that Deutsche and Wells Fargo seem to point to an in line or soft PCE too. I however, have doubts, as CPI and PPI came hot and PCE can follow. This can push dollar higher to 104.6, but let’s see. If it comes soft, dollar can fall back to 103.3.
• JPY is rising today on BOJ’s Takata pointing to inflation target coming in view. This is increasing chances of monetary tightening. Risk reversal on USDJPY continues to point sharply lower. Definitely expecting this to sell off soon.
• A lot of todays price action will depend on PCE. We saw Market take a breather last couple of days, which is healthy. Skew on mag7 starting to point higher again. If we see good PCe can be a push back to nasdaq 18k and SPX 5100. NVDA taking a breather but the bullish positioning remains, so this can lead QQQ higher soon.
• Whilst hedge funds are rotating out of tech, SPX money flows continue to surge, which is bullish overall market. Not seeing big signs of a major pullback still. Just short term volatility ahead of uncertain data.
• Probably, if the market dips, it will eb short lived.
• Traders continue to be short VIX another sign that it won’t be a long term sell off.
• Main struggling Mag7 continue to be GOOGL and AAPL. Others show positive skew.
• SNOW getting killed right now on the CEO change. The guidance was a miss but they pointed to the fact it was conservative, as is common practice when new CEO comes in.
• I think can present an opportunity. For intraday plays, may be an idea to play another Cloud stock which is getting dragged, such as DDOG. MDB or ESTC
• On SNOW, 166 is my buy price, but it may not hit. Will be watching the candlesticks for intraday reversal.

• Came 1.1% mom, slight miss on 1.5% expectation.

• Came slightly hotter than expected
• YOY was 2.9% vs 2.7% expected
• MOM was 0.8%, vs 0.7% expected.

• Note: Despite hot CPI in France, PPI which was also released came at lowest level since 2009. So mixed inflation readings there. Maybe temporary tick up and PPI points to lower inflation after that.

• Swiss GDP growth rate YOY was 0.6% vs 0.7% expected
• QOQ was 0.3% vs 0.1% forecast.
• So mixed GDP numbers, CHF moved lwoer on the data initially, then recovered higher.
• GER Unemployment Rate (Feb) - expecting continued weakness. MOM unemployment change to be positive again after surprising negative last month. Effect will be overshadowed by German CPI.

The most prolific spies, in this day and age, are millions of computer scripts working in the background to serve up the most important information to their masters. While military and political bodies engage in this behavior on a technologically advanced scale, the truth is that anyone can tap into this infrastructure to perform their own “intelligence” activities to stay informed, make money, keep secrets, uncover secrets, etc.
In ancient history, leaders even considered “spiritual intelligence” from oracles, astrology and prophecy to be legitimate. It is curious to note the symbols from these groups still persist in the intelligence community, today. After a failure of this type of intelligence, the Romans admit, “trust in the gods, but verify.”

Good intelligence is a force multiplier and it is important to know the history to better understand how and why. The history of intelligence starts with battlefield intelligence through the use of scouts; where men were sent out ahead of the bulk of their forces to uncover useful information about enemy troop locations, numbers and supporting logistics. Compartmentalization is a primary consideration in all intelligence operations. There is no reason to have the group collecting the raw data interact with the group analyzing the data or either of those groups to act upon the data. It is in the interests of national security for the intelligence apparatus to root out and destroy corrupting influences; but that is not what is happening. Self-fulfilling prophecy, is a primary facet of "Systems Psychodynamics;" telling people something over and over again until they believe it and act on it. People act the way they think they are supposed to act, so working on the basic assumptions of how they are supposed to act can change behavior.
One example is every horror movie where everyone splits up and acts hysterical. Only idiots act this way. Normal people work together in a rational manner.
If you tell a small child they are worthless every single day, they will have low self-esteem. If you build them up, instead, every single day, they have better self-esteem and are more likely to succeed.
Faking a consensus through saturation and repetition is also a type of self-fulfilling prophecy. A child sees all the other kids on TV playing with a toy and they want that toy. A child hears everyone at school talking about something they saw on TV and they want to see it, too.
The media machine wants everyone to think they are worthless; a state of mind and perception that is more marketable and easier to emotionally manipulate.
The news, TV shows, movies, video games, etc., are all using social dreaming to bring about a view or reality that is a faked consensus, but since one group can control the narrative on all these platforms, the fake consensus is very easy to create.
Tension centered strategies (creating anxiety/angeetc.) are used to create "organizational paradoxes," which is the "splitting" of groups. These groups are then kept in a "vicious cycle" of arguing and insulting so that no progress can be made without the manipulation of those overseeing the tension.
I think the monkey and the grape experiment shows exactly how easy it is to manipulate people using their sense of injustice as a trigger.

The twenty-two boys in the study were unknown to each other and all from white middle-class backgrounds. They all shared a Protestant, two-parent background. None of the boys knew each other prior to the study. The boys were randomly assigned to one of two groups, although neither was aware of the other’s existence. They were then, as individual groups, picked up by bus on successive days in the summer of 1954 and transported to a 200-acre Boy Scouts of America camp in the Robbers Cave State Park in Oklahoma.
At the camp the groups were kept separate from each other and were encouraged to bond as two individual groups through the pursuit of common goals that required co-operative discussion, planning and execution. During this first phase, the groups did not know of the other group's existence. The boys developed an attachment to their groups throughout the first week of the camp, quickly establishing their own cultures and group norms, by doing various activities together like hiking, swimming, etc. The boys chose names for their groups, The Eagles and The Rattlers, and stenciled them onto shirts and flags.
Sherif now arranged the 'competition stage' where friction between the groups was to occur over the next 4-6 days. In this phase it was intended to bring the two groups into competition with each other in conditions that would create frustration between them. A series of competitive activities (e.g. baseball, tug-of-war etc.) were arranged with a trophy being awarded on the basis of accumulated team score. There were also individual prizes for the winning group such as a medal and a multi-bladed pocket knife with no consolation prizes being given to the "losers."

What this study found, though, was that both teams would work together to get food and water, as opposed to the original premise.
Unless someone was there to keep feeding the vicious cycle, people would, eventually, work together. What we learn from this, is that the news is primarily organized around creating tension to perpetuate a vicious cycle.

INTRODUCTION
1) Operation CUCKOO was part of the overall operation CLEANSWEEP, aimed at eliminating domestic opposition to activities undertaken by the Central Intelligence Agency's special activities division, in main regard to operation GUILLOTINE.

5) Special Activities Division was given the green light after briefing president Johnson on the situation. The situation report was forwarded to the Department of Defense and the Joint Chiefs of staff, who both approved of the parameters of the operation, as outlined under article C of secret executive order 37 (see attached copy of article).
PLANNING STAGES
6) 8 members of the special activities division handpicked by operation lead William King Harvey began planning for the operation on October 3rd, with planned execution before October 16th.
7) The (?) of the operation was set as the neighborhood of Georgetown along the Potomac river, where the operators would observe, take note on routines, and eventually carry the operation.
8) After nothing Meyer's routines, Edward "Eddy" Reid was picked as the operation point man who would intersect Meyer on her walk on October 12th, with lead William King Harvey providing long range support if necessary from across the Chesapeake and Ohio canal (see illustration A for detailed map).
9) Edward Reid was planned to be dressed in the manner of a homeless black man, due to his resemblances to local trash collector (later found out to be Raymond Crump) who inhabits the AO and the path that Reid was planned to intersect Meyer.

Background:
Been dealing with GERD since fall 2014 after coming back from a trip. Took Nexium 40 mg for about 5 years which used to work well and was able to stop taking it afterwards for about 2 years. However, for the past 2 years, GERD has made a come back and now I need to be on Dexilant 60 mg + Zantac 150 mg twice daily which still doesn't solve the issue. Latest gastroscopy saw irritated z-line and acid eroded back teeth, no hiatal hernia though. Diet is already good overall, avoiding most common trigger foods. Healthy weight.
For 2024:
Diet:

• Avoid all trigger foods
• low fat diet
• 30 different vegetables, fruits and nuts per week
• low-acidity fermented food once daily
• low-fat protein sources like chicken or tofu
• High omega-3 fish twice a week

Morning routine:

1. Wake up take PPI, while waiting the mandatory 30 min before breakfast
   1. Meditate (subconscious anxiety might be the biggest culprit)
   2. Japanese bridge exercise for LES strengthening
   3. full body mobility
2. Take Zantac
   1. small breakfast of oats, banana, nuts
3. Go to gym
4. Go to work

Will religiously track symptoms, what I eat and overall mood to possibly find sources.
I should also get the results of a gut biome analysis mid-January, will see what comes out of that and maybe take pro-biotic supplements.
Hopefully something in all this will help, will keep updating about every month.

5 week update …………………… Advanced diet possibly a little too quickly, I would say that the reflux I have is markedly reduced and I assume the wrap is working because I do not have ability to burp nor do I get large amounts of sour food/liquid but I still get the sensation of a burp that triggers a hiccup-like response. I have been taking 20mg Famotidine once a day and Gas-X/TUMS to try and limit the acidity but am out of PPIs so didn’t resume. Left message with surgical assistant that I am experiencing reflux and needed to know what to do next other than tracking what I’m eating as you would pre-surgery with standard reflux. I briefly ran for a few seconds on a walking trail chasing my son but stopped myself when I realized that’s probably a bad idea.

I had a partial (Toupet) Fundoplication and am honestly unsure how they measure 270 degrees during these surgeries.
…………………… At 17 (two decades ago) I was first diagnosed with acid reflux. The internist I saw readily prescribed Prilosec and figured it would knock the pain I felt when eating fried foods. It did but only for a year at which point I switched to Pantoprazole. Two years later I switched to Prevacid. Seven years later I switched to Zantac 150 just to try something different.
For the next ten years, I was taking Prevacid twice daily just to have normalcy but I still had reflux every meal. There was a brief six month period I switched to Nexium and I warn you all Nexium can cause lower esophageal sphincter spasms at night. I described it and my GI and PCP both said it wouldn’t cause severe spasms but I assure you it did more harm than good. During this time I developed a shortness of breath I can only describe as painful deep breathing. I began to notice burping sounds internally after meals deep in my throat and began to have liquids regurgitate regularly. Over the course of two years and two scopes including a useless inconclusive Bravo PH Study, I was diagnosed with LPR by an ENT. The problem with LPR is that the doctors try to treat it with PPIs and they may not help with these symptoms. I tried altering my diet completely, going gluten free, removing alcohol, sugar, etc. The symptoms of LPR remained and I began to feel hopeless. Finally, fed up, I switched my GI to another GI in the group and was immediately referred to a surgeon for consult. I teared up…
The first visit to the surgeon left me feeling defeated because after $10,000 in endoscopies I was told there was no evidence of reflux yet. I agreed to another set of tests determined to get answers as I do not want Barrett’s. The esophageal manometry showed I had a small hiatal hernia! The PH 24-hr study was very uncomfortable but worked better than the Bravo so I encourage you to just endure the discomfort. You can visibly see the acidity rising on your PH meter. This test showed I was experiencing regular reflux throughout the day.
Finally, I had conclusive answers after being told by my PCP and GI for three years I was just supposed to live this way. The surgeon literally said “You are not crazy.” Based on my low pressure readings but overall good swallow results from the manometry testing LINX and TIF were not advised as they’d likely cause issues swallowing for me. The Toupet Fundoplication was his suggested course of treatment as he felt it would provide a good balance of relief with fewer new symptoms of a Nissen Fundoplication. Most reviews of too tight wraps are the latter from what I saw and he’s been doing these surgeries for 20 years so I went with it.
December 4th I had my surgery. I stayed one night in the hospital and came home the next morning. I had many protein shakes and ate boxes of popsicles and blended up ice cream and Lactaid in addition to broths. The first week I thought I’d made a huge mistake. I couldn’t sleep the gas pain was so bad. I couldn’t swallow my Tylenol without crushing. On the 7th day things suddenly started to get better and I started to eat Cream of Chicken soup. The 13th day I ate mashed potatoes and also eggs. It was the best food ever as far as I was concerned.
I’m now at three weeks post-op. I do still feel the inflammation and shortness of breath but have had zero reflux or regurgitation. I have to chew everything solid for 30 seconds and take half the size bites I used to. You will learn quickly that food getting stuck is excruciatingly uncomfortable. You just have to wait it out for 10-15 seconds if you swallow air. I have discovered that I do burp although smaller than before. I was told it may take 6 months for the LPR symptoms to fully subside as the damage has been constant for years. Time will tell. However, given the choice, I would still get the surgery again instead of living with the reflux and burning daily.
Be your own advocate. If a doctor says nothing’s wrong but you still have symptoms switch doctors. You are the only person who knows how you feel. The medications do often have adverse effects and won’t fix low esophagus pressure.
