I have a question. I got denied for Gerd/ acid reflux 9 years ago. I have documented in medical records that I went to the ER and thought I was having a heart attack. The abdominal exam did show some mild epigastric discomfort to deep palpation. I was given Zantac 150 mg twice a day. Fast forward today. My acid reflux has gotten worst over time. I have changed my diet. I don’t eat foods that contains acidic foods, nothing spicy. Do I file a new claim since I was denied 9 years ago?

Can someone explain why D would NOT be a viable answer? No history of serious illness but guy hasn't seen a doctor in 20 years and A1c 8.9% with BP 150/90 - so I assumed diabetes/HTN and jumped to hyaline arteriolosclerosis (protein accumulation in the renal arteriolar walls). I see the logic of A (BPH -> hydronephrosis -> elevated Cr) but why are D and possibly hyaline arteriolosclerosis incorrect? Thanks!

12 months of misery, 12 months of pain. Malnourished, depressed, suicidal. 185cm and dropped to 150 pounds. Every night I was ready to die in my sleep from malnutrition.Even though smoking is bad I was doing 20-30 cigarettes a day, taking xanax, PPIs, zantac.
Finally my stomach decided to improve and although I am not healed and will never heal I feel alive again. I can digest foods and have energy. I am in my 30s and this chronic gastritis has made me stupid and immature.
But now that I feel human I want to run, lift weights and take vengeance from life. But I have fear. Will it come back? Will my heart and lungs collapse? I mean I was 1 year in bed eating nothing.

In the 15th Century, the term “intelligence” began to be used in reference to secret knowledge from spies. The processes of collection, interpretation and implementation of military and political knowledge has grown in scope, throughout human history, but many of the principles have been the same for thousands of years.

While many people often think of “spies” as James Bond like figures, the truth is usually a lot more boring and slow. The most prolific spies, in this day and age, are millions of computer scripts working in the background to serve up the most important information to their masters. While military and political bodies engage in this behavior on a technologically advanced scale, the truth is that anyone can tap into this infrastructure to perform their own “intelligence” activities to stay informed, make money, keep secrets, uncover secrets, etc.
In ancient history, leaders even considered “spiritual intelligence” from oracles, astrology and prophecy to be legitimate. It is curious to note the symbols from these groups still persist in the intelligence community, today. After a failure of this type of intelligence, the Romans admit, “trust in the gods, but verify.”

Good intelligence is a force multiplier and it is important to know the history to better understand how and why. The history of intelligence starts with battlefield intelligence through the use of scouts; where men were sent out ahead of the bulk of their forces to uncover useful information about enemy troop locations, numbers and supporting logistics. Compartmentalization is a primary consideration in all intelligence operations. There is no reason to have the group collecting the raw data interact with the group analyzing the data or either of those groups to act upon the data. It is wise to even isolate groups within these groups so that no one party has the full picture. Bion specifically identified three basic assumptions: dependency, fight-flight, and pairing. When a group adopts any one of these basic assumptions, it interferes with the task the group is attempting to accomplish.
This makes Bion's job easy; convince everyone they are dependent on government, should be fighting each other, should be scared of each other and should not pair with each other.
While the idea of a large intelligence apparatus used to apply force multipliers to military operations is a good one and very effective; we will never see its full potential fulfilled until corrupt influences are removed from the equation. As the Romans learned, politicians with the reigns of intelligence are easily influenced and are a national security threat, when manipulated by enemies of its countryy's citizens.
It is in the interests of national security for the intelligence apparatus to root out and destroy corrupting influences; but that is not what is happening. Instead, we have a veritable web of slime that has exposed itself as it becoming drunken-mad with new, vast powers at their fingertips. The corrupted influences within the US government have turned the intelligence apparatus and the good, hard-working people on themselves; when it should be the other way around.
The only way to reverse this trend is to implement transparency and the only way to achieve that is to apply persistent and continuous pressure to the powers that be.

By funneling “intelligence” to increasingly smaller compartments, it becomes possible for the top compartment to control all others. Manipulating votes, then becomes a basic matter of group psychology.

Now, imagine if you were a peaceful team and only the other team was being manipulated. Political operatives call this strategy, "othering."
Self-fulfilling prophecy, is a primary facet of "Systems Psychodynamics;" telling people something over and over again until they believe it and act on it. People act the way they think they are supposed to act, so working on the basic assumptions of how they are supposed to act can change behavior.
One example is every horror movie where everyone splits up and acts hysterical. Only idiots act this way. Normal people work together in a rational manner.
If you tell a small child they are worthless every single day, they will have low self-esteem. If you build them up, instead, every single day, they have better self-esteem and are more likely to succeed.
Faking a consensus through saturation and repetition is also a type of self-fulfilling prophecy. A child sees all the other kids on TV playing with a toy and they want that toy. A child hears everyone at school talking about something they saw on TV and they want to see it, too.
The media machine wants everyone to think they are worthless; a state of mind and perception that is more marketable and easier to emotionally manipulate.
The news, TV shows, movies, video games, etc., are all using social dreaming to bring about a view or reality that is a faked consensus, but since one group can control the narrative on all these platforms, the fake consensus is very easy to create.
Tension centered strategies (creating anxiety/angeetc.) are used to create "organizational paradoxes," which is the "splitting" of groups. These groups are then kept in a "vicious cycle" of arguing and insulting so that no progress can be made without the manipulation of those overseeing the tension.
I think the monkey and the grape experiment shows exactly how easy it is to manipulate people using their sense of injustice as a trigger.

Robber's Cave Experiment

The twenty-two boys in the study were unknown to each other and all from white middle-class backgrounds. They all shared a Protestant, two-parent background. None of the boys knew each other prior to the study. The boys were randomly assigned to one of two groups, although neither was aware of the other’s existence. They were then, as individual groups, picked up by bus on successive days in the summer of 1954 and transported to a 200-acre Boy Scouts of America camp in the Robbers Cave State Park in Oklahoma.
At the camp the groups were kept separate from each other and were encouraged to bond as two individual groups through the pursuit of common goals that required co-operative discussion, planning and execution. During this first phase, the groups did not know of the other group's existence. The boys developed an attachment to their groups throughout the first week of the camp, quickly establishing their own cultures and group norms, by doing various activities together like hiking, swimming, etc. The boys chose names for their groups, The Eagles and The Rattlers, and stenciled them onto shirts and flags.
Sherif now arranged the 'competition stage' where friction between the groups was to occur over the next 4-6 days. In this phase it was intended to bring the two groups into competition with each other in conditions that would create frustration between them. A series of competitive activities (e.g. baseball, tug-of-war etc.) were arranged with a trophy being awarded on the basis of accumulated team score. There were also individual prizes for the winning group such as a medal and a multi-bladed pocket knife with no consolation prizes being given to the "losers."
What this study found, though, was that both teams would work together to get food and water, as opposed to the original premise.
Unless someone was there to keep feeding the vicious cycle, people would, eventually, work together. What we learn from this, is that the news is primarily organized around creating tension to perpetuate a vicious cycle.

INTRODUCTION
1) Operation CUCKOO was part of the overall operation CLEANSWEEP, aimed at eliminating domestic opposition to activities undertaken by the Central Intelligence Agency's special activities division, in main regard to operation GUILLOTINE.
2) Operation CUCKOO was approved by the Joint Chiefs of Staff, Department of Defense and the office of The President of the United States as a covert domestic action to be under taken within the limits of Washington D.C as outlined by Secret Executive Order 37.
3) Following the publishing of the Warren Commission, former special agent Mary Pinchot Meyer (Operation MOCKINGBIRD, Operation SIREN) also was married to Cord Meyer (Operation MOCKINGBIRD, Operation GUILLOTINE) threatened to disclose the details of several Special Activities Divisions' operations, including but not limited to, Operation SIREN and GUILLOTENE.
4) It was deemed necessary by senior Directorate of Operations members to initiate Operation CUCKOO as an extension of Operation CLEANSWEEP on November 30th. After Mary Pinchot Meyer threatened to report her knowledge of Operation GUILLOTENE and the details of her work in Operation SIREN from her affair with the former President.
5) Special Activities Division was given the green light after briefing president Johnson on the situation. The situation report was forwarded to the Department of Defense and the Joint Chiefs of staff, who both approved of the parameters of the operation, as outlined under article C of secret executive order 37 (see attached copy of article).

Posting this to keep myself accountable, explore healing possibilities and update the community.
Background:
Been dealing with GERD since fall 2014 after coming back from a trip. Took Nexium 40 mg for about 5 years which used to work well and was able to stop taking it afterwards for about 2 years. However, for the past 2 years, GERD has made a come back and now I need to be on Dexilant 60 mg + Zantac 150 mg twice daily which still doesn't solve the issue. Latest gastroscopy saw irritated z-line and acid eroded back teeth, no hiatal hernia though. Diet is already good overall, avoiding most common trigger foods. Healthy weight.
For 2024:

Morning routine:
  1. Wake up take PPI, while waiting the mandatory 30 min before breakfast
    1. Meditate (subconscious anxiety might be the biggest culprit)
    2. Japanese bridge exercise for LES strengthening
    3. full body mobility
  2. Take Zantac
    1. small breakfast of oats, banana, nuts
  3. Go to gym
  4. Go to work
Will religiously track symptoms, what I eat and overall mood to possibly find sources.
I should also get the results of a gut biome analysis mid-January, will see what comes out of that and maybe take pro-biotic supplements.
Hopefully something in all this will help, will keep updating about every month.

5 week update …………………… Advanced diet possibly a little too quickly, I would say that the reflux I have is markedly reduced and I assume the wrap is working because I do not have ability to burp nor do I get large amounts of sour food/liquid but I still get the sensation of a burp that triggers a hiccup-like response. I have been taking 20mg Famotidine once a day and Gas-X/TUMS to try and limit the acidity but am out of PPIs so didn’t resume. Left message with surgical assistant that I am experiencing reflux and needed to know what to do next other than tracking what I’m eating as you would pre-surgery with standard reflux. I briefly ran for a few seconds on a walking trail chasing my son but stopped myself when I realized that’s probably a bad idea.
Just updating anyone who is interested.
I had a partial (Toupet) Fundoplication and am honestly unsure how they measure 270 degrees during these surgeries.
…………………… At 17 (two decades ago) I was first diagnosed with acid reflux. The internist I saw readily prescribed Prilosec and figured it would knock the pain I felt when eating fried foods. It did but only for a year at which point I switched to Pantoprazole. Two years later I switched to Prevacid. Seven years later I switched to Zantac 150 just to try something different.
For the next ten years, I was taking Prevacid twice daily just to have normalcy but I still had reflux every meal. There was a brief six month period I switched to Nexium and I warn you all Nexium can cause lower esophageal sphincter spasms at night. I described it and my GI and PCP both said it wouldn’t cause severe spasms but I assure you it did more harm than good. During this time I developed a shortness of breath I can only describe as painful deep breathing. I began to notice burping sounds internally after meals deep in my throat and began to have liquids regurgitate regularly. Over the course of two years and two scopes including a useless inconclusive Bravo PH Study, I was diagnosed with LPR by an ENT. The problem with LPR is that the doctors try to treat it with PPIs and they may not help with these symptoms. I tried altering my diet completely, going gluten free, removing alcohol, sugar, etc. The symptoms of LPR remained and I began to feel hopeless. Finally, fed up, I switched my GI to another GI in the group and was immediately referred to a surgeon for consult. I teared up…
The first visit to the surgeon left me feeling defeated because after $10,000 in endoscopies I was told there was no evidence of reflux yet. I agreed to another set of tests determined to get answers as I do not want Barrett’s. The esophageal manometry showed I had a small hiatal hernia! The PH 24-hr study was very uncomfortable but worked better than the Bravo so I encourage you to just endure the discomfort. You can visibly see the acidity rising on your PH meter. This test showed I was experiencing regular reflux throughout the day.
Finally, I had conclusive answers after being told by my PCP and GI for three years I was just supposed to live this way. The surgeon literally said “You are not crazy.” Based on my low pressure readings but overall good swallow results from the manometry testing LINX and TIF were not advised as they’d likely cause issues swallowing for me. The Toupet Fundoplication was his suggested course of treatment as he felt it would provide a good balance of relief with fewer new symptoms of a Nissen Fundoplication. Most reviews of too tight wraps are the latter from what I saw and he’s been doing these surgeries for 20 years so I went with it.
December 4th I had my surgery. I stayed one night in the hospital and came home the next morning. I had many protein shakes and ate boxes of popsicles and blended up ice cream and Lactaid in addition to broths. The first week I thought I’d made a huge mistake. I couldn’t sleep the gas pain was so bad. I couldn’t swallow my Tylenol without crushing. On the 7th day things suddenly started to get better and I started to eat Cream of Chicken soup. The 13th day I ate mashed potatoes and also eggs. It was the best food ever as far as I was concerned.
I’m now at three weeks post-op. I do still feel the inflammation and shortness of breath but have had zero reflux or regurgitation. I have to chew everything solid for 30 seconds and take half the size bites I used to. You will learn quickly that food getting stuck is excruciatingly uncomfortable. You just have to wait it out for 10-15 seconds if you swallow air. I have discovered that I do burp although smaller than before. I was told it may take 6 months for the LPR symptoms to fully subside as the damage has been constant for years. Time will tell. However, given the choice, I would still get the surgery again instead of living with the reflux and burning daily.
Be your own advocate. If a doctor says nothing’s wrong but you still have symptoms switch doctors. You are the only person who knows how you feel. The medications do often have adverse effects and won’t fix low esophagus pressure.
